An Adolescent Case of Castleman Disease of the Plasma Cell Type Treated with Radiotherapy

Castleman disease (CD) is a disorder with distinct lymphoid hyperplasia of unknown origin, and it is rare in children. There are two major forms of CD: unicentric (localized) and multicentric. Pathologically, it can be categorized into three subtypes: hyaline-vascular, plasma cell, and mixed types. We experienced a 15-year-old boy with histopathologically proven mediastinal CD of the plasma cell type. He was referred to our hospital because of mediastinal widening in a chest x-ray found by chance. Surgery was performed to resect two right paratracheal masses and subtotal excision was done. As complete surgical resection was not possible, the patient subsequently received three-dimensional conformal radiation therapy (total 5,400 cGy in 27 fractions). The size of the mass decreased, and his clinical symptoms regressed without any side effects from radiotherapy. Here we report a case of unicentric plasma cell type CD, treated with radiotherapy due to incomplete surgical resection.

In the manufacturing process of steel rod, a number of micro-scale defects occur along the axial direction on the surface of the rods. To prevent failures in advance, a method to detect the micro-scale cracks remaining on the surface of rod is required. This work employs an Electromagnetic acoustic transducer (EMAT) to evaluate the cracks of the wire rods with the noncontact Ultrasonic reverberation spectroscopy (URS). The resonance at the cross section of the cylindrical rods is generated by horizontally polarized shear waves driven in the circumferential direction. Through a series of URS experiments, changes in the resonant frequency, the amplitude of the resonant peak, and the attenuation of the ultrasonic reverberation were observed at intact and defected parts of specimens. The results showed that the URS technique may be one of the effective tools to detect the axial surface microcrack with its depth of a few tens of micrometers.

The on-line monitoring for the wall thinning in secondary system has been considered one of main issues for the safety of nuclar power plants. To establish the on-line monitoring technique for the pipe wall thinning, the development of the ultrasonic transducer working in high-temperature is very important. In this investigation, the magnetostrictive transducer is concerned for high temperature condition up to 300℃. The magnetostrictive transducer has many advantages such as high working temperature, durability, cost-effectiveness, and shear waves, most of all. A thin Fe-Co alloy patch whose Curie temperture is over 900 ℃ was employed as a ferromagnetic material for magnetostriction. Wave transduction experiments in various temperature were carried out and the effect of bias magnets was considered together with the dry coupling performance of the transducer. From experimental results, consequently, it was found that the magnetostrictive transducer works stable even in high temperature up to 300 ℃ and can be a promising method for the on-line monitoring of the wall thinning in nuclear power plants.

A 70-year-old man presented with acute dysarthria and dizziness. He denied any history of trauma or cervical manipulations within several weeks before symptom onset. We could make a presumptive diagnosis of left Wallenberg syndrome through the results of neurologic examination, which include left limb ataxia, alternating hyp(o)esthesia, spontaneous nystagmus to right side, and left side Honer’s syndrome. Initial diffusion weighted imaging performed at admission showed small and discrete high signal lesions in left lateral medulla, left cerebellar hemisphere, and bilateral occipital areas. Contrast enhanced MRA demonstrated a fi lling defect in long segment of distal left vertebral artery. On 4th days after symptom onset, the patient developed a severe form of ipsilateral hemiparesis. Follow-up brain MRI showed a downward extension of the initial ischemic lesion in upper medulla to upper cervical region. This case suggests that a severe form of ipsilateral hemiparesis may be complicated in the clinical setting of acute lateral medullary infarction with vertebral artery occlusion. Isolated pulsatile tinnitus is a rare condition in patients with neurological disorders but can be the only clue to a potentially life-threatening disease such as intracranial vascular malformation. We report a patient with pulsatile tinnitus caused by an arteriovenous fistula of the external carotid artery, which was successfully treated with coil embolization.

Pipe wall thinning in the secondary piping system of a nuclear power plant is currently amajor problem that typically affects the safety and reliability of the nuclear power plantdirectly. Regular in-service inspections are carried out to manage the piping system onlyduring the overhaul. Online thickness monitoring is necessary to avoid abrupt breakagedue to wall thinning. To this end, a transducer that can withstand a high-temperatureenvironment and should be installed under the insulation layer. We propose a thin platetype of embedded ultrasonic transducer based on magnetostriction. The transducer wasdesigned and fabricated to measure the thickness of a pipe under a high-temperaturecondition. A number of experimental results confirmed the validity of the present transducer.
