      • Efficacy of Long-Term Tenofovir-Based Therapy in Chronic Hepatitis B Patients with Previous Nucleos(t)ide Analogues Treatment Experience

        Aims: Tenofovir disoproxil fumarate (TDF) is considered as the preferred treatment option for chronic hepatitis B (CHB) patients with treatment failure or resistance to prior nucleos(t)ide analogues (NAs) treatment. We investigated the efficacy of long-term TDF-based therapy in CHB patients with previous NAs-experience. Methods: This study included total 251 patients who had previous history of NAs therapy and were treated with TDF mono (n=173) or TDF combined with other NA (n=78) from August 2012 to March 2017. Virologic response (VR) was defined as undetectable serum HBV DNA by PCR (<20 IU/mL). Results: Mean age of patients was 49.3 years, median duration of TDF therapy was 27.2 months, 75.7% were HBeAg-positive, and median HBV DNA was 3.7 log10IU/mL. The cumulative rates of VR were 188/244 (77.0%), 180/211 (85.3%), and 146/161 (90.7%) at 12, 24, and 36 months, respectively. Multivariate analysis showed that body mass index (OR 0.77, 95% CI 0.61-0.95, p=0.0189) and duration of TDF therapy (OR 1.09, 95% CI 1.02-1.18, p=0.0221) was significantly associated with VR. TDF monotherapy, HBeAg-positivity, platelet count, serum albumin was associated with VR in the univariate analysis, but not significant in the multivariate analysis. In relation to renal safety, patients showed renal impairment (7, 3.0%), mild hypophosphatemia (15, 7.2%), severe hypophosphatemia (1, 0.4%). Conclusions: Long-term TDF-based therapy demonstrated highly effective in viral suppression and relatively favorable renal safety in CHB patients with previous NA-experience. The body mass index and duration of TDF therapy was independent factors associated with VR.

        Donepezil, Tacrine and α-Phenyl-n-tert-Butyl Nitrone (PBN) Inhibit Choline Transport by Conditionally Immortalized Rat Brain Capillary Endothelial Cell Lines (TR-BBB)

        In the present study, we have characterized the choline transport system and examined the influence of various amine drugs on the choline transporter using a conditionally immortalized rat brain capillary endothelial cell line (TR-BBB) in vitro. The cell-to-medium (C/M) ratio of [3H]choline in TR-BBB cells increased time-dependently. The initial uptake rate of [3H]choline was concentration-dependent with a Michaelis-Menten value, Km, of 26.2 ± 2.7 µM. The [3H]choline uptake into TR-BBB was Na+-independent, but was membrane potential-dependent. The [3H]choline uptake was susceptible to inhibition by hemicholinium-3, and tetraethylammonium (TEA), which are organic cation transporter substrates. Also, the uptake of [3H]choline was competitively inhibited with Ki values of 274 µM, 251 µM and 180 µM in the presence of donepezil hydrochloride, tacrine and α-phenyl-n-tert-butyl nitrone (PBN), respectively. These characteristics of choline transport are consistent with those of the organic cation transporter (OCT). OCT2 mRNA was expressed in TR-BBB cells, while the expression of OCT3 or choline transporter (CHT) was not detected. Accordingly, these results suggest that OCT2 is a candidate for choline transport at the BBB and may influence the BBB permeability of amine drugs.

        가정연계 신체활동이 유아의 기초체력에 미치는 영향

        The awareness of the decreasing physical activities of children today introduced the application of Home-Based physical activities as one of the solutions to the problem. This Home-Based physical activity program provided the families with training materials and wall posters for physical activities as well as the eurhythmics videos accessible through provided QR codes in attempt to assist the families to practice the physical activities taken in children educational institutes from homes. 4910 children of both sexes across the nation partook in this research while the researchers were determined to test the change on children’s physical strength before/after the application of the program. In order to track the change, the researchers have adjusted and supplemented the research tools of Na-young An, Eun-jung Kim, Tae-seop Park (2013) to measure the endurance, muscle strength, flexibility, agility, adaptation, and balance of the children. The research revealed that the children that partook in the program for household physical activities showed an overall superiority on all 6 fields - endurance (scissor jump for 30 seconds), muscle strength (grasping power), flexibility (sitting trunk flexion), agility (30meters shuttle run), adaptation (standing high jump), balance (standing on one foot) - compared to the children who haven’t engaged in the program. Also the researchers found that the general physical strength of the children in the program to have increased periodically during the test. Most of all, there was a significant difference on overall body endurance and right-left balance from which the deduction of the relatedness between the two and the household physical activities was made. In conclusion, Home-Based physical activities have shown to have positive influence on the general physical strength of the children.

