        폴리페놀을 다량 함유한 커피박 및 녹차박 추출물의 수준별 첨가가 반추위 발효 및 메탄 발생량에 미치는 영향

        This study was conducted to investigate the effect of addition levels of coffee and green tea by products extract including polyphenols through hot water extraction on rumen fermentation. The treatment groups consisted of coffee extract (CO), green tea extract (GR) and mixed extract (MIX), and the addition level was 10 μL, 20 μL and 30 μL of three levels. The experiment consisted of a total of 10 experimental groups including the control group, and a full factorial design was used. The effect of polyphenol addition in coffee and green tea by-products was analyzed through main and interaction effect of statistical analysis. The total polyphenol content of the extracts was 106.15, 79.10 and 185.25 μg GAE/g DM for coffee by-product, green tea by-product and mixture, respectively. Total gas production was significantly lower in the treatment groups than in the control (114.00 mL/gDM) (p<0.05). Methane emission tended to decrease as the polyphenol addition level increased. Moreover, the MIX showed the lowest methane emission when 30 μL was added (p<0.05). Volatile fatty acids showed a significant difference compared to the treatment group as a control (98.06 mM) (p<0.05), but there was no change according to the level of polyphenols. As a result of the main effect and interaction, it is thought that the effect on methane reduction and improvement of rumen fermentation in MIX20 can be expected. In a series of studies, the addition of 20 μL of a blended extract of coffee and green tea by-products is thought to reduce methane to levels that do not inhibit rumen fermentation.

        Usefulness of Smilax China Leaves Fermented Product as a Cosmetic Material for Scalp Hair Protection

        Purpose: Today, the population with hair loss is decreasing in age given various factors, and due to such, the market for scalp and hair cosmetics is exploding. Due to the growing interest in side effects such as allergies and environmental pollution, recently, many studies have been conducted to find cosmetic raw materials from natu-ral substances or to apply fermentation techniques to the cosmetic manufacturing process. Hence, after enzy-matic fermentation with Smilax china, which has been used for various pharmacological functions in the private sector for a long time and has secured clinical safety, the active ingredients and effectiveness(in vitro) were ex-amined. Method: After preparing a sample by the enzymatic fermentation of Smilax china, which is known to have various pharmacological actions, with malt, the active ingredients and effectiveness(in vitro, HaCaT cell protec-tive effect against oxidative stress, antibacterial activity, anti-inflammatory activity, dermal papilla cell prolifer-ation rate) was confirmed. Results: As a result of the experiment conducted, the fermented Smilax china enzyme had excellent antibac-terial activity, HaCat cell protective effect against oxidative stress, anti-inflammatory activity, and dermal papilla cell proliferation rate, respectively. Conclusion: It was confirmed that the fermented product obtained by fermenting Smilax china malt with malt offers the excellent HaCat cell protection effect against the oxidative stress, antibacterial activity, anti-inflam-matory activity, and the dermal papilla cell proliferation rate, and hence, it can be applied as a raw material for various cosmetics. In particular, the usefulness as a cosmetic material for scalp and hair that is effective for the health of the scalp and hair was confirmed. Future research is expected to articulate the pharmacological mech-anisms of the Smilax china fermented products through the physiological studies related to the fermentation process and the growth rate of dermal papilla cells.

        A Study on the Usefulness of the Natural Product Fermentation Mixture as Scalp and Hair Care Cosmetic Material Protection

