      • Myelin basic protein as a binding partner and calmodulin adaptor for the BK<sub>Ca</sub> channel

        <P>The activity and localization of large-conductance Ca<SUP>2+</SUP>-activated K<SUP>+</SUP> (BK<SUB>Ca</SUB>) channels are known to be modulated by several different proteins. Although many binding partners have been identified via yeast two-hybrid screening, this method may not detect certain classes of interacting proteins such as low affinity binding proteins or multi-component protein complexes. In this study, we employed mass spectrometry to identify proteins that interact with BK<SUB>Ca </SUB>channels. We expressed and purified the ‘tail domain’ of the rat BK<SUB>Ca </SUB>channel α-subunit, a 54-kDa region that is crucial for expression and functional activity of the channel. Using rat brain lysate and purified ‘tail domain’, we identified several novel proteins that interact with the BK<SUB>Ca </SUB>channel. These included the myelin basic protein (MBP), upon which we performed subsequent biochemical and electrophysiological studies. Interaction between the BK<SUB>Ca </SUB>channel and MBP was confirmed in vivo and in vitro. MBP co-expression affected the Ca<SUP>2+</SUP>-dependent activation of the BK<SUB>Ca</SUB> channel by increasing its Ca<SUP>2+</SUP> sensitivity. Moreover, we showed that calmodulin (CaM) interacts with the BK<SUB>Ca</SUB> channel via MBP. Since CaM is a key regulator of many Ca<SUP>2+</SUP>-dependent processes, it may be recruited by MBP to the vicinity of the BK<SUB>Ca</SUB> channel, modulating its functional activity.</P>

        A Comparative Study on Alexithymia in Depressive, Somatoform, Anxiety, and Psychotic Disorders among Koreans

        Objective Little is known about the characteristic differences in alexithymic construct in various psychiatric disorders because of a paucity of direct comparisons between psychiatric disorders. Therefore, this study explored disorder-related differences in alexithymic characteristics among Korean patients diagnosed with four major psychiatric disorders (n=388). Methods Alexithymic tendencies, as measured by the Korean version of the 20-item Toronto Alexithymia Scale (TAS-20K), of patients classified into four groups according to major psychiatric diagnosis were compared. The groups consisted of patients with depressive disorders (DP; n=125), somatoform disorders (SM; n=78), anxiety disorders (AX; n=117), and psychotic disorders (PS; n=68). Results We found that substantial portions of patients in all groups were classified as having alexithymia and no statistical intergroup differences emerged (42.4%, 35.9%, 35.3%, and 33.3% for DP, SM, PS, and AX). However, patients with DP obtained higher scores in factor 2 (difficulties describing feelings) than those with SM or AX, after adjusting for demographic variables. Conclusion These findings suggest that alexithymia might be associated with a higher vulnerability to depressive disorders and factor 2 of TAS-20K could be a discriminating feature of depressive disorders.

        섬망 치료에서 Aripiprazole과 Haloperidol의 효과 : 무작위 개방 연구

        Objectives:Delirium is a common condition frequently seen in consultation-liaison psychiatry. Antipsychotic drugs are the primary treatment for symptoms of delirium. Typical antipsychotics, especially haloperidol, have been used as the mainstay of pharmacological treatment of delirium. With the introduction of atypical antipsychotics, the clinical usefulness of these medication has also been investigated for delirious patients. The purpose of this randomized, open-clinical trial is to compare the clinical efficacy and tolerability of haloperidol and aripiprazole for the treatment of delirium. Method:45 patients with a diagnosis of delirium were enrolled between July 2005 and May 2007 in this study. They were randomly assigned to be treated with a flexible-dose regimen of haloperidol or aripiprazole. The Korean version of Delirium Rating Scale(K-DRS) and other measures related with delirium had been used repeatedly until the K-DRS score reached 12 or less which we defined as the criteria for recovery from delirium. 22 subjects in haloperidol group and 19 in aripiprazole group completed the study. Results:The mean dosage of antipsychotics for delirium was 3.0±1.6㎎ of haloperidol and 5.5±1.5㎎ of aripiprazole which was relevant to 150.5±83.6㎎ and 73.7±20.9㎎ of chlorpromazine respectively. Aripiprazole was effective to subside the delirium symptoms at less dosages than haloperidol. There were no significant group differences in the score of K-DRS, K-DRS-R-98 and MMSE-K at baseline as well as final assessments. There was no significant difference in duration of treatment and clinical improvement between the groups. Most of the patients were well tolerated to the adverse events of each antipsychotic drug but only two patients in haloperidol group showed extrapyramidal symptom. Conclusion:Aripiprazole may be more useful than haloperidol in patients with delirium because of its safety and low dose effectiveness, especially in aged patients. Further double-blind placebo-controlled studies with a large sample size will be necessary.

