        제 1형 비타민 D 의존성 구루병 1례

        “Rickets” is the term applied to impaired mineralization at epiphyseal growth plate, resulting in deformity and impaired linear growth of long bones. Rickets may arise as a result of vitamin D deficiency or abnormality in metabolism. Vitamin D-dependent rickets(VDDR) is rare autosomal recessive disorder in which affected individuals have clinical features of vitamin D deficiency. In 1961, Prader first described this disorder including severe clinical features of rickets, such as hypophosphatemia, hypocalcemia, muscle weakness and seizure. Two distinctive hereditary defects, type I VDDR and type II VDDR have been recognized in vitamin D metabolism. Type I VDDR may be due to congenital defects of renal 1 α-hydroxylase, the enzyme responsible for conversion of 25(OH)D3. These patients have low to detectable 1,25(OH)2D3 in presence of normal to raised 25(OH)D3. In type II VDDR, renal production of 1,25(OH)2D3 is intact but 1,25(OH)2D3 is not used effectively and target organ resistant to 1,25(OH)2D3 is respectively derived from the abnormality in the vitamin D receptor. We report a case of a 25 month-old girl with typical clinical features of VDDR type Ⅰ rickets, hypocalcemia, increased alkaline phosphatase and secondary hyperparathyroidism. 저자들은 구루병의 가족력이 있으면서 구루병의 전형적인 임상소견과 저칼슘혈증, 알칼라인 포스파타제의 상승, 2차적인 부갑상선 기능 항진증을 보인 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

        니트의 신축성에 따른 상의의 패턴 설계방법 연구

        With the purpose of developing a pattern alteration system for stretch fabrics, basic bodice pattern for knit was reduced in length and width. Nine different knits were used for the experiment. For each knit, six sleeveless shirts were made with patterns reduced in length and width by 6 different pattern alteration rates: 0%, 30%, 40%, 50%, 60%, or 70% of the stretch rate of the knits. The panel that was composed of 25 clothing specialists evaluated the fitness of the garments. The results of this study are as follows: The appropriate pattern alteration rate was different by the fabric stretch rate at wale and course. The different pattern alteration rate was needed at bust, waist, and hips. The knits with high stretch rate at course needed low alteration rate to have appropriate garment fit. The rib knits needed lower alteration rate than jersey or interlock to get the appropriate bodice width. The garment length was most appropriate at 20~30% of pattern length alteration rate.

        양측입가에 발생한 피부 결절아밀로이드증

        Primary cutaneous nodular amyloidosis is a rare variant of primary cutaneous amyloidosis and manifests as nodular, light-chain, amyloid deposits that are localized to the skin. The pathogenesis of the disease is still unclear. It usually originates in the lower extremities, face, scalp, and genitals and presents clinically as waxy, yellowish erythematous colored nodules. Histological characteristics include diffuse homogenous eosinophilic deposits in the dermis or subcutaneous regions along with interspersed plasma cells. We report a case of a 72-year-old man who presented with fissured, erythematous, waxy nodules on the bilateral lip angles. To the best of our knowledge, this is the first case report of primary cutaneous nodular amyloidosis of the lip angles in Korean dermatological literature.

        Impact of Longitudinal Changes in Metabolic Syndrome Status over 2 Years on 10-Year Incident Diabetes Mellitus

        Background: Metabolic syndrome (MetS) is a known predictor of diabetes mellitus (DM), but whether longitudinal changes in MetS status modify the risk for DM remains unclear. We investigated whether changes in MetS status over 2 years modify the 10- year risk of incident DM. Methods: We analyzed data from 7,317 participants aged 40 to 70 years without DM at baseline, who took part in 2001 to 2011 Korean Genome Epidemiology Study. Subjects were categorized into four groups based on repeated longitudinal assessment of MetS status over 2 years: non-MetS, resolved MetS, incident MetS, and persistent MetS. The hazard ratio (HR) of new-onset DM during 10 years was calculated in each group using Cox models. Results: During the 10-year follow-up, 1,099 participants (15.0%) developed DM. Compared to the non-MetS group, the fully adjusted HRs for new-onset DM were 1.28 (95% confidence interval [CI], 0.92 to 1.79) in the resolved MetS group, 1.75 (95% CI, 1.30 to 2.37) in the incident MetS group, and 1.98 (95% CI, 1.50 to 2.61) in the persistent MetS group (P for trend <0.001). The risk of DM in subjects with resolved MetS was significantly attenuated compared to those with persistent MetS over 2 years. In addition, the adjusted HR for 10-year developing DM gradually increased as the number of MetS components increased 2 years later. Conclusion: We found that discrete longitudinal changes pattern in MetS status over 2 years associated with 10-year risk of DM. These findings suggest that monitoring change of MetS status and controlling it in individuals may be important for risk prediction of DM.