        Purpose: The hair losing population is increasing due to stress, environmental pollution, and various cosmetic procedures, and the market for functional cosmetics for scalp and hair care is further growing due to the lower aging population. Recently, however, many studies have been conducted where many are found to seek cosmetic raw materials from natural substances or apply fermentation techniques to cosmetic manufacturing processes. Hence, after fermenting with Sasa Borealis, which is edible and has been used for various pharmacological func-tions in the private sector for a long time and has secured clinical safety, the effectiveness(in vitro) has been investigated, and the fermented mixture of natural products is used as a sample for damaged hair, and the re-covery effect has been examined and understood. Method: Examined in this study were the antibacterial activity, anti-inflammatory activity, and dermal papilla cell proliferation rate of Sasa Borealis leaf fermented product, which were mixed with the green coffee bean and Smilax china L. leaf fermented product, and after selecting the fermented mixed sample, the added scalp hair care product was treated with heat perm and dyeing to measure tensile strength and glossiness, where the re-covery effect of damaged hair was investigated by imaging with SEM. Results: Sasa Borealis leaf ferment was superior to M. canis, anti-inflammatory activity and dermal papilla cell proliferation. The product to which the fermentation mixture sample was added increased both the tensile strength and glossiness of the hair damaged by heat perm and dyeing, and as a result of confirming such via the SEM, it was found that there is an effect of restoring the damaged hair. Conclusion: Sasa Borealis leaf fermented product has excellent M. canis, anti-inflammatory ability, and der-mal papilla cell proliferation rate, and hence, it was confirmed that it may be applied as a raw material for various cosmetics. The cosmetics containing natural fermentation mixtures increased tensile strength and glossiness of damaged hair. Future research is expected to articulate the pharmacological mechanism of the fermentation mixture through the physiological studies related to the fermentation process and the growth rate of dermal papilla cells.

        Arthroscopic Partial Meniscectomy versus Physical Therapy for Degenerative Meniscal Tear: a Systematic Review

        Background: Meniscal tears are commonly observed in patients with knee osteoarthritis (OA), however, clinical significance of such lesions detected by magnetic resonance imaging is in many cases unclear. This study aimed to determine the clinical effectiveness of arthroscopic partial meniscectomy (APM) compared with non-operative care in patients with knee OA. Method: We used existing systematic reviews with updates of latest studies. Three randomized controlled studies were selected, where two studies compared the effects of APM plus physical therapy (PT) with PT alone and one compared APM alone and PT alone. While 1 study exclusively included OA patients, 2 studies included 21.1 and 12% of patients with no radiographic OA. Patients with knee locking were unanimously excluded. Results: Upon comparison of APM plus PT and PT alone, there was no significant difference observed in knee function, physical activity, or adverse events. Knee pain was observed to be significantly lower in the APM plus PT group at 6 months, but there was no difference between the two groups at 12 and 24 months. With respect to the comparison between APM alone and PT alone, PT was non-inferior based on the criteria for knee function during 24 months; however, knee pain was significantly reduced in the APM alone group. Conclusions: Our study showed that knee pain was significantly improved in the APM group compared to non-operative care group at 6 months and over 24 months. Our result was based on only 3 randomized controlled trials (RCTs) revealing a significant knowledge gap, hence demanding more high-quality RCTs in OA patients.

      • Poster Session : PS-1544 ; COPD : Effectiveness of Bronchoscopic Lung Volume Reduction Using Unilateral Endobronchial Valve: A Systematic Review and Meta-Analysis

        Background: Bronchoscopic lung volume reduction (BLVR) can be suggested as an alternative for surgical lung volume reduction surgery for severe emphysema patients. This article intends to evaluate the safety and effectiveness of BLVR using one-way endobronchial valve by systematic review. Methods: A systematic search of electronic databases including MEDLINE, EMBASE, and Cochrane Library, as well as eight domestic databases up to December 2013 was performed. Two reviewers independently screened all references according to selection criteria. The Scottish Intercollegiate Guidelines Network (SIGN) criterion was used to assess quality of literature. Data from randomized controlled trials (RCTs) were combined and meta-analysis was performed. Results: This review included 15 studies. The forced expiratory volume in one second (FEV1) improved in the intervention group compared to the control group (MD=6.71, 95% CI: 3.31 to 10.11). Six minute walking distance (MD=15.66, 95% CI : 1.69 to 29.64) and cycle workload (MD= 4.43, 95% CI: 1.80- to7.07) also improved. In addition, the St. George`s Respiratory Questionnaire (SGRQ) score decreased (MD -4.29, 95% CI: -6.87 to -1.71) in the intervention group. In a subgroup analysis of patients with complete fissure, the FEV1 change from baseline was higher in the BLVR group than the control group for both 6 month (MD=14.75, p <.001) and 12 months (MD=17.43, p <.001) whereas patients with incomplete fissure the FEV1 and 6MWD showed no change. One year follow-up randomized controlled trials reported deaths although the cause of death was not related to BLVR. Respiratory failure and pneumothorax incidence rate was relatively higher in the BLVR group but the difference was not significant. Conclusions: Bronchoscopic lung volume reduction may be an effective and safe procedure for the treatment of severe COPD patients with emphysema based on existing studies.

