Analysis of ICU Treatment on Resection of Giant Tumors in the Mediastinum of the Thoracic Cavity

Objective: The purpose of this study was to assess prognosis after resection of giant tumors (including lobectomy or pneumonectomy) in the mediastinum. Materials and Methods: Patients with resection of a giant tumor in the mediastinum of the thoracic cavity received ICU treatment including dynamic monitoring of vital signs, arterial blood pressure and CVP detection, determination of hemorrhage, pulmonary function and blood gas assay, treatment of relevant complications, examination and treatment with fiber optic bronchoscopy, transfusion and hemostasis as well as postoperative removal of ventilators by invasive and non-invasive sequential mechanical ventilation technologies. Results: Six patients were rehabilitated successfully after ICU treatment with controlled postoperative errhysis and pulmonary infection by examination and treatment with fiber optic bronchoscopy without second application of ventilators and tubes after sequential mechanical ventilation technology. One patient died from multiple organ failure under ICU treatment due to postoperative active hemorrhage after second operative hemostasis. Conclusions: During peri-operative period of resection of giant tumor (including lobectomy or pneumonectomy) in mediastinum ofthe thoracic cavity, the ICU plays an important role in dynamic monitoring of vital signs, treatment of postoperative stress state, postoperative hemostasis and successful removal of ventilators after sequential mechanical ventilation.

Purpose The purpose of this study was to compare the survival and toxicities in cervical esophageal squamous cell carcinoma (CESCC) treated by concurrent chemoradiothrapy with either three-dimensional conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT) techniques. Materials and Methods A total of 112 consecutive CESCC patients were retrospectively reviewed. 3D-CRT and IMRT groups had been analyzed by propensity score matching method, with sex, age, Karnofsky performance status, induction chemotherapy, and tumor stage well matched. The Kaplan- Meier method and Cox proportional hazards model were used for overall survival (OS) and progression-free survival (PFS). Toxicities were compared between two groups by Fisher exact test. Results With a median follow-up time of 34.9 months, the 3-year OS (p=0.927) and PFS (p=0.859) rate was 49.6% and 45.8% in 3D-CRT group, compared with 54.4% and 42.8% in IMRT group. The rates of grade ! 3 esophagitis, grade ! 2 pneumonitis, esophageal stricture, and hemorrhage were comparable between two groups, while the rate of tracheostomy dependence was much higher in IMRT group than 3D-CRT group (14.3% vs.1.8%, p=0.032). Radiotherapy technique (hazard ratio [HR], 0.09; 95% confidence interval [CI], 0.01 to 0.79) and pretreatment hoarseness (HR, 0.12; 95% CI 0.02 to 0.70) were independently prognostic of tracheostomy dependence. Conclusion No survival benefits had been observed while comparing IMRT versus 3D-CRT in CESCC patients. IMRT with fraction dose escalation and pretreatment hoarseness were considered to be associated with a higher risk for tracheostomy dependence. Radiation dose escalation beyond 60 Gy should be taken into account carefully when using IMRT with hypofractionated regimen.
Tanshinone IIA Reverses the Malignant Phenotype of SGC7901 Gastric Cancer Cells

Backgrounds: Tanshinone IIA (TIIA), a phenanthrenequinone derivative extracted from Salvia miltiorrhiza BUNGE, has been reported to be a natural anti-cancer agent in a variety of tumor cells. However, the effect of TIIA on gastric cancer cells remains unknown. In the present study, we investigated the influence of TIIA on the malignant phenotype of SGC7901 gastric cancer cells. Methods: Cells cultured in vitro were treated with TIIA (0, 1, 5, $10{\mu}g/ml$) and after incubation for different periods, cell proliferation was measured by MTT method and cell apoptosis and cell cycling were assessed by flow cytometry (FCM). The sensitivity of SGC7901 gastric cancer cells to anticancer chemotherapy was investigated with the MTT method, while cell migration and invasion were examined by wound-healing and transwell assays, respectively. Results: TIIA (1, 5, $10{\mu}g/ml$) exerted powerful inhibitory effects on cell proliferation (P < 0.05, and P < 0.01), and this effect was time- and dose-dependent. FCM results showed that TIIA induced apoptosis of SGC7901 cells, reduced the number of cells in S phase and increased those in G0/G1 phase. TIIA also significantly increased the sensitivity of SGC7901 gastric cancer cells to ADR and Fu. Moreover, wound-healing and transwell assays showed that TIIA markedly decreased migratory and invasive abilities of SGC7901 cells. Conclusions: TIIA can reverse the malignant phenotype of SGC7901 gastric cancer cells, indicating that it may be a promising therapeutic agent.

