        Background/Aims: Retroperitoneal fibrosis is a fibrosing disease process which frequently develops ureteral obstruction. Malignant retroperitoneal fibrosis has been reported that various types of cancer including cancer of the breast, stomach, prostate, lung, cervix, uteri, colon, pancreas, ovary, and even Hodgkin's disease. While the pathogenesis remains obscure, small foci of metastatic neoplasm in the retroperitoneal space can elicit a desmoplastic reaction in secondary form to malignancy. Methods: Recently we experienced 7 cases of retroperitoneal fibrosis with advanced gastric cancer. So, we report 7 cases of retroperitoneal fibrosis with clinical and radiologic characteristics. Results: The most common signs and symptoms were related irreversible renal dysfunction. The urologic finding noted medial deviation of the involved ureters and hydronephrosis Conclusions: Clinical manifestations are abdominal pain, back pain, hydronephrosis, and uremia. When sudden back pain and hydronephrosis develop in advanced malignancy, physicians should be considerd for retroperitoneal fibrosis.

        이상치에 따른 불확실성에 관한 연구

        딥러닝 모델의 성능을 평가하는 지표는 정확도를 평가할 뿐, 해당 모델의 불확실성을 평가할 수 없다. 불확실성은 정확도와는 별개로, 안정성 또는 신뢰도의 의미를 가질 수 있다. 불확실성을 측정할 수 있다면 기존 모델의 성능을 평가하는 지표 외에 새로운 지표가 될 가능성을 가지고 있다. 불확성을 측정하기 위한 딥러닝 모델로 베이지안 신경망을 사용한다. 본 논문은 불확실성이 딥러닝 모델에서 성능을 평가하는 지표가 될 수 있음을 확인해보려 하였다. 불확실성을 측정하기 위한 베이지안 신경망 모델로 베이지안 LSTM (Bayesian Long Short Term Memory)을 사용하였고, 모델의 학습에 실제 데이터를 적용하여 불확실성을 측정하였으며, 이를 기존 모델의 성능 지표와 비교분석 하였다. 실제 데이터를 이용한 실험에서 이상치가 추가됨에 따라 기존 모델의 성능지표는 증가하거나 감소하는 여러 가지 양상을 보이는 반면, 불확실성은 이상치에 따라 일관적으로 증가하는 결과를 보였다. Uncertainty is a separate measure of accuracy, and may have implications for stability or reliability. If uncertainty can be regulated, it has the potential to be a new metric for evaluating a performance of Deep learning models. Deep learning models for measuring uncertainty are Bayesian neural networks, which can be implemented by applying a drop-out scheme to each layer of existing deep learning models. This paper confirms that uncertainty can be an indicator of performance evaluation in deep learning models. We used Bayesian LSTM as a Bayesian neural network model to measure uncertainty, and measured uncertainty, which is compared to performance metrics from existing models.

        Change in Portal Pressure and Clinical Outcome in Cirrhotic Patients with Gastric Varices after Plug-Assisted Retrograde Transvenous Obliteration

        Background/Aims: Plug-assisted retrograde transvenous obliteration (PARTO) is widely used to manage gastric varices with a portosystemic shunt. It is not clear whether portal pressure and the incidence of complications increase after PARTO. The aim of this study was to determine the changes in portal pressure and the associated changes in liver function, ascites, hepatic encephalopathy, and especially esophageal varix (EV) after PARTO. Methods: From March 2012 to February 2018, 54 patients who underwent PARTO were analyzed retrospectively. The parameters collected included liver function and episodes of cirrhotic complications before and at 1 and 6 months after PARTO. Results: The analysis of 54 patients showed improvement in liver function during the 6-month follow-up period (Model for End-Stage Liver Disease score: change from 11.46±4.35 to 10.33±2.96, p=0.021). Among these 54 patients, 25 patients were evaluated for their hepatic venous pressure gradient (HVPG) before and after PARTO (change from 12.52±3.83 to 14.68±5.03 mm Hg; p<0.001). Twenty-five patients with portal pressure measured before and after PARTO were evaluated for risk factors affecting liver function improvement and EV deterioration. No factor associated with portal pressure was affected by liver function improvement. Post-PARTO portal pressure was a risk factor affecting EV deterioration (HVPG-post: odds ratio, 1.341; 95% confidence interval, 1.017 to 1.767; p=0.037). Conclusions: The artificial blockade of the portosystemic shunt evidently leads to an increase in HVPG. Liver function was improved over the 6-month follow-up period. Portal pressure after PARTO was a significant risk factor for EV deterioration. Portal pressure measurement is helpful for predicting the patient’s clinical outcome.

