        The causes of pyogenic liver abscess has been known as biliary tract disease or intrabadominal infection but the large proportions of the patients has no apparent underlying disorders. Recently colonic mucosal lesions were reported in patients with cryptogenic liver abscess and it has been suggested that colonic mucosal break may play a role in developing liver abscess in otherwise healthy patients. We experienced a patient of severe recurrent liver abscess complicated with endophthalmitis only 3 months after successful treatment of initial cryptogenic liver abscess and a polypoid colon cancer was discovered by chance. It seems prudent to proceed colonoscopic examination in patients with cryptogenic liver abscess especially when it is recurrent.

      • Platelet-to-lymphocyte ratio is related with the severity of OSA syndrome and hypertension

        Background: Chronic intermittent hypoxia, platelet activation and inflammation are the related mechanisms of pathogenesis in obstructive sleep apnea syndrome (OSAS), which may link to the risk of cardiovascular disease (CVD). The aim of this study was to evaluate the relationship of platelet- to-lymphocyte ratio (PLR), a new biomarker showing systemic inflammation, and platelet distribution width (PDW), an indicator of platelet activation, with the severity of OSAS assessed by polysomnographic parameters. Methods: This was a cohort study in which patients who had undergone a full night polysomnography for diagnosis of OSAS were recruited. According to apnea-hypopnea index (AHI) scores, individuals inwhom AHI was less than 5 were recruited as the control group, those in whom 5-15 as the mild OSAS group, those in whom 16-30 as the moderate OSAS group, and those in whom AHI was greater than 30 as the severe OSAS group. The presence of hypertension, diabetes mellitus, current smoking, and dyslipidemia were defined as CVD risk. Results: A total of 290 patients were involved in the study. There was significant linear increase of age, body mass index (BMI), systolic blood pressure and male proportion according to the higher AHI groups. PLR and PDW were also significantly different among the groups, and were significantly associated with AHI (r=0.417 for PLR and r=0.227 for PDW, all p’s<0.001) and Epworth sleepiness scale (ESS) (r=0.160 for PLR and r=0.189 for PDW, all p’s<0.05). Multivariate regression analysis showed that high AHI (≥9.2) (adjusted odds ratios [OR] 5.03, 95% confidential interval (CI) 1.67-15.2, p=0.004) and PLR≥159 (adjusted OR 2.81, 95% CI 1.34-5.91, p=0.006) were significant independent predictors of the hypertension. Conclusions: PLR and PDW were related with the severity of OSAS. Moreover, PLR might be useful as a systemic inflammation biomarker to predict hypertension in OSAS patients.         Effect of Phoxilium on prognostic predictors in patients undergoing continuous venovenous hemodiafiltration

        Background: Phosphorus-containing dialysis solution is used to prevent hypophosphatemia in patients undergoing continuous venovenous hemodiafiltration (CVVHDF). This study evaluated the effect of phosphorus-containing dialysis solution on mortality in pa-tients undergoing CVVHDF based on changes in phosphorus and red cell distribution width-coefficient of variation (RDW-CV) levels. Methods: We included 272 patients with acute kidney injury (AKI) who underwent CVVHDF at the medical intensive care unit from 2017 to 2019 and classified them according to Phoxilium (Baxter Healthcare Ltd.), as a phosphorus-containing dialysis solution, use within 48 hours after CVVHDF initiation. Clinical data were collected at baseline and 48 hours after CVVHDF initiation. The primary outcome was all-cause mortality during the follow-up period. Results: The non-Phoxilium (NP) group had higher phosphorus and lower RDW-CV levels than the Phoxilium (P) group (phosphorus, 7.3 ± 4.3 vs. 5.0 ± 2.8 mg/dL; RDW-CV, 14.6 ± 1.9 vs. 15.7 ± 2.6%; all p < 0.001). In the multivariable Cox proportional hazard re-gression of the NP group, an increase in phosphorus and RDW-CV at 48 hours of CVVHDF was associated with mortality (delta phos-phorus: median, >0 mg/dL vs. < -2.0 mg/dL; hazard ratio [HR], 8.62; 95% confidence interval [CI], 2.10-35.32; p = 0.003/delta RDW-CV: median, >0% vs. < -0.2%; HR, 4.34; 95% CI, 1.49-13.18; p = 0.008). Meanwhile, in the P group, an increase in delta RDW- CV was associated with mortality (delta RDW-CV: >0% vs. >-0.2% and <0%; HR, 2.65; 95% CI, 1.12-6.24; p = 0.03), while an in-crease in delta phosphorus was not. Conclusion: In patients with AKI undergoing CVVHDF, the risk factors for all-cause mortality differed according to the initial phospho-rus levels and use of Phoxilium.

        Objective: Present an unusual case Target: Beginning practicioners Methods: Conventionally bleeding control during laparoscopic myomectomy was achieved by suturing. Electro-cauterization without suturing during myomectomy was only limited to small myoma. Thus, suturing was considered essential for removal of myoma larger than 3cm. We report here on a case of myomectomy over 10cm without suturing in previous High-Intensity-Focused-Ultrasound (HIFU) state. The rare case of myomectomy without suturing was successfully treated by careful bipolar cauterization and drainage.

