      • 외상성으로 발생한 급성 요배통 환자에 대한 도침치료 3례 : 증례보고

        Objectives:The purpose of this study is to report the effect of acupotomy on pain, lumbar function, and quality of life in acute low back pain patients. Methods:We performed Acupotomy in three patients who have acute low back pain due to a traumatic motive within one week. NRS was used to evaluate patients' pain, ROM and RMDQ were used to evalutate lumbar function, EQ-5D and EQ VAS were used to evaluate quality of life, and a five-point Likert scale was used to evaluate treatment satisfaction. Results: Acupotomy reduced the patient's NRS, increased ROM and reduced RMDQ. EQ-5D increased in cases 1 and 3 but decreased in case 2 and EQ VAS increased in all cases. Conclusions: This study shows acupotomy has useful effect and acupotomy can be a effective treatment on acute low back pain patients. 이 증례보고의 목적은 도침 시술이 외상성으로 유발된 급성 요배통 환자의 통증, 기능, 삶의 질에 미치는 영향을 보고하기 위함이다. 1주일 이내의 외상성 계기로 인해 요배통을 호소하는 총 3명의 환자에게 도침 시술을 시행하여 NRS, ROM, RMDQ, EQ VAS에서 유의한 호전이 있었으며 EQ-5D의 경우 증례 1,3에서 유의한 호전이 있었다. 향후 보다 많은 증례보고 및 연구가 필요할 것으로 사료된다.

        신바로약침과 침치료를 이용한 족저근막염 환자 4례에 대한 임상증례보고

        Objects : This study is a report on treatment effect of the case that treated by using of Shinbaro pharmacopunture and acupuncture to plantar fasciitis patients. Methods : The four patients diagnosed with Plantar fasciitis were treated by using of Shinbaro pharmacopunture and acupuncture, Algometer Pressure and Numeric rating scale(NRS) were used as the evaluation tool of treatment effect. Results : Shinbaro pharmacopunture and acupuncture showed improvement of symptoms for all patient groups and brought about significant change on numerical value of Algometer Pressure and Numeric rating scale(NRS). Conclusions : The result shows that Shinbaro pharmacopunture and acupuncture can be effective treatment to Plantar fasciitis. Merely later, further study is needed.

        마황부자세신탕(麻黃附子細辛湯)의 각 구성약물별 항산화 및 항염 효능에 관한 연구

        ObjectivesThis study was carried out to find out the Antioxidant and Anti-inflammatory Effects of Components of Mahwangbujaseshin-tang in LPS-Stimulated RAW264.7 Macrophages.MethodsThere are 5 experimental groups. ; normal, control, EH (Ephedrae Herba), ALRP (Aconiti Lateralis Radix Preparata) and AR (Asiasari Radix). The extract of EH, ALRP and AR (100 μg/ml) was added to each group. We examined cytotoxicity, total phenolic contents, DPPH and ABTS free radical scavenging activity, Intracellular ROS (reactive oxy-gen species) production, NO (Total Nitric oxide), iNOS (inducible nitric oxide synthase), PGE2 (prostaglandin E2), COX-2 (cyclooxygenase-2), IL-1β (interleukin-1β), IL-6 (inter-leukin-6), TNF-α (tumor necrosis factor-α), MMP-9 (matrix metalloproteinase-9), TIMP-1 (tissue inhibitor of metalloproteinase-1) and HO-1 (heme oxygenase-1) expression level.Results1. Total phenolic contents of EH were in the highest level. 2. DPPH and ABTS free radical scavenging activity of EH was in the highest level. 3. ROS production was sig-nificantly decreased in AR. 4. NO production was significantly decreased in EH, ALRP, AR and iNOS expression was decreased in EH, AR. 5. PGE2 and COX-2 expression was de-creased in EH, AR. 6. IL-1β production was significantly decreased in EH, AR and IL-6 production was significantly decreased in AR. TNF-α production was significantly de-creased in ALRP, AR. 7. MMP-9 and TIMP-1 production were significantly decreased in EH. 8. HO-1 expression was significantly increased in EH. 9. With simultaneous usage of SnPP which is expression inhibitor of HO-1, NO, IL-1β, IL-6 and TNF-α production were partially increased in EH, ALRP, AR.ConclusionsAccording to this study, Components of Mahwangbujaseshin-tang have anti-oxidants and anti-inflammation effects in LPS-Stimulated RAW264.7 Macrophages.         Small non-coding RNA를 발현하는 형질전환 벼의 환경위해성 평가 방법

