WHO PROTESTS AND WHY: HIERARCHICAL GOVERNMENT TRUST AND PROTEST PARTICIPATION IN CHINA

We present a theory on how trust in the central government to remedy grievances combined with a lack of trust in local government to act motivates people to participate in local protests in China. Low trust in local government combined with high trust in the central government gives people expectation that protest will not be an exercise in futility. People protest to redress injustices when they believe that such protests have a chance of producing a favorable resolution of their grievances. Utilizing individual level data from the Asian Barometer Survey Wave 4, our analysis suggests that, in contemporary China, people who have greater trust in the central government than the local government are more likely than others to report having participated in protests. In a society without meaningful elections, participating in protest is an effective strategy for attracting the attention from the upper-level authorities in hope of redressing unfavorable situations.

Background: In intensive care units, mechanical ventilation is an important therapy to help patients with dysp- nea. However, long-term ventilator dependence would consume huge medical resources and increase the risk of morbidity and mortality. The aim of the study was to examine the efficacy of the acupuncture combined with western medical care on ventilator parameters in ventilator-dependent patients. Methods: In this clinical trial, 80 ventilator-dependent patients aged 20 to 80 years old were randomly assigned to acupuncture group and control group in the respiratory care center (RCC) of Changhua Christian Hospital. Besides regular medical care and therapy, participants in the acupuncture group received acupuncture therapy at the same 17 acu-points for 20 minutes once a day, a total of 12 sessions. The ventilator parameters were recorded to evaluate the respiratory efficiency for all participants. The primary outcome was rapid shallow breathing index (RSBI), and secondary outcomes were respiratory rate (RR), tidal volume (TV) and ventilation per minute (MV). Results: Though there was no significant difference in the parameter between the acupuncture group and the control group, we found the trend of decreasing RSBI in the acupuncture group. In subgroup analyses, the mean of RSBI significantly decreased 16.02 (with the SD in 60.84) in acupuncture group, while it increased 17.84 (with the SD in 39.38) in control group (p = 0.036) after 12 sessions. Conclusion: Acupuncture treatment can improve breathing ability of patients with respirator dependence in respiratory care center.

This paper presents the structural and optical properties of europium doped magnesium orthosilicate nanophosphor prepared by combustion method and further annealed at varying temperatures. The structural and optical properties were investigated by using X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FTIR), Field emission scanning electron microscope (FESEM), High resolution Transmission electron microscope (HRTEM), Diffuse Reflectance Spectra (DRS) and Photoluminescence spectra (PL) together with decay time. The XRD results confirmed the phase formation of Mg2SiO4. FESEM and TEM results indicated spherical particles in nanometer size. Photoluminescence spectra of Mg2SiO4:Eu3þ nanophosphors exhibited five groups of peaks 5D0 / 7FJ (J ¼ 0, 1, 2, 3, 4) transitions. Intensity parameters (U2 U4) and other radiative properties such as radiative transition probability, total radiative transition probability, radiative life time, branching ratio and stimulated emission cross-section were calculated using Judd- Ofelt theory. The lifetime measurement indicates a decreasing trend up to 5 mol% of Eu3þ concentration.

Very high frequency (VHF) PECVD has been demonstrated to be able to significantly increase the deposition rate without compromising the film quality for the manufacture of silicon heterojunction and silicon thin film solar cells. To further reduce the production costs by enhancing the throughput, larger electrode and higher frequency are often required at the same time. Nevertheless, raising frequency in large-area PECVD results in non-uniform discharge caused by the standing wave effect and deteriorates the processing uniformity. In this study, a technique that generates a traveling wave via superposing two specific standing waves launched simultaneously is proposed to resolve this issue. An industrial-scale linear plasma reactor with length and width of 125 and 10 cm, respectively, is adopted for experimental tests and two 80 MHz power supplies are utilized to separately control the standing waves. The experimental results show that the discharge gap is only partially covered by plasma discharge when only one standing wave is applied. However, as both standing waves are launched, the non-uniformity of plasma discharge can be effectively reduced to <±5%. In addition, numerical simulation is also conducted in this study to clarify whether the proposed technique can be applied to large-area rectangular PECVD (substrate size: 1.4 m 1.1 m). By arranging multiple feeding points on opposite sides of the powered electrode, the simulation results indicate the non-uniformity of electric field can be maintained within ±10%.