        State and Trait Anxiety of Adolescents with Autism Spectrum Disorders

        Objective This study examined how state and trait anxiety of adolescents with autism spectrum disorders (ASD) are associated with their demographic characteristics, repetitive and restricted behaviors (RRBs), and internalizing and externalizing problem behaviors.Methods A total of 96 participants with ASD (mean age=14.30 years; 91 males) completed a battery of tests including the State/Trait Anxiety Inventory (STAI), the Autism Diagnostic Interview-Revised, the Social Responsiveness Scale (SRS), and a cognitive test measuring intelligence quotient (IQ). Participants’ parents completed the Child Behavior Checklist (CBCL). Pearson’s correlations among age, IQ, two subscales of the STAI (i.e., STAIS and STAIT, measuring self-reported state and trait anxiety, respectively), and the Anxiety subscale of CBCL (i.e., CBCL-Anxiety, measuring parent-reported trait anxiety) were computed. Subsequently, Pearson’s correlations were computed among the three anxiety measures, RRBs, and problem behaviors, while controlling for participants’ age and IQ.Results The STAIS and CBCL-Anxiety were both significantly correlated with higher age, sensory sensitivity, depressive symptoms, somatic complaints, and aggressive behaviors. All three anxiety variables were significantly and positively correlated with total SRS RRB scores. Additionally, the STAIS and STAIT were significantly associated with more severe Compulsion/Adherence behaviors, and the CBCL-Anxiety was also significantly associated with more severe Rule-breaking Behaviors.Conclusion Self-reported state anxiety showed association patterns similar to those of parent-reported trait anxiety. Future studies investigating the precise operationalization of different anxiety instruments are needed to accurately measure the anxiety of adolescents with ASD.

      • Nanotopographic cues and stiffness control of tendon-derived stem cells from diverse conditions

        <P><B>Background</B></P><P>Tendon-derived stem cells (TDSCs) are key factors associated with regeneration and healing in tendinopathy. The aim of this study was to investigate the effects of mechanical stiffness and topographic signals on the differentiation of TDSCs depending on age and pathological conditions.</P><P><B>Materials and methods</B></P><P>We compared TDSCs extracted from normal tendon tissues with TDSCs from tendinopathic Achilles tendon tissues of Sprague Dawley rats in vitro and TDSCs cultured on nanotopographic cues and substrate stiffness to determine how to control the TDSCs. The tendinopathy model was created using a chemical induction method, and the tendon injury model was created via an injury-and-overuse method. Norland Optical Adhesive 86 (NOA86) substrate with 2.48 GPa stiffness with and without 800 nm-wide nanogrooves and a polyurethane substrate with 800 nm-wide nanogrooves were used.</P><P><B>Results</B></P><P>TDSCs from 5-week-old normal tendon showed high expression of type III collagen on the flat NOA86 substrate. In the 15-week normal tendon model, expression of type III collagen was high in TDSCs cultured on the 800 nm NOA86 substrates. However, in the 15-week tendon injury model, expression of type III collagen was similar irrespective of nanotopographic cues or substrate stiffness. The expression of type I collagen was also independent of nanotopographic cues and substrate stiffness in the 15-week normal and tendon injury models. Gene expression of scleraxis was increased in TDSCs cultured on the flat NOA86 substrate in the 5-week normal tendon model (<I>P</I>=0.001). In the 15-week normal tendon model, scleraxis was highly expressed in TDSCs cultured on the 800 nm and flat NOA86 substrate (<I>P</I>=0.043). However, this gene expression was not significantly different between the substrates in the 5-week tendinopathy and 15-week tendon injury models.</P><P><B>Conclusion</B></P><P>Development and maturation of tendon are enhanced when TDSCs from normal tendons were cultured on stiff surface, but not when the TDSCs came from pathologic models. Therapeutic applications of TDSCs need to be flexible based on tendon age and tendinopathy.</P>

        The Effects of Vascular Endothelial Growth Factor on Capsule Formation around Silicone Implant in Rats

        Background The purpose of this study was to examine the effects of a single administration of vascular endothelial growth factor (VEGF) in promoting the angiogenesis and thereby reducing the formation of capsular contracture. Methods We treated 24 female Sprague-Dawley rats with (1) 5 mM Tris Buffer and 150 mM NaCl 0.1 cc, (2) VEGF 15 μg/0.1 cc, (3) VEGF 150 μg/0.1 cc during placement of the implant, or (4) VEGF 150 μg/0.1 cc and VEGF 300 μg/0.2 cc. We histopathologically measured the thickness of the capsule and the number of blood vessels. Results All experimental groups had a significant difference in the thickness of the capsule compared to the control group (P<0.001). There was no significant difference between experimental group 2 and experimental group 3. The number of blood vessels formed around the capsule was significantly greater in all the experimental groups compared with the control group (P<0.05). There was no significant difference between the experimental groups. There was a significant negative correlation between the thickness of the capsule and the number of blood vessels (Spearman’s correlation coefficient, 0.2732; P<0.0001). Conclusions A single administration of VEGF reduced formation of the capsule and increased the vascularity around the implant, supporting the hypothesis that prevention of tissue ischemia can be a treatment strategy for capsular contracture.

      • Preparation of plasmonic monolayer with Ag and Au nanoparticles for dye-sensitized solar cells

        <P>We fabricated a plasmonic layer by immobilizing both Au and Ag nanoparticles via P4VP on a photoactive layer for dye-sensitized solar cells (DSSCs) to prevent aggregation of metal nanoparticles, to increase absorbance of N719 dye, and to enhance light harvesting and power conversion efficiency (PCE). The optimal conditions for immobilizing these nanoparticles were also examined. With plasmonic Au and Ag nanoparticles, the PCE increased by 8.05% and 5.78%, respectively (plasmonic Au nanoparticles: from 8.82% to 9.53%; plasmonic Ag nanoparticles: from 8.82% to 9.33%). When both Au and Ag nanoparticles were employed in the plasmonic layer, the PCE showed further improvement of 10.17%, corresponding to a 15.31% enhancement. This significant improvement of the PCE could be explained by a broader range of light absorption resulting from the presence of the plasmonic layer. (C) 2017 Elsevier B.V. All rights reserved.</P>