        Differences in Prostate Cancer between Korean Patients, the European Randomized Study of Screening for Prostate Cancer/Rotterdam Group, and a Dutch Clinical Cohort

        Purpose: We evaluated differences in biopsy-detected prostate cancer (PC) between a newly defined Korean clinical cohort (KCC) and two Western populations. Materials and Methods: The records of 723 Korean men aged 55 to 75 years who underwent an initial transrectal ultrasound-guided biopsy from 2004 to 2010 were retrospectively reviewed. Prostate biopsies were performed due to prostate-specific antigen (PSA) levels greater than 4.0ng/ml, or suspicious findings on rectal examination or transrectal ultrasonography. Characteristics of PC were compared between the KCC and Western groups (the Rotterdam group of the European Randomized Study of Screening for Prostate Cancer: ERSPC and Dutch Clinical Cohort: DCC). Results: The cancer detection rate was 26.4% (191/723) in the KCC, 20.9% (473/2268) in the ERSPC/Rotterdam group and 43.1% (138/320) in the DCC. The median PSA in patients with PC was 11.8ng/ml, 5.7ng/ml and 7.5ng/ml in the KCC, ERSPC/Rotterdam group, and DCC, respectively. The proportion of patients with a Gleason score of 7 or more was 57.1% (109/191) in the KCC, 37.5% (174/464) in the ERSPC/Rotterdam group and 37.0% (51/138) in the DCC. Conclusions: We found large differences in cancer detection rate, PSA, and Gleason score distribution between the KCC and Western cohorts. PC detected in Korean patients had more advanced clinical factors than in Western populations due to low availability of PSA screening programs.

        Effect of Tripolyphosphate (TPP) on the Controlled Release of Cyclosporin A from Chitosan-coated Lipid Microparticles

        Soybean phosphatidylcholine microparticles loaded with cyclosporin A (CsA) were prepared by the modified emulsion solvent diffusion and ionic gelation method, in which chitosan on the surface of the microparticles was crosslinked with various concentrations of tripolyphosphate (TPP). The morphology of the particles was characterized by scanning electron microscopy (SEM). The change of particle size and zeta-potential by chitosan on the surface of the lipid microparticles were systematically observed. The encapsulation efficiency and loading capacity of CsA in the particles were determined by high performance liquid chromatography (HPLC). In vitro release kinetics was studied using the dialysis method. In the results, the mean particle size and the zeta-potential of lipid microparticles increased when the attached chitosan was cross-linked (from 2.5 to 6.2 ${\mu}m$ and from -37.0 to +93.0 mV, respectively). The cyclosporin A-loaded lipid microparticles appeared discrete and spherical particles with smooth surfaces. The encapsulation efficiency of CsA was between 79% and 90% while the loading capacity was between 41% and 56%. In vitro release study showed that the crosslinkage of chitosan by TPP significantly delayed the release of CsA from the particles in a concentration-dependent manner. Thus, the release of CsA from the lipid microparticles could be controlled by tripolyphosphate used as a cross-linking agent.

        Genotype $\times$ Environment Interaction for Yield in Sesame (Sesamum indicum L.)

        Application of genotype by environment ($G\;{\times}\;E$) interaction would be used for identifying optimum test condition of the varietal adaptation in the establishment of breeding purpose. Yield and yield components were used to perform additive main effect and multiplicative interaction (AMMI) analysis. Significant difference for $G\;{\times}\;E$ interaction were observed for all variable examined. For yield, 0.18 of total sum of squares corresponded to $G\;{\times}\;E$ interaction. Correlation analysis was carried out between genotypic scores of the first interaction principal component axis (IPCA 1) for agronomic characters. Significant correlations were observed between IPCA 1 for yield and capsule bearing stem length (CBSL), number of capsule per plant (NOC). The biplot of grain yield means for IPCA1 which accounted for 34% of the variation in total treatment sums of squares showed different reaction according to $G\;{\times}\;E$ interaction, genotypes and environments. Taegu showed relatively lower positive IPCA1 scores, and it also showed smaller coefficient variation of yield mean where it is recommendable as a optimal site for the sesame cultivar adaptation and evaluation trial. In case of variables, Yangbaek and M1 showed relatively lower IPCA1 scores, but the score direction showed opposite each other on the graph. Ansan, Miryang1, Miryang4, and Miryang6 seemed to be similar group in view of yield response against IPCA1 scores. These results will be helpful to select experimental site for sesame in Korea to minimize $G\;{\times}\;E$ interaction for the selection of promising genotype with higher stability.

