N/A Objectives: EGFR is a transmembrane molecule and is characterized by tyrosine protein kinase activity and ligand-stimulated autophosphorylation. It was found in various cell types including fibroblast and keratinocytes. EGFR appears to have an important role in the development and proliferation of certain human malignancies, including those of neuroglia, bladder, and breast. The heterogeneity of EGFR expression is known to be related with proliferating fraction, histological grade, and susceptibility of anti- cancer drugs, etc. We investigated the biologic significance of the expression of EGFR in lung cancer tissues. Methods: The expression of EGFR was assessed immunohistochemically on paraffin imbedded lung cancer tissues including 28 primary sites with 16 lymph nodes using monoclonal antibody and indirect peroxidase technique. Results: 1) EGFR expression of primary tumor in squamous cell carcinoma was higher than in the others. 2) EGFR expression of primary tumor in stage III was higher than in stage I and stage II. 3) EGFR expression rates of primary and metastatic sites were 60.7% and 61.1%, respectively. No significant difference between primary site and metastatic one was noticed. 4) EGFR expression rates in primary tumor with metastasis was 64.7% and without metastasis was 54.5%. 5) EGFR expression rates of lung cancer by nodal stage were 50% in N1 and 60% in N2. But significant difference among nodal stages was not noticed. 6) EGFR expression rates of lymph nodes according to EGFR positivity of primary tumor were 50% in lymph nodes that have negative steins in lungs and 60% in lymph nodes that have positive stains in lungs. 7) No significant difference in 12-month survival between positive group and negative group was noticed. Conclusion: EGFR expression of routinely processed tissue is a simple technique for the assessment of cell growth and terminal differentiation in non small cell lung cancer. Whether EGFR expression has an independent prognostic value and deserves special attention in pathobiological evaluation in lung cancer remains to be investigated from large series with longer follow - up and to be correlated with multiple biological markers.

Corticosteroids are widely used in the treatment of bronchodilator-resistant bronchial asthma and various forms of anaphylaxis because of their antiinflammatory and immunosuppressive properties. Since Hollander reported 4 cases of urticaria out of a total of 10,000 intraarticular injections of hydrocortisone in 1953, several cases of bronchospasm with urticaria and angioedema after intravenous corticosteroids infusion have been reported. We recently experienced a case of bronchospasm after intravenous Solu-Cortef(Solu-Cortef, the Upjohn co., Kalmazoo, Mich.) infusion to 24 year-old female asthmatic patient with aspirin hypersensitivity for the purpose of symptom relief. After intravenous administration of Solu-Cortef 125mg, she complained tachypnea, severe dyspnea, and coughing with chest tightness. Several minutes later, cyanosis appeared. Two times of subcutaneous epinephrine(1: 1,000, 0.3cc) injections and manual bagging with endotracheal intubation were done. She was stabilized by mechanical venulauon 150 minutes after the attack of bronchospasm. And then she was admitted to ICU for mechanical ventilatory care. It was suggested that the bronchospasm be caused by intravenous Solu-Cortef infusion. Therefore, we emphasize the history taking of any corticosteroid hypersensitivity before its clinical use.
PCR을 이용한 Rat 기관지 세척액에서의 Pneumocystis carinii DNA의 검출 : 기초실험 A pilot study

Background: Pneumocystis carinii (PC) is a opportunistic pathogen causing serious pneumonia (Pneumocystis carinii pneumonia : PCP) in immunosuppressed patients including AIDS. Laboratory diagnosis of PCP is dependent on microscopic demonstration of the PC by using cytochemical staining or by immunocytochemical staining with monoclonal antibodies. However, these staining methods are not highly sensitive. The development of the polymerase chain reaction (PCR) has made it possible to detect very small numbers of pathogens in clinical specimens. To establish the usefulness of PCR for detection of PC DNA, We performed PCR as a pilot study. Methods: PCP was induced on Spraque-Dawley rats with prednisolone 5mg IM twice a week. Bronchial lavages, impression smears, and permanent sections were performed from the 3rd week to 7th week of cortisonized rats. PC DNA was extracted with bead beater / 10% CTAB method. Results: Impression smears showed PC cyst after the 4th week (100%, 28/28 rats), and permanent sections showed 27/28 rats. Control rats showed some PC cysts (2/7). PCR result was positive only one case among the 6th week rats (1/7), But positive 6 cases at the 7th week rats, (6/7). Among control rats. 2 cases were positive (2/7). ??This data is a pilot study for the PC DNA detection using a PCR. PCP were successfully cortisonized rats. If adequate bronchial washing technique will be used, PCR can be a sensitive for PC DNA detection.
중합효소연쇄반응을 이용한 도말양성 폐결핵 환자에서의 결핵균 DNA 검출방법에 관한 연구

Polymerase chain reaction (PCR) is the most sensitive and rapid method for the diagnosis of M.tuberculosis infection. Many reports have shown different methods for mycobacterial DNA extraction, but revealed many differences in simplicity and time-consumption. We studied which method was the best among 7 different ones for DNA extraction from 32 smear-positive sputa, using PCR of targeting 123bp DNA fragment of IS6110. The methods were the following : SDS-Microwave oven method, NaOH Lysis method, Triton X-100, Proteinase K method, Lysis buffer method. SDS-Proteinase K method, Bead Beater method, and Bead Beater/CTAB method. The amplification products were identified by electrophoresis on ethidium bromide-stained agarose gels, and confirmed by restriction enzyme, Southern blot hybridization, and DNA sequencing. All 32 cases were positive (100%) by the 7 different methods. It took 1 hour or more to detect DNA in SDS-Microwave oven method, NaOH Lysis method, and Bead beater/CTAB method. It took 2 hours or less in the others. SDS-Proteinase K method and Bead Beater/CTAB method were simpler and more convinient than the others. These results suggest that SDS-Proteinase K method and Bead Beater/CTAB method might be better than the others for the extraction of mycobacterial DNA in terms of simplicity and time-consumption, although all the methods were sensitive.

The Cefodizime is a cefotaxime derivatives with broad antibacterial spectrum similar to that third generation cephalosporins, and has immune stimulator effect, and it is effective againist pathogens responsible for respiratory tract infections like Streptococcus pneumoniae or Haemophilus influenzae. The antimicrobial efficacy of cefodizime was observed in 30 patients with respiratory tract infections in Chungnam National University Hospital from April to August, 1992. The resluts were as follows: 1. The cefodizime was highly effective in 27(90%) of 30 patients clinically in respiratory tract infections. 2. There were no significant side effects except skin rash (1 case), mild elevation of liver enzymes (2 cases), and mild elevation of creatinine (1 case). These side effects were improved after cefodizime withdrawal. As above results, cefodizime was considered to be effective antibiotics inpatients with respiratory tract infections.
