      • 1. 06㎛ Nd : YAG Laser를 이용한 요추간판탈출증의 경피적 수핵감압술

        The 1.06㎛ Neodymium-YAG Laser(800 micron hemisherical) is revealed a useful tool for percutaneous laser disc decompression(PLDD). Especially PLDD would be applied to the management of lumbar disc herniation with minimal intervention. Total irradiated energy to one level of disc space was bout 1000 to 2000 joules. We performed this method at 10 levels, in 7 cases of patients, consisting of 4 males and 3 females with an average age of 37.7 years. Five cases showed excellent and good results, were observed. In two cases, open surgery was required because of unsatisfactory results. The best results were obtained in the protrusion type of disc herniation. We have summarized some advantages of PLDD: a) safe and simple procedure in an outpatient setting, b) avoiding general anesthesia, late spinal instability, c) possibility of repetitive use.

        입원환자를 대상으로 한 대집단 싸이코드라마의 치료요인 연구

        연구목적 : 본 연구는 개인정신치료와 집단정신치료의 훈련을 받은 정신과 의사라면 누구든지 쉽게 이용할 수 있는 우리나라 입원환자에게 적합한 대집단 싸이코드라마의 모델을 개발하고, 이를 입원환자에게 적용한 후 치료요인을 조사하여, 입원환자 치료에 도움이 되고자 하는 목적으로 본 연구를 시행하였다. 방 법 : 본 연구는 1998년 3월부터 1998년 9월까지 계요병원 입원환자 싸이코드라마에 참가한 모든 환자들과, 보조자아로 참석한 모든 치료팀을 대상으로 하였다. 대상자는 환자가 346명(남자 152명, 여자 194명)이었고, 치료팀은 총 60명이었다. 환자에게는 13가지 치료요인 척도, 싸이코드라마에 대한 일반적인 태도 척도, 도움이 되었던 점에 대해 기술하도록 한 설문지를 사용하였으며, 치료팀에게는 13가지 치료요인과 환자에게 도움이 되었던 점에 대해 기술하도록 한 설문지를 사용하였다. 환자집단은 관객, 보조자아의 역할을 한 환자, 공유(sharing)시간에 참여한 환자, 주인공으로 나누어 치료요인을 비교하였다. 결 과 : 먼저, 환자의 관점에서 본 치료요인은 다음과 같다. 1) 싸이코드라마에서의 역할에 관계없이 거의 대부분의 환자집단에서 실존적 인지와 일차가족집단의 교정적 감정경험을 매우 중시하였다. 2) 주인공, 보조자아의 역할을 한 환자, 공유시간에 참여한 환자와 같이 어떤 형태로든 싸이코드라마에 직접적으로 참여한 환자들은 사회화 기술의 발달을 중시하였으며, 보다 직접적인 형태인 행동으로 참가한 주인공이나 보조자아의 경우 카타르시스를 중시하였다. 3) 주인공을 제외한 모든 환자들은 희망의 고취를 중시하였다. 4) 관객의 경우 치료자와 환자에 대한 동일시와 보편성을 중시하였다. 환자와의 동일시는 공유에 참가한 환자의 경우 특히 중시하였고, 보편성은 보조자아로 참가한 환자의 경우 또한 중시하였다. 다음으로 치료팀이 평가한 치료요인을 환자들이 평가한 치료요인과 비교하면, 치료팀은 환자들보다 집단응집력을 더 중시하였으며, 반면 환자들이 중시한 사회화 기술의 발달과 치료자나 환자에 대한 동일시는 매우 낮게 평가하였다. 결 론 : 환자들의 역할에 따라 세분화하여 조사한 치료요인을 통해 입원환자 대집단 싸이코드라마의 특성과 환자의 역할에 따른 치료요인의 특성을 파악할 수 있었다. 본 모델은 입원환자 치료에 쉽게 적용될 수 있었으며, 전체적으로 많은 환자들에게 도움이 되었다. Objectives : This study was performed to make a model of large group psychodrama suitable for Korean inpatients and easy for psychiatrists trained in the individual psychotherapy and the group psychotherapy to use, to investigate the therapeutic factors after applying this model to inpatients, and then to contribute to the treatment of inpatients. Methods : The study subjects were all the inpatients and the therapists(the auxiliary ego's) who participated in psychodrama from March 1998 to September 1998. The subjects were composed of 346 inpatients (152 males, 194 female) and 60 therapists. To the inpatients 13-Therapeutic Factors Scale, Scale of General Attitude to Psychodrama, and the Questionnaire describing helpful things in psychodrama were administerd. The inpatient group were divided into 4 groups(the audience, the auxiliary ego's, the participants in sharing, the protagonists). The therapeutic factors among 4 groups were compared. Results : First, the therapeutic factors described by the inpatients group were as follows : 1) Most inpatients made great account of ‘existential factor’ and ‘the corrective recapitulation of the primary family group’ regardless of their roles in psychodrama. 2) The inpatients who participated in psychodrama directly as the protagonists, the auxiliary ego's, or the participants in sharing made much account of ‘development of socializing techniques’. The inpatients who participated in psychodrama more directly as the protagonists or the auxiliary ego's made much account of ‘catharsis’. 3) All the inpatients except the protagonists made much account of ‘instillation of hope’. 4) The audience made much account of 'identification' and 'universality'. Especially,'identification with members' was much accounted of by the participants in sharing. ‘Universality’ also was much accounted of by the auxiliary ego's. Next, when we compared the therapeutic factors in the inpatient group with those in the therapist group, the therapist group made much account of ‘group cohesiveness’ which the inpatient group made little account of. And the therapist group made no account of ‘development of socializing techniques’which the inpatient group made much account of. Conclusion : By dividing the inpatient group according to their roles, we found the characteristics of the inpatient large group psychodrama and of the therapeutic factors according to the roles of the inpatients. This model could be applied to the treatment for the inpatients easily and was helpful to many inpatients on the whole.

