      • Association of CYP2C19 Polymorphisms with Survival of Breast Cancer Patients Using Tamoxifen: Results of a Meta-analysis

        Background: Previous studies accessing the association of CYP2C19 with outcomes of patients using tamoxifen for breast cancer have yielded conflicting results. The aim of this meta-analysis is to obtain a more precise estimate of effects of CYP2C19 polymorphisms and to clarify their effects on survival of the breast cancer patients using tamoxifen. Materials and Methods: A systematic search of PubMed and Embase was performed, comparing patients with or without $CYP2C19^*2$ and $CYP2C19^*17$, relevant articles searched for. The following outcomes were included from the eligible studies: disease-free survival (DFS) and overall survival (OS), expressed by hazard ratios (HR) with corresponding 95% confidence interval (CI). Subgroup analysis by genotypes was also performed. Pooled estimates were calculated using random-effect model in accordance to the heterogeneity. Results: Six studies met the inclusion criteria. The integrated OR on the association between CYP2C19 and DFS, calculated by the random-effect model, was 0.54 (95%CI=0.34-0.84, p=0.013). Subgroup analysis showed that both $CYP2C19^*2$ and $CYP2C19^*17$ were associated with increased survival. The pooled results of two studies for OS were OR=0.46 (95%CI=0.21-1.01, p=0.233). Conclusions: This meta-analysis suggests that the $CYP2C19^*2$ and $CYP2C19^*17$ genotypes are associated with increased survival in breast cancer patients using tamoxifen.

        Density Functional Theory Study on Triphenylamine-based Dye Sensitizers Containing Different Donor Moieties

        Density functional theory (DFT) and time-dependent DFT (TD-DFT) calculations have been employed to investigate the molecular structures and absorption spectra of two dyes containing diphenylaniline and 4-diphenylamino-diphenylaniline as donor moiety (TPA1 and TPA3). The geometries indicate that the strong conjugation is formed in the dyes. The electronic structures suggest that the intramolecular charge transfer from the donor to the acceptor occurs, and the electron-donating capability of 4-diphenylamino-diphenylaniline is stronger than that of diphenylaniline. The computed highest occupied molecular orbital (HOMO) energy levels are -5.31 and -4.90 eV, while the lowest unoccupied molecular orbital (LUMO) energies are -2.29 and -2.26 eV for TPA1 and TPA3, respectively, revealing that the interfacial charge transfer between the dyes and the semiconductor electrode are electron injection processes from the photon-excited dyes to the semiconductor conduction band. Furthermore, all the experimental absorption bands of TPA1 and TPA3 have been assigned according to the TDDFT calculations.

        Electromagnetic-Field Dependence of the Internal Excited State of the Polaron and the Qubit in Quantum Dot with Thickness

        The electromagnetic-field dependence of the ground and the first excited-state (GFES) energy eigenvalues and eigenfunctions of the strong-coupling polaron in a quantum dot (QD) was studied for various QD thicknesses by using the variational method of the Pekar type (VMPT). On this basis, we construct a qubit in the quantum dot (QQD) by taking a two-level structure of the polaron as the carrier. The results of numerical calculations indicate that the oscillation period of the qubit, T0, increases with increasing the thickness of the quantum dot (TQD) L, but decreases with increasing the cyclotron frequency of the magnetic field (CFMF) !c, electric-field strength F, and electron-phonon coupling strength (EPCS) . The probability density of the qubit | (, z, t)|2 presents a normal distribution of the electronic transverse coordinate , significantly influenced by the TQD and effective radius of the quantum dot (ERQD) R0, and shows a periodic oscillation with variations in the electronic longitudinal coordinate z, polar angle and time t. The decoherence time and the quality factor Q of the free rotation increase with increasing the CFMF !c, dispersion coefficient , and EPCS , but decrease with increasing the electric-field strength F, TQD L, and ERQD R0. The TQD is an important parameter of the qubit. Theoretically, the target, which is to regulate the oscillation period, decoherence time and quality factor of the free rotation of the qubit, can be achieved by designing different TQDs and regulating the strength of the electromagnetic field.

