        Evaluation of the Efficacy and Safety of DA-9601 versus Its New Formulation, DA-5204, in Patients with Gastritis: Phase III, Randomized, Double-Blind, Non-Inferiority Study

        <P>This study compared the efficacy of DA-9601 (Dong-A ST Co., Seoul, Korea) and its new formulation, DA-5204 (Dong-A ST Co.), for treating erosive gastritis. This phase III, randomized, multicenter, double-blind, non-inferiority trial randomly assigned 434 patients with endoscopically proven gastric mucosal erosions into two groups: DA-9601 3 times daily or DA-5,204 twice daily for 2 weeks. The final analysis included 421 patients (DA-5204, 209; DA-9601, 212). The primary endpoint (rate of effective gastric erosion healing) and secondary endpoints (cure rate of endoscopic erosion and gastrointestinal [GI] symptom relief) were assessed using endoscopy after the treatment. Drug-related adverse events (AEs), including GI symptoms, were also compared. At week 2, gastric healing rates with DA-5204 and DA-9601 were 42.1% (88/209) and 42.5% (90/212), respectively. The difference between the groups was −0.4% (95% confidence interval, −9.8% to 9.1%), which was above the non-inferiority margin of −14%. The cure rate of gastric erosion in both groups was 37.3%. The improvement rates of GI symptoms with DA-5204 and DA-9601 were 40.4% and 40.8%, respectively. There were no statistically significant differences between the two groups in both secondary endpoints. AEs were reported in 18 (8.4%) patients in the DA-5204 group and 19 (8.8%) in the DA-9601 group. Rates of AE were not different between the two groups. No serious AE or adverse drug reaction (ADR) occurred. These results demonstrate the non-inferiority of DA-5204 compared to DA-9601.         DW1029M, a novel botanical drug candidate, inhibits advanced glycation end-product formation, rat lens aldose reductase activity, and TGF-β<sub>1</sub> signaling

        <P>DW1029M is a botanical extract consisting of Morus bark and Puerariae radix, produced by Dong-Wha Pharmaceutical, for nephroprotective drug development; it has been in phase II clinical trials in Korea. In our mechanistic investigations, we found that DW1029M inhibits advanced glycation end products (AGEs), rat lens aldose reductase (RLAR), and transforming growth factor (TGF)-beta(1) signaling, all of which are implicated in diabetic complications such as diabetic nephropathy and diabetic retinopathy. DW1029M inhibits AGE formation via Fe2+ chelation. The extract contains 13 active constituents that inhibit AGE formation, 8 active constituents that inhibit RLAR activity, and 1 inhibitor of TGF-beta(1) signaling. Our results suggest DW1029M protects against diabetic nephropathy via blockade of AGE formation, RLAR activity, and TGF-beta(1) signaling.</P>

      • Metoclopramide가 휜쥐의 血壓에 미치는 影響

        Influences of metocopramide (MCP), which is a seletive dopaminergic antagonist, on blood pressure of the rat and its mechanism of action were investigated in the present study. MCP administered into a femoral vein of the rat caused markedly a dose-related fall in blood pressure followed by secondary transient pressor response. The depressor action evoked by MCP was not blocked by pretreatment of atropine or chlorisondamine, while the pressor action was inhibited significantly. Prazosin treatment reduced markedly both of depressor and pressor induced by MCP. MCP-evoked pressor action was not affected by clonidine, but the depressor response was attenuated significantly. Debrisoquin treatment made the inhibited response to MCP-induced pressor response, while did not affect the depressor action. Both of pressor and depressor responses evoked by MCP were not influenced by cyproheptadine. The hypertensive activity induced by nor epinephrine was weakened markedly by the infusion of MCP (1.50 mg/kg/30min.), but the pressor action evoked by dopamine not affected. These experimental data suggest that MCP produces biphasic responses (depressor and pressor) in blood pressure of the rat, and that the hypotensive activity is due to adrenergic alpha-receptors blockade, and that pressor activity is exerted through stimulation of cholinergic nicotinic receptors in autonmic ganglia.

        The aim of this study is to report 2 cases of acute promyelocytic leukemia who died from intracranial hemorrhage following leukapheresis and to provide proper preventive measures against hemorrhage following leukapheresis. From 1994 to 1997, a total of twenty-six patients with leukemia underwent leukapheresis to control hyperleukocytosis at Ajou University Hospital. Two patients with acute promyelocytic leukemia received all-trans retinoic acid but developed drug-induced hyperleukocytosis. Shortly after leukapheresis, they died from intracranial hemorrhage. The risk factors for fatal hemorrhage are thought to be coexisting disseminated intravascular coagulation(DIC), thrombocytopenia aggravated by leukapheresis, exacerbated coagulopathy related to mechanical trauma through leukapheresis and the excess use of citrate during leukapheresis. To reduce the risk of the bleeding associated with leukapheresis, it is necessary to replace platelet sufficiently before and after leukapheresis and to give calcium to correct coagulopathy induced by excess citrate which is used as anticoagulant as well as the correction of disseminated intravascular coagulation.

