Background: Churg-Strauss syndrome (CSS) is a systemic disease and gastrointestinal GI) involvement is frequent. Nonetheless, eosinophilic vasculitis-related esophagitis is very rare. Herein, we report a case of CSS presented as severe esophagitis with necrosis. Methods: We reviewed retrospectively the medical record of a patient with severe esophagitis associated with CSS from the department of gastroenterology, rheumatology, and otolaryngology in the university hospital. Results: A 75-year-old man was hospitalized because of recurrent oral bleeding. The patient had a history of asthma. Esophagogastroduodenoscopy (EGD) showed longitudinal ulcer, black pigmentation in the middle and lower third esophagus(Figure A). The mucosa was black and friable with adherent exudates. The esophageal and nasal mucosal biopsy specimens indicated vasculitis with extravascular eosinophilic infi ltration Figure B). Para-nasal sinus computed tomography demonstrated bilateral maxillary sinusitis. Despite proton pump inhibitor, diffuse esophageal ulceration improved slightly during follow-up period. One year later, the patient was re-admitted due to hematemesis. A chest X-ray revealed pulmonary infi ltrations. EGD showed diffuse esophageal ulceration coated with necrotic debris (Figure C). Electromyography showed peripheral polyneuropathy. Treatment with steroid in combination of cyclophosphamide was performed. On the follow-up EGD one month later, the esophagitis somewhat improved Figure D). Discussion: Although GI involvement in CCS is reported in 8 to 59%, esophageal involvement as the fi rst manifestation has rarely been reported. This case was characterized by the unusual esophageal involvement with extravascular eosinophils as the fi rst presentation. We reported here a case of recurrent severe necrotic esophagitis with extravascular eosinophil, which was fi nally diagnosed as CCS.

성인병 발병율의 증가원인은 식생활의 서구화에서 오는 영양과잉, 과학의 발달로 인한 낮은 신체활동에서 오는 운동부족과 스트레스의 증가와 같은 환경인자와 흡연, 음주 등을 들 수 있는데 이는 성인병의 예방과 치료를 위해서는 의학적 처치는 물론, 식이요법·운동요법·생활 습관의 개선 등 다각적인 대처가 필요하다. 본 연구는 NIDDM 환자에게 있어 유산소운동이 혈당, 인슐린, 혈청지질에 미치는 효과를 구명하고자 자전거 에르고미터 운동을 8주간, 주 3회, 1회 60분간 실시하였다. 체중은 64.8kg±4.89에서 8주 후 63.5±4.05kg로 1.3kg(2.01%), %fat는 35.4±2.57%에서 30.6±5.36%로 4.8%(13.56%)가 감소하였다. 체전굴은 9.4±5.44cm에서 8주후 9.7±5.63cm로 0.3cm당은 198.3±89.23mg/dl에서 8주 후 113.0±51.53mg/dl로 85.2mg/dl(43.00%), 인슐린은 12.1±3.38μU/ml에서 8주 후 9.8±3.69μU/ml로 2.30μU/ml(19.01%)가 감소하였다. Fructosamine(umol/L)은 339.5±79.70μmol/L에서 8주 후 259.8±54.0μmol/L로 79.75umol/L(23.49%)가 감소하였으며, Glucagon은 252.7±59.73mg/dl에서 287.1±79.16mg/dl로 34.4mg/dl(13.61%)가 증가하였다. T-C 는 0.85%, LDL-C은 0.56% 감소하였으며, HDL-C은 48.0±4.55mg/dl에서 8주 후 49.5±8.70mg/dl으로 1.5mg/dl(3.13%)가 증가하였다. 이상과 같이 HRmax 50^의 지속적이고 규칙적인 유산소 운동은 NIDDM 환자의 체지방량과 혈당을 감소시키고, 인슐린감수성과 HDL-C이 증가하여 당뇨병개선에 효과가 있는 것으로 나타났다.

