        Long-term outcomes of single-port laparoscopic myomectomy using a modified suture technique

        Objective To evaluate the long-term outcomes, including the pregnancy outcome and recurrence rate after single-port laparoscopic myomectomy (LM) using a modified suture technique with a Hem-o-lok clip (Choi’s LM) and conventional 4-port LM. Methods A retrospective study of patients who underwent Choi’s LM (n=55) and 4-port LM (n=102) in a single institutional hospital was conducted. Patients with <3 symptomatic myomas sized <10 cm each and operated on by a single surgeon were included. Recurrence was confirmed when a myoma measuring ≥3 cm was detected. Results The patients in both groups had similar demographic characteristics. Single (76.4% vs. 62.7%) and intramural (52.7% vs. 56.9%) tumors were commonly detected in both groups in the mean diameter (6.8±1.5 cm vs. 7.0±1.6 cm; P=0.40). In Choi’s LM, 16 patients (29.1%) needed an additional port; those who were nulliparous and/or had a large leiomyoma more frequently required an additional port (P=0.023 and 0.04, respectively). During a median follow-up period of 69 months, 17 patients (7.1% vs. 14.6%) had recurrence. The size of dominant myomas at recurrence was significantly smaller in patients who underwent Choi’s LM (3.4±0.7 cm vs. 5.7±2.4 cm; P=0.004). All 13 patients in both groups who successfully conceived had a full-term delivery. No major complications occurred during pregnancy. Conclusion Although an additional port was frequently used, the long-term outcomes of patients who experienced recurrence and pregnancy after Choi’s LM were acceptable. Considering its usability, Choi’s LM is feasible for the treatment of uterine leiomyoma.

      • Cytosolic Hsp60 orchestrates the survival and inflammatory responses of vascular smooth muscle cells in injured aortic vessels

        <P><B>Aims</B></P><P>Pro-inflammatory response of vascular smooth muscle cells (VSMCs) is triggered by endothelial damage and a causative step for thrombosis and neointimal thickening in the injured arterial vessels. Therefore, we investigate a role of cytosolic Hsp60 as a novel pro-inflammatory mediator in VSMCs.</P><P><B>Methods and results</B></P><P>Hsp60 was detected in the cytosol of VSMCs. The selective depletion of cytosolic Hsp60 in VSMCs reduced the IκB kinase activation, repressed the induction of nuclear factor (NF)-κB-dependent survival genes (MnSOD and Bfl-1/A1), and enhanced apoptotic death in response to TNF-α. Moreover, a quantitative RNA sequencing revealed that the expression of 75 genes among the 774 TNF-α-inducible genes was significantly reduced by the depletion of cytosolic Hsp60. In particular, the expression of pro-inflammatory cytokines/chemokines, such as CCL2, CCL20, and IL-6, was regulated by the cytosolic Hsp60 in VSMCs. Finally, the depletion of cytosolic Hsp60 markedly inhibited the neointimal thickening in the balloon-injured arterial vessels by inducing apoptotic cell death and inhibiting chemokine production.</P><P><B>Conclusions</B></P><P>This study provides the first evidence that cytosolic Hsp60 could be a therapeutic target for preventing VSMC hyperplasia and inflammatory response in the injured vessels.</P>

        Suppression of Azoxymethane-Induced Colorectal Tumors in iNOS<SUP>-/-</SUP> C57BL/6J Mice

        Nitric oxide (NO) is known to be involved in the pathogenesis of colorectal cancer in both rodents and humans. iNOS is responsible for the over production of NO in a variety of parenchymal cells and macrophages. In the present study, we utilized iNOS gene knockout mice to investigate the role of iNOS on chemical-induced colorectal polyposis. Azoxymethane (AOM) at a dose of 10 ㎎/㎏ body weight was administered to male and female iNOS<SUP>-/-</SUP> or iNOS wt C57BL/6J mice once a week for six weeks. The mice were sacrificed at 24 weeks after initiation of experiment and then examined with the incidence and multiplicities of colorectal polyps. The incidence of colorectal tumors were significantly reduced in iNOS gene knockout mice (22.9%), compared to that of control mice (61.4%). The multiplicity in colorectal polyps in the iNOS knockout mice were 0.37±0.77 (n = 35), being significantly less than wild type mice (1.02 ± 1.15, n = 44). The sizes of the polyps in the iNOS gene knockout mice were also decreased. However, according to histopathological observations, most of the adenocarcinomas from iNOS knockout mice were less differentiated compared to those of wild type mice. From the results, iNOS-mediated NO might have a promotive potential in the early stage, but a suppressive effect in the late stage of colorectal carcinogenesis.