<P>A 6-bit full-binary compact and low-power current-steering digital-to-analog converter (DAC) designed for 60-GHz Wireless Personal Area Network applications is presented. The closely located circuit components based on the stacked unit cell minimize the parasitic capacitance and enhance the high-frequency dynamic linearity. The proposed binary structure realizes a compact DAC by eliminating the need for additional circuits, such as thermometer decoders, and thus reduces power consumption. A prototype 6-bit 3.1-GS/s full-binary DAC was fabricated in a 90-nm CMOS process. The DAC exhibits a spurious-free dynamic range of >37.2 dB up to 3.1 GS/s over the Nyquist input. The chip consumes 17.7 mW of power and occupies 0.038 mm(2) of core size.</P>

This study was designed to evaluate the effects of an Schultz`s autogens traning and herbal medicine with various scales on symptoms of UL-syndrom(鬱證). We treated UL-syndrom(鬱證) with Schultz`s autogens traning, including herbal medicine and we evaluated the clinical progress of the patient using BDI. After treatment about 2-month, the chief complaint and most of symptoms were subsided and improved. BDI score were decreased 32 to 10. This study suggests that Schultz`s autogens traning and herbal medicine is significantly effective in the treatment of UL-syndrom(鬱證).
A 65-nm CMOS 6-Bit 20 GS/s Time-Interleaved DAC With Full-Binary Sub-DACs

<P>A 6-bit 20 GS/s two-channel time-interleaved current-steering digital-to-analog converter (DAC) with compact full-binary sub-DACs is presented. Optimally adjusted transition timings between the input data and the interleaving clock minimize glitches by the time-interleaving switches and enhance the high-frequency linearity. In order to prevent static linearity degradation by the leakage current through the time-interleaving switches, the relationship between the output current and the leakage current is analyzed. The proposed DAC architecture and the pseudo-differential logic gates for the high-speed data interface reduce the circuit complexity as well as the power consumption. The prototype 6-bit 20 GS/s DAC, fabricated in a 65-nm CMOS process, achieves a spurious-free dynamic range of 35.1 dB up to the Nyquist input, and consumes 136 mW given a 1.2-V power supply.</P>

최근 모든 연령대의 사람들은 미용적인 이유로 더 밝은 피부를 원하고 있으며, 천연 제품은 화학적으로 합성된 화합물보다 더 많은 관심을 받고 있다. 조구등은 아시아에서 전통 한약재로 널리 사용되어 왔다. 새로운 피부미백제를 찾기 위해 본 연구에서는 조구등의 항산화 활성과 잠재적인 tyrosinase 억제 작용을 확인하였다. 조구등을 70% 에탄올로 추출하여 항산화 활성을 분석하고 tyrosinase 활성과 melanin 합성에 미치는 영향을 평가했다. 총 mRNA 발현은 RT-PCR을 사용하였다. 그 결과 조구등 추출물은 B16F10 세포에서 뛰어난 항산화 능력과 상당한 수준의 폴리페놀 및 플라보노이드 화합물을 함유하였다. 또한, 세포 내 tyrosinase 활성을 억제하고 처리된 세포에서 melanin의 양을 감소시켰다. Tyrosinase의 mRNA 발현은 1 mg/ml 농도에서 현저히 감소하였다. 이와 같은 결과는 조구등이 미백 효과가 있는 화장품의 천연 소재로 사용될 수 있는 높은 잠재력을 가지고 있음을 시사한다. Many people of all ages wish to have lighter skin for cosmetic reasons, and natural products attract more attention than chemically synthesized compounds. Uncaria rhynchophylla is widely used in Asia as a traditional herbal medicine. In order to find novel skin whitening agents, the present study evaluated the antioxidant activity and potential tyrosinase-inhibiting properties of U. rhynchophylla. Specifically, this study analyzed the antioxidant capacity of a 70% ethanolic extract of U. rhynchophylla as well as its effects on tyrosinase activity and melanin synthesis. Total mRNA levels were examined using reverse transcription polymerase chain reaction. The results revealed that U. rhynchophylla extracts exhibit great antioxidant capacity and significant levels of polyphenol and flavonoid compounds. U. rhynchophylla extracts can also powerfully inhibit tyrosinase activity. This same capacity was observed in melanoma B16F10 cells; that is, U. rhynchophylla extracts suppressed intracellular tyrosinase activity and reduced the amount of melanin in treated cells. In addition, a 1 mg/ml concentration of U. rhynchophylla extract significantly reduced the mRNA expression levels of tyrosinase. U. rhynchophylla extracts decrease tyrosinase and inhibit melanogenesis in B16F10 cells. This finding suggests that U. rhynchophylla has great potential as a natural whitening agent in skincare products.
모작업장에서 휘발성 유기오염물질의 분석과 근로자들의 건강위해성 평가

This study was conducted to assess the health risk on the volatile organic compounds such as toluene, xylene, and styrene in painting workplace. It was monitored through personal air sampling during working time in selected 5 workplaces and analysed using gas chromatography. For the settlement of exposure situation, there were regarded working conditions such as working hours, yearly working days, and working years. Also, Monte-Carlo simulation was used for the induction of hazard index using toxicity value from IRIS(Integrated risk information system) database. The results of risk assessment were summarized as follows. 1. The air concentration of toluene was $7.096{\pm}15,644ppm,\;2.586{\pm}4.275ppm\;for\;xylene,\;1.914{\pm}5.320ppm$ for styrene in blast painting workplaces. The level of toluene was different significantly compared with the level of xylene and styrene. 2. Computated chronic daily intake values of 95th percentile on toluene, xylene and styrene treated by Monte-Carlo simulation were 9.616, 3.567, 2.782 mg/kg/day, respectively. 3. Computated hazard index values of 75th percentile on toluene, xylene and styrene treated by Monte-Carlo simulation were 3.5, 1.0 and 1.6, respectively. Adverse health effects on the toluene, xylene and styrene would be expected by working exposure in selected 5 workplaces since the hazard indices of three compounds were exceeded 1 in the surroundings of 75th percentile though having the low emerged frequency.
