        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

        새로운 알킬 실란의 3-aryl-1-silabutanes 1-4을 합성하고 6족 전이금속 착물인 ??을 사용하여 탈수소 중합을 시켰다. 전이금속 4족 착물인 metallocene계 촉매에 비해 분자량이 현저히 증가된 실리콘 고분자를 얻을 수가 있었다. 6족 전이금속 촉매에 의한 탈수소 중합 메카니즘을 제시하였다.

      • Dual Pickup 대물렌즈의 생산을 위한 주요 Parameter 및 특성에 관한 연구

        In order to operate CD and DVD compatibly in a pickup system, the objective lens comprise diffractive optical element(DOE) zone and aspheric curvature on its lens surface. The DOE objective lens is effective to simplify this dual-purpose pickup system of the 655nm and 785nm wavelength by using only one lens, but require more precision manufacturing process and system due to the complicated shape. This paper presents the overall manufacturing process of this objective lens and describes main parameters in each process, for the correction of the aspheric surface in its core, the shrinkage compensation after injection molding, and the uniformity compensation by adjusting molding conditions.

      • 播種期가 쇠비름의 生長과 競合에 미치는 影響

        In order to elucidate the effects of seeding time on growth and competition ability of common purslane(P.oleracea L.), sowings were made at five different dates in 1/2, 000 a pots in 1988. : As the sowing date delayed, plant height and number of branches were increased due to promoted early growth, and as planting densities were increased they were reduced by the intra - species competition regardless of sowing date. Effect of sowing date on dry weight of common purslane had the same tendency as plant height and number of branches, and dry weight per plant was decreased rapidly upto 4 plants/pot by increased planting density, but showed lower decreasing degree above 4 plants/pot. Changes of dry weight were affected by planting density and total dry weight per plant was not significantly affected by planting density. Seed productivity of common purslane per plant was decreased as planting density was increased, and seed productivity per pot showed same tendency affected by number of plant regardless of planting density. In inter - species competition with soybean, growth of common purslane had a same tendency as only commonpurslane was grown, growth degree and effect of planting density were low. Growth and yield of soybean showed differences between sowing date only and was not affected by intra - species competition in common purslane while interspecies competition ability of common purslane to soybean appeared to be low.

        효모시스템에서 Human Transglutaminase C(TGase 2)의 발현에 관한 연구 : C-말단부위의 결손효과 An Effect of a Domain from Carboxyl Terminal Deletion of the Enzyme

        Transglutaminase(TGase Ⅱ)는 펩타이드-결합 글루타민 잔기와 펩타이드-결합 라이신잔기 및 1차 아민류를 포함하는 다양한 화합물과의 교차결합에 의하여 단백질의 변형을 촉매하는 효소이다. 동물세포에는 구별가능한 5종류의 TGase가 존재한다[TGase Ⅰ(또는 K), Ⅱ(C), Ⅲ(E), Ⅳ(P)와 혈장단백 ⅩⅢa]. 그들은 아미노산 서열에서 N-말단에 약 70%의 상동성을 갖고 있으며, C-말단의 경우에는 거의 상동성이 부위(N-말단 2/3)와 비상동성 부위(C-말단 1/3 부위)의 역할을 알아보기 위해 전체효소 또는 C-말단이 결손된 형태의 효소를 Saccharmoyces cerevisiae에서 발현 하고자 하였다. Southern blot, 역전사 중합 연쇄 반응, Western blot등을 통해 전체효소 및 C-말단 결손형 효소가 형질전환된 효모에서 발현되는 것을 확인하였다. Succinylated casein에 [1,4-^14C]-putrescine이 통합되는 반응을 이용한 활성 측정에서 C-말단이 결손된 TGase Ⅱ는 같은 세포 농도에서 전체 TGase Ⅱ 효소와 비교하여 유사한 수준의 활성을 나타내었다. 이러한 결과들은 사람 TGase Ⅱ의 경우 N-말단의 높은 상동성부위가 효소의 역가결정에 중요하다는 것을 나타내 준다. 효모시스템에서 발현한 결과 효소의 역가가 나타났으나 높지 않았으며 발현된 양이 극히 낮아서 단백질의 정제나 특성연구를 위하여 발현벡터의 개량을 포함한 방법의 개선이 필요하다고 사료된다. 흥미롭게도 TGase Ⅱ의 발현이 효모의 성장을 저해하는 것으로 나타나 이러한 현상이 표피세포의 각질화에 수반된 세포의 죽음과 유관한지의 가능성에 대한 보다 자세한 연구가 필요하다. In an effort to understand the role of the conserved domain and of the heterologous one-third part of the carboxy terminal domain of transglutaminase C (TGase Ⅱ), attempts were made to express TGase Ⅱ cDNA of human origin in yeast Saccharmoyces cerevisiae as in a full-length form as well as in a form of C-terminal truncation. The 2μ-based expression plasmids which contained the TGase Ⅱ cDNA under the gal inducible promoter were introduced into yeast and the maintenance of the full-length and truncated form of the TGase Ⅱ gene plasmids were confirmed by Southern blot. The expression of e results indicate that the N-terminal homologous domain of human TGase Ⅱ retains a catalvtically active domain. The level of TGase Ⅱ expressed in yeast, however, was far lower than satisfactory and other expression system should be sought further characterization of the enzyme. The negative effect of TGase Ⅱ on the growth of yeast is interesting with respect to the physiological effect of TGase Ⅱ in the cornification f epidermal keratinocytes.

