        디스크 전지를 삼킨 소아에서 내시경적 이물 제거술의 비교

        목 적: 소아에서 디스크 전지를 삼키는 사고가 증가하고 있다. 식도에 걸린 경우 식도 궤양, 천공, 누공, 종격동염 등 심각한 합병증을 유발할 수 있으므로 즉각 제거해야 한다. 본 연구에서는 디스크 전지의 내시경적 제거에 있어서 기구에 따른 제거 속도의 차이와 안전성을 비교해 봄으로써 가장 빠르고 효율적으로 제거할 수 있는 방법을 찾아보고자 하였다. 방 법: 1997년 7월부터 2007년 6월까지 디스크 전지를 삼키고 경북대학교병원 소아과에 내원하여 내시경적 적출술을 시도한 24명의 환아를 대상으로 하였다. 평균 나이는 1.5세였고 남아가 9명이었다. 내시경 보조기구로써 magnetic extractor와 풍선을 이용하거나 retrieval net을 사용한 경우를 별도로 비교하여 제거에 걸린 시간과 안전성에 차이가 있는지 검토하였다. 결 과: 전지의 직경과 두께가 $20{\times}2$ mm (또는 2 mm) 인 것이 5건이었고 이 중 3예(60%)에서 식도에 걸려 식도 점막 괴사 또는 궤양을 초래하였다. 직경 20 mm 디스크 전지는 직경 10 mm 전지에 비해 식도에 걸려 손상을 초래하는 경우가 유의하게 많았다(p<0.01). 내시경적 제거방법으로 retrieval net을 사용한 경우가 10명으로 평균 1.5분이 소요되었으며 magnetic extractor와 풍선(또는 condom)을 이용한 경우가 6명이었는데 평균 소요 시간이 3.3분 걸려 retrieval net을 사용할 경우 더욱 신속한 제거가 가능하였다(p<0.05). 결 론: 식도의 디스크 전지는 내시경적 응급 상황으로 즉시 내시경으로 신속하고 안전하게 제거할 수 있도록 조치하여야 한다. 저자 등의 경험에 의하면 retrieval net을 이용한 위장내 이물 제거가 더 효과적이었으나, 시술자의 숙련도와 선호도에 따라 magnetic extractor와 풍선을 이용한 방법 또는 retrieval net을 이용한 제거 등을 선택하여 사용할 수 있을 것이다. Purpose: Any battery impacted in the esophagus must be removed urgently because of the possibility of serious complications such as perforation, fistula, and mediastinitis. The use of endoscopic procedures was compared to find the most rapid, effective, and safe method to remove disk batteries. Methods: The cases of 24 children (9 males, mean age, 1.5 years) that had accidentally ingested disk batteries were reviewed. The patients had visited the Department of Pediatrics, Kyungpook National University Hospital from July 1997 to June 2007 for the removal of disk batteries. Endoscopic removal was attempted using a retrieval net or a magnetic extractor with a balloon (condom). The procedure times for removing the ingested battery were compared. Results: Children that ingested batteries with a larger diameter (20 mm) had a greater chance to have esophageal impaction with serious injury, such as esophageal mucosal necrosis or ulcers, as compared to the ingestion of smaller diameter batteries (10 mm) (p<0.01). Endoscopic removal of disk batteries was attempted either using a retrieval net in 10 children or by using a magnetic extractor with a balloon in 6 children. Endoscopic removal using a retrieval net was more effective (mean procedure time: 1.5 min) as compared to using a magnetic extractor with a balloon (mean procedure time: 3.3 min) (p<0.05). Conclusion: Both procedures, either using a retrieval net or a magnetic extractor with a balloon were effective in removing ingested disk batteries. Based on our experience, an endoscopic procedure using a retrieval net was a more effective method for foreign body removal.

        Effects of Supplemental Fermented Agro By-products Diet on the Growth Performances, Blood Characteristics and Carcass Traits in Fattening Pigs

        Effects of a supplemental fermented agro by-products diet on growth performance, blood characteristics and carcass traits were investigated in fattening pigs. The fermented diet mainly contained 38.0% brewer's grain, 25.0% rice bran and 21.0% byproduct of king oyster mushrooms. The mixed ingredients were fermented at $40^{\circ}C$ for 7 days and fed to crossbred barrow pigs for 62 days. Ninety pigs were housed in 10 head per pen with three replicate pens per treatment. The pigs in the control group were fed with formula feed, while the pigs in T1 group were fed 20, 40, 60, 80 and 100% fermented diet substituted with formula feed on 1 week interval. Pigs in T2 group were fed 30, 60 and 100% fermented diet substituted with formula feed on 1 week interval. The fermented diet significantly (p<0.05) decreased body weight gain and feed efficiency of pigs. The blood characteristics differed with diet types. Carcass grade was significantly better (p<0.05) in the pigs fed fermented diet than in the pigs fed control diet as well as ratio of high grade was higher in the fermented diet groups. Therefore, although a dietary of fermented diet decreased growth performance and feed efficiency, it improved the carcass grade in pigs.

