      • Combination treatment for onychomycosis using 1,064-Nd:YAG laser and efinaconazole 10% topical Solution

        Onychomycosis is one of the most common nail disease around the world. No specific treatment other than drug therapy and surgery has been suggested till present. Systemic treatment with antifungal drugs for onychomysosis is effective. However, a long treatment time is required and it is not suitable for people with other systemic disease. Furthermore, it may cause many side effects such as liver diseases. For that reasons, topical drugs or physical treatments are preferred by many patients. Recently laser treatment for onychomycosis has been proposed. The efficacy of treatment is less than systemic drugs but, laser is more convenient, safe and accessible in treatment. As a result, the combination therapy using 1064 Nd:YAG laser and efinaconazole 10% topical solution was performed. Two patients with onychomycosis were treated with 1064 Nd:YAG laser (PinPointe FootLaser, Cynosure, Westford, MA, USA) once a month with application of 10% eifnaconazole solution(Jublia, Dong-A EST) once a day. All three patients were treated successfully. We considered a combination treatment of a 1064 Nd:YAG laser and efinaconazole 10% topical solutions would be an effective, safe, and convenient treatment method for onychomycosis.

        Background: Onychomycosis is one of the most common nail disease around the world. No specific treatment other than drug therapy and surgery was suggested till present. Objectives: The objective of this study was to evaluate the efficacy of combination treatment with a 1064 Nd:YAG laser and efinaconazole 10% Topical Solution. Methods: Ten patients with onychomycosis were treated with 1064 Nd:YAG laser (PinPointe FootLaser, Cynosure, Westford, MA, USA) once a month with application of 10% eifnaconazole solution(Jublia, Dong-A EST) once a day. The effectiveness of treatment were evaluated with before and after photographs. Results: Improvement were shown in 90% of patients after the treatment. However, 10% of patients had no response. Conclusion: This clinical study suggests that combination treatment of a 1064 Nd:YAG laser and efinaconazole 10% Topical Solutionis an effective, safe, and convenient treatment method for onychomycosis.

        Lightweight Printed Dipole Antenna Array With 3×2 Beamforming Network for Wide UAV Communication Coverage

        In this paper, a lightweight printed dipole antenna array with 3×2 beamforming network for wide unmanned aerial vehicle (UAV) communication coverage is presented. The proposed antenna consists of an upper substrate, a lower substrate, and two supports with a microstrip feeding. The upper substrate has a two-element printed dipole antenna array, and the lower substrate includes a beamforming feeding circuit and a ground plane as a refector of the proposed antenna array for gain improvement. For wide beam coverage, the proposed feeding network consists of three RF switches, one Wilkinson power divider, and one 90° hybrid coupler, and generates the wide beam using one broadside beam and two end-fre beams. Measured impedance bandwidth, peak gain, and half-power beamwidth (HPBW) of the proposed array operated from 5.03 to 5.15 GHz are approximately 2 GHz, 4.3 dBi, and 155°, respectively.

        Effect of Inhomogeneous Nucleation of Hydride at α/β Phase Boundary on Microstructure Evolution of Zr–2.5 wt%Nb Pressure Tube

        We analyzed the microstructural characteristics such as number density and length and width of hydrides in Zr–2.5 wt%Nbpressure tube. The hydrogen was charged cathodically and the hydride-contained sample was evaluated using the advancedanalysis methodologies. We performed a diff erential scanning calorimetry analysis to more quantitatively understand thethermodynamics of the hydride formation/growth process. We characterized the micrograph of hydride-contained Zr samplesto estimate the microstructural characteristics of the matrix and hydrides. We investigated eff ects of hydrogen concentrationand microstructure of matrix on determining microstructural measures of the hydrides. Particularly, we found that β phasein the matrix becomes isolated during the heat treatment same or above 475 °C and this change increases the inhomogeneousnucleation sites signifi cantly. We claim that the microstructure change of this matrix phase greatly increases the numberdensity of hydride.

        Effect of Highly Pressurized Hydrogen Gas Charging on the Hydrogen Embrittlement of API X70 Steel

        During the use of API X70 steel as a pipeline structural material for the transportation of natural gas,hydrogen embrittlement can occur due to the hydrogen contained in natural gas. The aim of this study is toinvestigate the effects of the hydrogen content under high-pressure hydrogen gas conditions on the hydrogenembrittlement of air-cooled API X70 steel. The air-cooled API X70 steel was manufactured by hot rollingand was then air-cooled to room temperature. Tensile test specimens were held for 0 h, 1000 h, and 2000 hwithin a pressure vessel filled with 100% hydrogen gas at a gas pressure of 10 MPa, with the tensile teststhen performed at room temperature. The microstructure of the API X70 steel consists of coarse polygonalferrite, coarse pearlite, and fine acicular ferrite. The yield and tensile strength increased and elongationdecreased considerably after a holding time of 2000 h compared to those of 0 h and 1000 h within the pressurevessel. The morphology of the fracture surface changed from ductile to brittle upon hydrogen gas charging. Secondary cracks were observed in both of the hydrogen-gas-charged specimens. No external cracks wereformed on the surface of the tensile-tested specimen with a 0 h holding time; however, many external crackswere observed on the specimen surface subjected to hydrogen gas charging.

