        Tracheostomy in infants and children is associated with a high rate of accidental decannulation in the early postoperative period. The most common complication in infants and children, which most often results in death, is accidental decannulation. Other authors advocate suturing tracheostomy tubes in place in infants and children under general anesthesia in operating room for avoiding decannulation. The authors designed tracheostomy tube for the prevention of accidental decannulation following tracheostomy in infant, so report with review of literatures.

      • 노령의 대퇴 전자간 골절 치료에서 압박고 나사못의 안정성 및 유용성

        목 적: 노령의 대퇴 전자간 골절에서 압박고 나사못의 안정성에 영향을 주는 인자들간의 상관 관계와 골절 양상에 따른 술후 고관절 기능을 평가 및 분석하였다. 대상 및 방법: 압박고 나사를 이용하여 수술적 치료를 시행한 231명의 환자 중 6개월 이상 추시가 가능하였던 60세 이상 84명의 환자들을 대상으로 했다. 방사선적 계측은 압박고 나사못의 활강 길이 및 내반각, 가압 나사 위치, 정복 상태, 원위 골편의 내측 전위를 측정하였으며 술후 기능은 Clawson의 기능 평가표로 하였고 골절 양상에 따라 방사선 계측과 기능 회복과의 상관 관계를 평가하였다. 결 과: 임상 결과는 불안정 골절에서 양호가 10례, 불량이 23례, 안정 골절에서는 양호 40례, 불량이 11례로 안정 골절에서 임상 결과가 좋았고(p<0.001), 압박고 나사 활강은 10 ㎜ 이상일 때 양호가 4례, 불량이 21례, 10 ㎜ 이하에서 양호가 46례, 불량이 13례로 10 ㎜ 이상 활강시에 술후 결과가 불량했다(p<0.001). 다르 방사선적 계측 요인은 결과에 의미있는 차이를 보이지 않았다. 또한, 불안정 골절 중에서 전위 양상 골절의 경우 10㎜ 이상 활강이 12례, 10㎜ 이하 활강이 8례, 분쇄 양상 골절의 경우는 10㎜ 이상이 11례, 10㎜ 이하가 2례로 분쇄 양상의 불안정 골절에서 10㎜ 이상 활강이 의미있게 많았다 (p<0.001). 결 론: 10㎜ 이상의 지연 나사 활강은 고관절 기능 회복 약화를 유발시킬 수 있으며 불안정 골절 분쇄 양상 골절은 이런 지연 나사의 활강이 과도하게 유발될 수 있으므로 불안정 분쇄 양상 골절에서 압박고 나사의 단독 사용은 재고해 보아야 할 것이다. Purpose: To evaluate the relationship between fracture stability and functional results, and analyze the correlation between stability factors and the outcome in intertrochanteric fracture of the elderly. Materials and Methods: Of the 231 patients, 84 patients with age above 60 were able to follow up for minimum 6 months. We measured the sliding length of the lag screw, varus degree, position of lag screw, reduction status and medialization of distal fragment radiologically. The functional outcome of the treatment was evaluated with the Clawson's result classification and we evaluated the correlation between the radiological results of measurement and the functional recovery depending on the Evans fracture classification. Results: There were good results in 40 cases out of 51 stable fractures, and in 10 cases out of 33 unstable fractures (p<0.001). In case of sliding of lag screw more than 10㎜, good results were obtained in 4 cases, and poor in 21. And in case of sliding less than 10㎜, good results were obtained in 46, and poor in 13. (p<0.001).But there was no relationship between other radiologic factors and clinical results. In unstable type, there were 12 cases with lag screw sliding more than 10㎜ and 10 cases with less than 10㎜. In comminuted type, there were 11 cases with lag screw sliding more than 10㎜ and 2 cases with less than 10㎜(p<0.001). Conclusion: The sliding of lag screw more than 10㎜ may result in poor outcome. As in comminuted unstable pattern, sliding of lag screw might be excessive, the use of compression hip screw alone is not good treatment option.

        The College of Engineering was specialized to have a sole function for supplying technical high school teachers in 1977 and referred to as the College of Industrial Education according to the Government's policy. The economic impact mainly due to the oil-crises caused a severe problem to this college, namely discrepancy of supply to demand scale. Accordingly the number of student enrollment was to be fluctuated from 479 in 1979 through 800 in 1979 and 900 in 1980 to 430 in 1981. And the engineering departments have been attached to this Engineering Education College since 1981. These circumstanses met singnificant inefficiency in facility operation. This program aims at providing an overall relocation of the college facilities and preparing the practical system to make the joint-operation possible taking into account circulation, academic stream, budget, maintenance, etc. However this study could not get optimal spacing but some trade-off, considering the current situation.

