        The purpose of this study was to explain the factors that effect on a academic achievement of Pusan National University(PNU) students. In order to achieve the above purpose, two research problems were established as follows: 1. What are the personal-internal factors which effect on a academic achievement of PNU students? 2. What are the personal-exteranl factors which effect on a academic achievement of PNU students? Statistical methods applied were general descriptive statistics, Pearson's correlation method, multiple regression analysis. The statistical computations were done by SPSS. The results of this study are summarized as follows: 1.The personal-internal factor that effect on a academic achievement of PNU students is the learning skill. 2. The personal-external factors that effect on a academic achievement of PNU students are professor's fulfilling prophecy and the times of review of studies.

        The purpose of this study was to explain the factors that effect on ego-identity of Pusan National University(PNU) freshmen. In order to achieve the above purpose 2,743 freshman class of 1988 were selected. Statistical methods applied were general descriptive statistics, Pearson's collelation method, multiple regression analysis. The statistical computations were done by SPSS. The results of this study are summarized as follows: 1. The personal and domestic factors that effect on ego-identity of PNU freshman class of 1988 are mother's attitude of rearing the adaptation confidence of the university life, religion, the participation of the social reality, the experience of the acquaintance with the other sex, the self-conceit and health. 2. The personality factors that effect on ego-identity of PNU freshman class of 1988 are the social introversion and the social responsibility. 3. The factors that effect on ego-identity of PNU freshman class of 1988 are the social introversion, the social responsibility, mother's attitude of rearing religion, the participation of the social reality, a sense of superiority, the experience of the acquaintance with the other sex, the adaptation confidence of the university life.

        The authors analized 78 patients with herpes zoster treated at Department of Dermatology, Chungnam National University Hospital from 1982 to 1985 to find out the epidemiology and clinical course. The results were as follows: 1. The annual prevalence rate of herpes zoster ranged 1.5% to 2.0%(mean 1.8%) and did not increase during 8 years from 1977 to 1984. 2. There was no sexual difference. 3. Herpes zoster was seen most frequently in the 50-59 year age group (24.4%), in July in a year (16.7%) and during summer(34.6%). 4. The dermatomic involvement of herpes zoster was most common on thoracic nerve(48.7%), and others were trigerminal nerve (28.2%), cervical nerve (14.1%), lumbar nerve(6.4%) and sacral nerve(2.6%) in order. 5. The interval of onset of subjective symptoms and vesicle appearance was 3.9 days in average. The mean duration of treatment was 13.8 days. There was a tendency that both interval and duration increased with increasing age. 6. The most common complication of herpes zoster was postherpetic neuralgia (6.4%) and others were keratoconjunctivitis(2.5%), herpes zoster with aberrant vesicles (2.5%) and recurrent herpes zoster (1.3%).

        The prevalence of prostate cancer is stiffly increasing in Korea. In contrasts to the frequency of occurrence, there is no definite guideline for the cut off value of serum PSA. So, we reviewed the usefulness of serum PSA and recently reported guidelines in the prostate cancer detection. Nowadays, the cut off value of PSA continued to change, moreover, it is difficult to satisfied both sensitivity and specificity, at the same time. Also, the value of PSA relies on the assay method or laboratory. Various methods such as the percent of free PSA, PSA velocity and PSA doubling time have been used to enhance the detection rate during prostate biopsy. However, these methods did not make certainty in deciding whether performing the prostate biopsy. To complement the uncertainty of PSA and its derivatives, the new ways such as proPSA, PCA3 and gene fusion have been recently documented. Currently, the clearly indication of prostate biopsy is placed in controversy. Therefore, in the future, new methods to overcome non-specificity of PSA should be considered.

        Predicting biochemical recurrence in patients with high-risk prostate cancer using the apparent diffusion coefficient of magnetic resonance imaging

        <P><B>Purpose</B></P><P>We aimed to investigate whether the apparent diffusion coefficient (ADC) value in diffusion-weighted magnetic resonance imaging predicts the prognoses of patients with high-risk prostate cancer.</P><P><B>Materials and Methods</B></P><P>A total of 157 patients with high-risk prostate cancer (based on D'Amico's criteria) were included in the analysis. Patients underwent preoperative 3.0 Tesla magnetic resonance imaging within 2 months before radical prostatectomy. Those who received neoadjuvant hormone therapy (33 persons) or radiation therapy (18 persons) were excluded. The ADC of the tumor calculated from 2 b-values (0 and 1,000 s/mm<SUP>2</SUP>) was measured. Areas under receiver operating characteristics curves were calculated to maximize the accuracy of the ADC value. Based on the obtained cutoff value, the patients were stratified into 2 groups: Group A consisted of patients with ADC values <746×10<SUP>−6</SUP> mm<SUP>2</SUP>/s and group B comprised those with ADC values ≥746×10<SUP>−6</SUP> mm<SUP>2</SUP>/s.</P><P><B>Results</B></P><P>Group A showed higher rate of lymph positive and biochemical recurrence (BCR) rates than group B. Kaplan-Meier analysis showed that the BCR-free survival rate of group A was much lower than that of group B (p<0.001). On Cox proportional regression analyses, ADC group A (hazard ratio [HR], 3.238, p=0.002) and pathologic lymph node positive (HR, 2.242; p=0.009) were independent predictors of BCR.</P><P><B>Conclusions</B></P><P>In patients with high-risk prostate cancer, ADC value is significantly associated with BCR-free survival. Therefore, the ADC value is a useful tool for predicting the prognoses of these high-risk patients.</P>

