      • 저강도 초음파 및 레이저를 이용한 장관골 불유합의 치료

        목 적: 저강도 초음파는 인체 및 동물 실험에서 골조직에 낮은 미세한 물리적 압력을 가하여 골절의 치유에 유리한 영향을 주고, 레이저 (LASER)는 심부 조직에 혈류를 증가시키고, 세포 내 잠재된 에너지를 증가시키는 것으로 알려져 있다. 이에 저자들은 임상적으로 확진된 장관골 붕류합의 치료에 있어 펄스형 저강도 초음파와 레이저를 적용하고 그 효용성을 알아보고자 하였다. 대상 및 방법: 초음파/레이저의 치료 효과를 비교하기 위하여 장관골 불유합으로 확진된 16예 중 8예는 연구군으로 골이식술 후 저강도 초음파와 레이저 치료를 추가하였고 대조군 8예에 대해선 일반적인 골이식술만 시행하여 조사하였다. 남자가 11예, 여자가 5예, 평균 연령은 41.7세 (19~62세)였고, 불유합 부위는 대퇴골이 10예, 경골이 6예였다. 첫 수상으로 인한 골절 후 초음파/레이저 치료까지 시간(골절 연력)은 평균 502일이었다. 결 과: 8예의 연구군에서는 완전 골유합이 6예에서 이루어졌으며 유합까지의 시간은 평균 141일 (101~202일)로 나타난 반면, 8례ㅢ 대조군에서는 5예에서 골유합이 이루어졌고, 유합까지의 시간은 평균 240일 (183~283일)이었다. 결 론: 저강도 초음파/레이져 골절 치료기가 반복된 수술로 인한 난치성 장관골 불유합의 치료에 있어 골이식술과 함께 사용될 경우 긍정적인 효과가 있었고, 특히 최초 가골 형성에 있어 대조군에 비해 그 시간을 줄일 수 있었따. Purpose: Low-intensity, pulsed ultrasound (LIPUS) has demonstrated a stimulation and acceleration of the normal fracture-repair process in cellular bases as well in animal and human models. In this study, the adjunctive effect of LIPUS and LASER was investigated in established nonunion of the long bones. Materials and Methods: Study group consisted of eight patients to whom conventional bone graft and adjunctive LIPUS and LASER was applied. On the other hands, eight patients in the control group underwent bone graft only. Eleven men and five women were included in this study and the average age of the patients was 41.7 years (range, 19~62 years). Six of the tibias and ten of the femurs met the criteria for established nonunion. The average fracture age, the post-fracture period before the start of LIPUS/LASER treatment, was 502 days. Results: Seven of the ten nonunions who were treated by LIPUS and LASER healed completely in an average treatment time of 141 days (range, 101~202 days) and otherwise, in control group, five of the eight nonunions healed completely, in 240 days (range 183~283 days). Conclusion: This clinical study showed a positive effect of LIPUS and LASER on the rate of osseous repair, especially accelerated time to initial callus cortical bridging compared to that of conventional treatment.

        제 3형 구상돌기 골절의 수술적 치료

        목적: 8례의 제 3형 척골 구상돌기 골절에 대해 수술적 치료 후 임상적 방사선학적 결과를 보고하고자 한다. 대상 및 방법 : 8명의 구상돌기 골절에 대한 후향적 연구를 시행하였다. 대상은 모두 남자였으며 평균 나이는 33세였다. 3례에서 척골 구상돌기 단독 골절이었으며 2례에서 주관절 탈구가, 2례에서 요골 두 및 경부 골절이, 1례에서 내측 측부인대 손상이 동반되었다. 전례에서 전방 도달법을 통해 유관나사를 이용한 관혈적 정복 및 내고정이 시행되었다. 평균 추시 기간은 31개월 이었으며 (24~60개월), Mayo Elbow Performance Score로 임상적 결과를 판정하였다. 결과 : 최근 추시에서 평균의 능동적 주관절 가동범위는 105°였다. 평균의 Mayo Elbow Performance Score는 76.9점이었다(50~95). 1례에서 최우수, 4례에서 우수, 2례에서 양호, 1례에서 불량의 결과를 보였다. 결론 : 제 3형의 척골 구상돌기 골절에서 조기의 관혈적 정복 및 안정적인 내고정으로 신뢰할 만한 치료를 제공하였으며, 손상 주관절의 동반된 인대손상과 복합골절은 중요한 예후인자로 간주되었다. Purpose: To review the clinical results of eight cases of typeⅢ coronoid process fractures which were treated operatively. Materials and Methods : Eight patients with coronoid type Ⅲ were reviewed retrospectively. All were men with an average age of 33. There were three isolated fractures, two elbow dislocations, two radial head and neck fractures, and one medial collateral ligament rupture. Open reduction and internal fixation through anterior approach with canulated screws was used. The patients were followed up for a mean of 31 months(24 to 60). Results: Average active elbow joint motion at the most recent follow up was 105°. The average Mayo Elbow Performance Score was 76.9 (50 to 95). There was one excellent result, four good, two fair, and one poor. Conclusion: Early open reduction and stable internal fixation provided a reliable method for the treatment of typeⅢ coronoid process fractures. Any associated ligament injuries to the elbow and fracture comninution were considered as important prognostic factors.

