Purpose: Recent research has shown that there is a relationship between the level of preoperative serum TSH and a papillary thyroid carcinoma. Therefore, this study examined the correlation between the serum TSH and papillary thyroid carcinoma. Methods: The preoperative serum TSH level of papillary thyroid carcinoma and nodular hyperplasia of 418 patients from 2009 Jan. to 2011 Dec. was examined. The patients were divided into 3 groups, nodular hyperplasia, less than 1 cm micropapillary carcinoma and more than 1 cm papillary carcinoma, and their TSH levels were compared. Results: Nodular hyperplasia and total papillary carcinoma was found in 98 (23.0%) and 322 (77.0%) patients, respectively. After dividing the patients according to the size of the mass, there were 224 (53.6%) patients with a mass less than 1 cm in size and 98 (23.4%) patients with a mass more than 1 cm in size. The preoperative serum TSH level of the 3 groups showed a significant difference, which was 1.180±1.168 ՌIU/ml in the nodular hyperplasia group, 1.670±1.224 ՌIU/ml in the micropapillary carcinoma group and 2.279±2.837 ՌIU/ml in the papillary carcinoma group (P<0.001). On the other hand, there were no significant correlations between the preoperative serum TSH level and gender, age, metastasis to lymph node, number of masses and extrathyroidal extensions. Conclusion: The larger size of the papillary thyroid carcinoma, the higher the preoperative high serum TSH level. Therefore, the stimulation of TSH can affect the progression of papillary thyroid carcinoma but more study will be needed.

Background: Anatomic anterior cruciate ligament (ACL) reconstruction has been presented as a means to more accurately restorethe native anatomy of this ligament. This article describes a new method that uses a double bundle to perform ACL reconstructionand to evaluate the clinical outcome. Methods: Grafts are tibialis anterior tendon allograft for anteromedial bundle (AMB) and hamstring tendon autograft withoutdetachment of the tibial insertion for posterolateral bundle (PLB). This technique creates 2 tunnels in both the femur and tibia. Femoral fi xation was done by hybrid fi xation using Endobutton and Rigidfi x for AMB and by biointerference screw for PLB. Tibialfi xations are done by Retroscrew for AMB and by native insertion of hamstring tendon for PLB. Both bundles are independentlyand differently tensioned. We performed ACL reconstruction in 63 patients using our new technique. Among them, 47 participatedin this study. The patients were followed up with clinical examination, Lysholm scales and International Knee DocumentationCommittee (IKDC) scoring system and radiological examination with a minimum 12 month follow-up duration. Results: Signifi cant improvement was seen on Lachman test and pivot-shift test between preoperative and last follow-up. Onlyone of participants had fl exion contracture about 5 degrees at last follow-up. In anterior drawer test by KT-1000, authors found improvementfrom average 8.3 mm (range, 4 to 18 mm) preoperatively to average 1.4 mm (range, 0 to 6 mm) at last follow-up. AverageLysholm score of all patients was 72.7 ± 8.8 (range, 54 to 79) preoperatively and signifi cant improvement was seen, score was92.2 ± 5.3 (range, 74 to 97; p < 0.05) at last follow-up. Also IKDC score was normal in 35 cases, near normal in 11 cases, abnormalin 1 case at last follow-up. Conclusions: Our new double bundle ACL reconstruction technique used hybrid fi xation and Retroscrew had favorable outcomes.
Overview of Various Measurement Tools for Shoulder Kinematics

Shoulder kinematics is important, as it is associated with shoulder arthropathy and pain mechanisms. Various static and dynamic analysis methods are prevalent for shoulder kinematics. These include 2-dimensional plane x-ray, computed tomography, and magnetic resonance imaging, cadaver study, electromagnetic motion analysis, transcortical bone pins technique, and in vivo 3-dimensional motion analysis. Although these methods provide the value of the shoulder kinematics angle, they are unable to explain why such changes occur. Since each method has its pros and cons, it is important to understand all factors accurately, and to choose a method that best meets the purpose of the researcher.

