Lidocaine으로 Retrobulbar block時 發生한 過敏反應 1例

This report shows a fatal hyperergy occurring during retrobulbar block using lidocaine on a 42 years old male patient. When we injected 60㎎ of lidocaine into the retrobulbar space, following signs were noticed in one minute: hypertension, convulsion, dyspnea and loss of consciousness. In addition to the above symptoms, cardiac arrest developed in 2∼3 minutes and did not respond to any resuscitative effort. It is considered that 60㎎ of lidocaine for retrobulbar block can induce hyperergy and cardiac arrest. The clinical picture and treatment of such reaction is discussed with particular attention to the importance of close observation of all anesthetic patients whether under general or local anesthesia.

마이크로 팩토리 시스템에서 마이크로 방전 가공 시스템은 많은 장점을 가진다. 전통적인 가공 시스템과 달리 비접촉 가공 방식이기 때문에 소형화에 따른 강성저하 문제에서 자유로우며, 정밀 가공이 가능하다. 그러나 방전가공에서 사용하는 전극에서, 가공 중 발생하는 마모 문제는 생산성뿐만 아니라 가공 정밀도를 떨어뜨리는 문제를 발생시킨다. 이번 연구에서는 전극의 마모 문제를 보완할 수 있는 방전 드릴링 가공과 밀링 가공에서의 공정 제어기법을 제안하였다. 또한, 마이크로 팩토리 용 테스트베드의 방전 가공 모듈로 디스크형 미세 전극 드레싱 가공 시스템, 자동 공구 교환 장치 시스템, 소형 유체 순환 시스템 등도 개발하였다.

Objectives:Hypothalamic-Pituitary-Adrenocortical(HPA) system dysregulation plays an important role in the pathophysiology of major depressive disorder. Medically unexplained somatic symptoms comprised the predominant complaints of korean patients with major depressive disorder. It might be related to dysregulation of HPA system and somatic symptoms. The aim of this study was to investigate the association between Hamilton Depression Rating Scale severity and the Plasma Adrenocorticotropic Hormone(ACTH) & Serum Cortisol level in Korean patients with major depressive disorder. Methods:Our study design was prospective. A total 111 hospitalized patients with major depressive disorder (DSM-IV) at Anam Hospital, Korea University Medical Center between Jan 2009 and May 2011 were selected. Hamilton Depression Rating Scale was measured by psychiatrist. Plasma ACTH, Serum Cortisol levels were analyzed at the time of admission. Results:There are significant correlations(p<0.05) between Hamilton Depression Rating Scale of somatic component(HAM-D 11-15 item) and Plasma ACTH and Serum Cortisol level. However, other HAM-D sub items were not significantly correlated with Plasma ACTH and Serum Cortisol level except somatic component. Conclusion:Our Study results suggest that hyperactivity of HPA axis might be correlation with somatic symptoms in korean patients with major depressive disorder.

The neodymium:yttrium-aluminum-garnet (Nd:YAG) laser is most widely used for treatment of lesions affecting the oral mucosa. A pulsed Nd:YAG laser (SDL-300EN, Samsung Electronics, Korea) with a wavelength of 1.32㎛ was used. In all cases, contact mode was used. Four cases of soft tissue oral lesions are presented to document the clinical aspects. Surgery proceeded without complication and healing was uneventful. This study indicate that the Nd:YAG laser can be used successfully for intraoral soft tissue applications are well tolerated and minimal bleeding.

Tumor necrosis factor (TNF) receptor members have unique structures composed of 2-4 cysteine - rich pseudorepeats in the extracellular domain. On ligation by trimeric ligand molecules, oligomerization of three receptor molecules occurs, which in turn activates the receptor and recruits intracellular signaling molecules to the cytoplasmic tail to initiate biological events. Recently, the numbers of tumor necrosis factor receptor and ligand family members have been rapidly expanding. Functional characterization of the new members has indicated redundant roles with other known members as well as provided insights into novel functions. In particular, identification of soluble decoy receptors which have the ability to bind multiple ligands highlights a complex control mechanism of immune responses by these molecules. Studies of the new members have also revealed that the TNF receptor and ligand family members play an important role in other than the immune system.