Objective: We wanted to assess the usefulness of four-dimensional (4D) ultrasonography (US), i.e., real-time three-dimensional US, as an adjunct for performing various US-guided interventional procedures in superficial lesions. Materials and Methods: Thirty-three patients were referred for US-guided interventional procedures for superficial lesions, including core biopsy in 19, fineneedle aspiration in eight, therapeutic drug injection in four and needle puncture in two. The procedures were performed under 4D US guidance. We reviewed the pathologic/cytologic results of the core biopsies or needle aspirations, and also the outcomes of drug injection or needle puncture. Results: For all the patients who underwent 4D US-guided core biopsy, the specimens were adequate for making the pathological diagnosis, and specimens were successfully obtained for those patients who underwent 4D US-guided aspiration. The patients treated with 4D US-guided therapeutic drug injection or needle puncture had a good response. No major procedure-related complications occurred. The procedural times were similar to those procedural times with using two-dimensional US. Conclusion: Combining the two dimensional and 4D US techniques aids the physician when performing US-guided interventional procedures for the superficial lesions.

Background/Aims: In Taiwan, due to budget limitations, the National Health Insurance only allows for a limited period of biologics use in treating moderate to severe Crohn’s disease (CD). We aimed to access the outcomes of CD patients following a limited period use of biologics, specifically focusing on the relapse rate and remission duration; also the response rate to second use when applicable. Methods: This was a multicenter, retrospective, observational study and we enrolled CD patients who had been treated with adalimumab (ADA) according to the insurance guidelines from 2009 to 2015. Results: A total of 54 CD patients, with follow-up of more than 6 months after the withdrawal of ADA, were enrolled. The average period of treatment with ADA was 16.7±9.7 months. After discontinuing ADA, 59.3% patients suffered a clinical relapse. In the univariate analysis, the reason for withdrawal was a risk factor for relapse (P=0.042). In the multivariate analysis, current smoker became an important risk factor for relapse (OR, 3.9; 95% CI, 1.2-14.8; P=0.044) and male sex was another risk factor (OR, 2.9; 95% CI, 1.1-8.6; P=0.049). For those 48 patients who received a second round of biologics, the clinical response was seen in 60.4%, and 1 anaphylaxis occurred. Conclusions: Fifty-nine percent of patients experienced a relapse after discontinuing the limited period of ADA treatment, and most of them occurred within 1 year following cessation. Male sex and current smoker were risk factors for relapse. Though 60.4% of the relapse patients responded to ADA again. (Intest Res 2017;15:487-494)

Since the introduction of pancreas transplantation more than 40 years ago, surgical techniques and immunosuppressive regiments have improved and both have contributed to increase the number and success rate of this procedure. However, graft survival corresponds to early diagnosis of organ-related complications. Thus, knowledge of the transplantation procedure and postoperative image anatomy are basic requirements for radiologists. In this article, we demonstrate the imaging spectrum of pancreas transplantation with enteric exocrine drainage.

Steroids are commonly used in patients with eosinophilic meningitis caused by A. cantonensis infections. The mechanism steroids act on eosinophilic meningitis remains unclear. In this mouse experiments, expressions of 14-3-3 isoform β and γ proteins significantly increased in the CSF 2-3 weeks after the infection, but not increasedin the dexa- methasone-treated group. Expression of 14-3-3 β, γ, ε, and θ isoforms increased in brain meninges over the 3-week pe- riod after infection and decreased due to dexamethasone treatment. In conclusion, administration of dexamethasone in mice with eosinophilic meningitis decreased expressions of 14-3-3 isoform proteins in the CSF and in brain meninges.

Objective: Hospitalization of patients with delirium after visiting the emergency department (ED) is often required. However, the readmission risk after discharge from the ED should also be considered. This study aimed to explore whether (i) immediate hospitalization influences the readmission risk of patients with delirium; (ii) the readmission risk is affected by various risk factors; and (iii) the healthcare cost differs between groups within 28 days of the first ED visit. Methods: Using the National Health Insurance Research Database, the data of 2,780 subjects presenting with delirium at an ED visit from 2000 to 2008 were examined. The readmission risks of the groups of patients (i.e., patients who were and were not admitted within 24 hours of an ED visit) within 28 days were compared, and the effects of the severities of different comorbidities (using Charlson’s comorbidity index, CCI), age, gender, diagnosis and differences in medical healthcare cost were analyzed. Results: Patients without immediate hospitalization had a higher risk of readmission within 3, 7, 14, or 28 days of discharge from the ED, especially subjects with more severe comorbidities (CCI≥3) or older patients (≥65 years). Subjects with more severe comorbidities or older subjects who were not admitted immediately also incurred a greater healthcare cost for re-hospitalization within the 28-day follow-up period. Conclusion: Patients with delirium with a higher CCI or of a greater age should be carefully considered for immediate hospitalization from ED for further examination in order to reduce the risk of re-hospitalization and cost of healthcare.
