        Increased Readmission Risk and Healthcare Cost for Delirium Patients without Immediate Hospitalization in the Emergency Department

        Objective: Hospitalization of patients with delirium after visiting the emergency department (ED) is often required. However, the readmission risk after discharge from the ED should also be considered. This study aimed to explore whether (i) immediate hospitalization influences the readmission risk of patients with delirium; (ii) the readmission risk is affected by various risk factors; and (iii) the healthcare cost differs between groups within 28 days of the first ED visit. Methods: Using the National Health Insurance Research Database, the data of 2,780 subjects presenting with delirium at an ED visit from 2000 to 2008 were examined. The readmission risks of the groups of patients (i.e., patients who were and were not admitted within 24 hours of an ED visit) within 28 days were compared, and the effects of the severities of different comorbidities (using Charlson’s comorbidity index, CCI), age, gender, diagnosis and differences in medical healthcare cost were analyzed. Results: Patients without immediate hospitalization had a higher risk of readmission within 3, 7, 14, or 28 days of discharge from the ED, especially subjects with more severe comorbidities (CCI≥3) or older patients (≥65 years). Subjects with more severe comorbidities or older subjects who were not admitted immediately also incurred a greater healthcare cost for re-hospitalization within the 28-day follow-up period. Conclusion: Patients with delirium with a higher CCI or of a greater age should be carefully considered for immediate hospitalization from ED for further examination in order to reduce the risk of re-hospitalization and cost of healthcare.

        Impact of fatty liver on long-term outcomes in chronic hepatitis B: a systematic review and matched analysis of individual patient data meta-analysis

        Background/Aims: Chronic hepatitis B (CHB) and fatty liver (FL) often co-exist, but natural history data of this dual condition (CHB-FL) are sparse. Via a systematic review, conventional meta-analysis (MA) and individual patient-level data MA (IPDMA), we compared liver-related outcomes and mortality between CHB-FL and CHB-no FL patients. Methods: We searched 4 databases from inception to December 2021 and pooled study-level estimates using a random- effects model for conventional MA. For IPDMA, we evaluated outcomes after balancing the two study groups with inverse probability treatment weighting (IPTW) on age, sex, cirrhosis, diabetes, ALT, HBeAg, HBV DNA, and antiviral treatment. Results: We screened 2,157 articles and included 19 eligible studies (17,955 patients: 11,908 CHB-no FL; 6,047 CHB-FL) in conventional MA, which found severe heterogeneity (I2=88–95%) and no significant differences in HCC, cirrhosis, mortality, or HBsAg seroclearance incidence (P=0.27–0.93). IPDMA included 13,262 patients: 8,625 CHB-no FL and 4,637 CHB-FL patients who differed in several characteristics. The IPTW cohort included 6,955 CHB-no FL and 3,346 CHB-FL well-matched patients. CHB-FL patients (vs. CHB-no FL) had significantly lower HCC, cirrhosis, mortality and higher HBsAg seroclearance incidence (all P≤0.002), with consistent results in subgroups. CHB-FL diagnosed by liver biopsy had a higher 10-year cumulative HCC incidence than CHB-FL diagnosed with non-invasive methods (63.6% vs. 4.3%, P<0.0001). Conclusions: IPDMA data with well-matched CHB patient groups showed that FL (vs. no FL) was associated with significantly lower HCC, cirrhosis, and mortality risk and higher HBsAg seroclearance probability.

        Overdensity of submillimeter galaxies around the <i>z</i> ≃ 2.3 MAMMOTH-1 nebula : The environment and powering of an enormous Lyman-<i>α</i> nebula