        Comparative efficacy of biological agents in meth otrexate-refractory rheumatoid arthritis patients: a Bayesian mixed treatment comparison

        Background/Aims: Biological agents (biologics) targeting proinflammatory signaling have emerged as an important treatment option in rheumatoid arthritis (RA). Despite the clinical effectiveness of biologics for patients with RA who do not respond to `traditional` disease-modifying anti-rheumatic drugs (DMARDs), there are concerns regarding their cost and long-term safety. In this study, we aimed to compare the efficacy of various biologics and traditional DMARDs in RA patients refractory to methotrexate (MTX). Methods: Four DMARDs (hydroxychloroquine, sulfasalazine, MTX, lef lunomide) and five anti-tumor necrosis factor drugs (adalimumab, etanercept, golimumab, inf liximab, and certolizumab) were selected. A systematic search of published studies was performed from inception through July 2013. Randomized trials of adults with MTX-refractory RA comparing two or more of the selected medications were included. Among 7,938 titles identified, in total, 16 head-to-head trials were selected. Two reviewers independently abstracted the study data and assessed methodological quality using the Cochrane Risk of Bias. Comparative efficacy was analyzed using a Bayesian mixed treatment comparison (MTC). Results: In total, 9, 4, and 11 studies were included for the outcome measures of the Health Assessment Questionnaire (HAQ), Disease Activity Score 28-erythrocyte sedimentation rate (DAS28-ESR) < 2.6 (remission), and American College of Rheumatology (ACR) 70 response, respectively. The treatments with the highest efficacy for each outcome measure were certolizumab combined with MTX, golimumab combined with MTX, and certolizumab combined with MTX, respectively. Conclusions: Based on MTC analysis, using data from published randomized controlled trials, certolizumab and golimumab combined with MTX showed the highest efficacy in the three outcome measures (HAQ, DAS28-ESR < 2.6, and ACR 70 response) in MTX-refractory RA patients.

        GaN light-emitting diodes on glass substrates with enhanced electroluminescence

        <P>We report the enhanced electroluminescence (EL) of GaN light-emitting diodes (LEDs) on glass substrates by controlling GaN crystal morphology, crystallinity, and device fabrication. Depending on the degree of epitaxy, we studied three different GaN morphologies; randomly oriented GaN polycrystals, nearly single-crystalline pyramid arrays, and fully single-crystalline pyramid arrays, which were fabricated by controlling the epitaxial relationship with the substrates. At proper growth temperature, GaN crystallinity was improved with increasing GaN crystal size irrespective of the GaN crystallographic orientation, as determined by spatially resolved cathodoluminescent spectroscopy. All the different GaN morphologies were further fabricated into LEDs to investigate their EL characteristics. The optimized GaN LEDs on glass composed of the nearly single-crystalline GaN pyramid arrays exhibited excellent microscopic EL uniformity and luminance values of about 2700 and 1150 cd m<SUP>−2</SUP> at peak wavelengths of 537 and 480 nm, respectively.</P> <P>Graphic Abstract</P><P>GaN LEDs on glass exhibiting excellent microscopic and macroscopic uniformities by optimizing GaN pyramid crystallinity and related process. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c2jm34405j'> </P>

        Two Years of Experience and Methodology of Korean COVID-19 Living Clinical Practice Guideline Development

