        악성 림프종 환자에서 발생한 다형홍반

        A Case of Erythema Multiforme A ssociated with Malignant Lymphoma Un Cheol Yeo, M.D., Kwang Hyun Cho, M.D., Yoo Shin Lee, M.D., Dae Seog Heo*, M.D., Seoung Chul Lee**, M.D Department of Dermatology and Internal Medicine*, College of Medicine, Seoul National University, Seoul, Korea, Department of Dermatology**, College of Medicine, Inha University, Kyung Gi Do, Korea We report a case of erythema multiforme associated with malignant lymphoma. Fifty-six-year-old male patient visited our clinic complaining generalized skin lesions which were compatible with erythema multiforme clinically and histopathologically. The skin lesions were treatment-resistant, recurrent and persistent for 3 years. In consideration of old age, atypical features of erythema multiforme and no other demonstrable etiology of erythema multiforme, we studied for internal malignancy. Malignant lymphoma at the duodenum was revealed by UGI series, abdominal CT, and endoscopic biopsy. After chemotherapy, the tumor mass and active skin lesions were cleared simultaneously. Erythema multiforme may be regarded as one of the paraneoplastic syndromes.

      • 청국장 발효에 적합한 미생물의 분리 및 특성

        장류발효 식품으로부터 protease 및 amylase 활성이 높은 균주를 중심으로 348 균주를 1차 분리하고, 그 분리균 중 2% skim milk가 함유된 LB평판배지(SM배지)화 2%corn starch가 함유된 LB평판배지(CM배지)에서 halo zone이 크고, 두 효소의 활성이 동시에 높은 SW-251균주를 최종 선별하였다. SW-251균주의 생육 온도 범위는 10∼60℃ 이었으며, 최적 생육온도는 35∼40℃ 이었다. 그리고 최적생육 pH는 6.5 부근이었으나 pH6.0 이하나 pH8.0 이상에서는 생육이 저하하였다. SW-251균주는 호기성의 간균 (0.8∼1.3x1.8∼2.5㎛)으로 운동성이 있으며, 그람양성 반응을 나타냈다. 10% 이하의 NaCl에 내성을 갖고있으며, starch 및 casein을 빠르게 분해하는 세균으로 Bacillus속의 특성을 갖고 있었다. Among 348 strains isolated from fermented soy foods, SW-251 strain has a potent protease and amylase activities on SM medium(LB agar plate supplemented with 2% skim milk), and CM medium(LB agar plate supplemented with 2% corm starch0, respectively. The optimal temperature and growth temperature range for cell growth were 35∼40℃ and 10∼60℃, respectively. The optimal pH was 6.5 and pH range of its growth was 6.0 and 8.0. The isolate was an aerobic bacterium (rod type, 0.8∼1.3 x 1.8∼2.5㎛), then there was its mobility, Gram staining and hydrolysis of starch and casein were positive reaction. Salt tolerance was below 10% NaCl. The strain was identified as a Bacillus sp. SW-251 with respect to morphological, cultural and physiological characteristics.

      • Methylprednisolone 및 Levamisole 투여가 Bovine Gamma-Globulin으로 초래된 생쥐의 IgA 신병증에 미치는 영향

