동물성과 식물성 시판 체다 치즈의 향미 특성 및 조직감 분석

본 연구는 동물성과 식물성 시판 체다 치즈의 향미 특성과 조직감을 분석하기 위해 전자코와 전자혀 및 TPA 분석을 이용하였다. Milky, dairy, buttery, creamy, nutty, fruity의 향미속성을 바탕으로 전자코 분석 결과 확인된 화합물 5종을 주요 향기성분으로 선정하였으며, 모든 시료에서 acetaldehyde, 2-methylbutanal 및 n-butanol이 검출되었으나, 2,3-pentanedione과 pentan-2-one은 DT1, VT1 및 VT3에서만 검출되었다. 주성분 분석 결과 VT간에 군집을 이루지 않고 분산되어 있어 동일한 타입 내 시료 간 향기의 유사도가 떨어지는 것으로 나타내었다. VT의 맛 패턴은 DT와 유사한 경향으로 나타나지 않았으며, DT와 VT 내 시료 간에도 맛패턴이 원재료에 의해 구분되지 않았다. 시판 체다 치즈의 조직감 측정 결과는 경도, 부착성, 탄력성 및 응집성 항목에서 생산원료의 타입에 따라 구분되지 않는 것으로 나타내었으나, 조직감에 영향을 미치는 DT와 VT의 단백질 함량은 각각 18.0-20.0%와 1.0% 미만이었다. 본 연구결과는 동물성 치즈와 유사한 향미 특성과 조직감을 갖는 식물성 치즈 제품 개발의 기초자료로서 품질 지표 설정에 활용될 수 있을 것으로 판단된다. This study compared the quality characteristics of commercial Cheddar cheese of dairy types (DT) and vegan types (VT) using electronic sensory evaluation (nose and tongue) and texture profile analysis (TPA). The electronic nose detected acetaldehyde, 2-methylbutanal, and n-butanol in all samples, whereas 2,3-pentanedione and pentan-2-one were detected in DT1, VT1, and VT3. The electronic tongue detected various intensities of six tastes in the two types, regardless of the raw material. Principal component analysis by electronic sensory evaluation revealed the differences between the pattern for VT. The TPA results showed that the textures of DT and VT were not classified according to the raw material. However, the protein content affecting texture was <0.6% for VT compared with that for DT. These results are expected to be useful for establishing quality indicators in developing vegan cheese products with characteristics similar to those of dairy cheese.

다른 기저질환이 없는 환자에서 원인 미상의 급성 폐손상이 발생할 경우 신체 진찰뿐만 아니라 철저한 병력 청취가 중요하다. 저자들은 급성 폐손상으로 내원한 환자에게 원인 규명을 위한 검사에서 수은 중독으로 판단하고 적절한 치료를 통하여 호전된 것을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Mercury is traditionally used as a dye for making amulets in Korea. Inhaling the vapor produced by burning mercury damages major organs, such as the lungs, kidneys, and brain. We herein present a case of a 41-year-old man who complained of abdominal pain and dyspnea. A chest X-ray and computed tomography scan showed infiltration in both upper lung lobes. A thorough medical history revealed that the patient had made amulets prior to developing symptoms, and blood and urine tests confirmed elevated levels of mercury. Dimercaptosuccinic acid was used to chelate the mercury, and methylprednisolone was used to treat the acute lung injury. No kidney or nervous system complications were detected during follow-up. Inhalation of mercury vapor should be suspected in patients with acute lung injury involving both upper lobes. (Korean J Med 2015,89:563-566)

