Incidence and Risk Factors of Ocular Infection Caused by <i>Staphylococcus aureus</i> Bacteremia

<P>Staphylococcus aureus bacteremia (SAB) often leads to ocular infections, including endophthalmitis and chorioretinitis. However, the incidence, risk factors, and outcomes of ocular infections complicated by SAB are largely unknown. We retrospectively analyzed the incidence and risk factors of ocular involvement in a prospective cohort of patients with SAB at a tertiary-care hospital. Ophthalmologists reviewed the fundoscopic findings and classified the ocular infections as endophthalmitis or chorioretinitis. During the 5-year study period, 1,109 patients had SAB, and data for 612 (55%) who underwent ophthalmic examinations within 14 days after SAB onset were analyzed. Of those 612 patients, 56 (9% [95% confidence interval [CI], 7 to 12%]) had ocular involvement, including 15 (2.5%) with endophthalmitis and 41 (6.7%) with chorioretinitis. In a multivariate analysis, infective endocarditis (adjusted odds ratio [aOR], 5.74 [95% CI, 2.25 to 14.64]) and metastatic infection (aOR, 2.38 [95% CI, 1.29 to 4.39]) were independent risk factors for ocular involvement. Of the 47 patients with ocular involvement who could communicate, only 17 (36%) had visual disturbances. Two-thirds of the patients with endophthalmitis (10/15 patients) were treated with intravitreal antibiotics combined with parenteral antibiotics, whereas all of the patients with chorioretinitis were treated only with systemic antibiotics. No patients became blind. Among 42 patients for whom follow-up assessments were available, the ocular lesions improved in 29 (69%) but remained the same in the others. Ocular involvement was independently associated with death within 30 days after SAB onset. Ocular involvement is not uncommon among patients with SAB. Routine ophthalmic examinations should be considered for patients with infective endocarditis or metastatic infections caused by SAB.</P>

The nephrotic syndrome has been considered a hypercoagulable state since it may be complicated by thromboembolic events of the venous or the arterial circulations. Diverse pathogenetic factors leading to the hypercoagulable state in nephrotic syndrome have been recognized. Renal vein thrombosis is a serious complication, which might lead to either renal failure or to secondary thromboembolic processes like pulmonary thromboembolism. Although it may present acutely with flank pain and macroscopic hematuria, the majority of cases run an indolent course. Until relatively recently, the diagnosis could only be confidently confirmed or excluded with selective renal venography but, more recently, computerized tomography and magnetic resonance imaging have been used. Anticoagulant therapy with heparin and warfarin apparently halts the natural progression of the disease and allowing for slow recovery. The possibility of more rapid and complete resolution with thrombolytic agents warranted their application. We described a case of bilateral renal vein thrombosis diagnosed by the new technique of magnetic resonance angiography and successful treatment by thrombolytic agent.
고뇨산성 신병증 발병 기전에서 대식세포 유주 억제 인자의 역할

배 경 : 고뇨산혈증 환자에서 신장에 요산 결정 침착이 지속되면 만성 신세뇨관 간질성 손상과 거대 세포를 포함하는 육아종 발생이 특징적인 병리 소견이다. 이러한 육아종 형성은 요산 결정이 지연성 과민 유사 반응을 유발할 수 있음을 시사하는 소견이며, 대식세포 유주억제인자(macrophage migration inhibitory factor, MIF )는 최근 지연성 과민 반응에 가장 중요한 매개 물질로 밝혀진 바 있다. 따라서 저자들은 요산 결정 침착이 MIF를 국소적으로 발현시켜 육아종을 형성하고 신장 조직 손상을 유발한다는 가설을 세우고 이를 증명하고자 고뇨산성 신병증 동물 모델에서 이 연구를 시행하였다. 방 법 : 고뇨산성 신병증은 Sprague- Dawley 쥐에서 uricase 억제제인 oxonic acid(OA, 2%)와 요산(1.5%)을 식이에 보충하여 35일 동안 투여하여 유발하였고(UA군) 정상 식이를 투여한 군(대조군)과 비교하였다(각 군 n=6). 35일째에 신장을 절제하여 병리 소견을 관찰하였고 면역 조직화학 염색법과 in situ hybridization으로 MIF의 발현과 침착된 세포들을 관찰하였다. 결 과 : UA군에서는 신세뇨관내 요산 결정과 육아종을 동반한 광범위한 세뇨관 간질성 손상이 관찰되었다. UA군에서 신세뇨관 MIF mRNA 발현은 정상 대조군 보다 6배 이상 증가하였으며(p<0.001) MIF mRNA는 신세뇨관 손상 부위와 육아종내에서 주로 발현되었다. 또한 UA군에서는 대식세포(p< 0.001)와 T 세포(p<0.001) 밀집이 대조군에 비하여 심하였고 육아종 부위에서 주로 관찰되었다. 대조군에서는 모두 광학 현미경 소견상 정상 신장 조직 소견을 보였고MIF mRNA와 단백 발현은 미세하였다. 결 론 : 고뇨산성 신병증에서 발생하는 육아종 형성과 신손상은 MIF에 의해서 매개되는 지연성 과민 유사 반응과 관련이 있을 것으로 생각한다. A Study on Assessment of Composite Couplings for Helicopter Rotor Blades with Multi-cell Sections

In this work, a closed-form analysis is performed for the structural response of coupled composite blades with multi-cell sections. The analytical model includes the effects of shell wall thickness, transverse shear, torsion warping and constrained warping. The mixed beam approach based on Reissner's semi-complementary energy functional is used to derive the beam force-displacement relations. The theory is validated against experimental test data and other analytical results for coupled composite beams and blades with single-cell box-sections and two-cell airfoils. Correlation of the present method with experimental results and detailed finite element results is found to be very good.

<P> According to the advent of ubiquitous world, the requirement for the mobile information devices with high display quality and compact device size is surprisingly increased. Among the diverse candidates for this mobile display system for mobile devices, Liquid Crystal on Silicon (LCoS) is the most competitive device due to the high aperture ratio and simple fabrication process. The inorganic liquid crystal (LC) alignment layers are widely used for LCoS devices because of the thermal and photochemical stability. In this work, the reactive sputtering was selected for the preparation method of inorganic LC alignment layers. The nitrogen (N2) gas had the effect on the deposition process of SiO2 and the surface morphology of SiO2 thin layers were affected by the N2 mixing ratio of sputtering gas. In addition, the LC alignment properties on SiO2 thin layer were also closely related with the N2 mixing ratio and other sputtering conditions. In the case of high RF power of reactive sputtering, the N2 mixing ratio has little effect on the LC alignment on SiO2 thin layers. However, in the case of low RF power of reactive sputtering, the LC alignment properties were enfeebled by increasing the N2 mixing ratio. This result might be attributed to the change of the surface morphology of inorganic SiO2 thin layers.</P>
