Objectives : To see if there were any differences in waist circumference reduction when certain variable such as age, sex, BFR(BFR; Body Fat Rate), PIBW(PIBW; Percent Ideal Body Weight), WHR(WHR; Waist Hip Ratio) and BMI(BMI; Body Mass Index) were given. Methods : Among outpatients who admitted to department of Oriental Rehabilitation Medicine, Jeon-Ju Oriental Medicine Hospital, Won-kwang University from August 2003 to september 2004, 89 obese patients were chosen and electroacupunture was practiced to these groups in order to treat abdominal obesity and waist circumference reduction. Results : There were no significant correlation between sex, age and waist circumference reduction. Compared to groups who had shown lower BMI, higher BMI groups have shown greater change in waist circumference reduction. As BFR increase waist circumference reduction tended to increase as well. Conclusions : From the above results, higher BMI groups had shown greater change in waist circumference reduction compared to lower BMI groups.

Laparoendoscopic single-site surgery (LESS) is helpful if not an evidence of suspicious malignancy in huge adnexa tumors. This approach might be reasonable alternative to conventional laparoscopic surgery. We report two cases with LESS for huge cystic adnexal tumors in whole abdomen. A 58 years old multigravida woman in menopause underwent removal of a huge cystic adnexal mass that occupied her entire abdomen by LESS.(ca.5200ml aspiration) A 15 years old single nulligravida woman had a huge cystic adnexal tumor in whole abdomen reaching to the level of the xyphoid process. A needle through the transumbilical single port was inserted directly under the mass. Subsequently, 8000 ml of cystic fluid was aspirated without spillage.

Background In general, external rotation (ER) exercises are commonly used in rotator cuff retraining programs to improve stabilization ability. ER exercises at 90° of shoulder abduction are common and effective in strengthening the infraspinatus (ISP). However, if the ISP is weak during ER at 90° shoulder abduction, the posterior deltoid muscle (PD) is overactivated, which can lead to anterior translation of the humeral head. Therefore, the selective activation of ISP muscles is an important factor in rehabilitation. Objectives We investigated the muscle activities of the ISP, PD, and middle trapezius (MT), the ratio of the ISP to PD muscle activities, and the anterior translation of the humeral head in response to three resistance intensities during sitting external rotation (SITER) exercises. Methods A total of 12 healthy women recorded changes in the anterior translation of the humeral head using a load cell, the muscle activities of the ISP, PD, and MT, the ratio of the ISP to PD muscle activities using surface electromyography (EMG) during SITER under three different intensities [low (10-20%) medium (45-55%) high (60-70%)]. Results The ISP, PD, and MT amplitudes were highest at high intensity, followed by medium and low intensities during SITER (p < 0.001). The muscle activity ratio was significantly highest at medium intensity compared with low and high intensities (p = 0.031). The anterior translation of the humeral head was highest at high intensity, followed by medium and low intensities during SITER (p < 0.001). Conclusions Medium intensity (45-55%) was the most appropriate intensity during the SITER exercise. However, anterior gliding of the humeral head was the lowest at low intensity. Therefore, during ER exercises, we should think about how to control the anterior gliding of the humeral head.

Aims: Current AKI criteria using serum creatinine (Cr) has some limitations to predict reversibility of renal function and discriminate renal parenchymal injury in cirrhotic patients. The aim of this study is to evaluate whether urine biomarkers [N-acetyl-ß-D-Glucosaminidase (NAG)] can predict survival and response to terlipressin in cirrhotic patients with AKI. Methods: Two hundred sixty two cirrhotic patients who developed AKI were prospectively enrolled from 11 tertiary medical centers in Korea during 2016 to 2019. AKI was defined as increase in serum Cr (SCr) of 0.3mg/dL or 50% increase of baseline in SCr according to guideline. The patients with SCr ≥ 2.5 mg/dL were diagnosed as hepatorenal syndrome (HRS-AKI) and treated with terlipressin plus albumin. Urine and blood samples were collected at the diagnosis of AKI and/or HRS. Results: The mean MELD score was 25.27 ± 9.11, and mean SCr was 2.27 ± 0.87 mg/dL. The baseline urine NAG (AKI stage I, 17.22 ± 24.66 mg/dL; stage II, 32.12 ± 52.71 mg/dL; stage III, 53.23 ± 63.28 mg/dL, P<0.001) increased as the baseline AKI stage increased. Urine NAG level was significantly lower in survival group than who underwent death or transplant in 3-months (22.34 ± 36.73 mg/dL vs. 38.80 ± 55.90 mg/dL, P=0.005). In multivariate analysis, urine NAG was a significant risk factor for 3-month transplant free survival (TFS), especially in patients with Child-Pugh class ≤ B or MELD < 24. However, urine NAG did not predict the response to terlipressin or recovery of renal function. Conclusions: Urine NAG is strongly associated with severity of AKI in patients with liver cirrhosis and may be helpful to predict 3-months TFS in these patients.