      • 알레르기성 비염 환자에서 절지동물성 특이항원의 양성율 및 항원간 일치율

        The current study was designed to evaluate the positive rates of arthropod allergens by Korean inhalant panel of MAST-CLA in patients with allergic rhinitis. One hundred sixty patients, diagnosed to allergic rhinitis in an Out Patient Center of Soonchunhyang University Chunan Hopsital during Aug 1998 to July 2000, were studied. 1. The overall positive rate among 35 specific allergens was 59.4% (95 patients). 2. The arthropod related allergen accounted for 82.1% (78 patients) of the positive rate. 3. Each positive rate of Dermatophagoides farinae (Df), Dermatophagoides pteronyssinus (Dp), house dust (Hd), and cockroach mix (Cm) was 45.0% (72 patients), 43.1% (69 patients), 28.1% (45 patients), and 8.8% (14 patients), respectively. 4.Significant agreement between arthropod allergens was observer (P < 0.05). 5. The obseved agreement between Df and Dp was 95.6%, showing most significant Kappa index (Kappa = 0.911, P < 0.001). There results suggested that although MAST-CLA is a useful measure of identifying specific allergens, the specificity of two mite allergen relatively low, implying because of common allergen between mites in Korean panel of MAST-CLA. Our data also supported that arthropod is the major cause of allergen in patients with allergic rhinitis, however, the MAST-CLA should be modified in order to exclude common allergen of mite and to include more specific mite allergen.

        Compressive Properties of Nanoporous Gold Through Nanoindentation: An Analytical Approach Based on the Expanding Cavity Model

        We investigated the analytic relation between hardness and compressive yield stress using an expanding cavity model (ECM)for nanoporous gold (np-Au). We prepared three np-Au samples with ligament sizes 30.61, 59.36 and 116.33 nm by freecorrosiondealloying and post heat treatment. The indentation contact morphology was examined to estimate the hardnessaccurately from the nanoindentation load-depth curve. Unlike conventional dense metals, the deformation was confined to theprojected contact area, and the center of the residual impression was dominated by densification. The projected contact areaestimated by the Oliver–Pharr method was overestimated, so that a new contact area function was proposed that consideredthe indentation contact morphology of np-Au. It was confirmed that a hardness value taking into account the indentationcontact morphology of np-Au matches well with the hardness derived by direct measurement of the residual impression. We modeled the ratio of hardness to compressive yield stress for np-Au using an ECM. The scaling factors, which representthe extra strain-hardening in the core in the ECM, were analyzed for np-Au and dense metals. An ECM that better matchesnp-Au is suggested based on the scaling factor resulting from densification beneath the indenter.

      • Process Optimization of Integrated SiCr Thin-Film Resistor for High-Performance Analog Circuits

        <P>We investigated the characteristic variation of an integrated thin-film resistor (TFR), which is composed of silicon chromium (SiCr), according to process conditions and its effects on analog circuits. To improve TFR properties, such as temperature coefficient of resistance (TCR) and mismatch, the integrated TFR was examined under various process conditions. First, the sputtering power and the annealing temperature were optimized to control the TCR property. Secondly, the etching time and the dummy pattern were optimized to enhance the mismatch property. The measured TCR is -3.9 ppm/<SUP>°</SUP>C and the calculated mismatch slope from the standard deviation of resistance mismatch is 0.34%·μm. These results show that the fabricated TFR had near-zero TCR and a high matching constant. To verify the TFRs effects on analog circuits, low-pass filters (LPFs) were fabricated and measured as a test vehicle; the use of the optimized resistor considerably improved the performance of the resulting LPFs.</P>

        A Precision On-Chip Measurement Technique for Dielectric Absorption of an Integrated Capacitor

        <P>This paper presents a precision dielectric absorption measurement method to characterize integrated capacitors. Conventional methods using an oscilloscope have measurement error due to the resolution limitation of the oscilloscope and the off-leakage current of the control switches. We improve the measurement accuracy by employing a switch without off-leakage current and a readout amplifier to minimize the influence of installation noise and environmental noise. Test chips were fabricated using a 0.18-μm CMOS process technology. The test chip consists of a metal-insulator-metal capacitor and measurement circuits. The measured standard deviations of the recovery level and short-term repeatability are 43.2 and 31.0 ppm, respectively. These results show that the proposed method is very accurate and stable compared with the conventional one.</P>
