      • 화농성 슬관절염이 동반된 수막구균성 수막염 1예

        저자들은 최근 화농성 슬관절염과 병발한 수막구균성 수막염 1예를 체험하였고, 이는 국내에서 아직 보고되지 않은 드문 예로 사료되어 문헌고찰과 함께 보고하는 바이다. We recently experienced a case of meningococcal meningitis associated with pyogenic arthritis in a 37-year old male patient. Pyogenic arthritis developed in right knee joint during treatment for meningococcal meningitis. He was treated successfully by appropriate antibiotics and arthroscopic irrigation. To our knowledge, such a rare case has not ever been reported in Korea, Hence we report this case with a review of related literatures.

        한국어 명칭말하기 검사 개발과 타당도 및 신뢰도 평가

        Background: Confrontational naming test is useful in evaluating the language function. It reveals memory and cognitive processing dysfunctions in many neurologic diseases including dementia. Although the Korean version of the Boston Naming Test (K-BNT) has been a popular and useful test, it takes much time and effort in dementia patients of moderate to severe stage of disease or in elderly individual with low educational level. So we developed the Korean Naming Test (KNT). Methods: KNT consists of relatively easy 40 items. Each item was familar materials around us and simplified figures with black thick line. Ninety two of probable Alzheimer's patients received the Korean version of Mini-Mental State Examination (K-MMSE), the Korean version of expanded Clinical Dementia Rating Scale (CDR), K-BNT and KNT. We tested reliability and validity of KNT and also evaluated scores of the KNT along with CDR. Results: The KNT correlated with K-BNT [0.75(p;0.01)], CDR [-0.49(p;0.01)], K-MMSE [0.55(p;0.01)]. The internal consistency was 0.91 (Cronbach's alpha). The KNT score decreased as the disease progressed. Conclusions: The KNT is a brief and good test for evaluating language function of moderate to severe dementia patients with low educational level. Also it is sensitive to mild stage of disease and has a excellent reliability and validity.

        As a technology to machine the 3-dimensional shapes of general metal material more flexibly, machining technologies such as metal cutting, grinding, etc have been steadily improved in their accuracy as well. However, the working method using machining technologies are restricted in terms of surface roughness, metal working accuracy and working hours, if a workpiece has an irregular 3-dimensional shape. Magnetic abrasive finishing process is also one of the super-precision machining technologies which proceed with the finishing metal working of precision working piece using the action of magnetic field and the mixed type magnetic abrasive. However, such studies only focus on the improvement of machining accuracy but are still incomplete in investigating the roundness. This study aims to realize the high accuracy finishing machining focusing on the roundness after magnetic abrasive finishing process of circular workpiece and the surface roughness of precision machining surface.

        The determination of surface topography is believed to be extremely important in the areas of contact mechanics, adhesion and friction. In order to describe topography of various frictional surface, the harding test was carried out under different experimental conditions in dry friction. And fractal descriptors was applied to frictional surface of laser modified steel with image processing system. These descriptors to analyze surface structure are fractal dimension. Surface fractal dimension can be determined by sum of intensity difference of surface pixel. Topography of frictional surface can be effectively obtained by fractal dimensions.

        마멸입자 형태해석에 의한 유압피스톤용 모터의 상태감시

        Morphological analysis of wear particles is one of useful methods for machine condition monitoring because it is well reflected in machine driving state. This paper was undertaken to apply to the condition monitoring of hydraulic piston motor. The lubricating wear test was performed under different experimental conditions using the wear test device and wear specimens of the pin on disk type was rubbed in paraffinic base oil by three kinds of lubricating materials, varying applied load, sliding distance. The four shape parameters (50% volumetric diameter, aspect, roundness and reflectivity) arc used for morphological analysis of wear particles. The results showed that the four shape parameters of wear particles depend on a kind of the lubricating materials. It was capable of calculating presumed wear volume for three kinds of mate-rials on driving time to foresee a damage condition of lubricating materials.