        A report of 20 unrecorded bacterial species isolated from the coastal area of Korean islands in 2022

        Bacterial communities inhabiting islands play a vital role in the functioning and formation of a unique, isolated ecosystem. Nevertheless, there has been a lack of systematic research on the indigenous microbiological resources of the islands in Korea. To excavate microbial resources for further studies on the metabolism and biotechnological potential, a standard dilution plating was applied to coastal seawater samples collected from islands along the west coast of the Korean Peninsula, including Deokjeokdo, Baengnyeongdo, and Daebudo in 2022. A total of 2,007 bacterial strains were isolated from the samples as single colonies and identified using 16S rRNA gene sequence analyses. A total of 20 strains, with ≥98.7% 16S rRNA gene sequence similarity to bacterial species having validly published names but not reported in Korea, were designated as unrecorded bacterial species in Korea. The unrecorded bacterial strains were phylogenetically diverse and belonged to four phyla, five classes, 12 orders, 17 families, and 18 genera. The unreported species were assigned to Algimonas, Amylibacter, Notoacmeibacter, Roseibium, and Terasakiella of the class Alphaproteobacteria; Alteromonas, Congregibacter, Marinagarivorans, Marinicella, Oceanospirillum, Psychromonas, Thalassotalea, Umboniibacter, and Vibrio of the class Gammaproteobacteria; Lutibacter and Owenweeksia of the class Flavobacteriia; Paenibacillus of the class Bacilli; and Pelagicoccus of the class Opitutae. The taxonomic characteristics of the unreported species, including morphology, biochemistry, and phylogenetic position are provided in detail.

      • Traveling-wave antenna for the current drive using the fast wave in the lower hybrid-frequency range in the Versatile Experiment Spherical Torus (VEST)

        <P><B>Abstract</B></P> <P>An antenna using the fast wave in the lower hybrid-resonance frequency range is proposed for the significant attainment of the heating and current-drive efficiencies. For the operational stability regarding the change of the plasma load, the proposed antenna is designed as the combline-structure traveling-wave antenna. It consists of two tapered coaxial cables, two exciting straps of the folded-monopole structure, 12 radiating straps, 11 field isolators, and the Faraday shield. Regarding the vacuum load, the simulated and measured results show the wide impedance bandwidth (45 MHz, VSWR < 2) of the proposed antenna in conjunction with the traveling-wave-antenna structure and the folded-monopole exciting strap. Regarding the plasma load, the simulated results show that the proposed antenna provides the impedance bandwidth (35 MHz, VSWR < 2), and the fast wave is coupled with an input power from 51.5 − 90%. The measured and simulated coupling efficiencies are highly compatible in the Versatile Experiment Spherical Torus (VEST). According to the frequency-range changes, the simulated-variable range of the parallel refractive index (n<SUB>||</SUB>) is from 3 − 4.5 in the operational frequency band of 475–510 MHz.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The development of an efficient external-current-drive device is required. </LI> <LI> The Lower Hybrid Fast Wave can propagate into central region in higher-density plasma. </LI> <LI> The combline-type traveling-wave antenna (TWA) is proposed for the heating and current-drive efficiencies. </LI> <LI> The coupling research between the antenna and simulated plasma model is carried out. </LI> <LI> The simulation and experiment results are compared regarding the coupling efficiency. </LI> </UL> </P>