      • Plant Regeneration from Hypocotyls of common buckwheat, (Fagopyrum esculentum Moench) and annual wild buckwheat, (F. homotropicum Ohnishi)

        Plant regeneration from cultured hypocotyls may provide means for overcoming breeding barriers caused by heterostylic cross-incompatibility in interspecific crosses of common buckwheat(Fagopyrum esculentum Moench). Hypocotyles were excised from seed embryos of common buckwheat(cv. Miyazakizairai) and wild annual buckwheat(F. homotropicum Ohnishi) and cultured on MS medium supplemented with 0∼2.0 mg/l IAA and 0~2.0 mg/l BAP. Shoot was induced only on the medium containing 1.0 mg/l NAA and 1.0 mg/l BAP, shoot induction was promoted by 2.4-D and the frequency was lower in wild annual buckwheat than in common buckwheat. Roots were developed on White's medium containing 1.0 mg/l IBA. Whole plantlets were obtained following the production of adventitious roots from the shoots. When transferred from sterile culture condition to peat and soil medium, most plantlets were able to survive and grew to have flowers.       • 개에서 발생한 신우신염 증례

        A 6 years old Schunauzer dog (7.2kg, intact female) with history of hematuria for two days was referred to the Veterinary Medical Teaching Hospital of Chungnam National University. Complete blood cell count showed no significant abnormalities and serum chemistry profile showed increased phosphate profile. On urinanalysis, there was evidence of bacterial infection. On ultrasonographic evaluation, irregular renal contour and increased echogenecity, pelvic and uretral dilation, and thickening of urinary bladder wall were observed. Based on these finding, the dog was diagnosed as pyelonephritis. The dog was treated with antibiotics based on antimicrobial susceptibility test results, then successfully resolved after 8 weeks of treatment.

        몇가지 植物의 棲息地 造成에 關한 硏究

        종의 棲息密度가 낮아지고 있는 하늘나리, 섬초롱, 자란 3종의 식물을 인위적으로 棲息地를 造成하기 위하여 국립공원 속리산 등산로변 標高 200m, 400m, 600m 지점에 150주식 종당 450주를 조직배양 및 분주에 의하여 육묘 된 묘를 이식하여 생존상태 및 棲息地 造成의 가능성을 2년에 걸쳐 검토한 결과는 다음과 같다. 1. 造成 棲息地의 토양의 理化學的 造成은 pH 5.2∼5.6, 유기물 함량 29∼54g/㎏, CEC 9.4∼11.5㎝ mol(+)/㎏으로 산성을 띠고 있었으며, 유기물 함량은 높은 편이었고, 염기치환용량은 낮았으나 비교적 우리나라 典型的 山地土壤의 理化學的 造成과 유사하였다. 2. 棲息地內 생장기간의 投光率은 標高 200m 지역은 10.8∼11.6%, 400m lw역은 10.8∼11.4%, 600m 지역은 11.0∼12.8%로 식재식물의 生長環境에 부족한 상태였다. 3. 식재 식물 수는 식재 후 활착율은 3종 모두 양호하였으나, 점차 감소되었으며, 특히 자란의 감소율이 높았다. 감소의 원인을 분석한 결과 하늘나리, 섬초롱, 자란 모두 굴취나 절취의 피해가 많았으며, 특히 자란은 凍害에 의한 枯死가 1년차 72∼86주, 2년 차에서 19∼35주로 조사도어 자연상태에서의 越冬은 어려운 것으로 나타났으며, 집중호우에 의한 流失도 감소의 한 원인이었다. 4. 식재식물의 생장상태는 불량하였는데 이는 投光量 부족으로 판단되어 棲息地造成時에는 錄陰期 投光量에 대한 사전 검토가 중요한 요인으로 나타났다. 5. 개화율은 섬초롱과 자란에서 높았으며 標高가 높을수록 개화기는 2∼4일 늦어지고 휴면기는 빨라지는 경향이었다. 식물 棲息地 造成時에는 造成 식물의 생장에 적합한 토양, 光環境 등을 고려한 위치 선정이 중요하며, 上層林에 의한 遮光으로 下層 초본식물 遷移速度가 빨라짐으로써 光環境 개선을 위한 上層林의 間伐, 周邊樹種의 更新이 요구된다. In order to make the artificial habitats of Lillium concolor, Campanula takesimana and Bletilla striata that the number of plant became sparse, 150 plants per species were transplanted at the region of 200m, 400m and 600m above sea level. And the growth of plants and the potential as habitats creation were analyzed. In the physical and chemical properties of soils in the there, soil pH was 5.2 to 5.6, organic matter content was 29 to 54g/㎏ and cation exchange capacity(CEC) was 9.4 to 11.5㎝ mol(+)/㎏. The transmittance rates of sunlight of the habitats during the growing periods were 10.8 to 11.6% in the 200m region, 10.8 to 11.4% in the 400m region and 11.0 to 12.8% in the 600m region. These transmittance rates were deficient for normal growth of plants. After the plants were transplanted, the survival rates of three species were high in common but the rates decreased gradually. Especially the decrease rate was high in the Bletilla striata. The causes of decrease were digging and cutting by climbers, sweeping asay by rainfall and freezing damage. Especially, the cold tolerance of Bletilla striata was weak in the natural condition. The growth of planted plants was poor, and it was assumed that the reason was shortage of sunlight transmittance. The flowering rates were high in the Campanula takesimana and Bletilla striata. And as the elevations were high, the flowering was later by 2 to 5 days but the dormancy was earlier. In the artificial making of plant habitats, the growth environment of plants have to be considered to choose the suitable region. And, because the habitats are shaded by the trees and bushes, the thinning of trees and the change of species are needs to improve the light transmission.