        Cr, Co, Cu, Ni와 Ca의 첨가에 따른 지하매설배관의 전기화학적 특성에 관한 연구

        Aqueous corrosion characteristics of low carbon steel with small amounts of Cr, Co, Cu, Ni and Ca, in synthetic groundwater was studied by electrochemical corrosion tests (potentiodynamic test and electrochemical impedance spectroscopy (EIS) measurements) and analytical techniques. Neither carbon steel nor newly alloyed steels showed passive behavior in this synthetic groundwater. Newly alloyed steels, containing Cr-Co, Cr-Cu-Ni and Cr-Cu-Ni-Ca showed higher corrosion resistance than carbon steel in the potentiodynamic tests. EIS measurements showed that the Nyquist plot presented two time constants. The high frequency resistance component (R_(rsut)) and low frequency resistance component (R_(et)) were affected by the alloying elements. The polarization resistance (RP= R_(et) +Ra) of steels could be clearly ranked as Cr-Cu-Ni-Ca steel >> Cr-Cu-Ni steel > Cr-Co steel > carbon steel. Results of surface analyses (XPS and EPMA) showed that Cr and Cu were concentrated in the inner region of rust. However, Co and Ni were uniformly distributed all over the rust layer. These alloying elements improved corrosion resistance of low alloy steel in synthetic groundwater. Especially, Cr-Cu-Ni-Ca steel had the lowest corrosion rate due to the inner rust film formation during the initial stage of corrosion, which suggested a potential for substituting carbon steel in soil environment (Received October 29, 2004)

      • MgO-Cr₂O₃캐스타블의 화학적 손상에 관한 연구

        마그-크롬질 캐스타블에 대한 내알칼리성 증진에 의한 수명 향상의 가능성을 검토하였다. 골재로서 전융 알루미나, 크롬질, 소결 스피넬, 마그-크롬질, 마그네시아질 순으로 알칼리 침식에 대한 저항성이 우수하였다. 마그-크롬질은 다른 골재에 비하여 알칼리에 대한 침식저항성 뿐만 아니라 열충격에 대한 저항성이 양호하였다. 장시간 적용 시험시 β-2CaO · SiO2.3CaO · SiO2 및 K2SO4가 존재하였고 조직 취화를 초래하여 가동면에서 손상을 가속하였다. 전융 마그네시아를 사용한 경우 소결 마그네시아질에 비하여 재질 변경 및 조직 치밀화로 인하여 알칼리에 대한 침식 억제 효과가 있었다. For long life of applications in MgO-Cr2O3 castables, the improvement of chemical resistance against alkali salts were investigated. Chemical resistance against alkali salts was excellent in order, i.e fused alumina, chrom ore, sinterded spinel, Magnesia-chrome, magnesia based castables. magnesia-chrome based tastable was comparatively good in the chemical attacks and the thermal resisatnce. As working for long time, β-2CaO · SiO2, 3CaO · SiO2 and K2SO4 were existed, and they accelerated chemical wear in the working area of magnesia-chrome castables caused the degreaded microstructure. The chemical resistance of fused magnesia in MgO-Cr2O3 castables showed the more excellent than that of the sintered magnesia due to the densification of mircrostructure and the change of materials

        상수도 배관의 부식감지시스템 개발 (Ⅱ)

        In order to develop a new corrosion sensor for detecting and monitoring the corrosion of buried pipeline, the electrochemical property of sensors and the correlation of its output to corrosion rate of steel pipe, were evaluated by electrochemical methods in synthetic groundwater, two soils of varying resistivity (5,000 ohm-cm, 10,000 ohm-cm), and synthetic tap water. In this paper, two types of electrochemical probes were used: galvanic cells containing of pipeline steel-copper and pipeline steel-stainless steel (Type 304). The results of EIS measurement indicated that the sensor current was inversely related to sensor resistance, which was governed by the corrosion behavior of cathode. In galvanic corrosion tests, the galvanic current of Cu-CS probe was higher than that of SS-CS probe. The comparison of the sensor output and corrosion rates revealed that a linear relationship was found between the probe current and the corrosion rates. A good linear quantitative relationship was found between the Cu-CS probe current and the corrosion rate of pipeline steel coupons in the soil resistivity of 5,000 ohm-cm, and synthetic tap water. In the case of the soil resistivity of 10,000 ohm-cm, although the SS-CS probe showed a better linear correlation than that of Cu-CS probe, the Cu-CS probe is more suitable than SS-CS probe, due to the high current output.         In order to observe hydrological characteristics in the Gulf of Yoeng-il, measurements of water temperature have been carried out from February to August 1983, and from August 1985 to December 1986. Surface water temperatures are closely linked to air temperature and solar-radiation for all littoral stations. Seasonal and annual variations of all horizontal stations are very similar to one another. Vertical variations of water temperatures for the stations located along the central-line of the Gulf are characteristic (and observed) as thermoclines and thermal stratification in the hottest summer-time (August). It is interesting to note that water column temperature of July and September are rather warmer than August, on account of a cold water mass coming from NE direction. For example, surface temperature in June 1982 was above 19℃, but the water below a depth of about 30m drastically drops to 4℃. It is indicative of invasion of water mass from cold current or deep sea.
