      • 세포브러쉬와 면봉도말법에 의한 부인과 세포학적 검사의 효율비교

        A study was undertaken to evaluate the efficiency of the sytobrush compared with the cotton swab for obtaining cells from the uterine cervix. In 300 patients, the cytobrush technique was employed and in control 300 patients was smeared by the cotton swab. In 12 out of 300 patients(4%) endocervical cells were found on the cotton swab technique. A total of 252 out of 300 cytobrush sampler specimen(84%) contained endocervical cells. With the X²method for statistical comparison, the difference in endocervical cell yield between the cotton swab and the cytobrush sampler was significantly different(p<0.01). This was particularly true in older patients. We conclude from study that the cytobrush sampler is of value for obtaining as adequate cervical cytologic specimen.

        이 硏究는 1992학년도의 한국항공대학교 신입생 의식구조의 조사를 토대로 한 새로운 학생생활지도 방안을 設定하기 위해서 마련된 것이다. 특히 이 연구의 진행과정에서는 최근 東北아시아의 국제정치 및 경제사정등의 극적인 變化가 학생생활에 미치는 영향 도 깊이 고려하였다. 뿐만 아니라 지난 오랜동안 한국항공대학교가 위치한 이 캠퍼스지역에 가로놓인 많은 어려운 장애 물과 악조건들이 학생들의 학습활동이나 연구생활에 미치는 악영향등도 충분히 유의하였다. 이 硏究의 두번째 목적은 무엇보다도 학생들의 進路지도를 적극 강화하고 보완하는 방안이 생활의 勉學분위기를 조성 하고 활기찬 학습 및 연구생활에 더욱 크게 이바지 하게 될 것이라는 점을 크게 유념하였다. This study is to eatablish "New Directions of Guidance for Students Life on the Basis of Research into Freshmen's Way of Thinking(1992)" in Hankuk Aviation University. In the process of this research, Particularly, we have deeply considered into the great influence upon the students life from the recent dramatic change of international polities and economic circumstances of Northeastern Asian countries. On the other hand, we have kept in mind very difficult conditions and circumstances laid in this campus area which have greatly influenced upon the very hard and inconvenient studing life of students for a long time past. The second objective of this research is to study of New Directions of Guidance for Making New job of Graduates and future new plan for variable social life after graduation. We believe such methods of New Guidance will remarkably devote on the effective and industrious studing life of students of Hankuk Avation University.

      • 신부전이 동반된 당뇨병 환자에서 발생된 침습성 모균증 : 수술과 Liposomal amphotericin B 및 GM-CSF 병합 요법에 의한 성공적인 치험 1예

        저자들은 신부전이 동반된 당뇨병 환자에서 발생된 부비동형 모균증에 대하여 수술과 함께 liposomal amphotericin B 및 GM-CSF의 복합치료를 하여 성공적으로 치료한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Mucormycosis (zygomycosis) primarily affects diabetic or immunocompromised patients and typically progresses rapidly, necessitating surgical excision and antifungal therapy with amphotericin B. Large doses of amphotericin B are needed for cure, but it has the risk of causing significant renal toxicity. The recent development of liposomal amphotericin B allows antifungal therapy to be administered with potentially improved efficacy and reduced nephrotoxicity. We have experienced a case of paranasal mucormycosis successfully treated with surgery, liposomal amphotericin B and GM-CSF. A 59-year-old male suffering from diabetes mellitus for 6 years was admitted with pain at left maxillary area. He was diagnosed as mucormycosis after cytologic exam on the necrotic nasal mucosa, which showed typical hyphae. He have had diabetic nephropathy with macroproteinuria and had rapidly rising serum creatinine levels with the amphotericin B treatment: creatinine levels reverted to basal level with the use of liposomal amphotericin B. Despite surgical excision and continued antifungal therapy, his infection was not effectively controlled. Therefore, GM-CSF was administered additionally to improve phagocytic activity of leukocytes. He was finally cured after receiving a combination of aggressive surgery, liposomal amphotericin B and GM-CSF. To our knowledge, this is the first detailed clinical description of the treatment of mucormycosis with liposomal amphotericin B in Korea.

