강박증 환자에서 신경 심리 검사를 통한 전두엽 기능에 관한 연구

연구목적 : 강박증 환자에서 비언어적 기억의 장애, 실행능력(executive function)의 장애, 시공간능력의 장애등의 신경 인지 기능의 장애 및 뇌 영상 연구에서 전두엽-기저핵 신경로의 기능장애가 있다는 결과들이 보고되어 왔다. 저자들은 전두엽의 기능을 반영하는 것으로 알려진 신경 심리 검사를 통하여 강박증 환자의 전두엽의 기능적 변화의 유무 및 기타 인지기능에 미치는 영향에 대해서 평가하고자 하였다. 방 법 : 서울대학교병원 강박증 클리닉에 내원한 32명의 강박증 환자군과 28명의 정상 대조군을 대상으로 강박증상의 중증도에 대한 평가와 함께 한국판 단축형 지능검사(Short-form of K-WAIS)와 주로 전두엽 기능을 평가하는데 민감한 것으로 알려진 신경 인지 기능 검사군(위스콘신 카드 분류 검사, Rey-Os-terrieth Complex Figure test, Controlled Oral Word Association test, 선로 잇기 검사, Luria-Nebraska 신경심리 바테리 중 언어학습능력검사)을 시행하였다. 이 검사들의 결과를 연령과 교육연수를 통제한 후에 ANCOVA로 분석하였다. 결 과 ; 강방증 환자군은 범주적 언어 유창성 점수(ANCOVA, F=15.07, df=58, p<0.001), Rey-Os-terrieth Complex Figure test의 즉각 회상 점수(ANCOVA, F=6.33, df=57, p<0.015), Rey-Os-terrieth Complex Figure test의 지연 화상 점수(ANCOVA, F=5.33, df=57, p<0.022)가 정상 대조군에 비하여 유의하게 저하되어 있었다. 결 론 : 이 결과는 강박증 환자들이 전두엽의 특징적인 기능으로 알려진 실행기능보다는 비언어적 기억능력의 장애 및 언어능력의 장애를 가지고 있음을 시사한다. Objective : Non-verbal memory deficits, impairments in executive function and deficits in visuospartial functions have been repeatedly reported in subjects with obsessive-compulsive disorder(OCD). The present study aimed to evaluate the frontal lobe function using neuropsychological test in subjects with OCD and normal control. Methods : A battery of neuropsychological test(Wisconsin Card Sorting test, Rey-Osterrieth Complex Figure test, Controlled Oral Word Association test, Trail Making test, Verbal Learning test) reflecting frontal lobe function was administrated to 32 OCD patients and 28 healthy comparison subjects. Result : There were no significant differences in age, years of education, or estimated IQ between the groups. Scores in category fluency, immediate recall and delayed recall of Rey-Osterrieth Complex Figure test in OCD subjects were significantly lower than those of healthy comparison subjects(ANCOVA, F=15.07, df=58, p<0.001 ;ANCOVA, F=6.33, df=57, p<0.015 ; ANCOVA, F=5.33, df=57, p<0.022, respectively). Conclusion : OCD patients had selective deficits in task involving non-verbal memory and categorical word fluency relative to healthy comparisons.

<P><B>Abstract</B></P> <P>A novel series of blended polymer electrolyte membranes (PEMs) from crosslinked sulfonated poly(arylene ether) (cSPAE100) and triazole-containing poly(arylene ether sulfone) (PAES-DTM) were prepared for fuel cell applications. cSPAE100 with a high degree of sulfonation was synthesized via aromatic condensation. PAES-DTM densely functionalized with di-triazole moieties was blended with cSPAE100. The blend ratios of the fabricated PEMs varied from 2 wt% to 10 wt%, and the blend PEMs (cSPAE-DTMs) displayed sufficiently high ion exchange capacities (IEC = 2.07–1.86 mequiv/g). Introducing the triazole moieties, the proton conductivities of cSPAE-DTM2 and cSPAE-DTM5 increased at high relative humidity (95% RH), whereas their IEC values decreased. Furthermore, evaluation of the cell performance at 80 °C revealed that the performance of the cSPAE-DTMs was superior to that of the pristine membrane under 50% RH. <P><B>Purpose</B></P><P>The American College of Surgeons Oncology Group Z0011 trial reported that complete dissection of axillary lymph nodes (ALNs) may not be warranted in women with clinical T1-T2 tumors and one or two involved ALNs who were undergoing lumpectomy plus radiation followed by systemic therapy. The present study was conducted to identify preoperative imaging predictors of ≥ 3 ALNs.</P><P><B>Materials and Methods</B></P><P>The training set consisted of 1,917 patients with clinical T1-T2 and node negative invasive breast cancer. Factors associated with ≥ 3 involved ALNs were evaluated by logistic regression analysis. The validation set consisted of 378 independent patients. The nomogram was applied prospectively to 512 patients who met the Z0011 criteria.</P><P><B>Results</B></P><P>Of the 1,917 patients, 204 (10.6%) had ≥ 3 positive nodes. Multivariate analysis showed that involvement of ≥ 3 nodes was significantly associated with ultrasonographic and chest computed tomography findings of suspicious ALNs (p < 0.001 each). These two imaging criteria, plus patient age, were used to develop a nomogram calculating the probability of involvement of ≥ 3 ALNs. The areas under the receiver operating characteristic curve of the nomogram were 0.852 (95% confidence interval [CI], 0.820 to 0.883) for the training set and 0.896 (95% CI, 0.836 to 0.957) for the validation set. Prospective application of the nomogram showed that 60 of 512 patients (11.7%) had scores above the cut-off. Application of the nomogram reduced operation time and cost, with a very low re-operation rate (1.6%).</P><P><B>Conclusion</B></P><P>Patients likely to have ≥ 3 positive ALNs could be identified by preoperative imaging. The nomogram was helpful in selective intraoperative examination of sentinel lymph nodes.</P>