        Since the RNA interference (RNAi) had been discovered in many organisms, small non-coding RNA-mediated gene silencing technology, including RNAi have been widely applied to analysis of gene function, as well as crop improvement. Despite the usefulness of RNAi technology, RNAi transgenic crops have various potential environmental risks, including off-target and non-target effects. In this study, we developed methods that can be effectively applied to environmental risk assessment of RNAi transgenic crops and verified these methods in 35S::dsRNAi_eGFP rice transgenic plant we generated. Off-target genes, which can be non-specifically suppressed by the expression of dsRNAi_eGFP, were predicted by using the published web tool, pssRNAit, and verified by comparing their expressions between wild-type (WT) and 35S::dsRNAi_eGFP transgenic rice. Also, we verified the non-target effects of the 35S:: dsRNAi_eGFP plant by evaluating horizontal and vertical transfer of small interfering RNAs (siRNAs) produced in the 35S::dsRNAi_eGFP plant into neighboring WT rice and rhizosphere microorganisms, respectively. Our results suggested that the methods we developed, could be widely applied to various RNAi transgenic crops for their environmental risk assessment.

        요추부 질환에 적용하는 추나 치료에 대한 국내 연구 동향

        Objectives : The present study examines the domestic trend of Chuna treatments on lumbar spinal disorder in Korea. Methods : We investigated the studies on Chuna treatments for lumbar spinal disorder via searching 10 Korean web databases. As a result, 63 research papers were found to be analyzed according to their published year, the titles of journals, the types of study, the techniques of Chuna, the instruments for assessment, the Chuna technique and the number of the treatment trials by the cases of lumbar spinal disorder and ethical approvals. Results : The number of the research papers published tends to increase every year. The studies on Chuna treatments were mainly published in The Journal of Korea CHUNA Manual Medicine for Spine & Nerves. The most frequently adopted technique of Chuna in the examined studies was Cox flexion & distraction technique. Visual analogue scale(VAS), oswestry disability questionnaire(ODI) were used as primary means of assessments. The ethical problems of the examined studies needed to be improved. Conclusion : Reviewing the domestic trend of studies on Chuna treatments for lumbar spinal disorder and examining the strong and weak points of those treatments are essential for the future studies. It is anticipated that this review benefits the future in-depth study on the treatments for Chuna in Korean medicine.

        하지방사통을 동반한 요추 추간판 탈출증 환자에 대한 이상근 MSAT 치료군과 비치료군의 후향적 호전도 비교 연구

        Objectives The purpose of this study was to compare the effects of MSAT (Mostion style acuputure treatment) on piriformis muscle regarding leg radiating pain with HIVD of L-SPINE. Methods Forty patients who received inpatient treatment from Febuarary 1 to March 31, 2014 in the Daejon-Jaseng oriental medicine hospital were divided into 2 groups by blocked randomization : Group A (n=20)=routine treatment on HIVD of L-SPINE with MSAT on piriformis muscle and Group B (n=20)=routine treatment on HIVD of L-SPINE (acupuncture, Shinbaro pharmacopuncture, herb medication, chuna treatment and physiotherapy). The MSAT (Mostion style acuputure treatment) on piriformis muscle was administered each day and both groups were received routine treatment each day. Evaluations were made before treatment, after 3 days and 7 days, and before discharge using the NRS (numeric rating scale) and ODI (oswestry disability index). The statistically significance was evaluated by SPSS 18.0 for windows. Results Compared to before treatment, the NRS (numeric rating scale) and ODI (oswestry disability index) of both A and B groups after 3, 7 days and before discharge of treatment significantly decreased (p<0.05). Regarding group comparison, the NRS (numeric rating scale) and ODI (oswestry disability index) of A group was significantly lower than the B group at 3, 7 days, before dischanre (p<0.05). Regarding improvement of the NRS (numeric rating scale) and ODI (oswestry disability index), A group was significantly lower than the B group at 3 day (NRS), 7day (ODI) (p=0.000) but there was no significant difference between the two groups at 7 days (NRS) and before discharge (p>0.05). Conclusions Compared to just routine treatment, the MSAT (Mostion style acuputure treatment) on piriformis muscle significantly improved the NRS (numeric rating scale) and ODI (oswestry disability index) after 3, 7 days, and before discharge. Because early intervention determines the grade of Leg radiating pain with HIVD of L-SPINE, the MSAT (Mostion style acuputure treatment) on piriformisat the early stage will be clinically helpful to patients.         요통 환자의 요추 주변 근육 단면적과 신체안정성(Stability), 체중심(Center of Pressure)과의 관련성 : 동적평형검사(Tetrax)를 중심으로

