        Concise Bedside Surgical Management of Profound Reperfusion Injury after Vascular Reconstruction in Severe Trauma Patient: Case Report

        For an orthopaedic surgeon, the critical decisions to either amputate or salvage a limb with severe crushing injury with progressive ischemic change due to arterial rupture or occlusion can become a clinical dilemma at the Emergency Department (ED). And reperfusion injury is one of the fetal complications after vascular reconstruction. The authors present a case which was able to save patient`s life by rapid vessel ligation at bedside to prevent severe reperfusion injury. A 43-year-old male patient with no pre-existing medical conditions was transported by helicopter to Level I trauma center from incident scene. Initial result of extended focused assessment with sonography for trauma (eFAST) was negative. The trauma series X-rays at the trauma bay of ED showed a multiple contiguous rib fractures with hemothorax and his pelvic radiograph revealed a complex pelvic trauma of an Anterior Posterior Compression (APC) Type II. Lower extremity computed tomography showed a discontinuity in common femoral artery at the fracture site and no distal run off. Surgical finding revealed a complete rupture of common femoral artery and vein around the fracture site. But due to the age aspect of the patient, the operating team decided a vascular repair rather than amputation even if the anticipated reperfusion time was 7 hours from the onset of trauma. Only two hours after the reperfusion, the patient was in a state of shock when his arterial blood gas analysis (ABGA) showed a drop of pH from 7.32 to 7.18. An imminent bedside procedure of aseptic opening the surgical site and clamping the anastomosis site was taken place rather than undergoing a surgery of amputation because of ultimately unstable vital sign. The authors would like to emphasize the importance of rapid decision making and prompt vessel ligation which supply blood flow to the ischemic limb to increase the survival rate in case of profound reperfusion injury.         The Clinical Results of Conservative Treatment of Frozen Shoulder Using Continuous Passive Motion

        Background: The purpose of this study is to administer conservative treatment in 30 patients diagnosed with idiopathic frozen shoulder, following the suggested frozen shoulder rehabilitation program and to assess the clinical outcome using a prospective study. Methods: Thirty patients diagnosed with idiopathic frozen shoulder, treated with steroid hormone injection on the articular joint with an intra-articular steroid (triamcinolone 40 mg+lidocaine 4 ml) injection and started on stepwise shoulder extension exercise were chosen. The subjects were divided into two groups of 15 people each with one group undergoing rehabilitation with continuous passive motion (CPM) and the other group without it. Follow-ups were done before rehabilitation and at 4-week intervals with the 24th being the final week. At every follow-up, passive range of motion (ROM) was measured and surveys on pain and clinical score were administered. Results: In the last follow-up, both groups showed statistically significant improvements in all evaluation criteria. However, no statistical difference in all values of the ROM and Constant score evaluation criteria was observed between the groups. Only in the last follow-up, group 1 had a visual analog scale (VAS) score of 2.4 ± 2.1 points, which was lower, with statistical significance, than the VAS score of group 2, which was 4.4 ± 3.1 points (p<0.001). Conclusions: Study using CPM in treatment of frozen shoulder has been inadequate, meaning that there is still room for improvement and need for more study on setting a more specific protocol and guidelines for this procedure.

        Cellular-uptake Behavior of Polymer Nanoparticles into Consideration of Biosafety

        Nanoparticles have tremendous potential in cancer prevention, detection and augmenting existing treatments. They can target tumors, carry imaging capability to document the presence of tumors, sense pathophysiological defects in tumor cells, deliver therapeutic genes or drugs based on the tumor characteristics, respond to external triggers to release an appropriate agent, document the tumor response, and identify the residual tumor cells. Nanoparticles < 30 nanometers in diameter show unexpected and unique properties. Furthermore, particles < 5 nanometers in size can easily penetrate cells as well as living tissues and organs. This study evaluated the safety of nano materials in a living body and the relationship between the living tissue and synthetic nano materials by examining the in-vitro cytotoxicity of poly(lactic-co-glycolic) acid (PLGA) nano-spheres and fluorescein isothiocynate(FITC)-labeled dendrimers as polymer nanoparticles. PLGA was chosen because it has been used extensively for biodegradable nanoparticles on account of its outstanding bio-compatibility and its acceptance as an FDA approved material. The dendrimer was chosen because it can carry a molecule that recognizes cancer cells, a therapeutic agent that can kill those cells, and a molecule that recognizes the signals of cell death. Cytotoxicity in L929 mouse fibroblasts was monitored using MTT assay. Microscopic observations were also carried out to observe cell growth. All assays yielded meaningful results and the PLGA nanoparticles showed less cytotoxicity than the dendrimer. These nano-particles ranged in size from 10 to 100 nm according to microscopy and spectroscopic methods.

