        2D X-선 조영영상에서 정확한 혈관 구조 추출 기법

        본 논문에서는 2D X-선 조영영상의 혈관 구조를 정확하게 추출하는 방법을 제안한다. X-선 조영영상에 혈관 증강 필터를 적용하여 혈관 그래프를 생성하고, 다익스트라 알고리즘을 사용하여 최적의 혈관 경로를 탐색한다. 혈관 경 로 트리의 각 서브트리를 혈관 길이에 따라 반복적으로 자르고, 병합한다. 2D 조영영상에서 추출된 혈관 구조를 3D CT 조영영상에서 분할된 혈관과 강체 정합을 수행하고, 2D 혈관 트리 구조와 3D 혈관 구조에 대한 유사도를 측정한다. 그리고 3D 혈관 세그먼트의 후보군들을 2D 세그먼트의 초기 분할 구조를 기반으로 유사도를 재측정하여 2D 혈관 트리 구조를 재구성한다. 실험으로 10명의 환자 데이터에 대하여 실험한 결과 제안 기법은 혈관 분지에 대한 precision은 평균 0.911765이고, recall은 평균 0.403644로 정확한 혈관 구조 추출 결과를 보여주었다. In this paper, we propose an accurate method for extracting the vessel structures in 2D X-ray angiogram. As the vessel enhancement filter is applied to X-ray Angiogram, the vessel graph is generated. Using Dijkstra’s algorithm, the optimum vessel path is investigated. Each Sub-tree of the vessel path tree is pruned and merged repeatedly according to the vessel length. The rigid registration is performed for the vessel structures that are extracted in 2D X-ray Angiogram and 3D CT Angiogram. Then, the similarity between the 2D and 3D vessel structures is measured. And the similarity remeasurement is performed between 3D vessel segment and the candidates of 3D vessel segment based on the initial segmentation structure of 2D segments. The 2D vessel tree structure is reconstructed according to the result of similarity remeasurement. In the experiment, our method showed the accurate vascular structure extraction result using ten patients' data. Precision and recall of vessel branches were 0.911765, 0.403644 in average, respectively.

        폐경 후 골다공증 및 골감소증 여성의 denosumab 약물 사용 평가

        Background: The indication of denosumab for osteoporosis was expanded from second-line to first-line therapy in 2019. The aim of this study was to evaluate the efficacy of denosumab as both first- and second-line therapy in postmenopausal women with osteoporosis and osteopenia with risk factors by using the Fracture Risk Assessment Tool (FRAX). Methods: We conducted a medication use evaluation of denosumab in 98 patients who had been treated three or more times for osteoporosis or osteopenia at Chungnam National University Hospital from July 1<sup>st</sup>, 2017 to January 31<sup>st</sup>, 2020. Risk factors were identified using quantitative N-gram analyses of FRAX estimations. Patient information, including menopause status and results of bone mineral density tests (T-score), was obtained from electronic medical records. Results: Age, body mass index (BMI), prior medication use, and T-score were identified as risk factors and were included as variables in the evaluation of denosumab use. Since no significant differences were detected between groups, denosumab is likely effective regardless of age or BMI. In addition, no significant difference was detected in T-scores following denosumab treatment, between groups who took bisphosphonates and selective estrogen receptor modulators (SERMs) with denosumab as first-line therapy for postmenopausal osteoporosis. Denosumab may, therefore, be effective as second-line therapy. Conclusion: Efficacy of denosumab was evaluated in postmenopausal women with osteoporosis. Denosumab may be used as first- and second-line therapy regardless of age, BMI, and prior use of bisphosphonates and SERMs.

        골프 연습장 환경이 드라이버 샷에 미치는 영향

        The purpose of this study was to target housewife membership golfers attending beginner’s class and was also to ask them to join a 15-week golf training course in indoor and outdoor golf range (forming two groups), so that it could find out which group had better golf shot performance. To achieve the goals, total 16 subjects were divided into indoor group and outdoor group (8 subjects respectively), but both groups were asked to join same golf training program at same place for 15 weeks. Summing up, it is found that outdoor exercise group shows higher improvement in all factors of driver shot than indoor exercise group, and the former also shows higher improvement in both distance and head speed than the latter. But it is found that both groups show lower impact after exercise. Hence, it is concluded that outdoor group shows higher improvement in overall golf shot performance than indoor group after training program.