        Locoregional recurrence is the most common type of recurrence in surgical operation of gastric adenocarcinoma, and peritoneal dissemination is one of the most difficult problems in advanced gastric adenocarcinoma treatment. Because the peritoneal cavity is the most common site of the first recurrence after gastric cancer resection, intraperitoneal chemotherapy seems a logical choice for cancer chemotherapy. In this study, Mitomycin C (MMC)-activated charcoal (CH)-alginate (ALG) beads were prepared by the mixtures of CH particles adsorbed with MMC as an anti-cancer drug and aqueous alginate solution. The alginate is recognized as biodegradable, nontoxic, and biocompatible. The release of MMC from the beads in 0.1 M Tris buffer was stable and continuous until about 1 week. The MMC-CH-ALG beads can be applied in the peritoneal cavity for intraperitoneal chemotherapy since they provide a good adhesiveness on the tissue and controlled release pattern of the drugs.

        A common form of relapse in adenocarcinoma of the stomach is intraperitoneal dissemination, in fact, among gastric adenocarcinoma patients who have undergone surgery intended to cure, approximately 50% of the patients develope initial recurrence in the peritoneal cavity regardless of the anatomic site of the primary tumor within the stomach. The efficacy of systemic postoperative chemotherapy to prevent peritoneal recurrence of gastrric adcnocarcinoma is not satisfactory. There is still a great need for improved therapeutic strategies on the disseminated microscopic disease and small miliary nodules remaining on the peritoneal surface or lymphatics after operation. The authers have made fluorouracil-polyglycolic acid composite disks(Fu-PGA disks) with fluorouracil and biodegradable polymer: polyglycolic acid for more effective intraperitoneal chemotherapy. We inserted the Fu-PGA disk(s) in the peritoneal cavity of rat and pharmacokinetic study was performed to measure fluorouracil concentration in the peritoneal fluid, plasma, liver, kidney and heart tissue at 24 hour, 72 hour and 168 hour after insertion of Fu-PGA disk(s). Myelosuppressive action of this composite also was determined following its administration. The data of this study suggested that Fu-PGA composite will be a new device releasing drugs in a controlled manner and having targetability to peritoneum, and this device will be improving the efficacy of intraperitoneal chemotherapy for gastric adenocarcinoma.

Because the peritoneal cavity is the most common site of the first recurrence after gastric cancer resection, intraperitoneal chemotherpy seems a logical choice for cancer chemotherapy. The Mitomycin C(MMC) adsorbed by the activated charcoal particles(CH) is relatively released when the drug concentration surrounding the carbon particles becomes low in the peritoneum of the peritoneal cavity. For the intraperitoneal chemotherapy on the advanced gastric adenocarcnoma, mitomycin C adsorbed on activated carbon particles was administered in the peritoneal cavity just before abdominal wall closure. The closed drainage tubes were inserted in the peritoneal cavity and clamped for tuo hours after completion of operation. MMC concentrations were serially measured in peritoneal fluid, plasma and urine at 2hour, 48 hour, 72 hour and 168 hour following its administration in order to study the efficacy of the MMC-CH as a drug delivery system. There were minimal toxicities in born marrow, liver, and gastrointestinal system after intraperitoneal MMC-CH administration. The data of this study suggested that MMC-CH may have a somewhat more beneficial effect than surgery alone when administered in optimal dose and schedules, but the MMC concentration of the peritoneal fluid was not sufficient to eradicate remnant cancer cells, and effective duration of maintenance was only below 24 hours in the peritoneal fluid and plasma.