        A Low-Jitter and Fractional-Resolution Injection-Locked Clock Multiplier Using a DLL-Based Real-Time PVT Calibrator With Replica-Delay Cells

        <P>A low-jitter and fractional-resolution injection-locked clock multiplier (ILCM) with a delay-locked-loop (DLL)-based process-voltage-temperature (PVT)-calibrator is proposed. The ring-type voltage-controlled oscillator (VCO) and the voltage-controlled delay line (VCDL) of the DLL consist of identical delay cells, and they share the same control voltage. Thus, by changing the ratio between the numbers of stages of the VCDL and the VCO, the frequency of the VCO can be calibrated at any target frequencies, noninteger times the reference frequency. As the amount of the unit delay is adjusted continuously by the DLL, the VCO can overcome real-time frequency drifts as well as static process variations; thus, excellent jitter performance can be sustained during any environmental variations. The proposed ILCM, designed in the 65 nm CMOS process, generated output frequencies that range from 1.2 to 2.0 GHz with a frequency resolution of 40 MHz and a 400 MHz reference clock. When injection locked, the integrated jitter from 1 kHz to 40 MHz of the 1.6 GHz signal was 440 fs. The proposed real-time PVT calibrator restricted the degradations of phase noise and jitter over the temperature and the supply variations to less than 0.7 dB and 20%, respectively. The active area was 0.032 mm(2) and the power consumption was 3.6 mW.</P>

        A Study on the Typification of Corporate Social Responsibility (CSR) Strategies of the World TOP 20 Airports

        지난 50 년간 다양한 산업 분야에서 CSR 전략의 중요성이 강조되어 왔으며 CSR에 대한 연구 역시 활발하게 이어져 왔다. 그러나 항공업계의 CSR 연구는 상대적으로 활발하지 않았으며 진행되어온 연구 역시 CSR보고서 분석을 중심으로 이루어졌다. 이 연구의 목적은 TOP 20 공항의 CSR 전략의 특성을 비교하여 각 공항이 표방하고 있는 사회적 책임의 공통점과 차이점을 제공하는 것이다. 각 공항의 CSR 및 지속 가능성 보고서를 기반으로 한 비교 분석 및 내용 분석과 TBL (Three-bottom-lines)을 사용하였다. 세계 20 공항이 표방하는 CSR 전략 적용 범위에 따라 전략을 그룹화하였으며 이를 통해 그룹별 공항의 사회적 책임 전략에 대한 공통점이 발견되었다. 각 공항은 다양한 공항, 중간 집중 공항 및 고도로 집중된 공항으로 나뉘어지며, 이것은 사회적 책임 전략 및 환경 전략에 적용될 수 있지만 공항의 지역사회활동은 각 공항이 처한 상황에 따라 진행되고 있어 유형화시키기에는 적합하지 않았다. 생물 다양성은 아시아보다 유럽에서 더 적극적으로 채택되었으며 지역적 특성으로는 일본의 공항들이 환경분야 활동에는 다양하게 추진하고 있는 반면 사회적 활동은 추진하고 있지 않음이 발견되었다. 본 연구에서 처음으로 공항의 사회적 책임 전략의 분석과 유형화를 시도하였으며, 이는 세계 주요 공항들이 추진하고 있는 사회적 책임 전략의 공통적이고 차별적인 특성을 발견함으로써 최근 몇 년 동안 업계 전반에서 사회적 책임이 강조되어 왔고 이는 공항의 사회적 책임이 더 이상 환경에 국한된 수동적 대응으로서가 아니라 공항의 경쟁력을 가져가기 위한 전략으로서 활용되고 있음을 의미한다. The importance of the CSR strategies has been increased and progressive studies have been developed for CSR in various industries last five decades. However the CSR studies in aviation industry relatively haven’t been active and the researches have done mainly in CSR reporting analyses. The purpose of this research is to compare and provide examples and characteristics of TOP 20 airports’ CSR strategies. It is discussed using Comparison analysis and Content analysis as well as Three-bottom-lines (TBL) based on CSR/sustainability reports of each airport. It is suggested that characteristics of CSR strategies promoted by the world TOP 20 airports be implemented by grouping strategies according to the degree of coverage of the strategy. In addition, common points of social responsibility strategies of the TOP 20 airports over the group were found. Each airport could be divided into diversified airports, intermediate concentrated airports, and highly concentrated airports. This could be applied to social responsibility strategy and environment strategy, but it could not be applied to the type of coverage based on the social sector strategy of the airport. The characteristics of social responsibility according to regional characteristics were also found. In addition, the biodiversity part is more actively promoted in Europe than in Asia. However, there is no regional feature of the grouping of social responsibility and environmental strategies. It is meaningful to try to analyze and do typification of the airport social responsibility strategy for the first time. This means that social responsibility is emphasized throughout the industry in recent years by discovering the common and discriminative characteristics of the social responsibility strategies promoted by the world s leading airports.