      • PBD가 설치된 연약지반의 원심모형실험에 관한 연구

        This study is the result of experimental and numerical study on the consolidational behavior of multi-layered soft soil ground installed with plastic board drains (PBD). Centrifuge model tests with a marine clay sampled from field were performed to investigate the consolidational behavior of multi layered ground where a dredged soil was placed on the soft clay ground and PBDs were installed. Test results were compared with those of numerical analyses, using the 2-D equivalent model previously proposed. From test resuts, it was found that the amount of consolidation settlement occurred in the original ground due to embankment surcharge loads was in the range of 38% of total settlement in the whole ground. From the results of cone penetration tests executed after finishing the centrifuge model tests, the cone resistance was found to increase with depth. The measured water contents inbetween PBDs were in the ranges of 38~50% and their values tended to increase with increasing the distance between PBDs.

      • 경호업 종사자의 직무유형과 동기수준 및 직무만족의 관계

        The purpose of this study was to analyze the relationship between motivation level, job satisfaction along with private security employees job style. For accomplishing this purpose of the study, the 262 subject were selected by the cluster random sampling method from private security employees in 6 private security companies located in Seoul and Kyoung-gi do. The survey questionnaires were used to collect data. The questionnaire for motivation level was developed by Frills and Knox(1972), Quinn and Shephard(1978) and standardized in Korean language by Young-In, Hur(1995) and the job satisfaction was developed by Rizzo(1970) and standardized in Korean language by Yong-Nam, Joo(1993). The statistical methods utilized in this study for analyzing the collected data were reliability analysis, descriptive statistics, Chi-square, t-test, One-way ANOVA, and standard multiple regression analysis. From the analysis of this study, the following conclusions were obtained: Firstly, the motivation level, job satisfaction along with private security employees are partially different by the socio-demographic characteristics. Secondly, the motivation level is not different by the private security employees job style. Thirdly, the job satisfaction is partially different by the private security employees job style. In detail, in job condition along with job style, safe employees are more satisfaction than security employees. Lastly, the motivation along with private security employees influence on the job satisfaction. In detail, the achievement needs, role conflict, and autonomy influence on the job condition and also the role conflict and autonomy influence on the role value.

      • 고점성 청동분말을 이용한 알루미늄 합금의 레이저 클래딩 특성

        Laser cladding processing allows rapid transfer of heat to the material being minimum conduction into base metal. The effects of CO_2 laser cladding with high ?? powder were investigated. High viscosity bronze powder consists of bronze powder a?? used at a high temperature. The material has a high viscosity so that it can be e?? substrate. Therefore, Laser cladding can be processed on a curved or slope surface. CO_2 laser cladding was designed. It consists of the high viscosity bronze powder f?? the shielding gas system and the preheating system. The high viscosity powder properly at 0.35g/s and 0.50g/s. Because of the metallic bond between bronze powder the hardness of dilution layer was suddenly increased. Experimental as results in?? viscosity mixed powder can be a useful cladding material.

        Recently, the social threat has been significantly increased by the natural disasters such as earthquakes, floods, and storms in addition to an unusual weather change, a global warming, a rapid change of infrastructure, and terrors. Therefore, the strategic and comprehensive approach are required to build the national disaster prevention system to minimize the damages. In this study, the disaster prevention system has been categorized to scientific, integral, and self-regulation system. The prevention system should be established ahead of the recovery system to minimize the damages and a scientific system should be introduced by developing new technology and self-regulation system should lead a participation of citizens.

      • 풍화암에 근입된 그라운드 앵커의 인발거동 연구

        This study is an numerical study of predicting the behavior of anchor embedded in weathered rocks, subjected to uplift loads, about ultimate pullout capacity and the failure mechanism. Factors influencing the behavior of anchors were investigated by reviewing the data about in-situ anchor tests performing numerical modelling with changing the bondage length of anchor, diameter of anchor body and diameter of tenden, and by Correlations between those factors were evaluated to apply them to predict the behavior of anchors. As results of numerical analysis, a linear relationship between bondage length, diameter of anchor body and diameter of tenden with ultimate pullout capacity was obtained on the one hand, from the result of numerical analysis changing the Young's modulus of weathered rock, this parameter was found to inflence to load-displacement and ultimate pullout capacity within the range of 10%, which was mot so significant to affect.
