Adrenal gland is a common site of metastatic tumors such as breast cancer, lung cancer, and colon cancer. When adrenal mass is found incidentally, adenoma is the most common among single adrenal masses. But in the case of bilateral adrenal masses, infection, bilateral metastases and hemorrhage are common. Secondary involvement of the adrenal gland is found in 25% of autopsy cases of non-Hodgkin's lymphoma. However, adrenal insufficiency is rare because it becomes apparent only when approximately 90% of adrenal cortex is destructed. We exprienced two cases of malignant lymphoma which involved the adrenal glands bilaterally. One case in which adrenal insufficiency was suspicious, was accompanied by hypovolemic shock and sepsis at the initial presentation. He died of sepsis combined with DIC even though hydrocortisone, intravenous saline infusion, and antibiotics therapy were started immediately. The other one was found incidentally, in which adrenal infiltraion was confirmed by CT scan. Hormonal level was normal and adrenal masses disappeared after chemotherapy(J Kor Soc Endocrinol 15:121-127, 2000).

Background: Balneotherapy is widely used for the management of inflammatory skin diseases, but the mechanism is partly understood. Korea has abundant resources of hot spring(HS) and among them are sea salt type of Na(Ca)-Cl. We previously reported that different HS water affected the secretion of proinflammatory cytokines from HaCaT cells. Objectives: We evaluated effect on proinflammatory cytokine secretion in primary keratinocytes(KCs) of three different sea salt type HS water, i.e., Haeundae, Magumsan, and Seokmodo HS area, selected by salt concentration gradient. Methods: To know whether primary KCs express Toll-like receptors(TLRs), we isolated mRNA from KCs and evaluated the expression of TLR1 to 10 by RT-PCR. Next, we investigated the effect of three different HS waters on theexpression of cytokines(IL-8, TNF-α, IL-1β, IL-6, and GM-CSF) in the primary KCs under TLR1, 2, 3, 5, and TLR6 stimulation by ELISA. Results: The treatment of HS waters induced suppression of proinflammatory cytokines under TLR stimulation in the primary KCs. Cytokine secretion from primary human KCs to TLR3 ligand was the most prominent. Decrease of IL-8, TNF-α, IL-1β, IL-6, or GM-CSF secretion under TLR stimulation were observed to all HS waters. Conclusion: Three different sea salt type HS water treatment affected the secretion of proinflammatory cytokine production from KCs under TLR stimulation. These parameters and protocols can be used to evaluate anti-inflammatory effect of sea salt type HS water.

Background: Balneotherapy has long been utilized as one of safe and effective alternative therapies of various inflammatory skin diseases in Korea. There are about 400 spas in Korea and many of them are recognized as having specific therapeutic effects. But, the beneficial clinical effects of spa therapy as well as their underlying mechanisms are still poorly defined and understood. Objectives: Natural hot spring water in Korea can be classified into five main hydrochemical groups. They are mostly alkaline in pH and have low chemical constituents. We performed a series of studies to have better understanding of the clinical effects as well as possible mechanisms of their beneficial effects of several hot springs. Methods: They are Haeundae(Na-Cl), Seokmo-do(Na-Cl, Ca-Cl), Suanbo(Na-HCO3), Baekam(Na-HCO3), and, Dukku(Na-Cl). We evaluated the effects of hot spring water on the changes of clinical symptoms of dermatitis and their skin barrier functions, skin infiltrating cells and related histologic findings, proinflammatory cytokine production and helper T cell subset differentiation and proliferation in the skin, spleen and peripheral blood. Results: We found that studied hot spring waters had tendency to downregulate the production of skin inflammatory cytokines and to increase the Foxp3+Treg cell population while to decrease the Th1, Th2, and Th17 populations. Conclusion: These results suggest that balneotherapy can be used as an effective treatment modality to improve skin inflammation.

Acquired generalized lipodystrophy is a rare disease in which a significant amount of adipose tissue is lost from a relatively large region of the body. The disease typically manifests during childhood and adolescence. Metabolic abnormalities, including insulin resistance, diabetes mellitus and hypertriglyceridemia develops in the early course of the disease. A 26-year-old woman with a 12 year history of type 2 diabetes mellitus and hyperlipidemia visited our outpatient clinic with a nonhealing diabetic foot ulcer. She was noticed to have fat loss from the face at 11 years of age. Over the next few years, it progressed to involve the trunk and extremities. Her hair became curly, and she developed diabetes mellitus, hypertriglyceridemia, and hepatomegaly at 14 years of age, and was placed on insulin therapy. On physical examination, her height was 1.60 m and weight was 30.0 kg. Her appearance was muscular with marked loss of fat from the face, trunk, and extremities. Her mother and younger sister had the same appearance. Diagnosis of lipodystrophy was made. Herein, we report a rare case of acquired generalized lipodystrophy in a 26-year-old female.

Background: No nationwide epidemiological study has investigated the urinary albumin/creatinine ratio in association with atopic dermatitis. Objectives: This study aimed to investigate the relationship between atopic dermatitis and albumin/creatinine ratio in South Korea. Methods: The Korean National Health and Nutrition Examination Survey examined a representative sample of the Korean population. A total of 1976 male participants aged 19 to 39 years were categorized into two groups according to the presence of atopic dermatitis. Multiple variables were compared between the two groups and the odds ratios for an increased albumin/creatinine ratio were calculated using weighted logistic regression analyses. Results: Among the 1976 participants, 103 participants were categorized as in the atopic dermatitis group. The percentages of ever smokers and current alcohol users (>30g/day) were significantly lower in the atopic dermatitis group (P=0.002 and 0.006, respectively). Weighted logistic regression analyses demonstrated that an increased urinary albumin/creatinine ratio had a higher risk of atopic dermatitis after adjusting for multiple variables (OR 1.363, 95% confidence interval 1.078- 1.724). The increased prevalence of atopic dermatitis in the higher ACR quartiles was statistically significant (P=0.0246) Conclusion: Early detection of increased urinary albumin/creatinine ratios in patients with atopic dermatitis has the potential to prevent progression to systemic disease.

In this study, the authors investigated changes in the non-specific immune states of 35 acute leukemia patients(19 AML; 16 ALL) according to the state of their disease (25 at diagnosis; 10 in complete remission with three off-treatments) and compared them to those of normal controls. The natural killer(NK) cell activity aganist K562 target cells and antibody-dependent cellular cytotoxicity(ADCC) against L1210 target cells were measured by four-hour chromium release cytotoxic assays. The number of NK cells was estimated by immunofluorescence techniques using monoclonal antibodies of Leu-7a, while Leu-11 and T-cell subpopulations were estimated by monoclonal antibodies ctf CD3, CD4, CD8. Interleukin-2 (IL2) productivity was measured with peripheral mononuclear cells from five patients. The results were as follows; 1) The NK and ADCC activities significantly decreased in the initial state, which might be due to a decreased number of NK cells and also to a diluting effect by the circulating blasts in ADCC. 2) In the complete remission state, patients who had maintenance chemotherapy showed a lower NK cell number, which resulted in decreased NK activity during this period without any effect on ADCC. 3) Compared to the controls, such depression of nonspecific immune state was more pronounced in the AML group than in the ALL group. In conclusion, the non-specific cellular immune states differed between the initial state and the complete remission state, to which maintenance chemotherapy might be attributed. The mechanism and the clinical significance of such an altered immune state according to the diseased state in relation to maintenance treatment remain to be elucidated.