        목적 : 본 연구는 생쥐를 대상으로 실험적 IgA 신병을 유발시키고 부신피질제재 및 levamisole 등의 면역제재를 투여하여 이들 약제의 IgA 신병증에 미치는 영향을 알아보고자 시행하였다. 대상 및 방법 : 실험동물은 체중 20-30gm 생쥐로서 암수 구별없이 사용하였다. 실험에 사용한 Bovine Gamma-Globulin (BGG, Sigma, U.S.A.)은 0.1% 농도로 식수에 섞어 매일 섭취시켰으며 제Ⅰ군은 BGG만을 섭취시켰고, 제Ⅱ군은 BGG섭취와 같은 시기부터 methylprednisolone을 격일로 존대를 통하여 경구 투여하였으며, 제Ⅲ군은 BGG 섭취와 같은 시기부터 levamisole을 격일로 존대를 통하여 경구 투여하였다. 실험 제 100일째 실험동물을 도살하고 신장을 적출하여 병리조직학적 검사를 시행하였다. 결과 : 실험개시후 100일째에 채취한 혈액검사상 혈청 BUN과 creatinine치는 각군간에 유의한 차이가 없었다. 병리조직학적 변화는 광학현미경 소견상 세군 모두에서 사구체의 미세한 변화만이 관찰되었다. 전자현미경적 소견은 세군 모두에서 기저막의 비후는 없었으며 족돌기도 잘 유지되어 있었다. 기저막쪽으로 향한 메산지움 영역에서 전자밀도가 높은 침착물이 흔히 관찰되었다. 면역형광현미경적 소견은 형광물질의 강약에 따라 약양성 (+), 중등도 양성 (++) 및 강양성(+++) 으로 구분하였다. 제Ⅰ군에서 형광물질은 IgA에서만 실험동물 9마리 모두에서 관찰되었다. 반면 IgM. IgG및 C_3는 모두 음성이었다. 면역형광물질은 모두 메산지움 및 기저막 일부에서 관찰되었으며 과립형이었다. 특히 강양성으로 보인 경우는 5마리였다. 제Ⅱ군에서도 결과는 같았으나 강양성은 20마리중 8마리였다. 역시 IgG, IgM, C_3는 음성이었다. 제 Ⅲ군에서도 결과는 같았으며 강양성은 20마리중 11마리였다. IgG, IgM은 모두 음성이었으며 단지 1마리에서 C_3가 약양성이었다. 결론 : 본 연구결과에 의하면 BGG투여로 초래되는 생쥐에서의 IgA 신병증의 실험모델에서는 methylprednisolone과 levamisole은 신장조직 소견상 IgA 침착정도나 병변의 경중에는 그 차이를 보이지 않았다. The pathogenesis of IgA nephropathy, first described by Berger and Hinglais in 1968. has not been defined clearly and appropriate treatment modality also not established. Present study has been conducted to induce IgA nephropathy in experimental animal and to see the effectiveness of methylprednisolone and levamisole on the experimentally induced IgA nephropathy. Mice weighing 20-30gm were used as experimental animal and Bovine gamma-globulin (BGG) (Sigma U.S.A.) was given as 0.1% drinking solution (Group Ⅰ). Group Ⅱ received methylprednisolone (5㎎/㎏) and Group Ⅲ levamisole (5㎎/㎏) on alternate day in addition to BGG-containing solution. On the 100th day of experiment, animals were sacrificed and blood chemistries and renal histopathologic examinations were done. Histopathologic examination: In all three groups, light microscopic, electron-microscopic and immunofluorescent microscopic findings were similar. Light microscopy showed mild focal mesangial proliferation and electron microscopy showed electron-dense materials deposited in the mesangial area near the basement membrane. Immuriofluorescent stain using fluorescin isothiocyanate goat anti-mouse IgA (Biodesign, U.S.A.) demonstrated prominent IgA staining in the mesangial area in all three group. Staining with IgG, IgM and C_3 were all negative. In conclusion, present experiment did not show any effectiveness of methylprednisolone nor levamisole on the BGG-induced IgA nephropathy in mice. And the effectiveness and its acting mechanism of methylprednisolone on human IgA nephropathy has to be studied further.

      • 수돗물과 정수기물의 미생물학적 수질

        The author examined microbiological water quality in Chinju National University. Water temperature was lowest at site 1 as 9.45℃, and highest at site 7 as 26.11℃ on average. pH was neutral being in the range of pH 6.28∼7.75. There was a little relationship between sampling sites and sampling time. General bacterial number exceeded the standard of water quality in every sites. And not a few bacteria were detected in the purified water. Coliform bacteria were detected at every sites and the number increased after rainfall. These results suggest that the water quality should be appropriately managed, although the present microbiological water qualigy is not so serious a problem.

      • 西部慶南地域의 느타리버섯 裁培 및 疾病 發生 現況

        The authors surveyed the present situation of Agaricus sp cultivation and its disease occurence in Western Kyong-Nam area. 192 farmers, which is 66% of the mushroom-cultivating farmers, were growing Agaricus sp. Especially there were 124 farms in Western Kyong-Nam around Chinju-City. The greater part of mushroom-farmers cultivated Agaricus sp. on tables using cotton as major medium. And they purchased the eldest mycelium from other districts. About 60% of the farmers were suffering some losses due to the development of disease. The major disease seemed to be brown blotch. When a mushroom-disease has developed, most farmer coped with it by removing the cotton media, and sometimes by treating with chemicals. Mushrooms seemed to be a major source of farmers' income, and some govenmental and scientific supports are needed for the development of mushroom cultivation.

