7β-(5-Phenyl-2H-tetrazolyl-2-acetamido) cephalosporanic acid was synthesized by condensation of 7-aminocephalosporanic acid(7-ACA) with(5-phenyl-2H-tetrazol-2-yl) acetyl chloride. This compound was tested for antimicrobial activity in vitro against Bacillus licheniformis ATCC 14580, Bacillus subtilis ATCC 6633, Candida albicans ATCC 10231, Escherichia coli ESS, Micrococcus luteus ATCC 9341, Pseudomonas aeruginosa IFO 13130, Staphylococcus aureus ATCC25923, Staphylococcus aureuss FDA-209p and Salmonella typhimurium SL 1102. This compound revealed excellent antibacterial activity against Bacillus subtilis ATCC 6633, Micrococcus luteus ATCC 9341, Staphylococcus aureus ATCC 25923 and Staphlococcus aureus FDA 209p. This compound have no antimicrobial activity against Candida albicans ATCC 10231, Pseudomonas aeruginosa IFO 13130 and Salmonella typhimurium SL 1102.
Surgical Treatment of Lumbar Spinal Stenosis in Geriatric Population : Is It Risky?

Objective : Lumbar spinal stenosis is increasingly recognized as a common cause of low back pain in elderly patients. Conservative treatment has been initially applied to elderly patients, however, surgical treatment is sometimes indispensable to relieve severe pain. We retrospectively examine the age-related effects on the surgical risk, and results following general anesthesia and operative procedure in geriatric patients for two different age groups of at least 65years old. Methods : Consecutive 51 patients [${\ge}$ 65years], who underwent open surgical procedure for degenerative lumbar spinal stenosis, were selected in the study. Patients were divided into two groups. Group A included all patients who were between 65 and 69years of age at the time of surgery. Group B included all patients who were at least 70years of age at the time of surgery. We reviewed medical history including preoperative American Society of Anesthesiologists[ASA] classification of physical status, anesthetic risk factor, operative time, estimated blood loss, transfusion requirements, hospital stay, operated level, and clinical outcome to look for comparisons between two age groups [$65{\sim}69$ and over 70years]. Results : In preoperative evaluation, mean anesthetic risk factor of patients was numerically similar between the groups. The American Society of Anesthesiologists classification of physical status was similar between two groups. There was no difference in operated level, operative time, estimated blood loss, hospital stay, and anesthetic risk factor between the two groups. The clinical successful outcome showed 82.7% for Group A and 81.8% for group B. The overall postoperative complication rates were similar for both group A and B. Conclusion : We conclude that advanced age per se, did not increase the associated morbidity and mortality in surgical decompression for spinal stenosis.

Background: Interferon-${\gamma}$ assays based on tuberculosis (TB)-specific antigens have been utilized for diagnosing and ruling out latent TB and active TB, but their utility is still limited for TB incidence countries. The aim of this study is to understand the clinical utility of enzyme-linked immunospot (ELISpot) assays among patients with clinically suspected TB and healthy adults in clinical practices and community-based settings. Methods: The ELISpot assays (T SPOT.TB, Oxford Immunotec, UK) were prospectively performed in 202 patients. After excluding those with indeterminate results, 196 were included for analysis: 41 were TB patients, 93 were non-TB patients, and 62 were healthy adults. Results: The sensitivity and negative predictive values of the T SPOT.TB assays for the diagnosis of TB were 87.8% and 89.1%, respectively, among patients with suspected TB. The agreement between the tuberculin skin test (10-mm cutoff) and the T SPOT.TB assay was 66.1% (kappa=0.335) in all participants and 80.0% (kappa=0.412) in TB patients. Among those without TB (n=155), a past history of TB and fibrotic TB scar on chest X-rays were significant factors that yielded positive T SPOT.TB results. There was a significant difference in the magnitude of T SPOT.TB spot counts between TB patients and non-TB patients or healthy adults. Conclusion: The T SPOT.TB assay appeared to be a useful test for the diagnostic exclusion of TB. A positive result, however, should be cautiously interpreted for potential positives among those without active TB in intermediate TB incidence areas.

Esophageal diverticulum is a disease that occurs mainly in adults. According to its location, it is named as pharyngoesophageal, paratracheal and epiphrenic diverticulum. Epiphrenic diverticulum is especially accompanied with esophageal leiomyoma sometimes. In this case. esophageal leiomyoma is combined with suhphrenic diverticulum. The authors present the case with review of literature. The diagnosis of subphrenic diverticulum was confirmed by esophagogastroscopy with biopsy, esophagogram and abdominal CT. Postoperative fin- dings showed it to be accompanied with esophageal leiomyoma.