        자유학기제 참여 학생의 발달 궤적과 영향 요인 분석: 진로체험의 참여 횟수와 만족도를 중심으로

        본 연구는 2017년 중학교 1학년을 대상으로 1학기 초, 말, 2학기 겨울 방학 시작 직전의 3개 시점에서 학생의 행복도, 학교생활만족 및 진로성숙의 하위 변인의 변화를 추적, 관찰하여 학생 발달 궤적을 분석하고자 하였다. 그리고 자유학기제 시행 이후 활발히 수행되고 있는 진로체험의 참여 횟수와 만족도가 이러한 학생 발달 궤적에 미치는 영향을 분석하여 자유학기제 및 진로체험 활동에 관한 시사점을 도출하고자 하였다. 연구 결과, 연구 변인에 따른 다양한 변화 모형을 도출하였고, 진로체험활동 참여 횟수는 진로포부, 합리적 의사결정의 초기치에 영향을 미치고, 변화율에는 통계적으로 유의미한 영향을 미치지 않는 것을 확인하였다. 진로체험활동 만족도는 학교생활만족, 상호협조성, 진로포부, 자기이해의 초기치와 변화율에 통계적으로 유의미한 영향을 미치는 것으로 나타났다. 이러한 연구 결과를 토대로 자유학년제, 연계학기 등 지속적인 학생 발달을 도모할 수 있는 정책 방안과 학생 발달의 궤적과 개인 차이를 고려한 교육과정의 필요성을 논의하였다. 그리고 진로체험 참여 횟수보다 만족도가 학생 발달에 정적 영향을 미치는 주요한 요인임을 고려하여 진로체험의 질 제고에 관한 논의를 첨하였다. The purpose of this study is to analyze the developmental trajectories of first-year middle school students who participated in the Free semester in 2017. The study also tried to identify how the number of participation and satisfaction of career experience programs through the school curriculum influenced by changes of students’ happiness, school satisfaction, learning attitudes and career maturity. The study collected data on student happiness, school satisfaction, learning attitude and career maturity for three times; firstly at the beginning of the school year, secondly at the end of the first semester and lastly, right before winter vacation. The results are as follows: The growth longitudinal analysis indicated that students’ life happiness, school satisfaction, and career maturity had increased, but the initial value and rate of change were different. The number of participation in career education program was statistically significant in the initial value of career aspiration, rational decision making, and satisfaction but not a significant in change of rate. However, satisfaction with career education programs was a statistically significant both an initial value and in a change rate. Based on the analysis results, the study emphasized the importance of quality of career education programs and discussed extending a free semester to a free year in middle school to stimulate students’ growth and development.

        소아 특발성 고칼슘뇨증에서 골밀도의 변화

        Purpose : Idiopathic hypercalciuria is a common clinical disorder in children with hematuria or nephrolithiasis. Recently, it has been reported that idiopathic hypercalciuria may disturb the metabolism of bone and induce its loss. We measured the biochemical markers of bone metabolism and bone mineral densities (BMD) by dual energy X-ray absorptiometry (DEXA) in children with idiopathic hypercalciuria to elucidate the prevalence and clinical significance of osteopenia. Methods : From 1997 to 2005, 52 children with idiopathic hypercalciuria were included in this study. The biochemical markers of bone metabolism were measured in all 52 patients. BMD was measured in 21 patients initially and additional 6 patients during the follow-up. Results : Mean age of patients was 6 years 7 months, and the male to female ratio was 1.26:1. BMD Z-scores were less than -1 in 10 of initial 21 patients (osteopenia), and -2.5 in 2 of 10 osteopenic patients (osteoporosis). There was a negative correlation between BMD Z-score and the level of osteocalcin. However, there were no significant differences in BMD Z-scores and other biochemical markers of bone metabolism among groups divided by sex, the type of idiopathic hypercalciuria, and the presence of urinary stone. Conclusion : Idiopathic hypercalciuria can be not only the cause of hematuria, but also of osteopenia in children. We suggest that the measurement of BMD in children with idiopathic hypercalciuria should be performed routinely with regular follow-up.