        Objectives : To identify the association between cross-sectional area of lumbar regional muscle and stability(ST), center of pressure(COP) assessed by tetrax. Methods : Patients(n=55) who had taken L-SPINE MRI and Tetrax(Sunlight, Israel) were analyzed retrospectively. To evaluate the cross-sectional area of lumbar regional muscle, L-SPINE MRI was used. Data of ST, COP were accumulated by using dynamic equilibrium analysis by Tetrax. Of the lumbar regional muscles, the Psoas muscles and the Paraspinalis muscles were examined. Using Pearson correlation, we analyzed COP, ST and the difference between the cross-sectional area of lumbar regional muscles. Results : The variance of cross-sectional area of both sides of Psoas muscle and COP had moderate positive correlation(r=0.621). Between variance of cross-sectional area of both sides of Paraspinalis and lumbar regional muscle and COP there was low positive correlation(r=0.287, r=0.329) ST also had low correlation with variance of cross-sectional area of both sides of Psoas muscle. Conclusion : The variance of cross-sectional area between both sides of Psoas muscle had moderate correlation with COP.

        호프의 탄닌과 폴리페놀류의 분리

        호프의 탄닌 및 다른 폴리페놀류를 실온에서 70% 아세톤으로 추출하였으며 Ellagic acid, Narigenin, Quercetin, Morin, Flavonol과 Gallic acid가 TLC와 역상 HPLC 분석에 의해 확인되었다. 아세톤 추출물을 여러 용매를 사용하여 다시 분획하였고 Ellagic acid, Morin, Quercetin, Narigenin 과 Flavonol이 헥산 분획에 포함되어 있었다. 에틸 아세테이트층에서는 Narigenin, Flavonol이 있었다. 부탄올층에서는 Ellagic acid, Flavonol, Morin과 Quercetin이 발견되었다. 탄닌의 주성분은 Gallic acid와 Ellagic acid인 것으로 판명되었다. 부탄올 층에서 Quercetin과 Quercetin-3-Rhamnoside를 sephadex LH-20과 MCI gel CHP-20으로 크로마토그라피 되었으며, 이들의 구조를 ¹HNMR, ^(13)CNNMR과 Mass spectroscopy로 확인하였다. Tannin and other polyphenols in Hop was extracted by 70% of aquous acetone at room temperature. Ellagic acid, Narigenin, Quercetin, Morin, Flavonol and Gallic acid were identified by TLC or reversed phase HPLC. In addition, the aquous acetone extract was further extracted with various organic solvent. Ellagic acid, Morin, Quercetin, Narigenin and Flavonol contained in the hexane extraction. The ethyl acetate layer contained Narigenin and Flavonol. Ellagic acid, Flavonol, Morin, Narigenin and Quercetin were found in the fraction of buthanol. Gallic acid and Ellagic acid were detected as main tannin compound in Hop. Quercetin and Quercetin-3-Rhamnoside were isolated by sephadex LH-20 and MCI gel CHP-20 chromatography in the fraction of buthanol and their structure were identified by ¹HNHR, ^(13)CNMR and Mass spectroscopy.
