      • 급성 백혈병 환자에서 발생한 감염 양상의 분석(1986~1992)

        Infection is a major complication and potentially life-threatening in patients with acute leukemia. The patients require prompt broad-spectrum antibiotics therapy when fever develops. One of the keys to successful patient management is the recognition of institutional trends of the spectrum of infections and infecting microorganisms. So we evaluated 138 patients with acute leukemia and blastic crisis of chronic myelogenous leukemia admitted to Seoul National University Hospital from July 1986 to June 1992. The results are following: 1) Total number of febrile episodes was 224 and 60% of them occurred after chemotherapy. 2) Microbiologically-defined infection, clinically-defined infection, and unexplained fever accounted for 28%, 45%, 27% of the febrile episodes respectively. 3) Fifty-eight percent of microbiologically-defined infections were caused by gram-negative bactria, and 36% were gram-positive bacteria. There was a tendency to a greater proportion of gram (+) organisms than that of the last study (p>0.1). Escherichia coli was the most common organism, and coagulase-negative staphylococci, Pseudomonas aeruginosa, Klebsiella pneumoniae were common in decreasing order. 4) Pneumonia was the most common type of infection, followed by skin and soft tissue infection, perianal infection, gingivitis and primary septicemia. 5) Seventy-two percent of total infections improved with therapy and this success rate was higher than that of the last study(p<0.001).

      • 관해유도 항암요법을 받는 백혈병 환자에서 진균 감염증의 예방 : 무작위 배정법과 이중 눈가림법에 의한 Fluconazole과 Nystain의 효과에 관한 다기관 공동연구 A Randomized, Double-blind, Multicenter Trial to Compare Fluconazole with Nystatin

        Fluconazole의 진균 감염증 예방 효과와 안전성에 관하여 3개 대학병원에서 관해유도화학요법을 받는 급성 백혈병환자를 대상으로 무작위 배정법과 너도나도 누가림법에 의하여 연구하였다. 모두 62명의 환자에게 fluconazole(100㎎ bid) 또는 nystatin(1,000,000IU/day)을 무작위로 투여하였다. 투약은 관해유도화학요법과 같은 날짜에 시작하여 호중구수가 1,000㎣이상으로 회복되거나 진균 감염증이 확인되거나 의심되어 Amphotericin-B를 시작하거나, 약과 관련된 부작용이 나타날 때까지 계속하였다. 진균 colonization은 fluconazole군에서 감소하였으나 nystqatin군에서는 증가하였다, 표재성 진균감염증으로 nystatin군에서 C. albicans 진균혈증 1례와 C.parasilosis 진균혈중 1례가 발생하였다. 경험적 항진균요법으로 Amphotericin-B를 투여한 경우는 fluconazolerns 34명중 7례(21%), nystatinrns 28명중 10례(36%)였다(p<0.05). Fluconazole군과 nystatin군 사이에 부작용이나 사망률에 차이는 없었다. 결론적으로, fluconazole은 관해유도화학요법을 받는 급성 백혈병환자에서 진균의 colonization을 줄이는데 효과적이고 안전한 항진균제이다. We made a randomized, double-blind, multicenter trial to compare the efficacy and safety of fluconazole with nystatin for prevention of fungal infections in patients with acute leukemia. Sixty-two adult undergoing remission induction chemotherapy for cute leukemia were enrolled. Patients were randomly assigned to receive either fluconazole (100㎎ bid) or nystatin(1,000,000IU×6/day) with corresponding placebo. The study drug was started in initiation of chemotherapy and continued until recovery of neutrophil counts(>1,000/㎣), development of proven or suspected invasive fungal infection, or the occurrence of drug-related toxicity. Fungal colonization decreased in fluconazole(F) group, however increased in nystain(n) group. Superficial fungal infection occurred in 1 of 34 F group, whereas invasive fungal infection developed in 3 of 28 N group. Empirical amphotericin-B therapy was given in 7 of 34(21%) F group and 10 of 28(36%) N group(p>0.05). The incidence of drug-related side effects and overall moratlity were similar in both study groups.