      • Hereditary nonpolyposis colorectal cancer in endometrial cancer patients

        <P>Endometrial cancer is the second most common cancer in hereditary nonpolyposis colorectal cancer (HNPCC). It has often been overlooked to explore the possibility of HNPCC in endometrial cancer patients. Our study was to investigate how many HNPCC patients existed among endometrial cancer patients. Among patients who underwent hysterectomy for endometrial cancer at Seoul National University Hospital from 1996 to 2004, 113 patients were included, whose family history and clinical data could be obtained and tumor specimens were available for microsatellite instability (MSI) testing and immunohistochemical (IHC) staining of MLH1, MSH2 and MSH6 proteins. There were 4 (3.5%) clinical HNPCC patients fulfilling the Amsterdam criteria II, and 2 (2/4, 50%) of them carried MSH2 germline mutations. There were also 8 (7.1%) suspected HNPCC (s-HNPCC) patients fulfilling the revised criteria for s-HNPCC, and one (1/8, 12.5%) of them revealed MLH1 germline mutation. In 101 patients, who were not clinical HNPCC or s-HNPCC, 11 patients showed both MSI-high and loss of expression of MLH1, MSH2 or MSH6 proteins, and 2 (2/11, 18.2%) of them showed MSH6 germline mutations. In 113 patients with endometrial cancer, we could find 5 (4.4%) HNPCC patients with MMR germline mutation and 2 (1.8%) clinical HNPCC patients without identified MMR gene mutation. Family history was critical in detecting 3 HNPCC patients with MMR germline mutation, and MSI testing with IHC staining for MLH1, MSH2 and MSH6 proteins was needed in the diagnosis of 2 HNPCC patients who were not clinical HNPCC or s-HNPCC, especially for MSH6 germline mutation. © 2007 Wiley-Liss, Inc.</P>

        Resonance spectrum of a three-dimensional photonic quantum ring laser with an equilateral triangle microcavity

        <P>We have fabricated three-dimensional (3D) photonic quantum ring lasers with an equilateral triangle microcavity. Their spectra were well explained by combining the off-normal resonance and hexagonally bounced in-plane whispering-gallery-mode condition. The angular distribution of the emission modes and their discrete wavelengths were shown to be in excellent agreement with a 3D Rayleigh Fabry-Perot model. We confirmed that the allowed modes in the equilateral triangle microcavity decrease by decreasing the length of equilateral triangle side, L, and the spectral mode spacing linearly increases with the mode index m and is inversely proportional to L2.</P>

        단백상실성 위장증으로 발현된 속발성 위장관 유전분증 1예

        저자들은 15년간 류마티스 관절명으로 치료받던 환자에서 단백상실성 위장증을 유발한 속발성 위장관 유전분증 1예를 경험하였기에 문헌고찰과 함께 보가하는 바이다. We report a case of protein-losing gastroenteropathy due to secondary gastrointestinal amyloidosis in a patient with rheumatoid arthritis(RA). A 66-year-old woman with RA for 15 years was admitted to our hospital with complaints of abdominal pain and diarrhea. She had taken bilateral total knee replacement 4 years ago and right shoulder synovectomy 1 year ago. She han been treated with nonsteroidal anti-inflammatory drugs, gold derivatives, or methotrexate. On laboratory examintion, serum total protein was 3.9g/dl and albumin was 1.6g/dl. Stool occult blood was detected but no evidence of infection was present. No urinary peotein including Bence-Jones protein, was detected. Serum and urinary protein electrophoresis were within normal limits. Alpha-l-antitrypsin clearance rate was increased upto 60ml/24hr. Small bowel series revealed thickened mucisal folds and multiple nodular filling defects on the entire small bowel. Histopathologically amyloid deposied, which were proved on immunofluorescence and immunohistochemistry, were present on the biopsied tissuse from gastric antrum, terminal ileum and rectum. Accordingly we established the diagnosis of proteinlosing gastroenteropathy due to secondary amyloidosis, which is the first case so far descripted in Korea to the best of our knowledge.

        Common dental anomalies in Korean orthodontic patients: An update

        Objective: The aim of this study was 1) to investigate the prevalence and pattern of dental anomalies (DAs), 2) to compare DAs according to the type of malocclusion, and 3) to investigate the correlation between tooth impaction and other DAs in the Korean orthodontic population. Methods: A total of 3,240 orthodontic patients were classified as Class I, Class II, or Class III malocclusion groups. The presence and location of common DAs, including impaction, microdontia, agenesis, supernumerary tooth, transposition, and fusion, were identified by examining diagnostic records. Furthermore, samples were classified as Group 1 without impaction or Group 2 with impaction. The prevalence of other DAs concurrent with impaction was investigated and compared to Group 1. Results: Impaction was the most prevalent DA, followed by microdontia, agenesis, and supernumerary. Class I and Class III groups showed the same order of prevalence, but agenesis was more frequent than microdontia in the Class II group. The prevalence of the four DAs was lowest in the Class III group. Overall, 8.6% of patients were classified into Group 2. The incidence of DAs other than impaction and the prevalence of multiple concurrent DAs were significantly higher in Group 2. Impaction showed a significant relationship with supernumerary tooth, transposition, and fusion. Conclusions: The prevalence and pattern of DAs varied depending on the type of malocclusion. As there was a higher risk of other DAs in patients with impacted teeth, early detection of the impacted tooth and a detailed diagnosis of other possible DAs may be essential.