        <P>In the hierarchical model of structure formation, giant elliptical galaxies form through merging processes within the highest density peaks known as protoclusters. While high-redshift radio galaxies usually pinpoint the location of these environments, we have recently discovered at <I>z</I> ∼ 2−3 three enormous (> 200 kpc) Lyman-<I>α</I> nebulae (ELANe) that host multiple active galactic nuclei (AGN) and that are surrounded by overdensities of Lyman-<I>α</I> emitters (LAE). These regions are prime candidates for massive protoclusters in the early stages of assembly. To characterize the star-forming activity within these rare structures - both on ELAN and protocluster scales - we have initiated an observational campaign with the <I>James Clerk Maxwell</I> Telescope (JCMT) and the Atacama Pathfinder EXperiment (APEX) telescopes. In this paper we report on sensitive SCUBA-2/JCMT 850 and 450 <I>μ</I>m observations of a ∼128 arcmin<SUP>2</SUP> field comprising the ELAN MAMMOTH-1, together with the peak of the hosting BOSS1441 LAE overdensity at <I>z</I> = 2.32. These observations unveil 4.0 ± 1.3 times higher source counts at 850 <I>μ</I>m with respect to blank fields, likely confirming the presence of an overdensity also in obscured tracers. We find a strong detection at 850 <I>μ</I>m associated with the continuum source embedded within the ELAN MAMMOTH-1, which - together with the available data from the literature - allow us to constrain the spectral energy distribution of this source to be of an ultra-luminous infrared galaxy (ULIRG) with a far-infrared luminosity of LFIR<SUP>SF</SUP> = 2.4−2.1<SUP>+7.4</SUP>×10<SUP>12</SUP><I>L</I>⊙, and hosting an obscured AGN. Such a source is thus able to power a hard photoionization plus outflow scenario to explain the extended Lyman-<I>α</I>, He II<I>λ</I>1640, and C IV<I>λ</I>1549 emission, and their kinematics. In addition, the two brightest detections at 850 <I>μ</I>m (<I>f</I>850 > 18 mJy) sit at the density peak of the LAEs’ overdensity, likely pinpointing the core of the protocluster. Future multiwavelength and spectroscopic datasets targeting the full extent of the BOSS1441 overdensity have the potential to firmly characterize a cosmic nursery of giant elliptical galaxies, and ultimately of a massive cluster.</P>

        Inhibition of the prostaglandin-degrading enzyme 15-PGDH potentiates tissue regeneration

        <P><B>A shot in the arm for damaged tissue</B></P><P>Tissue damage can be caused by injury, disease, and even certain medical treatments. There is great interest in identifying drugs that accelerate tissue regeneration and recovery, especially drugs that might benefit multiple organ systems. Zhang <I>et al.</I> describe a compound with this desired activity, at least in mice (see the Perspective by FitzGerald). SW033291 promotes recovery of the hematopoietic system after bone marrow transplantation, prevents the development of ulcerative colitis in the intestine, and accelerates liver regeneration after hepatic surgery. It acts by inhibiting an enzyme that degrades prostaglandins, lipid signaling molecules that have been implicated in tissue stem cell maintenance.</P><P><I>Science</I>, this issue 10.1126/science.aaa2340; see also p. 1208</P><P>Agents that promote tissue regeneration could be beneficial in a variety of clinical settings, such as stimulating recovery of the hematopoietic system after bone marrow transplantation. Prostaglandin PGE2, a lipid signaling molecule that supports expansion of several types of tissue stem cells, is a candidate therapeutic target for promoting tissue regeneration in vivo. Here, we show that inhibition of 15-hydroxyprostaglandin dehydrogenase (15-PGDH), a prostaglandin-degrading enzyme, potentiates tissue regeneration in multiple organs in mice. In a chemical screen, we identify a small-molecule inhibitor of 15-PGDH (SW033291) that increases prostaglandin PGE2 levels in bone marrow and other tissues. SW033291 accelerates hematopoietic recovery in mice receiving a bone marrow transplant. The same compound also promotes tissue regeneration in mouse models of colon and liver injury. Tissues from 15-PGDH knockout mice demonstrate similar increased regenerative capacity. Thus, 15-PGDH inhibition may be a valuable therapeutic strategy for tissue regeneration in diverse clinical contexts.</P>

        Serotonin Modulates the Correlations between Obsessive-compulsive Trait and Heart Rate Variability in Normal Healthy Subjects: A SPECT Study with [123I]ADAM and Heart Rate Variability Measurement