      • 지역사회 획득 폐렴의 원인으로서 Chlamydia pneumoniae의 중요성에 관한 연구

        연구 배경 : Chlamydia pneumoniae(TWAR)는 급성 호흡기 감염증을 일으키는 병원체로, 혈청역학적 연구 결과에 의하면 세계적으로 성인의 20-50%가 C. pneumoniae 대한 IgG 항체를 가지고 있다. 우리나라의 경우, 건강한 성인의 약 50%가 항체 양성임을 고려하면 C. pneumoniae는 우리나라에서도 호흡기 감염증의 흔한 원인중 하나일 것으로 생각된다. 대상 및 방법 : 건강인 108명과 1993년 2월부터 5월 말까지 신병 훈련소에 입대한 군인 중 폐렴으로 인해 후송병원에 입원한 환자를 대상으로 하였다. C. pneumoniae에 대한 IgG 및 IgM 항체는 microimmunofluorescent 법을 이용하여 측정하였다. 항체검사에 사용한 elementary body 항원은 Washington research Foundation으로부터 분양을 받았다. 현증 감염은 항체 역가가 4배 이상 상승하거나, IgG항체 역가 ≥512, 또는 IgM항체 역가>16으로 정의하였다. 성적 : 건강인 108명 중 IgM 항체 양성자는 한 명도 없었으나, IgG >16인 사람은 67명 (62%) 이었다. 폐렴에 걸린 신병 79명의 증상은 기침, 객담이 100%, 발열 87%,수포음 78%, 흉부 X-선상 침윤이 87%였고, 증상 발현 후 입원까지의 기간은 1주일이내가 27%, 8-13일 27%, 14-20일 27%, 21일 이상 20% 였다. 이들 폐렴 환자들 중 C. pneumoniae에 대한 lgG 항체 역가가 4배 이상 상승한 환자는 11명, lgG 항체 역가 ≥512인 환자가 4명 (3명은 항체 역가가 4배 이상 상승)으로, 전체 대상환자 중 15%(12/79)가 급성 간염증의 소견을 보였다. 결론 : C. pneumoniae 감염은 비교적 흔히 발생하며, 지역 사회 획득 폐렴의 중요한 원인 중 하나로 생각된다. Background : Chlamydia pneumoniae strain TWAR cause acute infections of the respiratory tract. Previous study showed that prevalence of TWAR antibody in adults from different areas of the world ranged from 25% to 50%. In Korea, positive rate of poly- IgG antibodies against TWAR was reported to be about 50% among healthy adults. These suggest that infections caused by TWAR may be quite prevalent in Korea. We tried to determine whether TWAR is an important pathogen of community-acquired pneumonia. Methods : We studied 108 healthy persons and 79 military recruits with pncumonia who were evacuated from the training camp to the military hospital from February to April 1993. IgG and IgM antibodies to TWAR were measured using the microimmunofluorescence method. Elementary body antigen from TWAR was provided by the Washington Research Foundation. Serologic evidence of current infection was defined as a four-fold rise in antibody titer. In addition, a titer of 16 or greater in the IgM fraction and of 512 or greater in the IgG fraction were considered presumptive evidence of current or recent infection. Results : Of the 108 healthy subjects, none had IgM TWAR antibody, while 67 (62%) had IgG antibody at a titer of 16 or greater. Of the 79 military recruits with pneumonia, cough and sputum developed in 100%, fever in 87%, and crackles in 78% Chest X-ray infiltration was noticed in 87%. Duration of symptoms before admission was less than 7days in 27%, 8-13 days in 27%, 14-20 days in 27%, 21 days or longer in 20%. Of the military recruits with pneumonia, 15% (12/79) showed evidence of acute TWAR infection: 11 showed a four-fold antibody titer rise, and 1 had IgG antibody titer greater than 512. Conclusion : These results suggest that TWAR infections are quite prevalent and TWAR may be one of the important pathogens of community-acquired pneumonia in Korea.