본 연구에서는 국내 자생식물 7종의 정유 중 우수한 항산화 효과가 있는 온주밀감, 육계나무 정유 2종을 선별하였고, 피부염을 유발하는 Candida albicans에 대한 항미생물 활성 평가를 통해 식물정유의 항미생물 활성성분을 구명하고자 하였다. 1,1-diphenyl-2-picrylhydrazyl (DPPH) 라디칼 소거활성 측정 결과 SC<sub>50</sub>값이 온주밀감 정유 0.010 mg/mL, 육계나무 정유 0.09 mg/mL을 나타냈으며, 2,2'-azinobis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) 라디칼 소거활성 측정 결과 온주밀감 정유 0.09 mg/mL, 육계나무 정유 0.06 mg/mL로 높은 항산화 활성을 나타냈다. Minimum Inhibitory Concentration는 육계나무 정유 1.25 mg/mL, 온주밀감 정유 5 mg/mL 값으로 육계나무 정유에서 뛰어난 항미생물 활성이 나타났다. Thin layer chromatography (TLC) bioassay를 통해 2종 정유의 성분 분획에 따른 C. albicans에 대한 항미생물 활성을 평가하였고, preparative TLC (prep. TLC)를 통해 활성분획을 획득하고, 동일 R<sub>f</sub>값을 가지는 성분들에 대해서 GC/MS 분석을 수행하여 항미생물 활성성분을 구명하였다. 그 결과 항산화 및 항미생물 활성을 나타내는 주성분은 각각 육계나무 정유의 경우 cinnamyl acetate, eucalyptol, linalool, citral 온주밀감 정유의 경우 linalool이었다. 2종 정유에 공통적으로 항산화 및 항미생물 활성을 나타낸 분획부 성분분석 결과 linalool은 항산화, cinnamyl acetate, eucalyptol, citral, geranyl acetate은 항산화와 항미생물 능력이 있다고 사료된다. In this study, we selected two essential oils, Citrus unshiu and Cinnamomum cassia with superior antioxidant effects from the essential oils of 7 wild plants in South Korea and examined their antimicrobial activity against Candida albicans, which causes dermatitis to identify the antimicrobial components in the essential oils. As a result of measuring DPPH (1,1-diphenyl-2-picrylhydrazyl) radical scavenging activity, SC<sub>50</sub> value of the Citrus unshiu essential oil was 0.010 mg/mL, while for the Cinnamomum cassia essential oil, SC<sub>50</sub> value was 0.09 mg/mL. In addition, when ABTS (2,2'-azinobis (3-ethylbenzothiazoline-6-sulfonic acid) radical scavenging activity was measured, SC<sub>50</sub> value of the Citrus unshiu essential oil was 0.09 mg/mL, while for the Cinnamomum cassia essential oil, it was 0.06 mg/mL, exhibiting high antioxidant activity. For the minimum inhibitory concentration (MIC), the essential oil of Cinnamomum cassia was 1.25 mg/mL and that of Citrus unshiu was 5 mg/mL, demonstrating a high antimicrobial activity of the Cinnamomum cassia essential oil. Through the thin layer chromatography (TLC) bioassay, we assessed the antimicrobial activity against C. albicans according to the fraction components of the two essential oils. Also, by using preparative TLC (prep. TLC), we obtained the active fractions, and by performing GC/MS analysis of the components with the same R<sub>f</sub> value, we identified the antimicrobial-active components. As a result, the main components having antioxidant and antimicrobial activities were cinnamyl acetate, eucalyptol, linalool, and citral of the Cinnamomum cassia essential oil and linalool from the Citrus unshiu essential oil. Also, based on the analysis of the fractional components that showed antioxidant and antimicrobial activities in both of the two essential oils, it was found that linalool has antioxidant activity, while cinnamyl acetate, eucalyptol, citral, and geranyl acetate have antioxidant and antimicrobial activities.

The purpose of this study was to investigate the influence of different hydrophilic polymers on the swelling, bioadhesion and mechanical strength of hydrocolloid wound dressings (HCDs) in order to provide an appropriate composition for a hydrocolloid wound dressing system. In this study, the HCDs were prepared with styrene-isoprene-styrene copolymer (SIS) and polyisobutylene (PIB) as the base using a hot melting method. Additionally, numerous SISIPIB-based HCDs were prepared with six hydrophilic polymers, and their wound dressing properties were assessed. Finally, the wound healing efficacy of the selected formulations was compared to a commercial wound dressing. The swelling ratio, bioadhesive force and mechanical strengths of HCDs were increased in the order of sodium alginate> sodium CMC = poloxamer = HPMC > PYA = PVP, sodium alginate> sodium CMC = poloxamer > PYA> HPMC = PVP and sodium alginate > PYA >PVP = HPMC = sodium CMC> poloxarmer, respectively. Among the hydrophilic polymers tested, sodium alginate most enhanced the swelling capacity, bioadhesive force and mechanical strengths. Thus, the hydrophilic polymers played great role in the swelling, bioadhesion and mechanical strength of SISIPIB-based HCDs. The HCD formulation composed of PIB, SIS, liquid paraffin and sodium alginate at the weight ratio of 20/25/12/43 gave better wound dressing properties and more excellent wound healing efficacy than the commercial wound dressing. Therefore, the novel HCD formulation could be a promising hydrocolloid system for wound dressings.
