Objective-Several lines of evidence suggest that brain-derived neurotrophic factor (BDNF) plays a role in the pathophysiology of anxiety. We analyzed the association of the BDNF gene val66met polymorphism, in the coding region of exon XIIIA in chromosome 11q and panic disorder (PD). Methods-One hundred and six patients with PD and 160 control subjects were tested for the BDNF (val66met) polymorphism. A clinical interview and MINI international neuropsychiatric interview were conducted by trained psychiatrists to diagnose panic disorder according to DSMIV. Information about the symptomatic characteristics of panic disorder was also gathered by measuring various clinical scales (Hamilton anxiety rating scale, Beck anxiety inventory, Spielberg State-Trait Anxiety inventory and Anxiety sensitivity index). Results-There were no significant differences in the frequencies of the genotypes (χ2=4.16, df=2, p=0.13), alleles (χ2=0.79, df=1, p=0.37) or allele (Met) carriers (χ2=0.28, df=1, p=0.59) between the patients and controls. In addition, in comparing the severity of panic disorder with the genotypes of the BDNF gene, we could not find any significant differences between the genotypes. Conclusions-These results suggest that BDNF (val66met) polymorphisms do not play a major role in the susceptibility to or severity of panic disorder in our Korean population.
Oral lichen planus treated with alitretinoin (Alitoc®): A case report

Lichen planus (LP) is a chronic inflammatory mucocutaneous disease. Oral lesions, compared with cutaneous lesions, rarely undergo spontaneous remission. Clinical classification of oral LP consists of 3 types of lesions: reticular, atrophic or erythematous and erosive LP. Alitretinoin is an endogenous vitamin A derivative, 9-cis-retinoic acid. It is a pan-agonist of both known nuclear retinoid receptor families: retinoic acid receptors (RAR) and retinoid X receptors (RXR). Alitretinoin, compared to acitretin or isotretinoin, is expected to be more effective in retinoid-responsive dermatoses such as LP due to its dual receptor activity. We present a case of 48-year-old man who had painful erythematous crusted erosions on the lip for 6 months. Histopathologically, the lesion showed hyperkeratosis in the epidermis and lichenoid perivascular lymphohistiocytic infiltration in the dermis. He was diagnosed with oral LP. He was treated with oral steroid and cyclosporin combined with topical tacrolimus for 6 months but the lesion remained without much improvement. Oral steroid and cyclosporine were discontinued and oral alitretinoin (Alitoc®) was started in a dose of 30mg daily. Within 3 weeks of taking oral alitretinoin (Alitoc®), the erythematous scales had disappeared without noticeable adverse effect. To our knowledge, this is the first case report of oral LP treated with alitretinoin in the Korean literature.
Acrodermatitis continua of Hallopeau: A case series

Acrodermatitis continua of Hallopeau (ACH) is a rare form of pustular psoriasis clinically characterized by the presence of periungual and subungual sterile pustules with paronychia, onychodystrophy and osteolysis of the distal phalanx of the fingers, toes and atrophic skin. In this case series, 16 cases of ACH are included. 13 out of 16 patients were female and the range of age was from 9 to 61. All patients had subungual pustulosis with nail changes such as onycholysis(69%), trachyonychia(69%) or yellowish chromonychia(44%). Two patients had additional psoriatic lesions on the other body parts. Histopathological results of the subungual biopsy of 9 patients showed acanthosis, inflammatory cell infiltrates and pustular formation. Treatment included dapsone(69%), NSAIDs (63%), methotrexate(31%) and acitretin(13%). 62.5% of the patients required long-term treatment duration over 5 months. Among these patients, 60% showed poor treatment response of <25%. ACH is often difficult to treat and tends to show recalcitrant prognosis because of its failure or only partial response to various therapies. Therefore, further studies regarding effective treatment regimen for ACH are needed due to lack of established therapy.
Lobular capillary hemangioma after automicroneedle therapy system in an oriental medicine clinic

Lobular capillary hemangioma(LCH) is a common benign vascular tumor presenting as a rapidly growing angiomatous papule. Trauma and underlying microscopic arteriovenous malformations have been implicated in LCH. Automicroneedle therapy system(AMTS), usually used to treat acne scars and wrinkles, improves transdermal delivery making multiple mechanical holes into the skin. In these days, AMTS is currently being used for cosmetic purposes mainly in oriental medicine clinics. We present a case of 41-year-old woman who had two erythematous papules on the preexisting infantile hemangioma site of the neck for several months. She had history of AMTS procedure on the lesion in an oriental medicine clinic several months ago. Histological examination demonstrated granulation tissue, mainly composed of proliferative capillaries, in the papillary dermis. The patient is scheduled for the pulsed dye laser treatment of the remaining infantile hemangioma. With the rising trends of herbal procedures conducted without accurate knowledgements regarding cutaneous lesions, dermatologists need to understand the current status of oriental medicine clinics and caution the public against indiscreet procedures.

