      • Prevalence and Risk Factors of Atrophic Gastritis and Intestinal Metaplasia in a Korean Population Without Significant Gastroduodenal Disease

        <P>BACKGROUND AND AIM: The prevalence of gastric cancer and Helicobacter pylori infection is unacceptably high in Korea. This study was performed to evaluate the prevalence of atrophic gastritis (AG) and intestinal metaplasia (IM) and to identify their risk factors with respect to H. pylori virulence factors, and environmental and host factors, in Korean population without significant gastroduodenal disease. METHODS: The study cohort consisted of 389 subjects (> or = 16 years). AG and IM were scored histologically using the Sydney classification in the antrum and body, respectively. Prevalences and bacterial factors (i.e. cagA, vacA m1, and oipA), environmental factors (i.e. smoking and alcohol), and host factors (i.e. genetic polymorphisms of IL-1B-511, IL-1RN, TNF-A-308, IL-10-592, IL-10-819, IL-10-1082, IL-8-251, IL-6-572, GSTP1, p53 codon 72, and ALDH2) were evaluated. RESULTS: Prevalences of AG in the antrum and body were 42.5% and 20.1%, and those of IM were 28.6% and 21.2%, respectively. The presences of AG and IM were significantly higher in H. pylori-positive than in the H. pylori-negative subjects. Multivariate analysis showed that the risk factors for AG were H. pylori infection, age > or = 61 years, and cagA and vacA m1 positivity. For IM the risk factors were H. pylori infection, age > or = 61 years, a smoking history (rather than current smoking), strong spicy food, occupation (unemployed or nonprofessional vs. professional), and the presence of IL10-592 C/A as opposed to A/A. In addition, IL6-572 G carrier was found to have a protective effect against IM development as compared with C/C. CONCLUSION: H. pylori infection was most important risk factor of AG and IM. Bacterial factors were found to be important risk factor for AG but environmental and host factors were more important for IM.</P>

        To evaluate antibiotic-induced endotoxin release(AIER) and its correlation with some cytokines, we measured endotoxin level and tumor necrosis factor alpha(TNF-α) and interleukin6(IL-6) production in mononuclear cells in vitro after exposure of Pseudomonas aeruginosa to antibiotics belonging to different class with two extreme concentrations. The tested concetration of antibiotics were set up according to peak serum level. The low concetration of ceftazidirne and low concentration of imiperiem increased AIER, but high concentration of ceftazideme, high concentration of ciprofloxacin, high concentration of cefoperazone/sulbactam, high concentration of amikacin, and high concentration of meropenem reduced AIER.Interestingly, combined treatment of these antibiotics markedly reduced AIER, But the major cyotkines, TNF-α and IL-6 were not affect by type and concettration of antibiotics, combined treatment of antibiotics, and level of endotoxin released by antiboitics. In this study, we observed AIER was different according to type of antibiotics, concentration of antibiotics, and combination of antibiotics, But AIER had poor correlation with TNF-α and IL-6 in Pseudomonas aeruginosa. It suggests that cytokine release is not solely dependent to endotoxin, but more complex cascade is needed. More invesfigations, such as endotoxin induced cytokine mRNA expression, relationship with penicillin-binding proteins and endotoxin-neutralizing effect of antibiotic itself, must be performed.

      • Characterization of effector memory CD8+ T cells in the synovial fluid of rheumatoid arthritis.

        <P>Little is known about the cellular characteristics of CD8(+) T cells in rheumatoid arthritis (RA). We addressed this by investigating whether the frequency of the CD8(+) T cell subsets and their phenotypic characteristics are altered in the peripheral blood and synovial fluid (SF) from patients with RA. In this study, CD8(+) T cells, mainly CD45RA(-) effector memory (EM) CD8(+) T cells, were increased significantly in the SF, but not in the peripheral blood from RA patients, compared with healthy controls. The synovial EM CD8(+) T cells were activated phenotypes with high levels of CD80, CD86, and PD-1, and had a proliferating signature in vivo upon Ki-67 staining, whereas the Fas-positive cells were prone to apoptosis. In addition, EM CD8(+) T cells in the SF were less cytotoxic, as they expressed less perforin and granzyme B. In particular, the proportions of synovial fluid mononuclear cells that were CCR4(+)CD8(+) T cells and IL-4-producing CD8(+) T cells (i.e., Tc2 cells) were significantly higher than those in peripheral blood mononuclear cells of patients with RA and healthy controls. In addition, the number of IL-10-producing CD8(+) suppressor T (Ts) cells increased significantly in the SF of RA patients. Especially, CD8(+) T cells were inversely correlated with disease activity. These findings strongly suggest that EM CD8(+) T cells in the SF are increased, likely because of inflammation, and they may be involved in modulating inflammation, thereby affecting the development and progression of RA.</P>

      • Therapeutic effect of a novel histone deacetylase 6 inhibitor, CKD-L, on collagen-induced arthritis in vivo and regulatory T cells in rheumatoid arthritis in vitro

