영어 온라인 수업에 관한 교사와 학생 인식: 초등학교 사례연구

본 연구는 코로나19 상황에서 진행된 초등영어 온라인 수업에 대한 교사와 학생의 인식을 알아보기 위해 진행되었다. 본 연구를 위해 서울의 한 초등학교의 영어전담교사 2명, 원어민보조교사 1명을 대상으로 심층 면담을 실시하고 4∼6학년 학생 180명을 대상으로 설문을 시행하였다. 연구 결과를 요약하면 다음과 같다. 교사들의 교수 경력과 행정업무 참여 여부에 따라 달랐지만, 처음 도입된 영어 온라인 수업 경험에 대해 대체로 긍정적으로 인식한 반면, 한계점도 깨달았다고 밝혔다. 온라인 수업 자료 제작과 관련해 세 교사 모두 각자 패턴이 생기며 익숙해졌고, 포스트코로나 시기에도 온라인 수업과 등교 수업 병행을 긍정적으로 인식했다. 교사와 학생들은 온라인 수업에 대해 유사하게 인식했으나 온라인 수업에서의 역할이 달라 차이점을 보여주기도 했다. 영어 온라인 수업에 따른 장점은 개인차를 고려한 학습이 가능하고 학생들에게 심리적인 안정감을 제공했다는 점이었다. 이에 반해 영어 온라인 수업의 단점은 교사의 학생에 대한 피드백이 감소했고, 학생 학업 성취도가 양극화가 심화되었으며 학생 특성과 학업 성취도 파악이 어려워졌고, 학생들 간 상호작용 기회가 감소하였으며 영어에 대한 기초 학습 지원이 부족했다는 점이다. 이 연구 결과에서 도출된 영어 온라인 수업에 대한 교사와 학생의 인식 및 의견을 고려한다면 향후 온라인 수업 발전에 기여할 것이다. This study examined teachers’ and students’ perceptions of English online classes during the COVID-19 period at an elementary school. Interviews were conducted with two English teachers and one native-speaking teacher at an elementary school, and a survey was conducted with 180 4-6th grade students. The findings are as follows: The teachers felt the limitations while positively recognizing online classes depending on their teaching expertise and participation in administrative work. All three teachers developed their own processes when producing online class materials. They also felt positive about offering a combination of online and in-person classes post-pandemic. Teachers and students had similar views towards online classes, while showing differences depending on their roles in online lessons. The advantages of online classes were that students could progress at their own pace, free from potential ridicule by classmates. The disadvantages, however, were as follows: Lack of teacher feedback to students, polarized students’ academic performances, difficulties gauging each student's characteristics and academic achievement, limited opportunities for interaction between students, insufficient basic learning support for English. To improve future application of online classes, teachers’ and students’ perceptions and the online format’s advantages and disadvantages must be considered.

<P><B>Purpose</B></P><P>Alteration of biomarker status after primary systemic therapy (PST) is occasionally found in breast cancer. This study was conducted to clarify the clinical implications of change of biomarker status in breast cancer patients treated with PST.</P><P><B>Materials and Methods</B></P><P>The pre-chemotherapeutic biopsy and post-chemotherapeutic resection specimens of 442 breast cancer patients who had residual disease after PST were included in this study. The association between changes of biomarker status after PST and clinicopathologic features of tumors, and survival of the patients, were analyzed.</P><P><B>Results</B></P><P>Estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) status changed after PST in 18 (4.1%), 80 (18.1%), and 15 (3.4%) patients,respectively. ER and PR mainly underwent positive to negative conversion,whereas HER2 status underwent negative to positive conversion. Negative conversion of ER and PR status after PST was associated with reduced disease-free survival. Moreover, a decline in the Allred score for PR in post-PST specimens was significantly associated with poor clinical outcome of the patients. HER2 change did not have prognostic significance. In multivariate analyses, negative PR status after PST was found to be an independent adverse prognostic factor in the whole patient group, in the adjuvant endocrine therapy-treated subgroup, and also in pre-PST PR positive subgroup.</P><P><B>Conclusion</B></P><P>ER and HER2 status changed little after PST, whereas PR status changed significantly. In particular, negative conversion of PR status was revealed as a poor prognostic indicator, suggesting that re-evaluation of basic biomarkers is mandatory in breast cancer after PST for proper management and prognostication of patients.</P>
Evaluation of Modified Core-Needle Biopsy in the Diagnosis of Thyroid Nodules

