        경계골 소실이 동반된 안와내하벽골절에서 생체흡수성 코폴리머의 겹치기 방법을 이용한 정복술

        Purpose: To report the results of surgical repairing of inferomedial orbital wall fractures accompanied with loss of bony strut by an overlapping method using Resorb X<sup>® plates (poly(D,L)-lactic acid). Methods: To repair inferomedial orbital wall fractures with loss of bony strut, we bent the medial aspect of the plate for floor fracture according to the anatomical structure and then inserted the floor implant, which was supported by anterior, posterior and lateral margins of the floor fracture. Then, the medial implant, which was supported by the bent medial edge of the floor implant, was inserted. Postoperatively, we analyzed the outcomes of 21 patients regarding postoperative enophthalmos, limitation of eye movement (LOM) and diplopia. The diagonal lengths of the orbit in the operated side were compared with the non-traumatized side based on the coronal view of the postoperative CT scans to determine the success or failure of the reconstruction. Results: The average postoperative enophthalmic value was 0.5 mm, and no significant differences between the orbital diagonal lengths of the operated and non-operated eyes were observed. The LOM shown in 7 out of 8 patients was completely resolved during the postoperative follow-up period. All patients with diplopia, including one patient who showed partial remission of LOM, experienced complete symptom resolution. No other complications related to the implant were recorded. Conclusions: Inferomedial orbital wall fractures with the loss of bony strut can be successfully repaired by the overlapping method using Resorb X<sup>® implants.

        승마기구 운동이 뇌경색 환자의 동적균형에 미치는 영향

        Purpose: The purpose of this study was to determine the effect of mechanical horseback riding exercise on dynamic balance in patients with cerebral infarction. Methods: The subjects of this study were 30 cerebral infarction patients. Subjects in the control group received physical therapy for 45 minutes, three times per week, and those in the experimental group received mechanical horseback riding exercise for 30 minutes, three times per week, with physical therapy for 45 minutes, three times per week, during a period of six weeks. For evaluation of dynamic balance ability, limit of stability (LOS) was measured and TUG test was performed. Results: According to the test result, LOS showed a significant increase after the exercise, and TUG showed a significant decrease after the exercise. However, no significant correlation was observed between LOS difference and TUG difference. Conclusion: Mechanical horseback riding exercise is effective for improvement of dynamic balance in patients with cerebral infarction. Mechanical horseback riding exercise is a useful indoor exercise program for clinical use for improvement of dynamic balance.

        삼차신경통에 대한 경피적 신경차단술 후 발생한 외향신경마비 1예

        Purpose: To report a case of abducens nerve palsy after a percutaneous nerve blocking procedure for trigeminal neuralgia. Case summary: A 35-year-old female complaining of stabbing pain in the right maxillary area 4 months in duration was diagnosed with trigeminal neuralgia at a pain clinic. The patient underwent a percutaneous trigeminal nerve blocking procedure using alcohol at the right maxillary nerve. After the procedure, the patient was referred to an ophthalmologic service for horizontal diplopia and abduction defect of her right eye. Her corrected visual acuity, intraocular pressure, pupillary response, anterior segment and fundus were normal bilaterally. The patient had right esotropia of 38 prism diopters in primary gaze (70 prism diopters in right gaze, 20 prism diopters in left gaze) with limited abduction of -3 in the right eye. She was diagnosed with abducens nerve palsy of the right eye. Three months after initial presentation, the patient had intermittent esotropia of 4 prism diopters at right gaze and orthophoria at the other diagnostic gazes; she presented no diplopia. Conclusions: In the present case study, abducens nerve palsy following a percutaneous trigeminal nerve blocking procedure resolved over 3 months. Because the abducens nerve is adjacent to the trigeminal nerve near the foramen ovale based on anatomical structure, when performing a percutaneous trigeminal blocking procedure, the surgeon should be aware that deep needle puncture could cause abducens nerve palsy.

        악태모형과 측모두부 계측 방사선 사진상 스피만곡 분석에 관한 연구

        Clinically, the curve of Spee is widely applied as a determined level of the occlusal curvature when the oral rehabilitation and the reconstruction of the prosthesis is needed at the malalignment dentition due to the missing, extrusion, and the inclination of the teeth. The purpose of this study was to analyze the curve of Spee of the occlusal curvature which influences to the occlusal form and the location three dimensionally, and then was to measure the radius and the degree of curvature of the curve of Spee and also was to investigate the influence to the cuspal inclination according to the change of the inclination of the curve of Spee which was analyzed by AutoCAD R.13 program at the gnathological cast and the cephalometric radiograph. The following results were obtained : 1. The radius of the curve of Spee was the mean of $11.74{\pm}3.64cm$ in the model, $12.75{\pm}4.63cm$ in the radiograph and there was no significant difference statistically between the model and the radiograph(P>0.001). 2. The radius and the degree of curvature of the curve of Spee showed negative correlation(r=-0.80), while the radius and the degree of curvature of the curve of Spee in relation to the length of the curve of Spee did not show correlation. 3. The case of the curve of Spee inclined to the posterior, that is. $Post.M{\theta}$ group showed the mean of $4.73{\pm}3.64$, positive correlation to the P2m, M1mm, M1dm, M2dm, and especially the greatest correlation coefficient to the mesial inclination angle of the mesio-buccal cusp tip of the first molar(r=0.70). 4. The case of the curve of Spee inclined to the anterior, that is, $Ant.M{\theta}$ group showed the mean of $3.28{\pm}3.59$, positive correlation to the P2m, M1mm, and also the greatest correlation coefficient to the mesial inclination angle of the mesio-buccal cusp tip of the first molar(r=0.78