        Low beta superconducting cavity system design for HIAF iLinac

        A superconducting ion-Linac (iLinac), which is supposed to work as the injector in the High Intensity heavy-ion Accelerator Facility project, is under development at the Institute of Modern Physics (IMP), Chinese Academy of Sciences. The iLinac is a superconducting heavy ion linear accelerator approximately 100 meters long and contains 96 superconducting cavities in two types of 17 cyromodules. Two types of superconducting resonators (quarter-wave resonators with a frequency of 81.25 MHz and an optimal beta β = v/c = 0.07 called QWR007 and half-wave resonators with a frequency of 162.5 MHz and an optimal beta β = 0.15 called HWR015) have been investigated. The cavity design included extensive multi-parameter electromagnetic simulations and mechanical analysis, and its results are described in details. The fundamental power coupler and cavity dynamic tuner designs are also presented in this article. The prototypes are under manufacturing and expected to be ready in 2023.

        Fluoroscopically Guided Three-Tube Insertion for the Treatment of Postoperative Gastroesophageal Anastomotic Leakage

        Objective: To retrospectively evaluate the feasibility and effectiveness of three-tube insertion for the treatment of postoperative gastroesophageal anastomotic leakage (GEAL). Materials and Methods: From January 2007 to January 2011, 28 cases of postoperative GEAL after an esophagectomy with intrathoracic esophagogastric anastomotic procedures for esophageal and cardiac carcinoma were treated by the insertion of three tubes under fluoroscopic guidance. The three tubes consisted of a drainage tube through the leak, a nasogastric decompression tube, and a nasojejunum feeding tube. The study population consisted of 28 patients (18 males, 10 females) ranging in their ages from 36 to 72 years (mean: 59 years). We evaluated the feasibility of three-tube insertion to facilitate leakage site closure, and the patients’ nutritional benefit by checking their serum albumin levels between pre- and post-enteral feeding via the feeding tube. Results: The three tubes were successfully placed under fluoroscopic guidance in all twenty-eight patients (100%). The procedure times for the three tube insertion ranged from 30 to 70 minutes (mean time: 45 minutes). In 27 of 28 patients (96%), leakage site closure after three-tube insertion was achieved, while it was not attained in one patient who received stent implantation as a substitute. All patients showed good tolerance of the three-tube insertion in the nasal cavity. The mean time needed for leakage treatment was 21 ± 3.5 days. The serum albumin level change was significant, increasing from pre-enteral feeding (2.5 ± 0.40 g/dL) to post-enteral feeding (3.7 ± 0.51 g/dL) via the feeding tube (p < 0.001). The duration of follow-up ranged from 7 to 60 months (mean: 28 months). Conclusion: Based on the results of this study, the insertion of three tubes under fluoroscopic guidance is safe, and also provides effective relief from postesophagectomy GEAL. Moreover, our findings suggest that three-tube insertion may be used as the primary procedure to treat postoperative GEAL.         Earthquake response analysis of series reactor

        A direct transfer substructure method is presented in this paper for analyzing the dynamic characteristics and the seismic random responses of a series reactor. This method combines the concept of FRF (frequency response function) and the transfer matrix algorithm with the substructure approach. The inner degrees of freedom of each substructure are eliminated in the process of reconstruction and the computation cost is reduced greatly. With the convenient solution procedure, the dynamic characteristics analysis of the structure is valid and efficient. Associated with the pseudo excitation algorithm, the direct transfer substructure method is applied to investigating the seismic random responses of the series reactor. The numerical results demonstrate that the presented method is efficient and practicable in engineering. Finally, a precise time integration method is employed in performing a time-history analysis on the series reactor under El Centro and Taft earthquake waves.