        시간 지도에 관한 초등수학교과서 비교 연구 - 한국, 싱가포르, 일본을 중심으로 -

        Our country has excessive amount of learning per hour compared with Japan and Singapore. And as there is no consistence for definition of time between grades, it deteriorates understanding of students. Our country teaches students focusing on time algorism whereas Japan and Singapore teaches their students focusing on flow of time. In composing of mathematics textbook in Korea, Japan and Singapore, textbook of our country contains far more of learning compared with the amount designated in textbooks. Textbooks of Japan contains less teaching elements, but instead it contains much activities to expedite time sense As time is distributed in activities of students, it is more important to construct textbooks with experience of students rather than algorism approaches. In addition, textbooks of Singapore contains various examples and clarified concepts compared with those of our country. Like above, time teaching deployment methods of Japan and Singapore gives us good lessons for teaching time in our country, and it is expected be good reference for future development of our textbooks. 본 연구에서는 시간 지도 개선에 관한 시사점을 찾고자 한국, 싱가포르, 일본의 초등학교 수학교과서를 비교 분석하였다. 연구 결과에 따르면, 우리나라는 두 나라에 비해 시간의 덧셈과 뺄셈의 사칙 계산과 그 알고리즘이 강조되고 있었다. 또한 두 나라에 비해 시간 지도 요소 항목이 많이 편이었으며, 소재와 시계 모형에 있어서도 유사한 것들이 반복 사용되고 있었다. 마지막으로 시간의 흐름이 적게 반영되어 있었다.

        파고지 추출물의 렛트에 대한 단회 경구 투여 독성 및 복귀돌연변이능 평가

        천연생리활성물질 소재로서 파고지 추출물의 안전성을 검증하기 위하여 렛트에 대한 단회 경구투여 독성 평가와 복귀돌연변이평가를 실시하였다. 파고지 에탄올 추출분말 2 g/kg, 5 mL를 암${\cdot}$수 각각 5마리씩 경구 투여한 후 14일 동안의 일반증상, 체중변화 및 부검시의 육안적 소견을 관찰한 결과, 사망례, 일반증상 및 체중변화는 관찰되지 않았고 부검결과에서도 특이할 만한 육안적 이상소견은 관찰되지 않아, 파고지 추출물은 $LD_{50}$값이 렛트 체중 kg당 2g 이상이며 단회 경구 투여에 대한 독성을 가지는 않는 것으로 평가되었다. 파고지 추출분말의 복귀돌연변이 유발성 여부는, 히스티딘 요구성 Salmonella Typhimurium TA98, TA100, TA1535, TA1537 균주 및 트립토판 요구성 Escherichia coli WP2uvrA(pKM101) 균주를 이용하여 S9 mix 첨가 여부에 따라 실험하였다. 용량설정시험 결과, S9 mix 첨가 여부에 관계없이 사용한 모든 시험균주에서 생육저해가 관찰되었다. 시험물질의 석출은 S9 mix 첨가 여부와 상관없이 사용한 모든 시험균주의 1,250, 2,500 및 5,000 ${\mu}g/plate$에서 관찰되었지만 콜로니 계수에는 영향이 없었다. 따라서, 본 시험의 용량은 S9 mix 미첨가 TA100, TA1537 균주 및 S9 mix 첨가 RA1535 균주의 경우에는 생육저해가 관찰된 625 ${\mu}g/plate$를, S9 mix 첨가 여부에 관계없이 TA98 균주 및 S9 mix 첨가TA100, TA1537, WP2uvrA(pKM101) 균주의 경우에는 생육저해가 관찰된 1,250 ${\mu}g/plate$를, S9 mix 미첨가의 TA1535 및 WP2uvrA(pKM101) 균주의 경우에는 2,500 ${\mu}g/plate$를 최고용량으로 하였고, 그 이하 공비 2로 최고용량을 포함한 5용량의 시험물질군과 음성대조군 및 양성대조군으로 하여 실험을 실시하였다. 그 결과, 모든 시험균주에서 시험물질에 의한 복귀변이 콜로니수는 S9 mix의 첨가 여부에 관계없이 용량의존적으로 증가되지 않았으며, 음성대조군과 비교하여 2배 이상의 증가를 보이지 않았다. 시험물질에 의한 균주의 생육저해는 S9 mix첨가 여부에 관계없이 사용한 모든 시험균주의 최고용량에서 관찰되었다. 시험물질의 석출은 S9 mix 첨가 여부에 관계없이 1,250 및 2,500 ${\mu}g/plate$에서 관찰되었지만 콜로니 계수에는 영향이 없는 것으로 나타났다. This study was performed to examine the toxicity of Psoralea corylifolia L. by the single-dose oral toxicity tests in rat and bacterial reverse mutation assay. In single-dose oral toxicity tests, 5 mL ethanol extract of P. corylifolia L. were directly injected into 10 rats (5 males and 5 females) at a dosage of 2 g/kg. Death practice was not detected during breeding periods (14 days), and $LD_{50}$ was calculated over 2 g/kg. No difference were observed with control group in the growth rate and histological observations. In bacterial reverse mutation assay, his(-) Salmonella Typhimurium TA98, TA100, TA1535, TA1537 and trp(-) Escherichia coli WP2uvrA (pKM101) were used for assessing the toxicity of ethanol extracts of P. corylifolia L.. No significant difference in formation of the colonies and no dose-dependent increase was observed regardless of the addition of S9 mix. The results showed that ethanol extracts of P. corylifolia L. did not have single-dose oral toxicity and mutagenic toxicity.

