Skin Necrosis after High Dose Vasopressor Infusion in Septic Shock -Two Case Reports-

Survival sepsis campaign recommends that vasopressor therapy is required to maintain mean arterial pressure (MAP) ≥ 65 mmHg. However, the absolute maximum dose of vasopressor is difficult to determine. Herein, we report 2 cases of severe skin necrosis after high dose vasopressor infusion to maintain the recommended MAP in septic shock. In our first case, norepinephrine 1.0-2.0 μg/kg/min and vasopressin 0.03-0.1 U/min were infused for 5 days; in the second case, dopamine 10-20 μg/kg/min and norepinephrine 0.25-2.5 μg/kg/min were infused for 7 days. Severe ischemic skin lesions, which required amputations, developed in both cases. The clinical appearance of the skin lesions in the 2 cases was different because of the unique distribution of target receptors for different vasopressors. Thus, when high dose vasopressors are required to achieve recommended MAP, extra vigilance is required. Further studies for dose adjustment are needed.

이 글은 『혼불』을 여성을 중심으로 하는 역사서사라는 사실에 착안하여 ‘여성’과 ‘공간’을 살피고자 하였다. 이를 위해 먼저 ‘장소’와 ‘공간’이라는 개념을 정리한다. 다음으로는 남성과의 관계맺음을 중심으로 하여 여성의 ‘장소상실’과 ‘공간’이라는 변화를 분석한다. 마지막으로 이러한 과정을 통하여 『혼불』의 여성인물들의 ‘장소 · 공간 분화’에 대해 살핀다. 이는 매안이씨 종가와 종가의 여성들의 관계에 주목하여 ‘장소’로서의 ‘집’이 공간으로 확장되고 축소되고 있음을 밝히는 과정이다. 『혼불』에서는 양반사회에 대한 천민의 반란이 두드러진다. ‘변동천하’로 요약되는 그러한 반란의 기운을 ‘매안이씨 종가’의 여인들과 ‘장소 · 공간 분화’와의 관계 속에서 살폈다는 점에 이 글의 의의가 있다고 하겠다. This study aims to examine ‘Women’ and ‘Space’ as a process of approaching the fact that Honbul is a history book focusing on women. Prior to approaching ‘Women’ and ‘Space’, we will examine the concept of ‘Place’ and ‘Space’. Then, we will examine changes in ‘Loss of Place’ and ‘Space’ of women, while focusing on their making relationships with men. Lastly, based on the contents examined earlier, we will examine ‘Place & Space Differentiation’ of the female characters in Honbul. It appears remarkable that the lowest class’s revolt against the aristocratic society in Honbul. This study has significance, in that such a spirit of ‘Byeondongcheonha (meaning a great upheaval all over the world)’ can examine changes from the upper, not a revolt from the lower, by examining the women of ‘the Head Family of Maeahn’ and ‘Place & Space Differentiation’.
Thermal Inactivation of Sodium-Habituated Staphylococcus aureus in Ready-to-Heat Sauces

The objective of this study was to evaluate the effect of sodium habituation on thermal resistance of Staphylococcus aureus in various ready-to-heat (RTH) sauces. The strain mixture of S. aureus strains KACC10768, KACC10778, KACC11596, KACC13236 and NCCP10862 was habituated up to 9% of NaCl. The inocula of NaCl-habituated and nonhabituated S. aureus were inoculated in 5 g portions of pork cutlet, meat and Carbonara sauces at 7 Log CFU/g, and the samples were vortexed vigorously. The inoculated samples were then exposed to 60 and 70oC in a water-bath, and survivals of total bacteria and S. aureus were enumerated on tryptic soy agar and mannitol salt agar, respectively, every 30 min for 120 min. At 60oC, the cell counts of total bacteria and the significant difference in survivals between sodium-habituated and non-habituated S. aureus were observed only in the Carbonara sauce; the tailing effect, which is the period of no reduction of bacterial cell counts, was observed in pork cutlet, meat and Carbonara sauces subjected to 60oC. RAS guanyl-releasing protein 3 (RasGRP3), a member of the Ras subfamily of GTPases, functions as a guanosine triphosphate (GTP)/guanosine diphosphate (GDP)-regulated switch that cycles between inactive GDP- and active GTP-bound states during signal transduction. Various growth factors enhance hepatocellular carcinoma (HCC) proliferation via activation of the Ras/Raf-1/extracellular signal-regulated kinase (ERK) pathway, which depends on RasGRP3 activation. We investigated the relationship between polymorphisms in RasGRP3 and progression of hepatitis B virus (HBV)-infected HCC in a Korean population. Nineteen RasGRP3 SNPs were genotyped in 206 patients with chronic liver disease (CLD) and 86 patients with HCC. Our results revealed that the T allele of the rs7597095 SNP and the C allele of the rs7592762 SNP increased susceptibility to HCC (OR=1.55, p=0.04 and OR=1.81${\sim}$2.61, p=0.01${\sim}$0.03, respectively). Moreover, patients who possessed the haplotype (ht) 1 (A-T-C-G) or diplotype (dt) 1 (ht1/ht1) variations had increased susceptibility to HCC (OR=1.79${\sim}$2.78, p=0.01${\sim}$0.03). In addition, we identified an association between haplotype1 (ht1) and the age of HCC onset; the age of HCC onset are earlier in ht1 +/+ than ht1 +/- or ht1 -/- (HR=0.42${\sim}$0.66, p=0.006${\sim}$0.015). Thus, our data suggest that RasGRP3 SNPs are significantly associated with an increased risk of developing HCC.