        녹음된 목소리를 통한 가족중재가 인공호흡기 이탈 과정 환자에게 미치는 효과

        Purpose: This study aimed to determine the impact of an intervention using voice recording of family members on pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation. Methods: A randomized control pre-post experimental design was implemented to 53 participants, with 27 and 26 participants in the experimental and control groups, respectively. A 70-second voice recording of a family member, repeated three times at 10-minute intervals was used as an intervention for the experimental group. Meanwhile, participants in the control group used headset for 30 minutes. Structured instruments were utilized to measure pain, anxiety, agitation, and the weaning process. Wilcoxon Signed Ranks test and the Mann-Whitney U test, or χ² test, were used for data analysis. Results: The experimental group exhibited significant decrease in pain (Z = - 3.53, p < .001), anxiety (t = 5.45, p < .001), and agitation (Z = - 2.99, p = .003) scores compared with those of the control group. However, there was no significant difference between groups in the weaning process’ simplification (χ² = 0.63, p = .727). Conclusion: Intervention using family members’ voice recording effectively reduces pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation. This can be actively utilized to provide a more comfortable process for patients.

        제주도 고산 습지에서 분리한 Bacteroidetes, Firmicutes, Actinobacteria 문에 속하는 신종후보 세균

        Although Sumunmulbangdui wetland at the Halla Mountain in Jeju Island, a kind of montane wetlands, has been considered to bear high biodiversity, no study has been reported on the bacterial diversity. In this study, soil and water samples were collected from the wetland in order to isolate novel bacterial species. Bacterial strains belonging to the phyla Bacteroidetes, Firmicutes, and Actinobacteria were isolated after spreading soil and water samples onto solid agar media. The 16S rRNA gene sequences of the strains assigned to the three phyla were compared to those of type strains of the species in the phyla. The strains that showed less than 98.7% 16S rRNA gene sequence similarity to the validly published species were considered to be novel species candidates. A total of 32 strains were regarded as novel species candidates in the phyla Bacteroidetes, Firmicutes, and Actinobacteria. Diversity of novel species candidates was very low; the candidates were confined to only few genera. In the Bacteroidetes, 13 novel candidate species were affiliated with the genera Mucilaginibacter, Sphingobacterium, Pedobacter, Flavobacterium, and Chryseobacterium. A total of 13 novel candidate species that assigned to the genera Paenibacillus, Lysinibacillus, and Bacillus were identified in the phylum Firmicutes. Only two candidate species that belonged to the genera Mycobacterium and Nocardia were excavated in the Actinobacteria. Cultural, physiological, and chemotaxonomic characteristics have been determined for the novel species candidates, and the characteristics are described in this study.

      • 피트모스와 바이오차의 단일 및 복합 처리가 발효 미생물 생육에 미치는 영향

        우사 깔짚으로 사용되는 여러 소재들은 비율에 따라 수분 흡수 속도와 사용기간, 부숙화 및 악취 저감 효율 등이 매우 다르기 때문에 적절한 비율을 찾는 것이 중요하다. 특히, 부숙화 효율에 큰 영향을 미치는 인자는 미생물로 퇴비화와 관련된 미생물의 활성이 높을수록 부숙화 기간이 단축되고, 악취가 저감된다. 이에 깔짚 소재의 비율을 다르게 하여 미생물 생육을 측정하고, 최적 혼합 비율을 확립하고자 하였다. 본 연구에서 사용된 미생물은 악취 저감 능력이 우수한 Bacillus subtilis, Saccharomyces cerevisiae 등 단일 미생물 2종과 단일 미생물 2종을 섞은 혼합미생물 1종을 사용하였다. 깔짚 제조 공정 확립을 위한 실험에서 깔짚 소재(피트모스:바이오차=8:2) 150 mL에 미생물 3%(v/v)를 첨가하고 하루동안 배양한 후 생균수를 측정하였다. 깔짚 소재 최적 혼합 비율을 확립하기 위한 실험에서 깔짚 소재는 왕겨, 바이오차, 피트모스의 단일 깔짚과 피트모스:바이오차=8:2, 7:3, 6:4의 혼합 깔짚을 사용하였다. 단일 깔짚 중 왕겨에서 미생물의 생육이 가장 높았으며, B. subtils의 생육이 가장 우세하였다. 혼합 깔짚에서 미생물 생육은 피트모스의 비율이 높아질수록 증가하는 경향을 보였다. 그 결과 피트모스:바이오차=6:4인 깔짚에서 가장 높은 미생물 성장이 나타났다. 따라서 본 연구의 결과를 통해 우사 깔짚의 최적 혼합비율은 피트모스:바이오차=6:4로 선정되었다.