        A New 5α,8α-Epidioxy Sterol from a Marine Sponge Psammocinia Species

        An investigation of the MeOH soluble fractions of a marine sponge Psammocinia sp. (Order:Dictyoceratida) led to the isolation of a new epidioxy sterol (1) and four known sterols (2-5). Their planarstructures were defined by analyses of the spectroscopic data. The 27-nor-24-methylcholestan type side chain withan epidioxy nucleus (1) was unprecedented. Compounds 1-5 were isolated from a sponge Psammocinia sp. for the first time.

        Case Report : Rupture of abdominal aortic aneurysm after spine surgery in the patient with Ehlers-Danlos syndrome

        Ehlers-Danlos syndrome (EDS) is a rare inherited disorder of the connective tissue that is characterized by hyper-extensible skin, hypermobile joints and abnormalities of the cardiovascular system. A 15-year-old girl with Ehlers-Danlos syndrome underwent thoracolumbar surgery for deformity correction. After surgery, an abdominal aortic rupture occurred, and she complained of abdominal distension had an abdominal circumference of 80 cm. Abdominal computed tomography revealed a pseudoaneurysm and a large hematoma at the retroperitoneum. She died of a massive hemorrhage during subsequent abdominal aortic surgery.         Postoperative radiotherapy dose correlates with locoregional control in patients with extra-hepatic bile duct cancer

        Purpose: To evaluate the results of postoperative radiotherapy in patients with extra-hepatic bile duct cancer (EHBDC) and identify the prognostic factors for local control and survival. Materials and Methods: Between January 2001 and December 2010, we retrospectively reviewed the cases of 70 patients with EHBDC who had undergone curative resection and received postoperative radiotherapy. The median radiation dose was 50.4 Gy (range, 41.4 to 54 Gy). The resection margin status was R0 in 30 patients (42.9%), R1 in 25 patients (35.7%), and R2 in 15 patients (21.4%). Results: The 5-year rates of overall survival (OS), event-free survival (EFS), and locoregional control (LRC) for all patients were 42.9%, 38.3%, and 61.2%, respectively. The major pattern of failure was distant relapses (33 patients, 47.1%). A multivariate analysis showed that the postradiotherapy CA19-9 level, radiation dose (≥50 Gy), R2 resection margins, perineural invasion, and T stage were the significant prognostic factors for OS, EFS, and LRC. OS was not significantly different between the patients receiving R0 and R1 resections, but was significantly lower among those receiving R2 resection (54.6%, 56.1%, and 7.1% for R0, R1, and R2 resections, respectively). Conclusion: In patients with EHBDC who had undergone curative resection, a postoperative radiotherapy dose less than 50 Gy was suboptimal for OS and LRC. Higher radiation doses may be needed to obtain better LRC. Further investigation of novel therapy or palliative treatment should be considered for patients receiving R2 resection.

