This study was compared pre-operative MRI finding with operative finding on 36 cases with the failed back surgery syndrome (FBSS) retrospectively. The purpose of this study was to determine the accuracy of the MRI on the FBSS including early complications such as hematoma of infection. Of the 51 patients with FBSS, we analyzed pre-operative MRI finding compared with operative finding on 36 cases excluding nonunion, instability, metal failure and pseudoarthrosis who underwent an operation for the FBSS from December 1994 to June 1997. There were 25 men (69.4%) and 11 women (30.6%), aged from 16 to 68 years (average 43.6 years). These were divided into 5 sub-groups and calculated sensitivity, specificity and positive predictability. MRI accuracy in recurred disc was 84%, scar adhesion, recurred or developed stenosis, infection and hematoma 100% each and all, overall accuracy of the MRI 93%, respectively. Average interval of re-operation in FBSS WAS 4.3 years. In the 18 cases (50%), symptoms persist without pain-free interval after first operation. Early complications including hematoma and infection are easily detected with MRI. Especially if patient complains of severe leg pain or neurologic deficit, it is a good diagnostic procedure to check the MRI. The MRI is a useful method for evaluation of most cases of the FBSS, but it has limitations to evaluate recurred disc or scar adhesion only in T1, T2 weighted image. Therefore Gd-DTPA enhancement is necessary for the accurate diagnosis.

<P><B>Aims</B></P><P>At present no proven standard treatment for drug-eluting stent (DES) restenosis is available, and the efficacy and safety of everolimus-eluting stent (EES) and zotarolimus-eluting stent (ZES) for DES restenosis are limited. The purpose of this prospective, randomized 9-month intracoronary ultrasound (IVUS) and 3-year clinical follow-up study was to compare the effects of EESs and ZESs on neointima volume and major adverse cardiovascular events (MACEs) such as death, myocardial infarction (MI), target lesion revascularization (TLR) and stent thrombosis in DES restenosis patients.</P><P><B>Methods and results</B></P><P>Patients were eligible for this study if they were between 40 and 75 years old with in-stent restenosis >50% by quantitative coronary angiographic analysis in DES or within 5 mm of the stent edges with signs of ischaemia. Eligible patients (<I>n</I> = 304, 146 women and 158 men) were randomly assigned to receive either EES (158 patients) or ZES (146 patients). The primary endpoint of the study was to compare neointima volume between the EES and ZES groups at the 9-month follow-up IVUS. MACEs, including death, non-fatal MI, stent thrombosis and the need for repeated TLR within 3 years, were noted. The 9-month angiographic and IVUS follow-up showed no significant differences in late lumen loss (0.40 ± 0.56 vs. 0.45 ± 0.61 mm, <I>P</I> = 0.57, respectively) and neointima volume (0.51 ± 0.48 vs. 0.56 ± 0.54 mm<SUP>3</SUP>/1 mm, <I>P</I> = 0.47, respectively) in the EES and the ZES groups. Composite MACEs such as death, MI, stent thrombosis and TLR during 3-year follow-up were comparable between the two groups [15.8% (<I>n</I> = 25) in the EES group and 22.6% (<I>n</I> = 33) in the ZES group, <I>P</I> = 0.276], independent of <I>de novo</I> DES type, sex, age, body mass index, presence of diabetes, hypertension and dyslipidaemia.</P><P><B>Conclusions</B></P><P>Patients with first- and second-generation DES restenosis, both EES and ZES implantation were effective and safe in reducing neointima volume and late loss with a comparable rate of MACEs independent of cardiovascular risk factors. </P>

Localized corrosion behavior of Ni-based Inconel 718 alloy was investigated by electrochemical anodic polarization techniques in NACE TM 0177 A solution of 5 wt% NaCl + 0.5 wt% acetic acid at room temperature. After the solution heat treated at 1080 ℃ for 2.5 h, Inconel 718 was age-hardened at 780 ℃ for 8 h. The microstructure of the alloy surface was investigated by optical microscopic or scanning electron microscopic technique. The austenitic phase with the presence of metal carbides was observed on the surface of Inconel 718. Metal-carbides such as Nb-Mo and Ti-carbide with diameters of approximately 10 and 3 μm, respectively, were formed in Inconel 718. Anodic polarization results revealed that localized corrosion was observed at the interface between austenitic phase of a substrate and metal carbides. Difference in electrochemical property between a metal carbide and an austenitic substrate could provide an initiation site for localized corrosion of Inconel 718 surface.

The combustor, diffuser and nozzle are main components of the ramjet engine. The objective of this study is thermo structure analysis of the combustor. The combustor is composed of 17-4 PH stainless steel housing and Inconel 718 liner. Temperature conditions of housing and liner are quite different. The combustor liner is rapidly heated up to 700℃, so that is reached plastic zone locally. The temperature of combustor housing rises to 400℃. The thermal stress of liner is large than housing stress. The nonlinear thermal analysis with mechanical properties of both housing steel and liner inconel such as Young’s modulus, thermal expansion coefficient and thermal conductivity is conducted. The analysis of the transient temperature and thermal stress of combustor is carried out the finite element method with code Nastran. The structural assessment of combustor is evaluated.