        Serial Magnetic Resonance Imaging to Determine the Progression of Neglected Recalcitrant Rotator Cuff Tears: A Retrospective Multicenter Study

        Background: To determine the natural progression of conservatively treated rotator cuff tears, we evaluated changes in radiologic and clinical parameters in patients whose recalcitrant tears were neglected after conservative treatment. Methods: A total of 73 patients with recalcitrant rotator cuff tears in spite of conservative treatment were included in this study. We measured changes in tear size and in the extent of fatty infiltration of the rotator cuff by comparing the initial and final follow-up magnetic resonance imagings (MRIs). To determine factors influencing the change in tear size, we collected the medical history of patients taken at the time of initial admission. Results: The average follow-up period was 20.1 months, and the average increase in tear size across this period was 6.2 mm. In terms of steroid injection, we found that the increases in tear size of the steroid injection group (p=0.049) and of the sub-group that had received more than three steroid injections (p=0.010) were significantly greater than that of the non-steroid injection group. Conclusions: We found that the increase in cuff tear size was on average 6.2 mm across the follow-up period, indicating that neglecting cuff tears may cause them to progress into more severe tears. We also observed that a history of steroid injection might be a possible risk factor for a worse prognosis of cuff tears. Therefore, we suggest that patients with rotator cuff tears and a history of steroid injection are recommended aggressive modes of treatment such as surgery.

        Nonpalpable supraclavicular lymph nodes in lung cancer patients: preoperative characterization with 18F-FDG PET/CT.

        <P>OBJECTIVE: Our purpose was to evaluate the usefulness of integrated 18F-FDG PET/CT in the detection and characterization of nonpalpable supraclavicular lymph node metastasis in patients with the initial diagnosis of lung cancer. SUBJECTS AND METHODS: This study was conducted from May 2005 to May 2006 and included 32 consecutively registered lung cancer patients in whom supraclavicular lymph nodes were not palpable but were identified on contrast-enhanced CT or exhibited increased FDG uptake on integrated PET/CT. Three patients had bilateral nodes, for a total of 35 nodes in the 32 patients. Results of cytologic analysis of a specimen obtained with sonographically guided fine-needle aspiration (n = 27), normal initial and follow-up sonographic findings (n = 3), and no change in the size of supraclavicular lymph nodes on follow-up sonography (n = 2) were the reference standards. The presence of supraclavicular lymph node metastasis was determined with integrated PET/CT (uptake greater than that of surrounding tissue) and contrast-enhanced CT (node short-axis diameter of 5 mm or more). The diagnostic efficacies of these methods in the detection of supraclavicular lymph node metastasis were compared. RESULTS: Supraclavicular lymph node metastasis was diagnosed cytologically in 12 (34%) of 35 lesions. The diagnostic accuracies of integrated PET/CT and contrast-enhanced CT in the detection of supraclavicular lymph node metastasis were 71% and 66%, respectively; the difference was not statistically significant. Although the difference was not statistically significant, the sensitivity (92%) and negative predictive value (93%) of integrated PET/CT were higher than those of contrast-enhanced CT. CONCLUSION: Because of its high sensitivity and negative predictive value, integrated PET/CT is useful in the detection and characterization of nonpalpable supraclavicular lymph nodes in lung cancer patients.</P>

      • The Adequate Resection Margin of Hepatocellular Carcinoma According to the Tumor Microenvironment

        Aims: There is no consensus on the safe resection margin in patients with hepatocellular carcinoma (HCC). We investigated the change of tumor microenvironment according to the resection margin. Methods: We prospectively collected the specimen of 60 patients with HCC. We selected three portions of specimens as follows: tumor, 1cm and 2cm margin normal tissue. We investigated the expression status of tumor microenvironment genes. We compared the expression status according to recurrence, HCC gross type and positron emission tomography (PET) positivity. We divided the patients into two groupsas follows: group 1 included expanding and vaguely nodular types whereas group 2 included nodular with perinodular extension, multinodular confluent and infiltrative types. Results: Group 2 had a higher prevalence of PET positive [6 (37.5%) vs 10 (62.5%)] and recurrence [5 (16.7%) vs 17 (56.7%)]. However, in cases with more than 1cm resection margin, there was no difference of recurrence rate [9 (75%), P=0.017 vs 8 (44.4%), P=0.06]. Beta-catenin was significantly decreased and E-cadherin was significantly increased according to the resection margin in group 1. Group 2 and PET positive patients showed Group 1 did not show any significant change of beta-catenin and E-cadherin until 2cm resection margin. Conclusions: Our data suggest that patients with HCC of expanding and vaguely nodular gross types may safely undergo surgical resection with a narrow resection margin and patients with HCC of nodular with perinodular extension, multinodular confluent and infiltrative gross type must undergo surgical resection with more than 2cm resection margin because of tumor microenvironment condition.