        임상 ; 조기분만 시 융모양막염 유무에 따른 제대정맥혈장내 Interleukin-6, C-reactive Protein 및 지질과산화물의 변화

        목적: 조기분만 시 제대정맥혈장내 Interleukin-6(IL-6)와 C-reactive protein (CRP) 및 지질과산화물을 측정하여 조직학적 융모양막염 유무에 따른 이들의 변화와 주산기 예후에 어떤 역할을 하는가 규명하고자 하였다. 방법: 양막파열이 없는 조기분만 임부 39명(조기분만군), 만삭전 조기양막파열 임부 27명(양막파열군)에서 분만시 태반조직과 제대정맥혈을 채취하여 IL-6, CRP, 지질과산화물 농도를 측정하고 비교하였고, 태반조직의 병리학적 소견을 얻었다. 결과: 조기분만군 39예 중 8예, 양막파열군 27예 중 16예에서 조직학적 융모양막염이 있었으며, 양막파열군에서 조기분만군보다 증가하였다. 조기분만군과 양막파열군 간에 IL-6, CRP, 지질과산화물의 차이가 없었다. 조직학적 융모양막염이 있는 군에서 없는 군보다 IL-6, CRP, 지질과산화물 농도가 증가하였다. 조직학적 융모양막염이 있는 조기분만군에서 없는 조기분만군보다 IL-6, CRP, 지질과산화물 농도가 증가하였다. 조직학적 융모양막염이 있는 양막파열군에서 없는 양막파열군에 비해 CRP는 증가하였으나, IL-6와 지질과산화물 농도는 차이가 없었다. 선천성 패혈증이 의심스러웠던 태아 3명의 IL-6와 지질과산화물 농도가 증가된 소견을 보였다. 조직학적 제대염을 동반한 경우의 IL-6 농도는 동반하지 않은 경우보다 증가하였으며, 제대염이 있었던 6예 중 신생아 패혈증 의증 2명, 호흡곤란증후군 1명, 뇌실내출혈 1명으로 주산기 이환율이 높았다. 결론: 조직학적 융모양막염이 동반된 조기분만에서 제대정맥내 IL-6, CRP 및 지질과산화물이 증가하였으며, 이에 따라 주산기 이환율이 영향을 받을 것으로 사료된다. Objective: To investigate interleukin-6 (IL-6), C-reactive protein (CRP), and lipid peroxide levels in the umbilical venous plasma of preterm birth with or without histologic chorioamnionitis and to evaluate their roles in the pathophysiology in preterm labor and perinatal outcome. Methods: This cohort study included 66 cases of preterm delivery with preterm labor and intact membranes (PTL) (n=39) and preterm premature rupture of membranes (PPROM) (n=27). The umbilical venous blood samples were collected at the time of delivery. IL-6, CRP, and lipid peroxide levels were measured by ELISA Kit, latex agglutination assay, and thiobarbituric acid reaction. Histologic chorioamnionitis was diagnosed by the presence of neutrophil infiltration into the subamnionic space. Results: The prevalence of histologic chorioamnionitis was significantly higher in PPROM (59.3%, 16/27) than in PTL (20.5%, 8/39). IL-6, CRP, and lipid peroxide levels in the umbilical venous plasma of histologic chorioamnionitis were significantly higher than those without histologic chorioamnionitis. IL-6, CRP, and lipid peroxide levels in the umbilical venous plasma of PTL with histologic chorioamnionitis were significantly higher than those of PTL without histologic chorioamnionitis. CRP levels in the umbilical venous plasma of PPROM with histologic chorioamnionitis were significantly higher than those of PPROM without histologic chorioamnionitis. Three suspected neonatal sepsis patients have increased IL-6 and lipid peroxide levels in the umbilical venous plasma compared with patients without neonatal sepsis. IL-6 levels in the umbilical venous plasma of histologic funisitis were significantly higher than those without funisitis. Conclusion: Preterm birth with chorioamnionitis is associated with an increased level of IL-6, CRP, and lipid peroxide in umbilical venous blood. Preterm birth with chorioamnionitis may have an effect on perinatal outcome.