본 연구에서는 임의의 관측지에서의 조위를 예측하기 위하여 인근 조위 관측소의 조위 관측자료를 입력으로 하는 딥러닝 모델을 제시한다. 시계열 데이터 예측에 뛰어나다고 알려져 있는 순환신경망(RNN, Recurrent Neural Network) 모델을 사용하였으며, 모델의 훈련 및 검증을 위하여 울산 조위관측소와 부산 조위관측소의 조위 데이터를 이용하였다. 조위 예측의 정확도를 평가하기 위하여 평균 제곱급 오차(RMSE, Root Mean Square Error) 및 상관계수(CC, Correlation Coefficient)를 계산한 결과, 각각 5cm, 0.95로 높은 정확도가 나타났다. 또한 예측된 데이터를 조화 분해하여 실제 조위 데이터의 결과와 비교하였을 때 4대 분조 모두에서 진폭 정확도 95% 이상, 위상차 4° 이하가 나타났다. In this research, tidal level of an arbitrary station is predicted by using a deep learning model. RNN (Recurrent Neural Network) model is used and tidal level data of Ulsan tide station and Busan tide station is used for model training and validation. To evaluate the model performance, accuracy of the developed model is discussed in terms of root mean square error (RMSE) and correlation coefficient (CC) of the predicted tidal level. The results indicated that the model had a high accuracy. Also, harmonic analysis is conducted to compare similarities with observed data. As a result, accuracy of the amplitude was more than 95% and the phase difference was less than 4° in all four largeness tide value.

The aim of this study was to examine how customers perceive storytelling at a real local restaurant and to understand how consumption reflects their evaluation by understanding their values. Participants received information from a real local restaurant in the Gangneung area to examine the effects of storytelling and to evaluate their affective attitudes towards local restaurant stories, word-of-mouth intentions, and purchasing intentions using a seven-point Likert scale. A total of 310 consumers were used. A factor analysis was performed to identify consumption value characteristics and factor structure, which consisted economic value, emotional value, and epistemic value. To test whether local restaurant consumers could be classified into homogenous groups based on their consumption values, a three cluster solution was selected, and a Kmeans cluster analysis was performed. As a result, three groups were identified and labeled appropriate for their scores based on each of the consumption values; emotional value-oriented consumers to cluster 1, epistemic value-oriented consumers to cluster 2, and economic value-oriented consumers to cluster 3. An analysis of variance was used to examine the differences in the affective attitudes towards storytelling at the local restaurant, purchasing intentions, and word-of-mouth intentions across the three clusters. The epistemic value-oriented consumers had the highest score for all three variables. In contrast, economic value-oriented consumers had the lowest scores for the three variables. A regression analysis revealed that affective attitudes towards storytelling were significantly affected by these three consumption values. It also showed that positive affective attitudes towards storytelling were associated with higher purchasing intention and word-of-mouth. The significance of this study was to show how customers perceive storytelling at a real restaurant and reflect on their evaluation by understanding their consumption values. As a result, this study examined the potential power of storytelling as an effective marketing communication tool for local restaurants.

<P><B>Abstract</B></P> <P>Achieving both high CO/CO<SUB>2</SUB> selectivity and large CO working capacity in an adsorbent is very challenging. In this work, we have loaded Cu(II) species into large pores of an iron-based metal-organic framework (MOF) with a large surface area and reduced them into Cu(I) species under mild conditions by utilizing Fe(II) sites in the pores. Remarkably, the Cu(I)-incorporated MOF (0.9Cu@MIL-100) exhibits a high CO/CO<SUB>2</SUB> selectivity (29 at 100 kPa) and a large CO working capacity (1.61 mmol/g at 10–100 kPa) simultaneously, which has not been observed for previously reported adsorbent materials. Moreover, 0.9Cu@MIL-100 also presents very high CO/CH<SUB>4</SUB> and CO/N<SUB>2</SUB> selectivities (87 and 677). Furthermore, breakthrough and cyclic adsorption-desorption experiments confirm that this material can efficiently separate CO/CO<SUB>2</SUB> mixtures under dynamic mixture flow conditions and can be easily regenerated under mild conditions.