        청소년 정신과 입원 환자들의 피부과 자문 의뢰에 관한 행태 분석 및 고찰

        연구목적 정신신체의학의 발전으로 최근에는 모든 신체적 질환에 대해 생물정신사회적 접근을 추구하고 있다. 그 중 피부과 영역에서는 원형탈모증, 아토피 피부염, 발모벽, 인공 피부염, 다한증, 만성 담마진, 건선, 심상성 좌창 등의 질환과 정신과적 질환과의 연관성이 특히 강조되며, 정신 피부질환(Psychocutaneous disease)이라는 명칭이 사용되나 아직 이에 대한 연구가 미미하다. 청소년기는 2차 성징으로 인해 많은 피부 증상이 발생할 수 있으며, 이는 자아상 형성에 영향을 미칠 수 있으므로, 청소년의 피부질환에 대한 적절한 중재 및 치료의 중요성이 더욱 강조된다. 이에 저자들은 정신과 청소년 환자들의 피부과적 질환의 특성을 알아보기 위해 정신과에서 피부과로 자문 의뢰되는 청소년 환자들의 자문의뢰의 행태를 분석하고자 하였다. 방 법 대학병원에서 정신과에 입원중인 청소년 환자 중 피부과에 의뢰된 22명의 환자와 타과에서 의뢰된 108명의 환자를 대상으로 연령별 및 성별 분포, 자문율, 이환된 피부질환, 자문이유를 비교 분석하였다. 결 과 정신과 입원 환자 중 피부과로 자문 의뢰된 환자들의 평균나이는 15.9세로 관찰되었으며, 남성과 여성의 비율은 1:1.44로 관찰되었다. 정신과에서 의뢰된 청소년 환자들의 진단의 경우 정신과적으로는 기분장애(59.1%), 피부과적으로는 여드름(40.9%)이 가장 많았다. 타과 청소년에 비해 정신과 청소년 환자의 피부과로의 자문율이 유의하게 높았고(11.8% vs. 1.5%), 환자의 요청에 의해 자문이 의뢰되는 비율도 높은 것으로 나타났다(49.9% vs. 16.7%). 결 론 다양한 정신병리적인 문제와 피부과적 질환이 연관되어 환자에게 영향을 미칠 수 있다. 피부과에서는 환자의 거부나 의사의 항정신약물 사용 시작에 불편감을 이유로 질환 치료가 지연될 수 있고, 정신과에서는 피부 문제를 경미한 증상으로 생각하여 간과할 수 있으므로, 두 과 간의 적절한 정보 교환 및 협진 체계를 마련하여 생물정신사회적 접근이 필요할 것으로 보인다. Objective:To review the patterns of the dermatologic consultations of psychiatric adolescent inpatient and to explore the relationship between the dermatologic disorders and psychiatric disorders. Methods:We retrospectively studied the data from 22 cases referred by psychiatric adolescent for a dermatologic consultation over 10 years in Daegu Catholic University Medical Center and compared with the data from 108 cases referred by the other department adolescent patients. Results:The mean age of patients was 15.9. The male to female ratio was 1:1.44. The most common psychiatric and dermatologic disorder was major depressive disorder and acne, respectively. The most frequent reason for consultation was to ask for dermatologic disease or condition(54.5%) followed by to perform cosmetic procedure of patients need(40.9%) and to perform dermatologic test(4.6%). Conclusions:More than just a cosmetic disfigurement, dermatologic disorders are associated with a variety of psychopathologic problems that can affect the patient. Increased understanding of biopsychosocial approaches and liaison among psychiatrists and dermatologists could be beneficial.

        The Effects of Temperature on Heat-related Illness According to the Characteristics of Patients During the Summer of 2012 in the Republic of Korea

        Objectives: This study was conducted to investigate the relationship between heat-related illnesses developed in the summer of 2012 and temperature. Methods: The study analyzed data generated by a heat wave surveillance system operated by the Korea Centers for Disease Control and Prevention during the summer of 2012. The daily maximum temperature, average temperature, and maximum heat index were compared to identify the most suitable index for this study. A piecewise linear model was used to identify the threshold temperature and the relative risk (RR) above the threshold temperature according to patient characteristics and region. Results: The total number of patients during the 3 months was 975. Of the three temperature indicators, the daily maximum temperature showed the best goodness of fit with the model. The RR of the total patient incidence was 1.691 (1.641 to 1.743) per $1^{\circ}C$ after $31.2^{\circ}C$. The RR above the threshold temperature of women (1.822, 1.716 to 1.934) was greater than that of men (1.643, 1.587 to 1.701). The threshold temperature was the lowest in the age group of 20 to 64 ($30.4^{\circ}C$), and the RR was the highest in the ${\geq}65$ age group (1.863, 1.755 to 1.978). The threshold temperature of the provinces ($30.5^{\circ}C$) was lower than that of the metropolitan cities ($32.2^{\circ}C$). Metropolitan cities at higher latitudes had a greater RR than other cities at lower latitudes. Conclusions: The influences of temperature on heat-related illnesses vary according to gender, age, and region. A surveillance system and public health program should reflect these factors in their implementation.