        Relationship between serum 25-hydroxyvitamin D and interleukin-31 levels, and the severity of atopic dermatitis in children

        Purpose: Atopic dermatitis (AD) is a chronic inflammatory relapsing skin disorder. Vitamin D plays a pivotal role in the development of AD, and interleukin (IL) 31 is known to be related to pruritus in AD. The aim of our study was to determine whether 25-hydroxyvitamin D (25(OH)D) levels are related to IL-31 levels or to the severity of AD. Methods: We enrolled 91 children with AD and 32 control subjects without history or symptoms of allergic diseases. Blood was drawn to evaluate complete blood cell count, total eosinophil count (TEC), and total IgE, specific IgE to common allergens, 25(OH)D, and IL-31 levels. Serum 25(OH)D and IL-31 levels were measured using high-performance liquid chromatography and enzyme-linked immunosorbent assay, respectively. The scoring atopic dermatitis (SCORAD) index was used to evaluate the severity of AD. Results: The mean 25(OH)D level was significantly lower in the AD group than in the control group; 25(OH)D decreased greatly in the moderate and severe AD groups compared with the mild AD group. Children with atopic sensitization showed significantly lower 25(OH)D levels than nonatopic children. However, serum IL-31 levels were not related to AD group, SCORAD index, or 25(OH)D levels. The SCORAD index was inversely correlated with serum 25(OH)D level and positively correlated with TECs and total IgE levels. Children with moderate and severe AD had significantly higher TECs than children with mild AD. Conclusion: Vitamin D is related to the severity of AD independently of IL-31.

        Low-Noise Characteristics of a 0.15 μm GaAs Power Metamorphic HEMT with a Double-Doped In0.52Al0.48As/In0.53Ga0.47As Structure

        The noise performance of 0.15 m GaAs power metamorphic high electron mobility transistor (MHEMT) devices with a double-doped In0:52Al0:48As/In0:53Ga0:47As structure has been investigated together with the DC and microwave characteristics. The 0.15 100 m2 MHEMT device shows a high gate-to-drain breakdown voltage of ..8.3 V, an extrinsic transconductance of 740 mS/mm, and a threshold voltage of ..0.97 V. The obtained cut-o frequency and maximum frequency of oscillation are 133 GHz and 235 GHz, respectively. A very low minimum noise gure of 1.11 dB and an associated gain of 9.51 dB at 30 GHz are obtained for the power MHEMT with an In content of 53 % in the InGaAs channel. This excellent noise characteristic is attributed to a drastic reduction in the gate resistance by the T-shaped gate with a wide head and to the improved device performance.

        Interpretation of Agronomic Traits Variation of Sesame Cultivar Using Principal Component Analysis

        This study was conducted to evaluate the growth characters and yield components of 18 collected sesame cultivars to get basic information on the variation for the sesame breeding using principal component analysis. All characters except days to flowering, days to maturity and 1,000 seed weight showed significantly different. Seed weight per 10 are showed higher coefficient of variance. Capsule bearing stem length and liter weight showed positive correlation with seed yield per 10 are. The principal components analysis grouped the estimated sesame cultivars into four main components which accounted for 83.7% of the total variation at the eigenvalue and its contribution to total variation obtained from principal component analysis. The first principal component ($Z_1$) was applicable to increase plant height, capsule bearing stem length and 1,000-seed weight. The second principal component ($Z_2$) negatively correlated with days to flowering and maturity by which it was applicable to shorten flowering and maturity date of sesame. At the scatter diagram, Yangbaek, Ansan, M1, M2, M4, M7 and M9 were classified as same group, but M10, Yanghuk, Kanghuk, M5, M6, M12 and M13 were classified as different group. This results would be helpful for sesame breeder to understand genetic relationship of some agronomic characters and select promising cross lines for the development of new sesame variety.

        대퇴골 간부 골절의 수술적 차이에 의한 치료결과

        Purpose: To evaluate and compare the bone union period, complications and functional assessments between compression plate and intramedullary nailing which are operative methods of the femoral shaft fracture. Material and Method: We evaluate the bone union period, complications and functional assessments of the 65 cases(63 patients). 30 cases were treated with compression plate and another 35 cases were treated with intramedullary nailing, who were diagnosed to have femoral shaft fracture. And they were treated in our hospital from January 1995 to August 1999, whose follow up was available more than 1 year. Result: Mean bone union period of compression plate fixations was 14.6 weeks and intramedullary nailing was 16.2 weeks. Complications in compression plate fixations group was deep infection in one case, metal failure in one case, refracture in one case, nonunion in one case, and there were 5 complications in intramedullary nailing fixations group which were nonunion in 2 cases, limb shortening in 2 cases and angular deformity of nail in one case. By using functional assessment according to Magerl et al(16) among 30 cases of compression plate fixations, 25 were excellent, 3 were good, 2 were fair, and among 35 cases of intramedullary nailing, 29 were excellent, 4 were good, 2 were fair, so we achieved excellent results in both techniques as 83.3% and 82.9% cases each. Conclusion: In operative management of femoral shaft fracture, compression plate fixations and intramedullary nailing shows average difference of 1.6weeks of bone union period, but both method didn't show significant difference in bone union period. Both methods achieved excellent results in functional assessments, so if we manage by knowing thoroughly the advantage and disadvantage of both technique, we can get a excellent results with reduced complications.
