        The value of prophylactic cranial irradiation in limited-stage small cell lung cancer: should it always be recommended?

        Purpose: Prophylactic cranial irradiation (PCI) is a standard treatment for limited-stage small cell lung cancer (LS-SCLC) showing a response to initial treatment, but many patients do not receive PCI due to comorbidities or refusal. This study aims to define the patient group for whom PCI can be omitted with minimal risk. Materials and Methods: Patients with LS-SCLC who underwent radiotherapy with curative aim at our institution between January 2004 and December 2015 were retrospectively reviewed. Patients who did not receive PCI were evaluated for brain metastasis-free survival (BMFS), progression-free survival (PFS), overall survival (OS), and prognostic factors for survival, and treatment outcomes were compared with a patient cohort who received PCI. Results: A total of 350 patients achieved a response following thoracic radiotherapy, and 190 of these patients did not receive PCI. Stage I–II and a complete response (CR) to initial therapy were good prognostic factors for BMFS and OS on univariate analysis. Patients with both stage I–II and a CR who declined PCI showed comparable 2-year BMFS to those who received PCI (92% vs. 89%). In patients who achieved CR, PCI did not significantly improve OS or PFS. Conclusion: There should be less concern about omitting PCI in patients with comorbidities if they have stage I–II or a CR, with brain metastasis control being comparable to those patients who receive PCI.

        Involvement of cAMP in the Human Serum-Induced Migration of Adipose-Derived Stem Cells

        Previously we observed that human adipose-derived stem cells (hADSCs) could form aggregation during culture in the presence of human serum (HS). In the present study, we have examined if the aggregation might result from the cell migration and analyzed the difference of cell adhesivity after culture in various conditions. When cells were cultured in fetal bovine serum (FBS) alone, there was no morphological change. Similarly, cells pretreated with FBS for 1 day or cultured in a mixture of FBS and HS showed little change. In contrast, cells cultured in HS alone exhibited formation of cell-free area (spacing) and/or cell aggregation. When cells cultured in FBS or pretreated with FBS were treated with 0.06% trypsin, almost cells remained attached to the dish surfaces. In contrast, when cells cultured in HS alone were examined, most cells detached from the dish by the same treatment. Treatment of cells with forskolin, isobutylmethyl xanthine (IBMX) or LY294002 inhibited the formation of spacing whereas H89 or Y27632 showed little effect. When these cells were treated with 0.06% trypsin after culture, most cells detached from the dishes as cells cultured in HS alone did. However, cells treated with IBMX exhibited weaker adhesivity than HS alone. Based on these observations, it is suggested that HS treatment might decrease the adhesivity and induce three-dimensional migration of hADSCs, in the latter of which cAMP signaling could be involved.

        광역자치단체 온실가스 배출량과 경제성장의 탈동조화 분석

        탄소중립을 위한 지역의 역할이 강조되고 있다. 국가 온실가스 감축목표를 효과적으로 달성하기 위해서는 지역의 온실가스 배출 특성과 감축 여건을 고려한 차별화된 전략과 접근이 필요하다. 본 연구는 광역지자체의 탈동조화 현상을 분석하고 시사점을 제시하는 데 목적이 있다. 온실가스 배출량은 경제활동과 밀접하게 관련되어 있어서 온실가스 배출과 경제성장의 관계를 나타내는 탈동조화(decoupling)는 기후변화 논의에서 중요한 주제이다. 이를 위해 2001~2018년 동안 연도별 단기 탈동조화 지수를 산정하여 추세를 파악한 다음 회귀분석을 통해 10년 주기로 지역별 탈동조화 정도를 비교 분석하였다. 분석 결과 16개 시도의 탈동조화 지수는 시간의 변화에 따라 전반적으로 개선되는 추세를 보였으나 지역별 탈동조화 정도는 차이를 나타내었다. 기간별로는 제주와 충남을 제외한 모든 지역에서 2001~2010년에 비해 2010년 이후의 기간에서 탈동조화 현상이 강화되었으며, 울산, 경남, 대구, 강원 등 4개 지역에서는 통계적으로 유의한 수준의 절대적 탈동조화 현상이 나타났다. 로그평균 디비지아 지수(LMDI) 분석 결과 에너지 집약도 개선 효과가 탈동조화를 추동하는 주요 요인으로 나타나 선행연구와 일치된 결과를 보였다. 이러한 지역별 감축 경로와 역량의 차이를 고려하여 탄소중립 이행을 위한 지역의 권한과 자율성을 부여하는 유연한 접근과 함께 탄소중립 목표 이행의 책임성을 강화할 수 있는 장기적인 정책목표와 수단이 필요하다. The role of local governments for carbon net-zero is being emphasized. To effectively achieve the national greenhouse gas reduction target, a differentiated strategy and approach that reflects the local greenhouse gas emission characteristics and reduction conditions are required. This study aims to analyze the decoupling degree of regional local governments and draw implications for carbon net-zero. Since greenhouse gas emissions are closely related to economic activities, decoupling, which indicates the relationship between the two, is critical in climate change discussions. To this end, we calculated the short-term decoupling index for each year from 2001 to 2018 to figure out the trend and then compared and analyzed the degree of decoupling by region through regression analysis on a 10-year cycle. As a result of the analysis, the decoupling indices of 16 metropolitan cities and provinces showed an overall improvement trend with the change of time, but there were differences in the degree of decoupling by region. By period, decoupling was strengthened in the period after 2010 compared to 2001 to 2010 in all regions except Jeju and Chungnam. In particular, a statistically significant level of absolute decoupling was observed in four regions: Ulsan, Gyeongnam, Daegu, and Gangwon. According to the logarithmic mean Divisia index (LMDI) analysis, the energy intensity improvement effect was found to be a major factor driving decoupling, showing results consistent with previous studies. Considering these differences in regional mitigation pathways and capacities, a flexible approach that grants local authority and autonomy for carbon net-zero is needed, along with long-term policy goals and means to strengthen accountability of regional local governments.

