        In this paper, we propose a user friendly object-based image retrieval system using the interaction between the cortex and the hippocampus. We propose a method of automatic color object extraction using CSB tree map(Color and Spatial based Binary tree map). Extracted objects are transformed to bit stream representing information such as color, size and location by region labelling algorithm and they are learned by the hippocampal neural network using the interplay between the cortex and the hippocampus. Proposed hippocampal neural network makes an adaptive fast content-based image retrieval system using excitatory learning method that forwards important features to long-term memories and inhibitory learning method that forwards unimportant features to short-term memories controlled by impression.

        소아에서 발생한 상지의ㅏ 재골절

        목적: 소아에서 발생하는 상지의 재골절과 관련된 임상적, 해부학적 요인 및 이를 예방할 수 있는 방법을 알아보고자 하였다. 대상 및 방법: 쇄골과 수부를 제외한 상지의 재골절로 내원하였던 만 15세 이하 소아 환자 중 전신적 골질환이 있었던 경우 및 처음 골절부위와 다른 부위에 골절이 발생한 경우를 제외한 18례를 대상으로 하였으며, 처음 골절이 발생한 부위에 따라 3개의 군 (상완골 과상부 골절, 상완골 외과 골절, 전완골 골절)으로 나누어 후향적 분석을 시행하였다. 이들은 다시 재골절의 유형에 따라 미성숙한 가골에서 발생한 조기 재골절과 골의 재형성 후 발생한 후기 재골절로 분류하였으며 골절양상과 동반된 기존 변형 등이 조사되었다. 결과: 상완골 과상부 골절의 경우 9례 중 2례는 과상부에서, 7례는 외과에서 재골절이 발생하였으며 이 중 6례에서 내반변형이 동반되어 있었다. 상완골 외과 골절의 경우 3례 중 1례는 과상부에서, 2례는 외과에서 재골절이 발생하였으며 이 중 1례에서 내반변형이 동반되어 있었다. 이러한 상완골 원위부 재골절의 경우 대부분 골유합이 완전히 이루어진 후 발생한 후기 재골절이었던 반면, 전완골 골절의 경우 총 6례의 재골절 중 4례가 골의 재형이 완전히 이루어지기 전에 발생한 조기 재골절이었다. 또한 이 중 5례는 골간부에서 발생하였으며 재골절 발생 전 모두 경도의 전방 각형성이 선행하였다. 결론: 상완골 원위부 재골절의 경우 내반변형이 가장 중요한 유발인자로서 특히 외과에서 발생하는 경우가 빈번하였고, 전완골 재골절의 경우 전방 각형성이 잔존된 골간부에서 미성숙한 기골을 통한 조기 재골절이 가장 흔하엿다. 이러한 재골절을 방지하기 위해서는 상완골 원위부의 경우 내반변형의 발생 예방을, 전완부의 경우 각형성이 남지 않도록 정확한 정복과 더불어 충분한 기간동안의 고정이 필요할 것으로 사료되었다. Purpose: To investigate the etiologic factors related to refractures of the upper extremity in children. Materials and Methods: 18 refractures of the upper extremity were divided into three groups according to the location of initial fractures: Supracondyle fractures of the humerus, lateral condyle fracture of the humerus, and the forearm bone fractures. They were analyzed in terms of the type of refractures(early refracture occurring at the immature callus and late refracture occurring at the remodeled bone), fracture patterns, and the existence of underlying deformity. Results: Nine supracondyle fractures had refractures at the supracondyle (2 cases) and the lateral condyle (7 cases), in which underlying cubitus varus were present in 6 cases. Three lateral condyle fractures had refractures at the supracondyle(1 case) and the lateral condyle (2 cases), in which one case had underlying cubitus varus. All but one cases in the group of humerus fractures were late refractures and treated operatively except one. Of 6 refractures of forearm, 5 were early refractures and occurred within 9 weeks at the original site: 4 at the diaphysis of both bones of forearm and 1 at the diaphysis of ulna. All cases in the group of forearm fractures had volar angulation before the refracture, and treated conservatively except one. Conclusion: In the humerus, underlying cubitus varus was the most important predisposing factor to refractures and the lateral condyle fractures were common. In the forearm, volar angulation of the diaphysis were related to refractures, and complete and circular consolidation of the primary fracture of forearm was thought to be important to prevent refracture.

      • 위 MALT 림프종에서 다발성 림프절 전이로 오인된 동시성 림프절 결핵 1예

        28세 남자가 내시경적 점막절제술을 통해 저등급 위MALT 림프종으로 진단되었다. 병기설정을 위해 18F-FDG PET 검사를 시행하였고 우측 쇄골상부, 우측상/하부 기관주위, AP window 부위에 강한 섭취가 관찰되어 전이성 림프절로 판단하였다. 그러나, 우측 쇄골상부 림프절에서 시행한 절제생검에서 만성육아종성염증소견이 관찰되었고 조직의 결핵 중합효소연쇄반응에서도 양성 반응을 보였다. 2개월간 항결핵제의 사용 후 시행한 추적 컴퓨터단층촬영 검사에서 림프절 크기의 감소를 보여 최종적으로 위 MALT 림프종에서 광범위 림프절 전이로 오인된 동시성 림프절 결핵으로 진단되었다. A 28-year-old man was diagnosed as gastric MALT lymphoma in diagnostic EMR (endoscopic mucosal resection). He subsequently underwent an 18F-FDG PET. 18F-FDG PET showed intense multifocal uptake in right supraclavicular, right upper and lower paratracheal, and AP window areas that was considered radiologically to represent widespread metastatic lymphadenopathy. Excisional biopsy of a supraclavicular lymph node, however, revealed chronic granulomatous lymphadenitis with a positive reaction of Tb-nested PCR. After antituberculosis treatment for 2 months, a follow-up CT scan showed complete resolution of the lesions. Intense 18F-FDG uptake could be due to an infectious process such as synchronous tuberculous lymphadenitis. Therefore, this could mimic a malignant condition such as lymphoma with extensive lymph node metastasis due to a false-positive finding, which may lead to a misdiagnosis.