        원위부 신세뇨관성 산증에서 산-염기 운반체의 결손

        The purpose of this study was to elucidate whether the molecular defect of acid-base transporters in renal tubules is related to the functional defect of urinary acidification in distal renal tubular acidosis(RTA). We performed NH₄Cl, furosemide, or bicarbonate loading test to evaluate renal acidification function, and immunohistochemistry using antibodies to H^+ -ATPase, Cl^-/HCO₃^- exchanger(band-3 protein), and Na^+/K^+ -ATPase in kidney tissue in 6 patients with RTA and renal cell carcinoma patients as normal controls. Kidney tissue was obtained either by percutaneous needle biopsy(RTA) or nephrectomy(NC). The results were as follows; 1) In all six RTA patients, proton secretory defect of distal acidification was shown by a failure to lower the urine pH after NHC1 loading or furosemide test or abnormally low urine-blood pCO₂ difference during bicarbonate loading. In two patients with RTA, proximal acidification defect was combined, which was demonstrated by increased fractional excretion of bicarbonate. 2) In mal control, intense H^+ -ATPase and band-3 protein staining was observed in collecting ducts. 3) In distal RTA patients, H6+ -ATPase and band-3 protein staining was not demonstrable or markedly decreased in the intercalated cells of distal nephron. 4) In two patients who had both proximal and distal RTA, H^+ -ATPase staining was markedly decreased in the brush border of proximal tubules as well as the distal nephron. In conclusion, the defect of acid-base transporters in renal tubule was related with the functional defect of urinary acidification in distal RTA.

      • Comparison of Virologic Response and Renal Safety of Long-Term Antiviral Therapy with Tenofovir and Entecavir in Naive Patient with Chronic Hepatitis B

        Aims: Tenofovir (TDF) entecavir (ETV) are considered as the preferred treatment options for treatment-naïve chronic hepatitis B (CHB) patients. We compared the virologic response and renal safety of long-term TDF versus ETV therapy in naïve CHB patients. Methods: This retrospective study included total 432 patients who were treated with TDF (n=205) or ETV (n=227) from August 2012 to March 2017. Virologic response (VR) was defined as undetectable serum HBV DNA by PCR (<20 IU/mL). Results: Mean age of patients, sex, baseline serum levels of AST, ALT, creatinine, and HBV DNA were not significantly different between TDF and ETV groups. The cumulative rates of VR between TDF and ETV groups were 71.6% vs. 61.8% (p=0.477), 88.1% vs. 79.6% (p=0.058), and 84.8% vs. 83.7% (p=0.966) at 1, 2, and 3 years, respectively. The cumulative rates of genotypic resistance between TDF and ETV groups were 0% vs. 0%, 0% vs. 1.5% (p=0.404), and 0% vs. 2.2% (p=0.447) at 1, 2, and 3 years, respectively. Incidences of renal impairment and hypophosphatemia during treatment up to 3 years were not significantly different between two groups. Multivariate analysis showed that HBeAg-positivity (OR 0.27, 95% CI 0.12-0.58, p=0.0012) and serum HBV DNA at 1 year 2000 IU/mL (OR 0.09, 95% CI 0.03-0.25, p<0.0001) were significantly associated with VR. Conclusions: Long-term TDF and ETV treatments appear to have similar virologic response and renal safety in naïve CHB patient. However, long-term ETV therapy might to be associated with genotypic resistance in 2.2% up to 3 years, while none of patients on TDF therapy did.

      • 골반경수술 146예에 대한 임상적 고찰

        Pelviscopic surgery has been reported to have minimal tissue damage and bleeding, and to reduce cost and recovery time for patients undergoing a variety of procedures. The field of gynecologic operations that can be performed by pelviscopic procedures has expanded rapidly. This report is a retrospective review of 146 cases of pelviscopic surgery from January, 1991 to February 1996 to evaluate the efficacy of pelviscopic surgery in gynecologic patients. The results of this review were summarized as follows; 1. The majority of patients was between 21 and 40 years of age(84.2%), mean age was 33.1 years. 2. Preoperative clinical indications were adnexal mass 77 cases(52.7%), ectopic pregnancy 48 cases(33.9%), uterine myoma 9 cases(6.2%), and etc. 3. Types of surgery were salpingectomy 43 cases(29.5%), salpingoophorectomy 41cases(28.1%), LAVH 9 cases(6.2%) and etc. 4. The previous operation history was found in 34 cases(23.3%), and the type of previous operation were Cesarean section 11 cases(32.4%), appendectomy 10 cases(29.4%), and there were 5 cases of previous history of ectopic pregnancy. 5. The mean duration of hospitalization was 5.8 days.