Idiopathic pulmonary fibrosis (IPF) is a progressive fibrosing interstitial lung disease characterized by worsening lung function and dyspnea. The prognosis of IPF patients is poor, as median survival is approximately 3 years. However, recently developed IPF-specific therapies have shown improved efficacies in terms of reducing lung function decline and mortality. Therefore, the early recognition and accurate diagnosis of IPF are crucial. In 2018, new guidelines for the diagnosis of IPF were published by the Fleischner Society and by the American Thoracic Society/European Respiratory Society/Japanese Respiratory Society/Latin American Thoracic Society (ATS/ERS/JRS/ALAT). Both guidelines emphasize the need for a thorough history taking and physical examination to exclude an alternative diagnosis, such as exposure-related or connective tissue disease. The most informative initial examination is high-resolution computed tomography, the results of which can indicate the need for bronchoalveolar lavage or surgical lung biopsy, based on a multidisciplinary discussion of the findings and the patient’s clinical condition. A multidisciplinary discussion of the clinico-radiologic-pathologic findings is currently the gold standard in the diagnoisis of IPF and will allow the more effective and timely treatment of these patients. Immediate-type hypersensitivity responses to systemic corticosteroids are rare despite their widespread use. It is still controversial whether the responses rarely occur or are underdiagnosed in clinical settings. Many cases probably remain underdiagnosed because the symptoms often mimic symptoms of underlying diseases. This case report describes a 73-year-old man who had immediate hypersensitivity reactions after intravenous administration of hydrocortisone, which was primarily intended to prevent hypersensitivity reactions to contrast media. Whole body rash with pruritus developed shortly after the steroid injection. Since the patient already experienced rash and itching sense after contrast media and antihistamine injection, we conducted skin testing to figure out which drug triggered the hypersensitivity reactions. Intradermal skin tests revealed a positive response to hydrocortisone sodium succinate, which suggested his hypersensitivity reactions were developed by hydrocortisone. Being a common therapy for allergic reaction, corticosteroids themselves are rarely suspected of causing hypersensitivity reactions. Considering there is no typical symptom or standard diagnostic test, awareness of corticosteroid hypersensitivity reactions is of importance to make the diagnosis. Drug-induced immune hemolytic anemia (DIIHA) is a rare side effect of drugs. DIIHA may cause a systemic inflammatory response that results in acute multi-organ failure and death. Ceftizoxime belongs to the class of third generation cephalosporins, which are the most common drugs associated with DIIHA. Herein, we present a case of a 66-year-old man who developed fatal DIIHA after receiving a second dose of ceftizoxime. He was admitted to receive photodynamic therapy. He had a history of a single parenteral dose of ceftizoxime 3 months prior to admission. On the day of the procedure ― shortly after the infusion of ceftizoxime ― the patient`s mental status was altered. The blood test results revealed hemolysis. Oliguric acute kidney injury developed, and continuous renal replacement therapy had to be applied. On the suspicion of DIIHA, the patient underwent plasmapheresis. Diagnosis was confirmed by a detection of drug-dependent antibody with immune complex formation. Although his hemolysis improved, his liver failure did not improve. He was eventually discharged to palliative care, and subsequently died.

Primary biliary mucosa-associated lymphoid tissue (MALT) lymphoma is extremely rare. We report a case of primary biliary MALT lymphoma with obstructive jaundice diagnosed by endoscopic biopsy, without surgical intervention. Obstructive jaundice was relieved by endoscopic drainage and endoscopic biopsy was done simultaneously during endoscopic retrograde cholangiopancreatography. Unnecessary surgical intervention can be avoided after pathological confirmation of lymphoma. The patient received radiotherapy, and is alive without any evidence of recurrence or biliary obstruction. Diagnosis of primary biliary lymphoma is very difficult because of its low prevalence. However, it should always be considered as a differential diagnosis, since when an accurate diagnosis is made, unnecessary surgical intervention can be avoided. Background: Superior colliculus is a part of midbrain, and participates in the visual reflexes, It receives afferent fibers from optic nerve, visual cortex, and spinotectal tract. After optic deprivation, the microscopic structure of the superior colliculus changed. Calcium-binding proteins (CBPs) Play an important role in the neuronal protection, differentiation and reorganization of the central nervous system, Objectives and Methods: The effects of neonatal retinal deafferentation on a CBPs, calbindm D-28k (CB), Parvalbumin (PB) and calretimn (CR), and the existence of colocalization between the CBPs were examined immunohistochemically in the rat superior colliculus. Results: On the experimental (contralateral to enucleation) side of superior colliculus, the number of CB-immunoreactive (IR) cells was reduced (77.4% compared to control), but not fibers. The number of PB-IR neurons and fibers was also reduced on the experimental side (88.5% compared to control), In the other hand, the CR-IR cells were dramatically increased (642% compared to control), but CR-IR fibers were markedly decreased on the experimental side. The colocalization between CB-CR and PV-CR was rarely observed in the superior colliculus Conclusion: These results suggest that the changes of retinotectal projection may alter the expressional pattern of CBPs in different manners; relatively stable in CB- and PV-IR neurons and plastic in CR-IR neurons.