        Neurodevelopment for the first three years following prenatal mobile phone use, radio frequency radiation and lead exposure

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Studies examining prenatal exposure to mobile phone use and its effect on child neurodevelopment show different results, according to child's developmental stages.</P> <P><B>Objectives</B></P> <P>To examine neurodevelopment in children up to 36 months of age, following prenatal mobile phone use and radiofrequency radiation (RFR) exposure, in relation to prenatal lead exposure.</P> <P><B>Methods</B></P> <P>We analyzed 1198 mother-child pairs from a prospective cohort study (the Mothers and Children's Environmental Health Study). Questionnaires were provided to pregnant women at ≤20 weeks of gestation to assess mobile phone call frequency and duration. A personal exposure meter (PEM) was used to measure RFR exposure for 24h in 210 pregnant women. Maternal blood lead level (BLL) was measured during pregnancy. Child neurodevelopment was assessed using the Korean version of the Bayley Scales of Infant Development-Revised at 6, 12, 24, and 36 months of age. Logistic regression analysis applied to groups classified by trajectory analysis showing neurodevelopmental patterns over time.</P> <P><B>Results</B></P> <P>The psychomotor development index (PDI) and the mental development index (MDI) at 6, 12, 24, and 36 months of age were not significantly associated with maternal mobile phone use during pregnancy. However, among children exposed to high maternal BLL <I>in utero</I>, there was a significantly increased risk of having a low PDI up to 36 months of age, in relation to an increasing average calling time (p-trend=0.008). There was also a risk of having decreasing MDI up to 36 months of age, in relation to an increasing average calling time or frequency during pregnancy (p-trend=0.05 and 0.007 for time and frequency, respectively). There was no significant association between child neurodevelopment and prenatal RFR exposure measured by PEM in all subjects or in groups stratified by maternal BLL during pregnancy.</P> <P><B>Conclusions</B></P> <P>We found no association between prenatal exposure to RFR and child neurodevelopment during the first three years of life; however, a potential combined effect of prenatal exposure to lead and mobile phone use was suggested.</P> <P><B>Highlights</B></P> <P> <UL> <LI> RFR exposure was measured by mobile phone use questionnaire and 24-h personal exposure meter among pregnant women. </LI> <LI> Child neurodevelopment was assessed by trained examiners at 6, 12, 24, and 36 months of age. </LI> <LI> Associations were not observed between prenatal exposure to RFR and child neurodevelopment during the first three years. </LI> <LI> A potential combined effect of prenatal exposure to lead and mobile phone use was suggested. </LI> </UL> </P>