        <P><B>Background</B></P><P>Histone deacetylase (HDAC) inhibitor has recently been reported to have a therapeutic effect as an anti-inflammatory agent in collagen-induced arthritis (CIA). We investigated the therapeutic effect of a new selective HDAC6 inhibitor, CKD-L, compared to ITF 2357 or Tubastatin A on CIA and regulatory T (Treg) cells in patients with rheumatoid arthritis (RA).</P><P><B>Methods</B></P><P>CIA was induced by bovine type II collagen (CII) in DBA/1 J mice. Mice were treated with HDAC inhibitor for 18 days. Arthritis score was assessed and histological analysis was performed by hematoxylin and eosin (H&E) stain. Cytotoxic T-lymphocyte associated protein (CTLA)-4 expression in induced Treg cells was analyzed and suppression assay was analyzed using Treg cells and effector T (Teff) cells isolated from naive C57BL/6 mice by flow cytometry. Cytokines were analyzed in peripheral blood mononuclear cells (PBMC) of five patients with RA by enzyme-linked immunosorbent assay (ELISA) and real-time polymerase chain reaction (PCR). Tumor necrosis factor (TNF) was analyzed using PMA- activated THP-1 cells by ELISA. Suppression assay was analyzed using Treg cells and Teff cells isolated from RA patients by flow cytometry.</P><P><B>Results</B></P><P>In the CIA model, CKD-L and Tubastatin A significantly decreased the arthritis score. CKD-L increased CTLA-4 expression in Foxp3<SUP>+</SUP> T cells and inhibited the proliferation of Teff cells in the suppression assay. In RA PBMC, CKD-L significantly inhibited TNF and interleukin (IL)-1β, and increased IL-10. CKD-L and Tubastatin A inhibited TNF secretion from PMA-activated THP-1 cells. CKD-L and ITF 2357 inhibited the proliferation of Teff cells in RA patients in the suppression assay. Tubastatin A had no effect on inhibition of proliferation.</P><P><B>Conclusion</B></P><P>CKD-L decreased the arthritis score in CIA, reduced the expression of TNF and IL-1β, and increased the expression of IL-10 in PBMC from RA patients. CKD-L increased CTLA-4 expression and the suppressive function of Treg cells. These results suggest that CKD-L may have a beneficial effect in the treatment of RA.</P>

        Immunomodulating Activity of a Fucoidan Isolated from Korean Undaria pinnatifida Sporophyll

        A fucoidan, isolated from Korean Undaria pinnatifida spoprophyll (UP-F), was investigated for its immunomodulating activity on murine macrophages and splenocytes, and its activity was compared with that of fucoidan from Fucus vesiculosus (FV-F). Treatment of UP-F resulted in inhibition of the growth of murine macrophage RAW 264.7 cells, but its cytotoxicity was not observed in normal murine splenocytes. FV-F was shown to be highly cytotoxic to both immune cells, and its cytotoxic activity was higher than that of UP-F. Treatment of UP-F induced TNF-α in a dose-dependent manner from two types of macrophages, RAW 264.7 cells and murine peritoneal macrophages. The TNF-α-inducing activity of UP-F was higher than that of FV-F. UP-F also actively induced chemokines (RANTES and MIP-1α) from RAW 264.7 cells. Furthermore, treatment of UP-F gave rise to activation of murine splenocytes to produce cytokine (IL-6) and chemokines (RANTES and MIP-1α), showing significantly higher activity than that of FV-F. These results indicate that UP-F is less cytotoxic to immune cells than FV-F, and possesses immunomodulating activity to produce cytokines and chemokines from macrophages and splenocytes.

        t-Butyldimethylsilyl triflate 존재에서 3-Cyano 및 3-Carbomethoxy Chromone(1a,b)과 Allenylstannane(2) 과의 반응에서 Chromone의 C-2위치에 propargyl기가 도입된 유도체(3a,b)를 합성하였다. 합성한 유도체의 상대적 구조는 ¹H-NMR을 통해 분석하였으며, T.S구조의 모델을 통하여 가능한 메카니즘을 제시하였다. Reaction of 3-Cyano and 3-Carbomethoxy Chromone(1a,b) with Allenylstannane(2) in the presence of tert-butyldimethylsilyl triflate afforded the corresponding the propargylation product(3a,b) in the moderate yield. The relative stereochemistry of 3a,b are obtained on the basis of ¹H-NMR analysis. And the possible reaction mechanism are suggested by the molecular models in the transition state.

      • Bismuth Germanate를 利用한 閃光檢出器의 分光特性

        The response of a 32×15×100㎣ bismuth germanate scintillator to gamma radiation from 0.122 to 1.332 MeV has been measured. The energy resolution of the bismuth germanate detector for ^60Co gamma rays was 14.5% at 1.17 MeV and 13.8% at 1.33 MeV. The bismuth germanate scintillator was coated with aluminium reflector and coupled to the RCA 8575 photomultiplier tube with silicon optical grease. The high detection efficiency of this material, together with its physical ruggedness and nonhygroscopicity, make it a strong candidate for application in radiation detector fields.