        Objective: The impact of serotonergic system on obsessive-compulsive disorder (OCD) is well studied. However, the correlation between OC presentations and autonomic nervous system (ANS) is still unclear. Furthermore, whether the correlation might be modulated by serotonin is also uncertain. Methods: We recruited eighty-nine healthy subjects. Serotonin transporter (SERT) availability by [123I]ADAM and heart rate variability (HRV) tests were measured. Symptoms checklist-90 was measured for the OC presentations. The interaction between HRV and SERT availability were calculated and the correlation between HRV and OC symptoms were analyzed after stratified SERT level into two groups, split at medium. Results: The interactions were significant in the factors of low frequency (LF), high frequency (HF), and root mean square of successive differences (RMSSD). Furthermore, the significantly negative correlations between OC symptoms and the above HRV indexes existed only in subjects with higher SERT availability. Conclusion: OC symptoms might be correlated with ANS regulations in subjects with higher SERT availability.

      • Reorientation of the poly(3-hydroxybutyrate-co-3-hydroxyhexanoate) crystal in thin film induced by polyethylene glycol

        Understanding the crystal structure and thermal behavior of poly(3-hydroxybutyrate-co-3-hydroxyhexanoate) (PHBHx) and its blends thin film under spatial confinement is crucial for its application in molecular scale devices and advanced nanotechnology. In this study, we investigate the influence of polyethylene glycol (PEG) on the morphology and molecular orientation of PHBHx thin films using scanning probe microscope and grazing-incidence wide-angle X-ray scattering. Unexpectedly, we found that with addition of PEG no hydrogen bonds were detectable in the thin film of PHBHx/PEG blend. PEG destroyed the edge-on lamellar orientation of PHBHx leading to the chain axis to pack preferentially in a direction between the out-of-plane and in-plane along with a flat surface. More importantly, analysis of the q space of (020) diffraction indicated that PHBHx crystal tends to form a densely packed lamellae after the addition of PEG. The thermal behaviors of such densely packed lamellae crystals were examined by temperature-dependent infrared-reflection absorbance spectroscopy and two-dimensional correlation analysis. It was revealed that the crystal structure of PHBHx/PEG blends were stable during the melting process despite the decrease of the crystallinity. We speculate that the reorientation and thermal stability of the crystalline PHBHx/PEG blends were due to PEG acting as an effective solvent that not only lowered the surface energy of chain folding surface but also changed the kinetics of PHBHx crystallization. This interesting observation will provide new insights into understanding of the crystallization kinetics of PHBHx thin film under spatial confinement.

        On the Disturbance Rejection Control of Flexible-joint Robot: A GPIO-based Approach

        A robust disturbance rejection control scheme is addressed for the trajectory tracking problem of a flexible-joint robot (FJR). The system is always severely affected by various types of unknown disturbances including model errors, couplings, changing working environments as well as unmodeled dynamics. These disturbances on the link and actuator side will deteriorate the control performance of FJR. By considering all the disturbances as an unknown lumped time-varying disturbance, a flatness description of FJR is developed. Then, a new output feedback controller is constructed through the estimates of unmeasurable states and unknown lumped disturbance provided by a generalized proportional integral observer (GPIO). The stability of the closed-loop system with the driven of the proposed control scheme is guaranteed under some mild assumptions. Compared with the conventional linear active disturbance rejection control (LADRC) scheme, test results are presented to demonstrate the feasibility and efficacy of the proposed control approach.

        <P><B>Summary</B></P><P>After activation, CD4<SUP>+</SUP> helper T (Th) cells differentiate into distinct effector subsets. Although chemokine (C-X-C motif) receptor 5-expressing T follicular helper (Tfh) cells are important in humoral immunity, their developmental regulation is unclear. Here we show that Tfh cells had a distinct gene expression profile and developed in vivo independently of the Th1 or Th2 cell lineages. Tfh cell generation was regulated by ICOS ligand (ICOSL) expressed on B cells and was dependent on interleukin-21 (IL-21), IL-6, and signal transducer and activator of transcription 3 (STAT3). However, unlike Th17 cells, differentiation of Tfh cells did not require transforming growth factor β (TGF-β) or Th17-specific orphan nuclear receptors RORα and RORγ in vivo. Finally, naive T cells activated in vitro in the presence of IL-21 but not TGF-β signaling preferentially acquired Tfh gene expression and promoted germinal-center reactions in vivo. This study thus demonstrates that Tfh is a distinct Th cell lineage.</P>