        둔상 이후 소아에서 발생한 급성 슬개골 골수염 - 증례 보고 -

        소아에서 발생하는 슬개골 골수염은 흔치 않은 질환이다. 게다가 비특징적이며 다양한 임상 양상을 보이는 골수염은 정확한 진단을 내리지 못하고 진단이 지연되기도 한다. 그리고 종종 슬개골 활액막염이나 슬관절의 화농성 관절염으로 오인되기도 한다. 그럼에도 불구하고 조기 진단과 초기에 적절한 치료는 그것이 만성 골수염으로 진행하는 것을 막는 데 중요하다. 저자들은 소아에서 둔상을 당한 후 슬개골에 발생한 급성 골수염을 초기 진단을 정확히 하지 못하고 치료한 케이스가 있어 이를 보고하고자 한다. Osteomyelitis of the patella is a very uncommon condition that occurs mostly in the pediatric population. In addition to its rarity, nonspecific and variable clinical presentations usually lead to postponement in making the correct diagnosis. Moreover, it is often missed as prepatellar bursitis or septic arthritis of the knee. Nonetheless making early diagnosis and initiating prompt treatment is most important to preventing this condition from becoming chronic. In this case report, the authors encountered this rare condition of the patella in a child that was first misdiagnosed with pyogenic arthritis or prepatellar bursitis of the knee. The delay in making the diagnosis led to intractable progression of the disease, and sequestrectomy was required to stabilize the condition.

        전통문화 교육을 위한 아동 교구디자인 및 교육프로그램 연구

        한국은 고도로 발달된 현대 서구 문명과 문화의 무분별한 유입 및 모방으로 전통문화의 단절과 함께 정체성 혼란의 위기를 초래하였다. 이러한 문제점들이 부각되면서 우리나라 역시 전통문화를 보존·계승하고 이를 활용할 수 있도록 다양한 시도를 하고 있으며, 이러한 관심과 이해가 지속될 수 있도록 하기 위해서는 교육이 전제되어야 한다. 이에 본 연구는 아동이 전통문화를 자연스럽게 경험하고, 창의성 증진에 효과적인 교구를 제작하여 이에 대한 효과를 극대화할 수 있는 프로그램을 개발하고 제안하는데 목적이 있다. 현재 전수교육관에서 교육을 진행 중인 전수자와 함께 종목을 선정하고 각 종목별로 교육, 디자인 등의 전문가그룹을 구성하여 교구디자인에 대한 세부 프로세스를 진행하였다. 단청은 문양을 맞출 수 있는 퍼즐, 자수는 타공판을 이용한 자수판과 도구, 목조각은 플라스틱 조각도와 신소재를 이용한 시연 점토판, 선소리 산타령은 일러스트를 사용한 플래시카드를 완성하였다. 이는 아동과 종목의 특성을 고려하여 제작하였으며 안전성, 심미성, 난이도, 재미·흥미 등을 반영하였다. 프로그램은 문양 맞추기, 기초 기법 배우기, 가사 순서 맞추기 등 종목의 특성을 배우는 기본 프로그램과 교구를 통해 다양하게 표현하고, 만들며, 게임을 할 수 있는 심화 프로그램으로 구성하였다. 이후 하나의 종목을 시범교육 한 결과 교육 만족도와 필요성에 대한 인식도가 높게 나타났다. 본 연구는 종목선정에 있어 연구범위가 다소 제한적이었지만, 향후에는 최소한의 연구범위를 적용하고 제작한 모든 교구의 시범교육을 통해 전통문화 이해도와 창의성 향상에 대한 검증이 필요하다. Korea caused confusion in identify along with traditional culture discontinuation as advanced and developed western culture and civilization inflows and are copied imprudently. As these problems appear, our country also tries in different ways to apply, preserve, and succeed this kind of traditional culture. To make this kind of interest and understanding persisting, education must be a premise. Therefore, this study aims to suggest developing programs which can maximize effect of efficient educational tools for creativity and make children experience traditional culture naturally. With an initiator who educates at succession education institute, items were selected and detailed processes were undergone for tool designs by grouping professional groups for education, design, etc. Dancheong is a puzzle which matches patterns. Embroidery is tool, and embroidery board. Wooden Sculpture are practical clay board by using plastic gravers and new materials, and Seonsori Santaryeong completed flash card by using flash card. Considering the features of children and types, they were produced. They reflected stability, esthetic feature, difficulty, interest/fun, etc. Programs express basic programs and educational tools differently such as pattern matching, basic methods study, lyric order matching, etc. They were made differently. Advanced programs were composed to play games. As a result of educating one item as an example, awareness of educational satisfaction and necessity appeared high. The range of minimal study was applied. Through manufactured educational tools’ model education, verification on understanding level about traditional culture and creativity improvement is required.

      • Application of Langmuir and Freundlich isotherms to predict adsorbate removal efficiency or required amount of adsorbent

        <P><B>Abstract</B></P> <P>The aim of this study was to investigate how basic adsorption isotherms could be applied to predict removal efficiency or required adsorbent mass under given sets of initial conditions. The intrinsic parameters of the Langmuir and Freundlich adsorption isotherms were experimentally obtained and subsequently utilized to predict removal efficiencies for other sets of initial solute concentrations, solution volumes, and adsorbent masses, or to estimate the adsorbent mass required to remove solute at a desired removal efficiency. This was accomplished by combining the isotherms with mass balance of solutes between liquid solution and solid adsorbent phases.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Modeling by combining the isotherm equations with mass balance to predict the removal efficiencies and adsorbent mass required. </LI> <LI> Contour plots of predicted removal efficiencies as functions of both initial solute concentration and adsorbent mass. </LI> </UL> </P>