<P>Though hyaluronic acid (HA)-based hydrogel has drawn great attention in biomedical society, it’s long molecular weights sometimes have been problematic due to its difficulty in handling. After reduction of its high molecular weight into smaller sizes with various concentrations of hydrogen chloride solutions, its chemical and biological properties have been examined by changes in viscosity, FTIR spectroscopy and gel permeation chromatography as well as cellular interactions. While FTIR analysis indicated maintenance of its original chemical structures, its viscosity has been remarkably reduced and its extent was dependent upon the employment of acid concentrations. After controlling its molecular weight to approximately 100 kDa and coupling of aminopropymethacrylate to the treated HA, we evaluated in vitro cellular interactions and cell proliferations of the HA-poly(ethylene oxide) (PEO) hydrogel.</P>
Schwannoma of the Ulnar Nerve in the Elbow

Schwannomas are soft tissue sarcomas arising from neurilemma of Schwann cells in peripheral nerves, and is the most frequent type of benign tumor found in these nerves. We report a case of a Schwannoma of the ulnar nerve in the elbow joint, and along this report, give a review of the literature. A 46-year-old male patient was hospitalized with complaints of swelling and pain in the left elbow and a tingling sensation and hypoesthesia of the fourth and fifth fingers. Physical examination of the patient showed he was positive for Tinel’s sign, and magnetic resonance imaging results demonstrated the presence of a Schwannoma. Subsequent biopsy and excision of the Schwannoma was carried out. The suspected mass, which had a clear margin separating it from the healthy nerve of the medial left elbow, was removed along with its 2 × 2 × 3 cm capsule after a histological diagnosis of a Schwannoma was made. Pathophysiological results confirmed the excised mass as a Schwannoma. Schwannoma of the ulnar nerve within the elbow joint is rare and differential diagnosis is difficult. Therefore, treatment can only proceed after the presence of Schwannoma has been confirmed by physical and radiological examinations.

<P>Characterization of an hyaluronic acid (HA)-poly(ethylene oxide) (PEO) hydrogel was performed by changing the number of side arms in a PEO molecule. Verification of grafting chemistry and mechanical strength, as well as swelling behaviors and surface morphologies of the HA-PEO hydrogels were analyzed by employing different x-linking molecular weights (MW) of PEO ranging from 1.7 to 5.0 kDa at a fixed low MW HA in the hydrogel network. Methacrylation to the HA successfully obtained via Michael type reaction between the methacrylate arm groups in HA and the thiol end groups in PEO was observed by XPS. Hydrogel formation was markedly dependent upon the numbers of thiol groups in the PEO molecule. Furthermore the lower MW PEO-based HA hydrogel demonstrated stronger mechanical properties but lower water absorption and the smaller pore sizes on its surface and cross section.</P>

<P>In vitro cell behaviors of calvarial osteoblasts (MC3T3) were evaluated by seeding them on both the surface and inside of in situ hyaluronic acid-poly(ethylene oxide) (HA-PEO) hydrogel, either after or before incorporation of mixture micro-particles of hydroxyapatite-β-tricalcium phosphate on/inside the hydrogel, respectively. Cellular behaviors such as adhesion and proliferation on the surface and inside the gel were evaluated with light microscopy and a microplate reader by focusing on the interactions of cell-HA-PEO as well as cell-hydroxyapatitetricalcium phosphate micro-particle surface in the gel. Cell adhesion and spreading seemed to be enhanced by supplying the micro-particles to the inside the HA-PEO hydrogel, compared to the results of the HA-PEO hydrogel itself.</P>

<P>Glioblastoma is a highly angiogenic tumor with a dismal prognosis. Temozolomide (TMZ), a methylating agent is one of the most effective chemotherapeutic agents against glioblastoma. To overcome the problem that most of these tumors become resistant to chemotherapeutic regimens within a year, we investigated the antitumor efficacy of metronomic administration of low-dose TMZ in in vitro cell proliferation/cytotoxicity assay and in vivo rat and nude mouse orthotopic glioma model. By in vitro assay, we elucidated that C6/LacZ rat glioma cells were more resistant to metronomic treatment of TMZ than U-87MG human glioblastoma cells and bEnd.3 mouse brain endothelial cells. Compared with the conventional chemotherapeutic regimen of TMZ, we found that frequent administration of TMZ at a low dose (metronomic treatment) markedly inhibited angiogenesis as well as tumor growth in a TMZ-resistant C6/LacZ rat glioma model. In addition, metronomic treatment of TMZ significantly augmented apoptosis of tumor cells in this model. For the TMZ-sensitive U-87MG cells, even with a very low dose of TMZ, which is not effective to reduce tumor mass, the metronomic treatment of TMZ reduced the microvessel density, i.e. angiogenesis, in a nude mouse orthotopic model. In conclusion, for both models, the metronomic treatment of TMZ decreased angiogenesis. Especially, in TMZ-resistant glioma cells, this regimen increased apoptosis of tumor cells and decreased tumor growth. The metronomic treatment of TMZ in orthotopic glioma models demonstrated a successful antiangiogenic effect which can overcome the chemoresistance in conventional TMZ chemotherapy.</P>