BACKGROUND/OBJECTIVES: Elevation of postprandial lipemia characterized by a rise in triglyceride (TG)-rich lipoproteins can increase the risk of atherogenesis. The objective of this study was to investigate postprandial lipemia response to a single dietary fat/sugar load test and monitor beneficial changes induced by the consumption of Platycodi radix (AP) beverage in healthy subjects. SUBJECTS/METHODS: A total of 52 subjects were randomly assigned to either placebo or AP beverage group with a high-fat shake in a randomized controlled crossover trial. Postprandial blood was collected at 0, 1, 2, 4, and 6 h and analyzed for TG and lipoprotein lipase mass. Inhibition of pancreatic lipase was determined in vitro. RESULTS: AP inhibited pancreatic lipase activity in vitro ($IC_{50}=5mg/mL$). Compared to placebo beverage, AP beverage consumption with a high-fat shake induced significant increase of plasma lipoprotein lipase mass (P = 0.0111, ${\beta}$ estimate = 4.2948) with significant reduction in very low-density lipoprotein (VLDL) TG concentration (P = 0.038, ${\beta}$ estimate = -52.69) at 6 h. Based on significant correlation between high-fat dietary scores MEDFICTS and postprandial TG responses in VLDL (P = 0.0395, r = 0.2127), subgroup analysis revealed that 6 h-postprandial VLDL TG response was significantly decreased by AP consumption in subjects with MEDFICTS ${\geq}40$ (P = 0.0291, ${\beta}$ estimate = -7214). CONCLUSIONS: AP beverage might have potential to alleviate postprandial lipemia through inhibiting pancreatic lipase activity and elevating lipoprotein lipase mass. Subgroup analysis revealed that subjects with high-fat dietary pattern could be classified as responders to AP beverage among all subjects.
The Development of the Korean Medication Algorithm for Major Depressive Disorder

There are many differences in the biological characteristics, clinical situations, and medical insurance systems between ethnic groups or countries. Consequently, there is a need for a specific Korean algorithm for the treatment of major depressive disorder. Therefore, the Korean society of depressive and bipolar disorder decided to develop the Korean Medication Algorithm for Major Depressive Disorder (KMA-MDD). The Korean Medication Algorithm Project for Major Depressive Disorder (KMAP-MDD) was designed with the goal of developing: 1) ideal algorithm, 2) Korean algorithm, 3) medication algorithm, 4) evidence-based and formal consensus algorithm. After collecting and reviewing many articles and reports by the evidence-based rule, we constructed a survey questionnaire designed to obtain the formal consensus of Korean experts. By employing panels of experts to review the collected evidences and survey results thoroughly, we used evidence based algorithm development as a component of the formal consensus development process. The KMA-MDD consists of two algorithms: one for major depressive disorder without psychotic features and the other for major depressive disorder with psychotic features. Clinical guidelines for the implementation of KMA-MDD were also developed. The KMA-MDD provides specific treatment strategies for each stage. The KMA-MDD is the first Korean algorithm for the treatment of major depressive disorder. It is based on evidence supporting the efficacy of each treatment modality and has obtained the consensus of Korean experts. We hope that the KMAMDD will be a good practical tool for clinicians who treat major depressive disorder in Korea.
Defoliation and Fruit Coloration in ‘Fuji’/M.9 Apple Affected by Cu-EDTA and Fe-EDTA Foliar Spray

The objective of this study was to determine whether two chelate compounds (Cu-EDTA and Fe-EDTA)applied as a foliar spray could effectively defoliate ‘Fuji’/M.9 apple trees and improve fruit qualityand fruit coloration. Foliar spray with these two chelate compounds at 30 days before harvestsignificantly defoliated fruit cluster leaves, which improved light interception of ‘Fuji’/M.9 appletrees. However, Cu-EDTA non-selectively defoliated bourse shoot leaves and true leaves, whichare involved in photosynthesis during the late period of fruit growth. Cu-EDTA markedly reducedfruit weight, soluble solids content, and fruit coloration compared to Fe-EDTA. Regarding Fe-EDTAconcentration, 3% Fe-EDTA was more effective at enhancing fruit coloration but lowered fruitweight compared to 1% Fe-EDTA. Consequently, we found that 1% Fe-EDTA induced moreselective defoliation of fruit cluster leaves than Cu-EDTA and was more effective in improving fruitquality and fruit coloration of ‘Fuji’ apple trees.