      • 금강유역에서 채집된 Lymnaea pervia, Lymnaeidae (애기물달팽이)외 3종(種)에 대한 계통유연성

        RAPD(Randomly Amplified Polymorphic DNA) method was used to find out the phylogenetic relationships and the genetic polymorphisms on 4 species, C. chinensis, H. cantori, P. acuta and L. pervia collected in Keum-River basins. It was awared that 4 random primers among the used 20 primers were available to characterize the genetic relationships of 4 species freshwater gastropods. The data obtained from the above experiments were analyzed by PAUP Genetic Similarity Index Program. According to these analysis, the genetic similarity distance index was ranged from 0.5 to 0.9; that is it was revealed that C. chinensis was closely related to P. acuta and H. cantori to L. pervia as being treated to the specific primer sequences. From these results, RAPD analysis method could be succesively applied to the classification and genetic analysis for the phylogenetic relationships of the freshwater gastropods.

      • 열처리에 따른 Ti-6Al-4V 합금의 전기화학적 부식거동

        In this paper the effect of aging heat treatment to the hardness and electro-chemical corrosion behavior of the Ti-6Al-4V alloy was studied. Its solution heat treatments were carried out 966'C 1066'C and its aging heat treatments was carried out 5hours on the temperature at 550℃, 600℃ and 650℃ with different time of 1hour, 2hours, 4hours, 8hours and 16hours respectively. The Electrochemical corrosion behavior by potentio dynamic polarization in 1N HNO₃+ 15ppm HF solution was accomplished. The obtained results were as follows; 1. As solution heat temperature increased, the corrosion potential was increased. whereas passive current density and critical current density were decreased. 2. As aging heat temperature increased, the corrosion potential were almost constant but passive current density was decreased. 3. As solution heat temperature increased, the hardness was increased from Hv 405 at 966℃ to Hv 512 at 1066℃, whereas the highest hardness time was reduced from 4hours to 2hours.

      • 열처리에 따른 Nimonic 80A 초내열재료의 전기화학적 부식거동

        In this paper, the Electro-chemical corrosion of the Nimonic 80A superalloy was studied. Its aging heat treatments was carried out at 650?, 700?,750?, 800? and 850? with different time of 20min., 1hour, 2hours, 4hours, and 16hours, additionally 64hours and 128hours at 650?. The obtained results were as follows : 1. In the Electro-chemical corrosion test as a function of aging heat treatment time and temperature, the corrosion potential was reversely proportional to Hardness which indicated the effects of ?/? coherency of base material and precipitate. 2. Initiation point of the pitting was observed at grain boundary, twin boundary and near ?' precipitates. The results of composition analysis by EDS at this point indicated that sulphur originated from 1N H₂SO₄ solution was found in abundance at the grain boundaries, and the pit which arouse in the near precipitates were lack of Al, Ti and Nickel which were the main element of ??, that the depletion of such element was caused breakdown of passive film. 3. As aging heat treatment time increased the passivation current density is increased, that is caused by sensitization of the ?? phase growth. 4. As result of comparison 1N H₂SO₄. 1N HCl electrolyte, both solutions showed similar corrosion potential and Flade potential but much higher rate of critical and passivation current density, which indicated that the early breakdown of passive film by Cl ion, originated from 1N HCl.

        대학생의 라이프스타일 유형에 따른 신용카드 사용과 의복구매

        This study aimed to classify the lifestyles of 1020 college students, and to analyze the effect of the lifestyles on the usages of credit card, price perception, purchasing satisfaction and the number of new clothing. The number of credit cards, total payment of credit cards and the attitude to credit card were different by the 7 clusters of college students, but the frequency of credit card use, the amount of cash service and arrearages were not different. The perception to the apparel price, purchasing satisfaction, and the degree of clothing purchasing varied according to the lifestyle clusters. For example, 6th cluster being highest in the pursuit of appearance showed the highest amount of credit cards usage significantly and tendency of highest arrearage, and used credit cards mostly in clothing purchase.