Objective: Core needle biopsy (CNB) of the thyroid is an additional diagnostic method for non-diagnostic or indeterminate cytology samples. We sought to evaluate a new modified core biopsy technique and compare the concordance of its diagnosis with the final diagnosis of the surgically resected specimen. Materials and Methods: A retrospective analysis was conducted on 842 patients who had a thyroid CNB with or without a previous fine-needle aspiration from August 2002 to March 2015; 38% of patients ultimately underwent thyroidectomy. We divided the patients into two groups for comparison: conventional group (n = 329) and new modified technique group (n = 513) that enabled sampling of not only the lesion but also the margin and surrounding parenchyma. The diagnostic conclusiveness of CNB and concordant rate with thyroidectomy was compared between the two groups. Results: The overall diagnostic conclusiveness did not exhibit a significant increase (77% in the conventional technique group and 75% in the modified technique group, p = 0.408). In terms of the diagnostic concordance rate between CNB and thyroidectomy, no overall significant increase was observed (83% in the conventional technique group and 88% in the modified technique group, p = 0.194). However, only in follicular-patterned lesions (nodular hyperplasia, follicular neoplasm, and follicular variant of papillary thyroid carcinoma), a significant increase in the diagnostic concordance rate was observed (83% in the conventional group and 94% in the modified technique group, p = 0.033). Conclusion: Modified CNB technique can be beneficial for the accurate diagnosis of follicular-patterned thyroid lesions.
Pulmonary Arteriovenous Fistula: Clinical and Histologic Spectrum of Four Cases

Pulmonary arteriovenous fistula (PAVF) is abnormally dilated vessels that provide a right-to-left shunt between pulmonary artery and pulmonary vein and is clinically divided into simple and complex type. Here, we report four cases of surgically resected sporadic PAVFs presenting various clinical and histologic spectrums. Cases 1 (a 57-old-female) and 2 (a 54-old-female) presented as incidentally identified single aneurysmal fistulas and the lesions were surgically removed without complication. On the other hand, case 3 (an 11-old-male) showed diffuse dilated vascular sacs involving both lungs and caused severe hemodynamic and pulmonary dysfunction. Embolization and surgical resection of the main lesion failed to relieve the symptoms. Case 4 (a 36-oldmale) had a localized multiloculated cyst clinically mimicking congenital cystic adenomatoid malformation. Microscopically, the lesion consisted of dilated thick vessels, consistent with the diagnosis of fistulous arteriovenous malformation/hemangioma.

Background/Aims: Neoadjuvant chemotherapy is increasingly utilized in patients with borderline or locally advanced pancreatic cancer (LAPC). However, the pathologic evaluation of tumor regression is not routinely performed or well established. We aimed to evaluate the prognostic value of three tumor regression grading systems frequently used in LAPC and to determine the correlation between pathologic and clinical response. Methods: We included a total of 38 patients with LAPC who were treated with neoadjuvant chemotherapy and subsequent resection. Pathologic tumor regression was graded based on the College of American Pathologists (CAP), Evans, and MD Anderson grading systems. Results: One out of 38 patients (2.6%) achieved a pathologic complete response. Unlike other grading systems (Evans, p=0.063; MD Anderson, p=0.110), the CAP grading system was a significant prognostic factor for overall survival (p=0.043). Pathologic N stage (p=0.023), margin status (p=0.044), and radiologic response (p=0.016) correlated with overall survival. In the multivariate analysis, CAP 3 was an independent predictor of shorter overall survival (p=0.026). The CAP grading system correlated with the radiologic response (p=0.007) but not the carbohydrate antigen 19-9 level (p=0.333). Conclusions: The four-tier CAP pathologic tumor regression grading system predicted the clinical outcome in LAPC patients who underwent resection after neoadjuvant chemotherapy. Therefore, a more comprehensive pathologic evaluation is warranted in these patients. (Gut Liver 2022;16:129-137)