Three experiments were conducted to determine the effects of non-ionic surfactant (NIS) supplementation on ruminal fermentation, nutrient digestibility and performance of beef steers fed high-roughage diets. The objective of experiment 1 was to investigate the effects of NIS supplementation on in vitro ruminal fermentation of cultures administered with corn and barley as grain substrate and rice straw and timothy hay as roughage substrate. The in vivo ruminal fermentation, nitrogen balance and digestibility of nutrients were also examined with steers fed a high-roughage diet in experiment 2. The aim of experiment 3 was to determine the responses to NIS of growing steers fed a high-roughage diet. In experiment 1, ammonia nitrogen concentration for NIS supplementation was higher (p<0.05) than for the control with all substrates. However, concentrations of total volatile fatty acid (VFA), acetate, butyrate and valerate of the incubated roughage substrates, rice straw and timothy hay, were higher (p<0.05) for NIS supplementation than for the control whereas VFA concentrations in the cultures of corn and barley were unaffected. These results indicated that effects of NIS on ruminal fermentation are diet dependent, specifically on roughage sources. In experiment 2, ruminal pH of steers supplemented with NIS was lower (p<0.05) than the control. Ruminal concentrations of ammonia nitrogen, acetate, total VFA and urinary concentrations of purine derivatives were increased (p<0.05) by NIS supplementation. In experiment 3, supplementation of NIS increased (p<0.05) intakes of total feed and corn silage, average daily gain, and feed efficiency of growing steers although they varied depending on supplementation level. Due to the roughage-specific feature of NIS effects, NIS appears to enhance ruminal fermentation of fibrous parts of feeds and, consequently, performance of steers fed a high-roughage diet.

Recently, sensorless induction motor drives have been much studied due to several advantages. Sensorless drives eliminate the additional mounting space, increase the reliability in harsh environments, and reduce the cost of a motor. This paper investigates an improved sliding mode observer for the sensorless speed control of an induction motor. The proposed control strategy is the sliding mode observer with a variable boundary layer for a low-chattering and fast-response control. The proposed sensorless-algorithm is verified through the simulation and experimentation.

Pericardial effusion (PCE) in neonates has various clinical presentations depending on the amount and speed of fluid accumulation and can cause cardiac tamponade (CT). We report a case of rapidly accumulating PCE and near-fatal CT with an umbilical venous catheter successfully resolved by emergent echo-guided pericardiocentesis in a term infant who had been hospitalized with meconium aspiration syndrome and persistent pulmonary hypertension. This case report suggests that if a patient with an intracardiac umbilical catheter shows sudden cardiopulmonary instability, the possibility of PCE and CT should be considered. Furthermore, if necessary, emergency drainage of the PCE and removal of the umbilical catheter should be immediately performed.
Nomogram for Predicting Breast Conservation after Neoadjuvant Chemotherapy

<P><B>Purpose</B></P><P>The ability to accurately predict the likelihood of achieving breast conservation surgery (BCS) after neoadjuvant chemotherapy (NCT) is important in deciding whether NCT or surgery should be the first-line treatment in patients with operable breast cancers.</P><P><B>Materials and Methods</B></P><P>We reviewed the data of 513 women, who had stage II or III breast cancer and received NCT and surgery from a single institution. The ability of various clinicopathologic factors to predict the achievement of BCS and tumor size reduction to ≤ 3 cm was assessed. Nomograms were built and validated in an independent cohort.</P><P><B>Results</B></P><P>BCS was performed in 50.1% of patients, with 42.2% of tumors reduced to ≤ 3 cm after NCT. A multivariate logistic regression analysis showed that smaller initial tumor size, longer distance between the lesion and the nipple, absence of suspicious calcifications on mammography, and a single tumor were associated with BCS rather than mastectomy (p < 0.05). Negative estrogen receptor, smaller initial tumor size, higher Ki-67 level, and absence of <I>in situ</I> component were associated with residual tumor size ≤ 3 cm (p < 0.05). Two nomograms were developed using these factors. The areas under the receiver operating characteristic curves for nomograms predicting BCS and residual tumor ≤ 3 cm were 0.800 and 0.777, respectively. The calibration plots showed good agreement between the predicted and actual probabilities.</P><P><B>Conclusion</B></P><P>We have established a model with novel factors that predicts BCS and residual tumor size after NCT. This model can help in making treatment decisions for patients who are candidates for NCT.</P>