        제주관광공사의 실시간 관광지 혼잡도 분석 서비스 사례

        The spread of COVID-19 has been changed the tourism industry. Travelers changed their traveling style and started to consider congestion of the spot for their health and safety. In Jeju, a famous tourist destination in South Korea, managing the congestion of tourists has become an important issue. This example introduces the Jeju Tourism Organization’s development of a system as a smart tourism information service that manages congestion in real-time big data. Combining with congestion theory and behavior immune system, we would like to assure the necessity of the system. Also, by analyzing the system, we understand how deducing congestion information from big data and the new paradigm of the tourism industry combined with congestion theory. Data was collected by Korea’s telecommunication company SKT to develop the system. The paper explains the reason for choosing the company and the pros of data quality. We expect this system to be a solution for any other city in the world under a similar situation. Finally, several suggestions for the system are included to promote and better future usage.

        Development of portable image-guidance system (PIGS) for preclinical small animal experiments with the cone-beam irradiation of X-RAD

        The purpose of this study is to develop a portable image-guidance system (PIGS) for preclinical experiments to identify anatomical structures of small animals and deliver beams accurately to their target locations more precisely. The PIGS consists of a gadolinium oxide (Gd2O3) scintillator, a transparent acrylic, a thin mirror, opaque polymethyl methacrylate (PMMA), and a charge-coupled device (CCD) camera. In addition, the PIGS can reduce dose-measurement efects using tissue-equivalent as the principal. Using 320 X-RAD devices, the X-ray beam was irradiated with 130~35 kVp /12.5~45.0 mA of tube parameters. With 3D-printed grids and homemade mouse phantom, we evaluated the accuracy between the actual position and the location in the imaging coordinate. The image quality of the actual mouse's anatomical images was also examined by setting a 5.0-mm region of interest (ROI) on images, such as the actual mouse Lt. brain, Lt. lung and Lt. femur for each energy. Furthermore, the EBT3 flm was used to compare the solid water phantom and PIGS with the dose measurements at a depth of 5.0 mm to confrm any diferences. Overall, PIGS showed excellent results in evaluating the accuracy and image quality of image coordinates of small animals. The accuracy of the image coordinates showed error values below the moving average±0.2 mm. Furthermore, the mouse imaging results showed the best anatomical image results with high CNR values and low SNR values at 35 kVp/45 mA conditions. COV was the best at 0.0028 in non-fltered 35 kVp/40 mA conditions. Overall, NPS showed similar patterns without signifcant diferences in all energy conditions but had less area noise with lower spatial frequency under lower energy conditions. In addition, the dose evaluation of PIGS and solid water phantom using EBT3 showed a Dmax error value of within 0.2% at a depth of 5 mm measured. This study showed that PIGS could accurately identify the main anatomical structures of mice to deliver radiation beams to the targets. In addition, we proved that the application of PIGS in 320 X-RAD causes no dosimetric infuence.