        Background: In Korea, during the early phase of the coronavirus disease 2019 (COVID-19) pandemic, we responded to the uncertainty of treatments under various conditions, consistently playing catch up with the speed of evidence updates. Therefore, there was high demand for national-level evidence-based clinical practice guidelines for clinicians in a timely manner. We developed evidence-based and updated living recommendations for clinicians through a transparent development process and multidisciplinary expert collaboration. Methods: The National Evidence-based Healthcare Collaborating Agency (NECA) and the Korean Academy of Medical Sciences (KAMS) collaborated to develop trustworthy Korean living guidelines. The NECA-supported methodological sections and 8 professional medical societies of the KAMS worked with clinical experts, and 31 clinicians were involved annually. We developed a total of 35 clinical questions, including medications, respiratory/critical care, pediatric care, emergency care, diagnostic tests, and radiological examinations. Results: An evidence-based search for treatments began in March 2021 and monthly updates were performed. It was expanded to other areas, and the search interval was organized by a steering committee owing to priority changes. Evidence synthesis and recommendation review was performed by researchers, and living recommendations were updated within 3–4 months. Conclusion: We provided timely recommendations on living schemes and disseminated them to the public, policymakers and various stakeholders using webpages and social media. Although the output was successful, there were some limitations. The rigor of development issues, urgent timelines for public dissemination, education for new developers, and spread of several new COVID-19 variants have worked as barriers. Therefore, we must prepare systematic processes and funding for future pandemics.

        Usefulness of the Green Coffee Bean Fermented Product as a Cosmetic Material for Scalp Hair Protection

        Purpose: The population experiencing hair loss has increased given the stress, environmental pollution, and the various cosmetic procedures, and the functional cosmetics market for the scalp hair care is further expanding due to the aging population. However, side effects such as allergies have occurred, and the interest in environmental pollution is rising. Recently, many studies have been conducted to search cosmetic raw materials from natural substances or applied fermentation techniques to the cosmetic manufacturing process. Accordingly, the green coffee beans which are edible and have been used for various pharmacological functions for and by the private sector for long and have secured clinical safety were fermented, then the active ingredients and effectiveness (in vitro) were investigated. Method: After fermenting the green coffee beans which are known to be effective for the scalp hair health by using EM, and preparing a sample using a double fermentation method of enzyme fermentation with malt, active ingredients (caffeine, polyphenol, flavonoid, and sugar) and effectiveness (in vitro, antioxidant activity, anti-inflammatory activity, and the dermal papilla cell growth rate) were verified. Results: As a result of the experiment, the caffeine and sugar content increased during the EM fermentation and enzyme fermentation, and there was no difference discovered according to the enzyme fermentation temper-ature. The total polyphenol and the total flavonoid content decreased during the enzyme fermentation, and enzyme fermentation at 60℃ of EM fermentation solution of green coffee bean extracts demonstrated an excellent rate of anti-inflammatory activity and dermal papilla cell growth. Conclusion: It was verified that the fermented product of the green coffee bean double fermented by using EM and malt has an excellent rate of antioxidant, anti-inflammatory, and dermal papilla cell growth, and hence, it may be applied as a raw material for various cosmetics. In particular, it was verified that it is effective for the scalp and hair health, while the usefulness as a cosmetic material for scalp hair was also verified.

        Korean Clinical Practice Guidelines: Current Status of Adherence to the RIGHT Checklist

        Background: The Korean Academy of Medical Sciences (KAMS) has been utilizing AGREE II to audit the quality of clinical practice guidelines (CPGs) developed in Korea. Monitoring the RIGHT Checklist adherence could help monitor the quality status and discover areas for improvement of CPG development. Methods: We included 129 CPGs from the past 5 years and assessed each item of the RIGHT Checklist. STATA version 15.0 was used for statistical analysis. Results: Among the seven sections of the RIGHT checklist, sections with a full compliance rate over 60% were ‘basic information’ (65%) and ‘background’ (66%). The other sections’ mean full compliance rates were ‘Evidence’ 52%, ‘Recommendation’ 35%, ‘Review and quality assurance’ 25% and ‘Funding, declaration and management of interest’ 17%. Sections with a partial compliance rate over 60% were ‘Recommendation’ (60%) and ‘Funding, declaration and management of interest’ (70%). Non-compliance was highest in the ‘Review and quality assurance’ (17%) domain. In comparison between groups 1 (under median group) and 2 (over median group), group 2 showed a tendency to have multi-stakeholder involvement and present sufficient information on financial resources and conflict of interest declarations. For the CPGs developmental methodology aspect, group 2 provided more pertinent information than group 1 about supporting evidence-making and the process from evidence to recommendation. Conclusion: This study evaluated adherence to the RIGHT Checklist of CPGs developed in Korea. It can provide helpful information to develop strategic plans for enhancing the capabilities of developing CPGs in Korea.
