Transcatheter arterial chemoembolization (TACE) is suitable treatment method of hepatocellular carcinoma (HCC) for patients with decreased underlying liver function or unresectable disease. But this procedure can induce some embolic events. We report a case of a gallbladder infarction caused by the embolic event in TACE procedure. A 61-year-old man had been going to undergo 6th TACE for marginal recurrence of HCC. After the procedure, gallbladder infarction occurred and was detected by abdominal computed tomography (CT) scan for response evaluation of the treatment at out-patient-clinic. Then, he was treated with oral antibiotics and conservative therapy at out-patient clinic. In 6th week after the event, abdominal CT scan showed improved state of gallbladder.
The effects of pulmonary rehabilitation during chest radiotherapy in patients with malignancy

Background: Radiotherapy become a treatment of choice for lung cancer or esophageal cancer when surgery cannot be an option due to poor lung function. Radiotherapy can affect pulmonary function and induce pneumonitis or pneumonia, which can be fatal in patients with poor lung function. The purpose of this study is to ensure that the reduction of pulmonary function tests (PFT) after radiotherapy can be minimized through pulmonary rehabilitation (PR). Methods: The patients, who initiated pulmonary rehabilitation with radiotherapy for lung cancer from January 2018 to June 2019, were enrolled. The results of the pulmonary function test and 6 minute walk test (6MWT) conducted within six months prior and after radiotherapy were analyzed. Results: A total of 10 patients performed pulmonary rehabilitation at least once a week during chest radiotherapy, and conducted pulmonary function test prior and after radiotherapy. The mean age was 68 and they were all male. Nine patients had non-small cell lung cancer, one had esophageal cancer and seven had COPD. The FEV1 (%) and FEV/FVC (%) were significantly increased by 10.5% (p=0.004) and 6.2% (p=0.038), respectively, compared with baseline (55.6%, 51.6%). The 6MWT results were also significantly increased from 405.6m to 466.9m (p=0.009). Conclusions: Pulmonary rehabilitation during chest radiotherapy improved pulmonary function and exercise capacity in patients with lung or esophageal cancer. This can be an important basis for further studies to identify the effects of pulmonary rehabilitation during chest radiotherapy through large study with long-term follow up period.
Depression and Quality of Life in Patients with Systemic Lupus Erythematosus

Objective. The objective of this study is to examine the prevalence of depression and its related factors including quality of life, brain-derived neurotrophic factor (BDNF), and vitamin D in patients with systemic lupus erythematosus (SLE). Methods. Depression was assessed using the center for epidemiologic studies depression (CES-D) scale. Disease activity, disease-related organ damage, the EuroQol-5 dimensions (EQ-5D), sociodemographic features, and laboratory tests including serum vitamin D level were surveyed. Serum BDNF was measured using an enzyme-linked immunosorbent assay. Results. Depression was observed in 22.8% of 180 SLE patients (n=41). Patients with marital status of single/divorced/separated/widowed, a higher patient global assessment (PGA) score, and extreme pain/discomfort showed significant association with depression. The EQ-5D index showed negative correlation with CES-D score (r=-0.56, p<0.05). In each EQ-5D dimension, depression showed significant association with moderate to severe problems in self-care and usual activities, and extreme pain/discomfort. Serum BDNF levels were not associated with depression (p=0.75) but associated with SLE disease activity index (SLEDAI; r=-0.21, p<0.05). Serum vitamin D levels were not associated with depression (p=0.60) but showed negative correlation with SLEDAI (r=-0.23, p<0.05) and mean glucocorticoid dose over the previous 3 months (r=-0.21, p<0.05) after adjustment for use of vitamin D supplement. Conclusion. Depression was prevalent in patients with SLE and was associated with low quality of life, and a higher PGA but not with SLEDAI. Serum BDNF and vitamin D levels were not associated with depression but showed negative correlation with SLEDAI. (J Rheum Dis 2015;22:346-355)
Improvement of Peptide Expression Vector by Mutagenesis

The goal of the present study is to improve the preS2-expression vector in such a way that purification of the recombinant preS2 peptide of hepatitis B virus surface antigen is facilitated. Previously we have succeeded in expressing the β-galactosidase-preS2 fusion protein in E. coli with a yield of 50% of total cellular proteins. Since there is no internal Met residue in the preS2 sequence, we could use CNBr cleavage in isolating the preS2 peptide out of the fusion protein. In amino acid sequence of the fusion protein encoded by pCTHB20 plasmid, there exist three internal Met residues: two in the β-galactosidase portion and one at the beginning of preS2 sequence. In the present study we altered those two Met codons in the lacZ portion by oligonucleotide-directed mutagenesis. DNA fragment containing the target sequence was cloned into single strand DNA phage M13 mp18 vector and two mutagenic oligonucleotides (17-mer) were used as primers in synthesizing the complementary strand. Mutant clones were selected by hybridization with ^(32)P-labeled mutant oligonucleotides as probes. DNA sequencing confirmed that those two target Met codons (ATG) were changed into Val codons (GTG). The mutagenized sequence of lacZ in the recombinant M13 vector was isolated and replaced for the wild type counterpart of pCTHB20 plasmid. The resulting plasmid was named as pCMHB20. Expression of β-galactosidase-preS2 fusion proteins was carried out and the presence of preS2 sequence in the fusion protein was confirmed by Western blotting method with preS2-specific monoclonal antibody and peroxidase-conjugated anti-mouse IgG antibody.