      • Mesial Temporal Lobe Epilepsy in a Patient with Peutz-Jeghers Syndrome

        포이츠-예거 증후군은 장관 용종증과 피부점막의 색소침착을 특징으로 하는 매우 드문 질환이다. 대부분의 임상 양상은 용종에 의한 장관 합병증이나 악성 종양으로 나타나며, 신경계 증상 발현에 대해서는 잘 알려지지 않았다. 저자들은 포이츠-예거 증후군 환자에서 해마 경화증에 의한 내측 측두엽 뇌전증의 발생을 경험하였기에 보고하는 바이다. 포이츠-예거 증후군으로 추적관찰 중이던 14세 여아가 비유발성 전신 긴장간대 발작을 주소로 내원하였다. 포이츠-예거 증후군 외의 다른 과거력은 없었으며, 가족력도 없었다. 이학적 검사, 신경학적 검사, 혈액검사는 이상이 없었으며, 뇌파에서 우측 측두-두정부의 극파가 관찰되었다. 2회 이상의 비유발 경련이 발생하여 뇌전증으로 진단 후 항경련제를 사용하였고, 3년간 치료 후 경련이 없어 약물 치료를 중단하였다. 약물 중단 후 경련이 재발하였으며, 재발 후 시행한 뇌자기공명영상에서 우측 해마경화증이 발견되어 해마경화증에 의한 내측 측두엽 뇌전증으로 진단되었다. 현재 약물치료를 지속 중이나 경련이 조절되지 않아 수술적 치료를 고려 중이다. 포이츠-예거 증후군에서 신경계 이상에 대해서는 잘 알려지지 않았으나, 내측 측두엽 뇌전증도 발생될 수 있으므로 정확한 진단을 통해 치료의 계획을 세우는 것이 도움되겠다. Peutz­Jeghers syndrome (PJS) is a rare disease characterized by intestinal polyposis and mucocutaneous pigmentation. Central nervous system manifestations of this disease are not yet fully known. A 14­year­old girl with PJS visited clinic after experi­encing the first unprovoked seizure. She experienced impairment of consciousness after an epigastric sensation followed by a generalized seizure for about 2 minutes. She had undergone small bowel segment resection because of the small bowel intussusception caused by small intestinal polyposis at the age of 5. She was diagnosed with PJS on the basis of presence of melanoic pigmented lip and multiple hamartomatous polyps in gastrointestinal tracts. The electroencephalography showed occasional sharp wave discharges on the right temporo­parietal areas. After her second seizure, she was diagnosed with focal epilepsy and began to receive anticonvulsants. With anticonvulsants, her seizures had been controlled for 3 years. However, when the anticonvulsants were discontinued, her seizures recurred after 6 months and persisted. The brain magnetic resonance imaging showed asymmetrical atrophy of the right hippocampus. The final diagnosis was the mesial temporal lobe epilepsy with right hippocampal sclerosis. Currently, seizures are not controlled completely with anticonvulsants, and surgical treatment is being taken into consi­deration. Mesial temporal lobe epilepsy might be considered one of central nervous system manifestations in PJS.

      • 축사 깔짚의 부숙화 효율 증진을 위한 미생물제의 기초 특성 평가

        축산업의 대규모화와 사육두수가 증가함에 따라 가축분뇨 발생량도 꾸준히 증가하고 있다. 현재 톱밥에 의한 깔짚 사용은 미완숙된 축산분뇨 퇴비 문제를 해결하는데 한계가 있으며, 여러 환경문제를 야기시키고 있다. 미생물이 혼합된 발효깔짚은 병원균의 증식을 억제하고, 가축으로부터 배설물이 배출되면 수분조절과 함께 저온 발효가 진행될 수 있게 하여 부숙 시간을 단축하고 부숙도를 안정적으로 관리할 수 있다. 이에 본 연구에서는 안정적인 부숙도 관리를 위해 미생물 제제를 깔짚과 혼합하여 가축분뇨 내의 유기물 분해를 가속화시키고자 하였으며, 우선 발효깔짚의 부숙화 효율을 증진하기 위해 필요한 단일 미생물 3종(B. subtilis, L. sakei, S. cerevisiae)과 3종(L. casei, S. cerevisiae, P. deserticola)이 혼합된 혼합미생물 1종의 퇴비화 시 기초 생육 특성을 평가하고자 하였다. 이를 위해서 탄소원, 질소원, 온도, pH에 의한 영향을 조사하였고, 각각의 실험조건에서 미생물의 생장속도는 OD<SUB>600nm</SUB>를 측정하였다. 미생물 생육특성은 탄소원 종류별(Sucrose, Lactose, Acetate, Citrate)로 조사하였으며, 질소원 종류별(Peptone, Yeast extract, Urea, NH₄Cl)로 조사하였다. 실험 결과 부숙화 효율 증진을 위한 미생물제의 최적 탄소원은 Sucrose, 최적 질소원은 Yeast extract, 최적 온도는 35℃, 최적 pH는 7~9 이었으며, 사용된 미생물 모두 퇴비 부숙에 적합한 것으로 판단되었다.