        Background: Most cases of Graves' disease have been treated by antithyroid drugs in Korea. The major drawbacks of antithyroid drugs are the relative high rate of relapse and the inconvinience of long term therapy. Measurement of TBII and TRH stimulation test have been considered as the most reliable tests predicting relapse, but the predictive power is not high enough to use in clinical situation. This study was performed to find good prognostic indicators in Graves' disease patients after the discontinuation of antithyroid drug therapy. Subjects and methods : We restrospectively evaluated 96 patients with Graves' disease who showed normal TRH test at the end of antithyroid drug therapy and were followed for more than one year. Serum T3, T4, T3/T4 ratio, TBII, Anti-TPO Ab and TGAb levels were measured at the time of diagnosis and at the end of therapy. Two to three months after withdrawal of antithyroid medication, serum TSH and free T3 responses to TRH were examined. These parameters were compared between the relapse group and the remission group. Results : Among the 96 patients, 22 patients(22.9%) relapsed(Group Ⅱ) and 74 patients(77.1%) remained in remmision(Group Ⅰ). No significant difference was observed between the relapse and the remission group in clinical parameters, serum T3, T4, TSH, T3/T4 ratio, TBⅡ and Anti-TPO Ab determined before and after treatment. Serum TSH and free T3 levels measured after TRH stimulation were also similar in both groups. Serum TGAb levels determined at the end of treatment were significantly higher in the remission group(p<0.05). Relapse following the discontinuation of therapy occured within 24 months in 72.7% of the relapsed cases. After 24 months, relapse rate was reduced significantly. Conclusion : These results suggest that high serum TGAb levels could be a favorable prognostic indicator for the long term remission of Graves' disease treated with antithyroid drugs, and may suggest that Hashimoto's thyroiditis is combined with Graves' disease in those patients.

        한국어 판 WHO-CIDI(Composite International Diagnostic Interview)-Alcohol의 개발:신뢰도 및 타당도 검사

        CIDI-Alcohol의 한국어 판 개발을 위해 이를 우리말로 번역, 수정하고, 정신병원에 입원한 환자 90명과 일반인 50명에게 시행한 후 임상 진단과 비교하여 일치도를 봄으로써 그 신뢰도 및 타당도를 조사하였다. 결과는 다음과 같다. 1) 주정 사용 각 진단 범주에 대한 한국어 판 CIDI-Alcohol의 일반 면담자간 진단 일치도는 97%∼100%의 높은 일치도를 보였다. 2) 임상 진단을 기준으로 한 한국어 판CIDI-Alcohol의 주정 사용 장애의 진단 일치도는 각 진단 범주별로 kappa치가 0.43∼0.55로 기존의 연구에 비해 비교적 낮았다. 3) CIDI-Alcohol의 진단 기준을 변화시켜, 각 진단 범주에 대한 임상 진단과의 일치도를 알아본 결과 ICD-10/harmful use는 진단 기준 항목의 수를 2개 이상으로 하였을 때, DSM-Ⅲ-R/주정 의존은 진단 기준 항목의 수를 4개 이상으로 하였을 때 임상 진단과 가장 일치하였다. 이런 결과는 한국어 판 CIDI-Alcohol의 진단 기준이 대체적으로 민감도는 높으나, 특이도가 떨어지는 것으로 생각할 수 있으며, 따라서 본 연구자들에 의해 작성된 한국어 판 CIDI-Alcohol는 일반인구 집단을 대상으로 하는 역학 조사보다는 주정 사용 장애로 이루어진 순수한 실험군의 연구에 적합하다고 생각된다. he authors translated a CIDI-Alcohol, a highly structured diagnostic interview, into Korean and examined the validity and reliability of the Korean version of CIDI-Alcohol. In order to examine the validity and reliability of CIDI-Alcohol, the authors calculated the degree of agreement of CIDI-Alcohol diagnoses between the two lay interviewers and sensitivity, specificity, kappa coefficient between clinical and lay CIDI-Alcohol diagnoses. The results were as follows: 1) Agreement rates of CIDI-Alcohol diagnoses between the two lay interviewers ranged from 97% to 100%. The kappa coefficient for each diagnosis was quite high(0.96-1.0). 2) The kappa value of agreement between clinical and lay CIDI-Alcohol diagnoses ranged from 0.43 to 0.55, which was lower than those of other studies. 3) With canging the diagnostic criteria of CIDI-Alcohol, The highest agreement rate between clinical and CIDI-Alcohol diagnoses was acquired when the authors used two or more diagnostic criteria of ICD-10/harmful use and four or more diagnostic criteria of DSM-Ⅲ-R/ alcohol dependence.