        Leptin Elongates Hypothalamic Neuronal Cilia via Transcriptional Regulation and Actin Destabilization

        <P>Terminally differentiated neurons have a single, primary cilium. The primary cilia of hypothalamic neurons play a critical role in sensing metabolic signals. We recently showed that mice with leptin deficiency or resistance have shorter cilia in the hypothalamic neurons, and leptin treatment elongates cilia in hypothalamic neurons. Here, we investigated the molecular mechanisms by which leptin controls ciliary length in hypothalamic neurons. In N1 hypothalamic neuronal cells, leptin treatment increased the expression of intraflagellar transport proteins. These effects occurred via phosphatase and tensin homolog/glycogen synthase kinase-3β-mediated inhibition of the transcriptional factor RFX1. Actin filament dynamics were also involved in leptin-promoted ciliary elongation. Both leptin and cytochalasin-D treatment induced F-actin disruption and cilium elongation in hypothalamic neurons that was completely abrogated by co-treatment with the F-actin polymerizer phalloidin. Our findings suggest that leptin elongates hypothalamic neuronal cilia by stimulating the production of intraflagellar transport proteins and destabilizing actin filaments.</P>

        <P>Hedgehog signaling plays critical roles during embryonic development. It is also involved in tissue regeneration and carcinogenesis in various adult tissues. Moreover, it regulates the maintenance of cancer stem cells and adult stem cells. Although hedgehog signaling is important in gastric carcinogenesis, its role in gastric regeneration has not been previously examined. In the present study, we evaluated the expression and roles of hedgehog signaling during gastric regeneration.</P>

      • 녹차로부터 분리한 루테인의 전립선암세포 증식억제 활성

        Background : Lutein, a xanthophyll, consists of chains with 8 conjugated double bounds containing closed rings on each end of the chain. This carotenoid is found in fruits and vegetables, especially dark green leafy vegetables such as green tea. In this study, we investigated the anticancer effects of purified lutein from green tea on human cancer cell lines containing prostate carcinoma cancer cells (LNCaP). Methods and Results : Prostate carcinoma cancer cells (LNCaP) were cultured and evaluated the inhibitory effect of lutein isolated from green tea compared other carotenoids (β-carotene and lycopene) on cell proliferation. Cyclin D1 and PCNA were evaluated as cell differentiation. In results, PCNA/cyclin regulates the initiation of cell proliferation by mediating DNA polymerase. Under cultural conditions, lycopene remarkably suppressed the PCNA expression prostate cancer cell line LNCaP in higher doses (20 μM - 100 μM) statistically. However, β-carotene and lutein presented the less inhibitory effects on PCNA expression. Determination of PCNA expression in control and treated cells demonstrates that lycopene did affect proliferation in LNCaP cancer cells in dose-dependent manner. However, β-carotene and lutein suppressed the cyclin D1 expression in dose-dependent manner but no in lycopene group. These results indicate that differ carotenoids presented the various suppressive ability of PCNA and cyclin D1 expression in cell proliferation. Conclusion : In conclusion, lutein suppressed the carcinogenesis of induced prostate cancer cell line by acting as a suppressor for inhibiting the expression of cyclin D.

      • Temperature-Dependent Raman Spectroscopic Study of the Nematic Liquid Crystal 4-<i>n</i>-Pentyl-4′-Cyanobiphenyl

        <P>The spectral behaviors of 4-n-pentyl-4'-cyanobiphenyl (5 CB) have been studied by means of temperature-dependent Raman spectros-copy in the range between --70 and 70 degrees C. The nu(C identical with N) bands in the Raman spectra were found splitting below the transition temperature from the solid to the nematic liquid crystalline phase at approximately 24 degrees C, suggesting the existence of solid crystalline polymorphism. The interfacial structures of 5 CB on metal plate surfaces have been reexamined by surface-enhanced Raman scattering (SERS) at different temperatures. On Ag and Au, the asymmetric shapes of the nu(C identical with N) bands suggest that there should exist different binding schemes for 5 CB on metal surfaces. These asymmetric bands in the nu(C identical with N) stretching region were found to vary in changing temperature.</P>