        Fustin flavonoid attenuates β-amyloid (1–42)-induced learning impairment

        <P>Natural flavonoids ameliorate amyloid-β peptide (Aβ)-induced neurotoxicity. We examined whether the fustin flavonoid affects Aβ-induced learning impairment in mice. Repeated treatment with fustin significantly attenuated Aβ (1–42)-induced conditioned fear and passive avoidance behaviors. This effect was comparable to that of EGb761, a standard extract of ginkgo. Fustin treatment significantly prevented decreases in acetylcholine (ACh) levels, choline acetyltransferase (ChAT) activity, and ChAT gene expression induced by Aβ (1–42). Fustin also consistently suppressed increases in acetyl cholinesterase (AChE) activity and AChE gene expression induced by Aβ (1–42). In addition, fustin significantly attenuated Aβ (1–42)-induced selective decreases in muscarinic M1 receptor gene expression and muscarinic M1 receptor binding activity (as determined by [<SUP>3</SUP>H]pirenzepine binding) by modulating extracellular signal-regulated kinase 1/2 (ERK 1/2) and cAMP response-element binding protein (CREB) phosphorylation and brain-derived neurotrophic factor (BDNF) expression. These effects of fustin were reversed by treatment with dicyclomine, a muscarinic M1 receptor antagonist, and SL327, a selective ERK inhibitor, but not by chelerythrine, a pan-protein kinase C (PKC) inhibitor. Taken together, our results suggest that fustin attenuates Aβ (1–42)-impaired learning, and that the ERK/CREB/BDNF pathway is important for the M1 receptor-mediated cognition-enhancing effects of fustin. © 2009 Wiley-Liss, Inc.</P>

        외상 후 스트레스 장애 난민에 관한 뇌 영상 연구 동향: 주제범위 문헌고찰

        Purpose: The purpose of this study was to analyze research trends and find whether Post-Traumatic Stress Disorder (PTSD) of refugees could affect structural or functional changes of brains of those under MRI, focusing on volumes, functional connectivities, and metabolites. Methods: A literature search was done using PubMed, Embase, RISS, and KMBase to identify studies that matched our research purpose. A total of eight studies were identified using Prisma flow diagram by two reviewers independently. Results: Eight studies were identified. Three studies were on North Korean defectors as subjects. The number of studies that observed structural changes, functional changes, and metabolite changes in brains was 2, 5, and 2, respectively. Although each study observed various parts of the brain, anterior cingulate cortex (ACC) was observed commonly in three studies. The PTSD group showed reduction of ACC volume and N-acetyl-aspartate (NAA) metabolite in ACC compared to the non- PTSD group. When exposed to negative stimuli, the PTSD group showed higher neural activity than the non-PTSD group, but not vice versa. Conclusion: ACC showed significant difference in volume, neural activity, and NAA metabolite between the PTSD and the non-PTSD group, resulting in significant differences in structural changes, functional changes, metabolite changes, respectively. This study showed the need for conducting more research using various biomarkers to clarify the relationship between PTSD of refugees and their brain changes.

        DEVELOPMENT AND ANALYSIS OF BUS WITH COMPOSITE-MATERIAL-REINFORCED FRAMES

        The bus frame structures suffer serious deformation or fracture and then damage the survival space of passengers. The superstructure of a bus is a frame member that supports the vehicle’s load and incorporates other parts for strength enhancement and impact absorption. Therefore, superstructures of bus play an essential role in protecting passengers in case of rollover accidents. Currently, the frames of large passenger vehicles are mostly manufactured using steel materials. The selection of a composite material that exhibits lightweight and outstanding structural strength has become an imperative topic in research on the reinforcement of large passenger vehicles’ frames. This study examined a vehicle frame reinforced using steel coated with composite materials. According to the relevant regulations in ECE R66 on the superstructure strength of large passenger vehicles, the LS-DYNA simulation was employed to perform structural analysis. A composite-materialreinforced frame was employed to simulate the crush test, thereby evaluating the effect of such reinforcement on enhancing the strength of vehicle frames. Finally, a rollover simulation test was performed according to ECE R66 on a vehicle whose entire frame was constituted by composite-material-reinforced steel. This study’s proposed vehicle frame material for design improvements can serve as references for manufacturers of large passenger vehicle bodies to improve the quality of their product development.