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

        To evaluate the in-vivo effects of mitomycin-C-polyglycolic acid composite(MMC-PGA) the disk: shaped composite were implanted into the peritoneal cavity of the Sprague-Dawley rats. The pathologic changes were examined at various time points up to 12 weeks. Initially the soft tissue around the inserted disks showed capillary congestion at 3 days. Inflammatory infiltrates with foreign body giant cells appeared from the 2nd week and reached peak response at 6-8 weeks. These reaction diminished prominently at 12 weeks. No specific pathologic change was found in the liver, the kidneys, and the heart. The above results suggest that the MMC-PGA composite can serve as a new device for intraperitoneal chemotherapy of various types of cancers.

        <P><B>Abstract</B></P> <P>Tumor Necrosis Factor-α (TNF-α) is a proinflammatory cytokine implicated in neuronal damage in response to cerebral ischemia. Ischemic preconditioning (IPC) provides neuroprotection against a subsequent severer or longer transient ischemia by ischemic tolerance. Here, we focused on the role of TNF-α in IPC-mediated neuroprotection against neuronal death following a subsequent longer transient cerebral ischemia (TCI). Gerbils used in this study were randomly assigned to eight groups; sham group, TCI operated group, IPC plus (+) sham group, IPC + TCI operated group, sham + etanercept (an inhibitor of TNF-a) group, TCI + etanercept group, IPC + sham + etanercept group, and IPC + TCI + etanercept group. IPC was induced by a 2-min sublethal transient ischemia, which was operated 1 day prior to a longer (5-min) TCI. A significant death of neurons was found in the stratum pyramidale (SP) in the CA1 area (CA1) of the hippocampus 5 days after TCI; however, IPC protected SP neurons from TCI. We found that TNF-α immunoreactivity was significantly increased in CA1 pyramidal neurons in the TCI and IPC + TCI groups compared to the sham group. TNF-R1 expression in CA1 pyramidal neurons of the TCI group was also increased 1 and 2 days after TCI; however, in the IPC + TCI group, TNF-R1 expression was significantly lower than that in the TCI group. On the other hand, we did not detect TNF-R2 immunoreactivity in CA1 pyramidal neurons 1 and 2 days after TCI; meanwhile, in the IPC + TCI group, TNF-R2 expression was significantly increased compared to TNF-R2 expression at 1 and 2 days after TCI. In addition, in this group, TNF-R2 was newly expressed in pericytes, which are important cells in the blood brain barrier, from 1 day after TCI. When we treated etanercept to the IPC + TCI group, IPC-induced neuroprotection was significantly weakened. In brief, this study indicates that IPC confers neuroprotection against TCI by TNF-α signaling through TNF-R2 and suggests that the enhancement of TNF-R2 expression by IPC may be a legitimate strategy for a therapeutic intervention of TCI.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Ischemic preconditioning (IPC) protects CA1 pyramidal neurons from ischemic damage. </LI> <LI> IPC attenuates TNF-α and TNF-R1 expressions in ischemic CA1 pyramidal neurons. </LI> <LI> IPC increases TNF-R2 expressions in pericytes in CA1 area after ischemic insult. </LI> <LI> IPC-mediated neuroprotective effect is reversed by etanercept (an inhibitor of TNF-a) treatment. </LI> </UL> </P>

      • Age-dependent decreases in insulin-like growth factor-I and its receptor expressions in the gerbil olfactory bulb

        <P>Insulin-like growth factor-I (IGF-I) is a multifunctional protein present in the central nervous system. A number of previous studies have revealed alterations in IGF-I and its receptor (IGF-IR) expression in various regions of the brain. However, there are few reports on age-dependent alterations in IGF-I and IGF-IR expressions in the olfactory bulb, which contains the secondary neurons of the olfactory system. The present study examined the cellular morphology in the olfactory bulb by using cresyl violet (CV) staining at postnatal month (PM) 3 in the young group, PM 6 in the adult group and PM 24 in the aged group in gerbils. In addition, detailed examinations were performed of the protein levels and immunoreactivities of IGF-I and IGF-IR in the olfactory bulb in each group. There were no significant changes in the cellular morphology between the three groups. The protein levels and immunoreactivities of the IGF-I and IGF-IR were the highest in the young group and they decreased with age. He protein levels and immunoreactivities of the IGF-I and IGF-IR were the lowest in the aged group. In brief, our results indicate that IGF-I and IGF-IR expressions are strong in young olfactory bulbs and significantly reduced in aged olfactory bulbs. In conclusion, subsequent decreases in IGF-I and IGF-IR expression with age may be associated with olfactory decline. Further studies are required to investigate the roles of IFG-I and IGF-IR in disorders of the olfactory system.</P>