      • TCP/IP 상에서 FMS 프로토콜 구현

        필드버스는 자동화 시스템에서 필드 디바이스들간의 실시간 통신을 제공하는 통신망이다. 본 논문에서는 Profibus와 FOUNDATION Fieldbus을 인터넷에 접속시키기 위해 이들의 응용계층인 FMS(Fieldbus Message Specification)를 TCP/IP 상에서 동작하도록 구현하였다. 본 논문에서 FMS 서비스는 서비스 기술 블록과 데이터 블록의 두 부분으로 나누어 구현하고, 이와함께 FMS 프로토콜의 사용자 접속 S/W를 개발하였다. 본 연구에서 구현된 FMS 프로토콜은 두 대의 PC를 Ethernet으로 연결하여 구성한 클라이언트-서버 모델에 탑재하여 그 성능을 검증하였다. Field bus is the network system which provides real-time communication to field devices in the automation system. In order to integrate fieldbus and Internet, this paper presents an implemetation method of FMS(Fieldbus Message Specification) protool, which is the application layer of Profibus and FOUNDATION Fieldbus, on the TCP/IP. In this study, FMS services are implemented in two blocks; Service Description Block and Data Block, and the user interface S/W is developed. The FMS protocol implemented in this paper is verified using two computers, connected with Ethernet card, each of which plays the role of client and server.

        Different Antiulcer Activities of Pantoprazole in Stress, Alcohol and Pylorus Ligation-Induced Ulcer Models

        Antiulcer effects of pantoprazole, a proton-pump inhibitor, on water-immersion restraint stress (WIRS)-, alcohol (ethanol)- and pylorus ligation-induced gastric ulcers were investigated in male rats. Rats were orally administered with pantoprazole 30 min prior to exposure to various types of ulcer inducers. In stress-induced ulcer model, rats were subjected to WIRS at 22℃ for 4 hours, and the degree of ulcer (in ㎜) was evaluated. In alcohol-induced ulcer model, rats were orally administered with pure (100%) ethanol (1 ㎖/㎏), and the ulcer lesions were measured 1 hour after ethanol challenge. In pylorus ligationinduced ulcer model, rats were subjected to pylorus ligation, and the degree of erosions and ulcers was scored 17 hours after the operation. Pantoprazole attenuated the ulcer lesions induced by WIRS in a dose-dependent manner, exhibiting a median effective dose (ED??) value of 0.78 ㎎/㎏. By comparison, pantoprazole was effective at relatively-high doses for the improvement of ethanol-induced ulcers, showing an ED?? value of 20.5 ㎎/㎏. Notably, pantoprazole was practically ineffective (ED??>50.0) in pylorus ligation model. Taken together, it was confirmed that pantoprazole showed inhibitory activity on gastric ulcers induced by stress and alcohol, but was ineffective on pylorus ligation-induced ulcer. Therefore, the results indicate that proton-pump inhibitors including pantoprazole might reveal highlydifferent effects according to the type of ulcer inducers, and that the prescription of antiulcer agents should be carefully selected.

        Effects of Hyperbaric Oxygen and $\alpha$-Tocopherol on Skin Antioxidant Enzymes Defence in Rats

        In order to test the effects of hyperbaric oxygen (HBO) and $\alpha$-tocopherol on full-thickness skin grafts in rats, we peeformed full-thickness skin grafts bilaterally on rats. After surgery, we analyzed the tissue-concentrations of superoxide dismutase (SOD), catalase, and glutathione peroxidase(GPx)/reductase(GPr) on days 0, 2, 4, 7, 10, 14, 21 and 28. The four groups had similar patterns of change in SOD, catalase, GPx and GPr values. SOD increased initially, and was significantly increased at day 7, returning to the preoperative activity level on day 14 (control, HBO, and $\alpha$-tocopherol treated alone) and 28 (HBO plus $\alpha$-tocopherol). Catalase had a similar pattern of change as the SOD enzyme activity, except for the surgical control on day 2. Glutathione peroxidase/reductase activity in the four groups had a similar pat-tern of enzyme activity, with a significant increase from preoperative level on day 4, peaking during days 7 to 10, and returning to preoperative level on day 21(surgical control, HBO, and $\alpha$-tocopherol-treated alone) and 28 (HBO plus $\alpha$-tocopherol treated group). Hence, the clinical use of HBO and $\alpha$-tocopherol mixture can be recommended as an adjunctive treatment for free skin grafts in rats. But, the antioxidant used, its dose, and the timing of its administration, as well as, the exposure time and the pressure of HBO, should be the subject of further research.

        Ginsenoside $Rg_5$, A Genuine Dammarane Glycoside from Korean Red Ginseng

        A genuine dammarane glycoside, named ginsenoside $Rg_{5}$, has been isolated by repeated column chromatography and preparative HPLC from the MeOH extract of Korean red ginseng (Panax ginseng C.A. Meyer). The chemical structure of ginsenoside$ Rg_{5}$ was determined as $3-O-[{\beta}-D-glucopyranosyl (1{\rightarrow}2)-{\beta}-D-glucopyranosyl]$ dammar-20(22), $24-diene-3{\beta},12{\beta}-diol$ by spectral and chemical methods. The stereostructure of a double bond at C-20(22) of ginsenoside $Rg_{5}$ was characterized as (E) from the chemical shift of C-21 in the $^{13}C-NMR $and a NOESY experiment in the $^{1}H-NMR$.