        N/A The 19 months clinical experiences about 634 cases of laparoscpic surgery were reviewed at Sung-Ae General Hospital from July 1991 to Jan. 1993 The results were as follows; Among 168 cases of laparoscopic cholecystectomy and 5 cases of laparoscopic CBD exploration, we obtained better results with advantages of laparoscopic surgery including few complications, less morbidity and early return to work. In series of 361 cases of laparoscopic appendectomy, we obtained superior results especially in the cases of 1) uncertainty of clinically suspected appendicitis, 2) right lower quadrant pelvic pain without other pathology and 3) patients who wanted early return to work. In series of spontaneous pneumothorax with bullae and recurrent pneumothorax, the thoracoscopic bullectomy was much superior than open bullectomy. The laparoscopic Taylor procedures and laparoscopic bandlysis will be better choice in the management of intractable pain in duodenal ulcer and postoperative adhesion. The diagnostic laparoscopy will be much useful in the blunt and penetrating abdominal trauma.

        Effect of simultaneous therapy of arthrocentesis and occlusal splints on temporomandibular disorders: anterior disc displacement without reduction

        Objectives: This study sought to evaluate the effect of simultaneous application of arthrocentesis and occlusal splint. Materials and Methods: A retrospective study of 43 patients (3 males, 40 females) whose symptoms had improved was conducted at the Department of Oral and Maxillofacial Surgery, Dong-A University Hospital between 2008 and 2010. Subjects were divided into three groups: Group A (17 patients with arthrocentesis and occlusal splints simultaneously applied), Group B (13 patients whose symptoms did not improve with occlusal splints, undergoing arthrocentesis after occlusal splint use for 8 weeks), and Group C (13 patients that only used occlusal splints). We compared these groups in maximum comfortable opening (MCO) and the visual analogue scale of pain and noise. Follow-up was performed at 1 week, 1 month, 3 months, and 6 months. Results: The improvement of symptoms was noted in all three groups, but Group A had a quicker improvement than the other groups, in terms of pain reduction and MCO increases. Conclusion: The simultaneous application of arthrocentesis and occlusal splints can reduce patient discomfort more quickly.

        SNL-EFDC를 이용한 군산항 유사퇴적고 계산의 비교 연구

        본 논문은 Ziegler, Lick, and Jones에 의해 개발된 SEDZLJ라 부르는 유사역학 알고리즘이 계산되는 SNL-EFDC 모델을 이용하여 군산항의 퇴적고 계산에 관하여 연구하였다. SEDZLJ모듈은 SEDflume 이나 Annular flumes로부터 얻은 현장 침식률 및 침식 한계전단응력 자료를 직접 결합할 필요가 있다. 군산항에서는 SEDflume 자료를 구할 수 없기 때문에 Ryu와 Yim에 의한 Annular flumes 실험으로부터 얻은 현장 침식률 및 침식 한계전단응력 자료를 이용하였다. SNL-EFDC 모델이 군산항의 유사퇴적고를 계산하기 위해 적용되었고 관측된 부유사 농도와 퇴적고를 사용하여 검증되었다. 유사 퇴적고 계산에 대한 모델의 적용성은 NSEC가 0.72로 침식과 퇴적의 물리적 변화가 적절하게 재현됨이 입증되었다. 부가적으로 검증된 모델은 저면 활성층두께에 대한 매개변수 α값의 영향을 조사하기 위해 사용되었고, Lee와 Park에 의해 수행된 SEDTM을 이용한 유사이동 계산의 결과와 비교하기 위해 사용되었다. 그 결과, 매개변수 α값의 변화는 유사퇴적고 계산에 미미한 영향이 미치는 것으로 나타났다. SEDZLJ에 의한 결과를 SEDTM의 것과 비교할 때 SEDZLJ모듈이 군산항에서 SEDTM 보다 NSEC가 0.03향상 되어 토사이송 과정을 재현함이 확인되었다. This paper was studied on the sediment deposition height computation at Gunsan port using recently advanced SNL-EFDC model which the sediment dynamics algorithm was called as SEDZLJ developed by Ziegler, Lick and Jones. SEDZLJ module needs to directly incorporate site-specific erosion rate and critical shear stress data from SEDflume or Annularflumes. Here this model attempted to use site-specific erosion rate and critical shear stress data from Annular flumes experiment by Ryu and Yim instead of unavailable SEDflume data at Gunsan port. SNL-EFDC was applicated to calculate deposition height at Gunsan port and verified using observed values for suspended sediment concentration and sediment deposition height. The model applicability for sediment deposition height computation was evaluated as NSEC 0.72 and verified that it reproduced the physical variations of erosion and deposition. In addition, the verified model was used to investigate the effect of parameter “α” on the active bed layer thickness and used to compare with the result of sediment transport computation using SEDTM done by Lee and Park. As a result, it was found that the change of parameter “α” value has a negligible effect on sediment deposition height computation. As results of SEDZLJ are compared with those of SEDTM, it was identified that SEDZLJ module reproduces the sediment transport process more appropriately than SEDTM at Gunsan port with advanced NSEC 0.03.