The aim of this study is to evaluate whether urine biomarkers [cystatin C, N-acetyl-β-D-Glucosaminidase (NAG)] can predict survival and response to terlipressin in cirrhotic patients with AKI. Methods: One hundred eleven cirrhotic patients who developed AKI were prospectively enrolled from 11 tertiary medical centers in Korea during 2016 to 2018. AKI was defined as increase in serum Cr (SCr) of 0.3mg/dL or 50% increase of baseline in SCr with a final value above 1.5 mg/dL. Among them, the patients with SCr ≥ 2.5 mg/dL were diagnosed as hepatorenal syndrome (HRS) and treated with terlipressin. Urine samples were collected for the measurement of urine cystatin C and urine NAG at the diagnosis of AKI and/or HRS. Results: Of 111 patients, 29 patients had HRS. The mean MELD score was 23.92 ± 8.90, and mean SCr was 2.20 ± 0.79 mg/ dL. The baseline urine NAG (AKI stage I, 16.37 ± 16.48 mg/ dL; stage II, 27.84 ± 36.70 mg/dL; stage III, 55.69 ± 62.93 mg/ dL, P=0.007) and urine cystatin C (AKI stage I, 0.50 ± 0.78 mg/ dL; stage II, 0.67 ± 2.29 mg/dL; stage III, 1.07 ± 2.16, P=0.558) increased as the baseline AKI stage increased. During median 2.6 months observational period, all-cause mortality was 39.4%. In patients with liver transplantation or death, urine NAG level tended to be higher than in survival group, but not statistically significant (30.56 ± 40.81 mg/dL vs. 20.80 ± 29.22 mg/dL, P=0.101). However, this urine biomarkers were not yet different depending on the improvement of renal function and survival. Conclusions: In the interim-analysis of our study, urine NAG and urine cystatin C are strongly associated with severity of AKI in patients with liver cirrhosis and may be helpful to predict transplant-free survival in these patients.

Purpose: The purpose of this study was to examine the impact of gluteus medius (GM) strengthening training using the pressure biofeedback unit for lower extremity muscle function and balance ability in stroke patients. Methods: Twenty-seven stroke patients (14 men, 13 women) volunteered to participate in this study. They were randomly assigned to 3 groups: pressure biofeedback unit GM training, general GM training, and control group. Experimental group 1 performed GM strengthening training using PBU in the sidelying position. Experimental group 2 performed GM strengthening training without PBU in the sidelying position. The training program lasted 4 weeks (20-minute sessions, three times a week). Muscle function was assessed by measuring muscle strength and endurance, and Berg balance scale (BBS), Lateral Reach Test (LRT), and limited of stability (LOS) velocity were measured for evaluation of balance ability. Results: Enhancement of muscle strength and endurance was significantly higher in experimental group 1 than in the other two groups (p<0.05). In comparison of the balance function, experimental group 1 showed a significantly enhanced ability to balance (p<0.05). After the intervention, experimental group 1 showed a significant reduction of LOS velocity in the anterior direction and affected direction (p<0.05). There was no significant difference than the other two groups but a large reduction was observed. Conclusion: These findings suggest that selective gluteus medius strengthening training using the pressure biofeedback unit is effective for lower extremity muscle function and balance ability in stroke patients.

Eukaryotic replication protein A (RPA) is a single-stranded(ss) DNA binding protein with multiple functions in DNA replication, repair, and genetic recombination. The 70-kDa subunit of eukaryotic RPA contains a conserved four cysteine-type zinc-finger motif that has been implicated in the regulation of DNA replication and repair. Recently, we described a novel function for the zine-finger motif in the regulation of human RPA's ssDNA binding activity through reduction-oxidation (redox). Here, we show that yeast RPA's ssDNA binding activity is regulated by redox potential through its RPA32 and/or RPA14 subunits. Yeast RPA requires a reducing agent, such as dithiothreitol (DTT), for its ssDNA binding activity. Also, under non-reducing conditions, its DNA binding activity decreases 20 fold. In contrast, the RPA70 subunit does not require DTT for its DNA binding activity and is not affected by the redox condition. These results suggest that all three subunits are required for the regulation of RPA's DNA binding activity through redox potential.
