Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a potentially life- threatening but reversible autoimmune disorder characterized by psychiatric symptoms, cognitive dysfunction, speech dysfunction, seizures, movement disorder, decreased level of consciousness, and autonomic dysfunction or central hypoventilation. It occurs predominantly in young women and approximately half of them have underlying tumors, mainly ovarian teratoma. A 24-year old woman was admitted because of fever, headache, abnormal movement and decreased mental status. Five cycles of plasmapheresis improved her neurological and mental status. Anti-NMDAR antibodies in her CSF and serum were positive, and computed tomography revealed a 1-cm sized mass suggestive of mature cystic teratoma arising from the right ovary. We promptly performed laparoscopic right ovarian cystectomy. She was discharged after 2 weeks with mild memory deficit. Prompt removal of ovarian teratoma and multidisciplinary care are particularly important for good outcome.

본 연구에서는 스크러버 내부의 열 유동 수치해석을 수행하였다. 고온의 배기가스와 저온의 세정수의 이상(2-phase)유동을 해석하였으며, 하나의 상이 다른 상에 비해 차지하는 VOF가 10% 미만임을 고려하여 해당 상에 대해 DPM 을 사용하였다. 스크러버는 전처리기와 본처리기의 2단으로 구성되어 있으며 각 처리기 내부의 노즐을 통해 유동장에 세정수가 분사된다. 스크러버의 본처리기 내부의 3단 노즐 위치가 각 단마다 수직, 수평방향으로 어긋나게 배치하여 세정수와 배기가스의 접촉면적을 높임에 따라 배기가스의 냉각이 활발히 진행되도록 설계하였다. 해석 결과로 배기가스의 스크러버의 입구 및 출구 온도를 측정하고 내부 압력 차를 확인하여 선박의 기본 승인을 득할 수 있는 조건을 갖출 수 있는 지 파악하였다. Background/Aims: Data regarding the prognosis of early esophageal cancer are lacking. This study investigated the long-term outcomes and factors affecting the survival of patients with mucosal esophageal squamous cell carcinoma (T1aESCC). Methods: We analyzed the clinical and tumor-specific parameters of 263 patients who received surgical resection (SR; n=63) or endoscopic resection (ER; n=200) for T1aESCC. Underlying comorbidities were scored using the Charlson comorbidity index (CCI). Overall survival (OS) was the primary outcome, and multivariate regression analysis was performed to predict factors for OS. Results: Of the study patients (age, 64.5±8.0 years), the CCI was 1.0±1.4 in the ER group and 0.6±0.9 in the SR group (p=0.107). The 5-year OS rate during follow-up (54.4±20.4 months) was 85.7% (ER group, 86.8%; SR group, 82.4%; p=0.631). The cumulative 5-year incidence of esophageal cancer recurrence was 10.5% in the ER group (vs 0% in the SR group). The overall mortality rate was 12.9% (ER group, 12.0%; SR group, 15.9%; p=0.399). The most common cause of mortality was second primary cancers in the ER group (75%) and organ dysfunction or postoperative complications in the SR group (70%). According to multivariate analysis, only CCI was significantly associated with OS (p< 0.001). The 5-year OS rate in patients with a CCI >2 and in those with a CCI ≤2 was 60.2% and 88.2%, respectively (p<0.001). The treatment method (ER vs SR) was not a significant affecting factor (p=0.238). Conclusions: The long-term prognosis of patients with T1aESCC was significantly associated with underlying comorbidities. (Gut Liver 2021;15:705-712)
Genetic diversity and distribution of edible scytosiphonacean algae from Ulleungdo Island, Korea

Despite the abundance of seaweeds from Ulleungdo Island, genetic diversity and distribution of edible brown algae from the island remain unstudied. We analyzed mitochondrial cox3 sequences from 86 specimens collected in the island and from the nearby Korean Peninsula. Our cox3 phylogeny for the first time confirmed the occurrence of fives species from Ulleungdo Island; Petalonia binghamiae, P. fascia, Planosiphon zosterifolius, and two cryptic species previously identified as Scytosiphon lomentaria. P. binghamiae was relatively homogeneous with three haplotypes. P. fascia comprised four haplotypes, which were grouped into two genetic lineages. S. lomentaria was heterogeneous with nine haplotypes and was divided into two cryptic species; one species clustered with taxa from cold waters while the other clustered with taxa from temperate and cold waters. Low genetic diversity in P. binghamiae while high genetic diversity in S. lomentaria from Ulleungdo Island are comparable to patterns observed from other species from the Korean peninsula. Ulleungdo Island, although small in size, is an ideal field laboratory to investigate genetic diversity and distributions of economic marine algae.