      • 공역 리놀레산 유도체의 세균생장 억제효과

        Antibacterial activity of CLA-K salt was surveyed in this paper. CLA-K(10%) exhibited antibacerial activity at the concentration more than 60 ㎕. The antibacterial activity of CLA-K in liquid culture was relatively proportional to its concentration. The resistance of bacteria against CLA-K increased as time goes by. Streptococcus mutans, Bacillus cereus KCCM-l1204, Lactobacillus plantalium and Escherichia coli ATCC-11835 were relatively sensitive to CLA-K. Staphylococcus aureus KCTC-l927 and Klebsiella pneumoniae KCTC-2208 sensitively reacted to the concentration of CLA-K.

      • 영지버섯 리큐르주의 침출시간에 따른 성분변화

        The physicochemical factors such as organic acid composition, pH, sugar content, acidity and color according to extraction time of Ganodema licidum liquor changed as followings; 1. The major general components of Ganodema licidum were analyzed to be crude fiber(47.5%), carbohydrate (29,4%), crude protein (14.8%), crude fat (6.5%) and crude ash (1.8%) in dry basis. 2. pH and sugar content in Ganodema licidum liquor slightly decreased, whereas acidity increased with extraction time. 3. The luminosity of color (L) showed no remarkable change. 4. The tone of color (a) decreased, whereas the chroma (b) increased as time goes by. 5. The concentration of total organic acid in Ganodema licidum liquor was 2.453%, and nine kinds of organic acids such as oxalic, citric, tartaric, succinic, lactic, formic, acetic, propionic and isobutyric acid were detected. Among them, the concentration of lactic acid was highest acetic and isobutyric acid were medium level, and that of citric, formic, succinc and tartaric acid was very low.

        리조트 산업의 활성화를 위한 美 Las Vegas 개발사례 연구

        As we studied Las Vegas with the changes of the times, we found that Las Vegas which tourist used to via has become the destination of journey, and that a city for man adults has become the comprehensive resort city for the whole family. The main causes for these changes lied on the hotels and casinos in Las Vegas. The hotels and casinos that simply supplied lodging and gaming seems to change into the place that has multidimensional mass resort spot with elements of sports and leisure. The changes of the character of the hotels and casinos seem to be in gear with both internal and external changes of game industry represented as casino. The internal change of game industry emerged from the change of visitor and user's preference of games; visitors to casino have moved to mid-low batting class, and to young women class and family tourist. They are reported to prefer somewhat soft slot machine and video games to traditional table games. Now casino games settle down as the part of entertainment for public. This is also related to the external change of game industry which means cognitional change of public. Due to the political endeavor of authority of Nevada and Las Vegas that recognized casino as an industry and foster it, the cognition that casino is not for the gamblers but for a kind of entertainment for public has been spreaded widely. By constant improving through market extension and adding sports-leisure facilities, authority of Las Vegas try to Las Vegas into the mass resort city with game industry. This seems to be great full of suggestions. Today, casino has become an object of active leisure activity, taking off its gambling image. In addition, from the point of economic view, it is in the spotlight as high value added industry that has great effects such as improvement of international payments, activation of local economy, increase of income of resident and insurance of tax revenues. As we studied about Las Vegas, casino games have become a part of mass resort industry by adding leisure-sports facilities and playing space for children and have changed into the factor of important entertainment that leads resort industry. Thus, to develops Korean resort industry, together with existed leisure-sports facilities, factor of game industry like casino seems to be considered.

        국민건강정보시스템의 개발과 효율적인 운영방안

        The purpose of this research is to analyze the work of institute of Exercise prescription and to evolve the computer program about Exercise prescription. For evolving program, examined and analyzed the Exercise prescription note paper with researcher of institute of Exercise prescription. This program consists of individual information, medical examination, an examination of th physique and fitness, maximal exercise test, graphic Evaluation, Exercise prescription Editor, and Data Base. The individual information consists of sex, age, measurement day, company, location and memo. Medical examination consists of disease cheak the Judgment of medical examination and attention Box. Judgment of medical examination describes to classify. Consists of height, weight, Ideal weight, and the chest length. Examination fitness consists of muscle strength(grip, back), flexibility(standing trunk flexion), trunk extension, balance(stork stand, two-foot standing), muscle endurance(sit-up, Push-up), power(surgent jump, stand broad jump), and Agility(side step, lom-ran). Maximal exercise test consists of HR, VE, VO₂max, Vco₂max, and RQ. This reserch suggest that continuously, this program have to up-grade by resercher of exercise prescription and programer. The reasercher of institute of Exercise prescription have to liarning computer education for using the program. We have mind that is to impromation on age of computer program, because we have to bring up ability that trans impromation from data.