        일반논문 : 체육시설법상의 영업양도와 회원권 승계에 관한 재고찰

        「체육시설의 설치. 이용에 관한 법률」 (2003. 5. 29. 법률 제6907호로 개정되기 전의 것) 제30조 제1항은 골프장 등 체육시설의 양도 · 양수가 있을 경우 양수인은 종전의 회원을 승계하도록 규정하고 있고, 그 제3항은 같은 법 제12조의 규정에 의한 사업계획의 승계에 관하여 준용한다고 규정하고 있다. 그런데, 근자에 IMF 당시 회원분양에 실패하여 도산한 모 골프장과 관련하여 일단의 대법원판결들이 선고되었는데, 그 주 요 내용은 골프장 부지를 경매절차에서 낙찰받아 취득한 경우에도 그 낙찰자가 기존 골프장사업자로부터 모집한 회원들에 대한 권리 · 의무를 승계하고, 특히 그 회원들이 위 골프장공사의 건설에 참여한 공사업자들이 그 공사대금을 변제받는 방편으로 회원 권을 취득한 경우에도 보호된다는 것이다. 그러나, 위 대법원판례들은 영업양도의 개념을 지나치게 확장하여 기존의 대법원판례에 배치될 뿐만 아니라 골프장부지를 낙찰 받은 자에게 불측의 손해를 입게 하고 회원의 탈을 쓴 일반 공사대금채권자들을 정상 적인 회원처럼 과도하게 보호하게 된다는 문제점이 지적되고 있다. 필자는 이와 같은 문제점에 관하여 2009년 학위논문1)에서 일부 거론한 바 있으나, 본고에서는 쟁점을 좁혀 중요한 문제점만을 거론하고자 한다. 법률의 통일적 해석을 주된 사명으로 하는 대법원이 전원합의체를 거치지 아니한 채 기존 대법원판례와 배치되는 판결을 하는 것은 법령의 통일적 해석이라는 측면에서나 법적 안전성의 측면에서 볼 때 결코 묵과 할 수 없는 문제임이 분명하다. Article 30, Section 1 of 「Act on Installation and Use of Sports Facilities」 (Before the change of the Law No.6907 on 29 May, 2003) states that, in the case of the transfer or taking over the sports facilities, such as golf course etc., the assignee succeeds the erstwhile members. The Section 3 states that the provision of Article 12 of same law shall apply in succession of business plan. These days, however, in reference to certain golf course, insolvency due to the failure of recruitment of members in IMF, it is sentenced Supreme Court judgment. The main content of the judgment is that, even if the site of golf course is received via auction process, the successful bidder succeeds the right and obligation of the erstwhile members, which were recruited from the erstwhile golf course operator. Especially, in the case of the building contractors of the erstwhile members, participated in the construction of golf course, and received the golf membership as a way of getting a refund of the cost of construction, they are also protected. Above the Supreme Court precedents, however, run counter to the existing Supreme Court precedents due to the excessive expansion of the concept of business transfer. Besides, the successful bidder of the site of golf course sustains unsuspected damage, and the general creditors of construction payment, play the hypocrite as a member, are protected excessively same as normal members.

        에너지 성능분석을 통한 개방형BIM 기반 초고층 건물 외피 디자인 방안 제시 및 적용

        As contemporary design gets more sophisticated and the policies of environment-friendly design have been established, the use of BIM (Building Information Modeling) technology is widely spreading in the current AEC industry. For instance, PPS(Public Procurement Service) announced that public projects that are larger than 50 billion won in total construction costs should apply BIM from the year 2012, which is expected to be applied for large scale projects like super-tall buildings in the future. Unlike the conventional process of making a blueprint, BIM technology is a concept that includes additional information as well as the shape information of a building. However, there are few researches conducted for alternative assessment through open BIM-based energy assessment in the initial planning stage. Eventually, it requires much effort and time to understand energy efficiency to come up with the optimal building alternative in super-tall buildings design. Therefore, this study suggests an open BIM-based design method of super-tall building’s envelopes through energy performance analysis. When creating building envelopes of a super-tall building from the initial planning stage, the users can efficiently select the most suitable alternative.

        An External Capacitor-Less Ultralow-Dropout Regulator Using a Loop-Gain Stabilizing Technique for High Power-Supply Rejection Over a Wide Range of Load Current

        <P>An external capacitor-less ultra low-dropout (LDO) regulator that can continue to provide high power-supply rejection (PSR) over a wide range of the load current is proposed. Using the loop-gain stabilizer (LGS) to fix the dc level of the output voltage of the error amplifier to the optimal value, the LDO can keep maximizing the unity-gain frequency, while the load current changes widely up to 200 mA. Despite the multiple poles in the regulating loop, the stability can easily be obtained due to an intrinsic left-half plane zero, generated by the auxiliary path of the LGS. The proposed LDO was fabricated in a 40-nm CMOS process, and it had an input voltage of 1.1 V. When the dropout voltage was 0.1 V and the load current was 200 mA, the measured PSRs were -60 and -35 dB at 1 and 10 MHz, respectively. Due to the LGS, the dc loop gain was maintained to be high, resulting in good load and line regulations of 19 mu V/mA and 0.75 mV/V, respectively. While the total current consumption of the LDO was 275 mu A, the LGS consumed only 7 mu A. The area was 0.008 mm(2) with 4-pF on-chip capacitance for compensation.</P>