        Glycated Albumin Is a More Useful Glycation Index than HbA1c for Reflecting Renal Tubulopathy in Subjects with Early Diabetic Kidney Disease

        Background: The aim of this study was to investigate which glycemic parameters better reflect urinary N-acetyl-β-Dglucosaminidase (uNAG) abnormality, a marker for renal tubulopathy, in subjects with type 2 diabetes mellitus (T2DM) subjects with normoalbuminuria and a normal estimated glomerular filtration rate (eGFR). Methods: We classified 1,061 participants with T2DM into two groups according to uNAG level—normal vs. high (>5.8 U/g creatinine)— and measured their biochemical parameters. Results: Subjects with high uNAG level had significantly higher levels of fasting and stimulated glucose, glycated albumin (GA), and glycosylated hemoglobin (HbA1c) and lower levels of homeostasis model assessment of β-cell compared with subjects with normal uNAG level. Multiple linear regression analyses showed that uNAG was significantly associated with GA (standardized β coefficient [β]=0.213, P=0.016), but not with HbA1c (β=–0.137, P=0.096) or stimulated glucose (β=0.095, P=0.140) after adjusting confounding factors. In receiver operating characteristic analysis, the value of the area under the curve (AUC) for renal tubular injury of GA was significantly higher (AUC=0.634; 95% confidence interval [CI], 0.646 to 0.899) than those for HbA1c (AUC=0.598; 95% CI, 0.553 to 0.640), stimulated glucose (AUC=0.594; 95% CI, 0.552 to 0.636), or fasting glucose (AUC=0.558; 95% CI, 0.515 to 0.600). The optimal GA cutoff point for renal tubular damage was 17.55% (sensitivity 59%, specificity 62%). Conclusion: GA is a more useful glycation index than HbA1c for reflecting renal tubulopathy in subjects with T2DM with normoalbuminuria and normal eGFR.

        CT Features of Vasculitides Based on the 2012 International Chapel Hill Consensus Conference Revised Classification

        Vasculitis, characterized by inflammation of vessel walls, is comprised of heterogeneous clinicopathological entities, and thus poses a diagnostic challenge. The most widely used approach for classifying vasculitides is based on the International Chapel Hill Consensus Conference (CHCC) nomenclature system. Based on the recently revised CHCC 2012, we propose computed tomography (CT) features of vasculitides and a differential diagnosis based on location and morphological characteristics. Finally, vasculitis mimics should be differentiated, because erroneous application of immunosuppressive drugs on vasculitis mimics may be ineffective, even deteriorating. This article presents the utility of CT in the diagnosis and differential diagnosis of vasculitides.

        Metabolic Syndrome Severity Score in Korean Adults: Analysis of the 2010–2015 Korea National Health and Nutrition Examination Survey

        Background: Continuous metabolic syndrome (MS) severity scores that can track metabolic risk in individuals over time have been developed for Western populations. The present study aimed to develop gender- and age-specific equations for MS severity scores in Korean adults. Methods: Using data from the Korea National Health and Nutrition Examination Surveys (KNHANES) IV (2010–2012) and VI (2013–2015), we performed a confirmatory factor analysis of single MS factor that allowed for differential loadings across groups to generate gender- and age-specific, continuous MS severity scores. Then, we validated this equation in a different dataset of Korean adults. Results: In confirmatory analysis, waist circumference had the highest factor loading, indicating that waist circumference had the strongest correlation with MS among Korean adults. Lower factor loadings (< 0.4) among Korean adults aged 40–59 years were noted for systolic blood pressure and fasting glucose. MS severity score values were significantly correlated with metabolic parameters, including high-sensitivity C-reactive-protein, glycated hemoglobin, and homeostasis model assessment of insulin resistance. Furthermore, MS severity scores well predicted traditional MS according to receiver operating characteristic analysis in a validation dataset (KNHANES VII). In a longitudinal cohort dataset, participants diagnosed with Adult Treatment Program III (ATP-III) MS after an initial assessment had progressively higher baseline MS severity scores in relationship to their time until ATP-III MS diagnosis. Conclusion: The new MS severity score equations for Korean adults proposed in this study provide a clinically-accessible continuous measure of MS for potential use in identifying adults at higher risk for MS-related diseases and following changes within individuals over time.

      • 인공와우와 보청기 양이착용이 K-HINT 수행도에 미치는 혜택

        The present study examined performance of 10 normal-hearing listeners (control group) and 10 cochlear implant (CI) users who had CI on one ear and HA on the opposite ear (CI+HA) in order to investigate the amount of binaural benefit on K-HINT (Korea-Hearing in Noise threshold) test. Additionally, a K-SSQ (SSQ translated in Korean) survey was also conducted to all CI participants in order to evaluate their subjective benefit from the use of CI+HA. Results of the K-HINT test for YNH and CI participants were as follows. For YHN listeners, -7.2 dB SNR, on average, was required to identify K-HINT sentences by 50%. For CI users, 15.32 dB SNR was needed for 50% of K-HINT sentence identification during CI-only listening condition whereas 11.82 dB SNR was required for 50% of K-HINT sentence identification during CI+HA listening condition. CI users’ binaural benefits calculated from K-HINT results were 4.6 dB signal-to-noise ratio (SNR), 3.4 dB SNR, and 3.8 dB SNR for head shadow effect, binaural squelch, binaural summation effects, respectively. Results of K-SSQ survey suggested that the degree of satisfaction was around 62%, regardless of three sections (speech, spatial and quality) in K-SSQ. CI users recieved a significant benefit from binaural use (CI+HA), regardless of the direction of noise.