Fabry disease is a X-linked lysosomal storage disorder caused by deficiency of α-galactosidase A. This abnormality in enzyme results intracellular accumulation of globotriaosylceramide and leads to severe painful neuropathy with progressive renal cardiovascular, and cerebrovascular dysfunction and early death. We report a 35 year-old man who had been suffered from acroparesthesia aggravated by body temperature elevation and with asymptomatic renal function impairment, which were proven to be due to Fabry disease. We performed gene analysis by PCR direct sequencing and confirmed missense mutation of GLA gene. Recently enzyme replacement of α-galactosidase was introduced and we think that the importance of early diagnosis and treatment should be emphasized.
Epidemiology and Clinical Features of Common Community Human Coronavirus Disease Before COVID-19

Backgrounds We would improve the understanding and management preparation of human coronavirus (HCoV) disease by analyzing the epidemiology, clinical aspects, and prognostic factors of patients of all ages who were recently hospitalized with coronavirus. Methods This study was retrospectively performed at five university teaching hospitals between 1 January 2018 and 31 March 2020. Routine molecular testing using for multiplex real-time reverse transcription-polymerase chain reaction Methods was conducted on the respiratory viruses. We assessed the demographics, laboratory findings, and treatment of patients infected with coronavirus. Results There were 807 coronavirus-infected patients admitted to five hospitals for 27 months and 553 (69%) of them were <10 years age. All-cause mortality rates of patients admitted for seasonal HCoV disease were 3.1% in all patients, and 10.8% in patients aged ≥ 18 years. The Cox proportional hazard regression analysis was performed in patients aged ≥ 18 years. After adjusting for other clinical variables, general weakness symptoms (hazard ratio [HR], 2.65; 95% confidence interval [CI], 1.14-6.13, P = 0.023) , National Early Waring Score (NEWS) ≥2 (HR, 6.00; 95% CI, 1.40-25.81, P = 0.016), and coronavirus subtype OC43 (HR, 2.82; 95% CI, 1.21-6.58, P = 0.016) were significantly associated with death from coronavirus. Conclusions Coronavirus infection can reveal a higher mortality rate in patients of ≥18 years than those of <18 years, thus, adult patients require more careful treatment. Furthermore, in adult patients, the factors associated with death from coronavirus include general weakness symptoms, NEWS higher than 2, and OC43 subtype.

Idiopathic pulmonary fibrosis (IPF) comprises an aggregate of mesenchymal cells. However, the cellular origin of these mesenchymal phenotypes remains unclear. Transforming growth factor 1 (TGF- 1) has been known as the main cytokine involved in the pathogenesis of IPF. We examined whether the potent fibrogenic cytokine TGF- 1 could induce the epithelial-to-mesenchymal transition (EMT) in the human alveolar epithelial cell line, A549, and determined whether snail expression is associated with the phenotypic changes observed in the A549 cells. EMT was investigated with cells morphology changes under phase-contrast microscopy, western blotting, and indirect immunofluorescence stains. E-cadherin and transcription factor, snail, were also evaluated by measuring mRNA levels using reverse transcriptase-polymerase chain rection (RT-PCR) analysis. The data showed that TGF- 1 induced A549 cells with epithelial cell characteristics to undergo EMT in a concentration-dependent manner. Following TGF- 1 treatment, A549 cells induced EMT characterized by cells morphological changes, loss of epithelial markers Ecaherin and cytokeratin, increased stress fiber reorganization by F-actin, and cytokeratin replacement by vimentin. Although IL-1 failed to induce A549 cells to undergo EMT, the combination of TGF- 1 and IL-1 showed synergy effects in cells morphology changes and the expression of mesenchymal markers. The snail expression study using RT-PCR analysis provided that loss of E-cadherin expression was associated with snail expression. Stimulation of A54 cells with TGF- 1 plus IL-1 revealed a higher level of snail expression. Our data showed that EMT of A549 cells might be closely associated with snail expression.

Background: Interplay of numerous constitutional and precipitating factors are involved in the development of atopic dermatitis (AD). Most of these etiologic factors are not controllable, but barrier dysfunction can be managed. Correction of barrier abnormalities has been accepted as a first-line therapy with effective anti-inflammatory therapy. Objective: We wished to establish standardized skin care guidelines for Korean patients who suffer from atopic dermatitis. Methods: The skin care guidelines were proposed by the Korean Atopic Dermatitis Association (KADA), with a particular emphasis for barrier dysfunction in AD from a review of more than 100 published studies and related documents in the clinical literature by a task force team (TFT) of the KADA. The TFT also evaluated the reliability of the studies based on scientific evidence and the size of the study populations. The TFT disregarded controversial findings and summarized all of the collected studies. Results: Skin care guidelines were proposed by the KADA, particularly for barrier dysfunction in AD. A warm bath for approximately 20 minutes was recommended once daily. Soap can be used two or three times per week and aggressive skin scrubbing should be avoided. A topical moisturizer should be applied at least twice a day. Immediate application of emollient is required after bathing. The donning of clothing made of cotton is recommended. Nails should be cut short to reduce skin damage caused by a skin scratch. Conclusion: Application of moisturizer immediately after appropriate bathing may be the most important treatment for skin care. A topical steroid should be used whenever AD develops. Nail care and proper clothing are also be helpful for the prevention of dermatitis aggravation. Proper ways to bathe, use of moisturizer and topical steroids and nail care and appropriate clothing were considered. (Korean J Dermatol 2009;47(5):531~538)