        Development of a Modified Korean East Asian Student Stress Inventory by Comparing Stress Levels in Medical Students with Those in Non-Medical Students

        Background: Medical students are usually under more stress than that experienced by non-medical students. Stress testing tools for Korean medical students have not been sufficiently studied. Thus, we adapted and modified the East Asian Student Stress Inventory (EASSI), a stress testing tool for Korean students studying abroad, and verified its usefulness as a stress test in Korean university students. We also compared and analyzed stress levels between medical and non-medical students. Methods: A questionnaire survey was conducted on medical and non-medical students of a national university, and the responses of 224 students were analyzed for this study. Factor analysis and reliability testing were performed based on data collected for 25 adapted EASSI questions and those on the Korean version of the Global Assessment of Recent Stress Scale (GARSS). A correlation analysis was performed between the 13 modified EASSI questions and the GARSS, and validity of the modified EASSI was verified by directly comparing stress levels between the two student groups. Results: The 13 questions adapted for the EASSI were called the modified EASSI and classified into four factors through a factor analysis and reliability testing. The Pearson’s correlation analysis revealed a significant correlation between the modified EASSI and the Korean version of the GARSS, suggesting a complementary strategy of using both tests. Conclusion: The validity and reliability of the EASSI were verified. The modified Korean EASSI could be a useful stress test for Korean medical students. Our results show that medical students were under more stress than that of non-medical students. Thus, these results could be helpful for managing stress in medical students.

        A Treatment Case of Delayed Aortic Injury: The Patient with Posterior Rib Fracture

        A 66-year-old male patient arrived at the emergency room with a crush injury to his chest. Multiple rib fractures, hemothorax on both sides, left scapular fracture, liver laceration, and retroperitoneal hematoma were found upon the radiologic examination. After closed thoracostomy, the patient had been initially admitted to the intensive care unit, but he was transferred to the general ward on the next day. On the 4th post-trauma day, the patient complained of severe pain and there was bloody drainage through the chest tube. This case is an exploration with the consideration of the possibility of major bleeding and the subsequent repair of the descending thoracic aorta. This case is regarded as a case in which the aorta wall was damaged as the sharp margin of the fractured ribs caused continuous irritation.

        A Treatment Case of Endoscopic Removal of Left Ventricular Thrombus, During Coronary Artery Bypass Graft

        Left ventricular thrombus is a common complication related to acute myocardial infarction. Removing this with an incision of the free wall of the left ventricle may cause fatal cardiac dysfunction or arrhythmias. Furthermore, performing incision and suture on the fragile myocardium of an acute myocardial infarction patient may cause serious bleeding complications. If there is a patient with left ventricular thrombus who needs thoracotomy for another reason, the case is attempted with the thought that if effective intraventricular visualization and manipulation can be done, fatalities caused by incision and suture may be reduced. For patients undergoing cardiopulmonary bypass, if intracardiac manipulation is required, an endoscope can be used, and given the potential complications after the incision and suturing of the infarcted tissue, the benefits are deemed sufficient.
