        Case I These muscles were 21.8cm long and 3.3cm wide and located on both sides. They arose from the tendon of the opposite sternocleidomastoid muscles on the manubrium between first and second ribs. They terminated on the aponeuroses of rectus abdominis muscles and external oblique' abdominal muscles on same sides at the level between 7th and 8th ribs. They formed a diamond shape on chest. Case II This muscle was located on the right side of sternum and run obliquely and inferolaterally. It was 9.9cm long and 2.5cm wide. It arose from the sternal border between 2nd and 3rd ribs and terminated on the 6th ribs.

        Awake GlidescopeⓇ intubation in a patient with a huge and fixed supraglottic mass -A case report-

        Intubating patients with a huge, fixed supraglottic mass causing an obstruction of the glottis is difficult to most anesthesiologists. We attempted awake fiberoptic orotracheal intubation assisted by GlidescopeⓇ Videolaryngoscope (GVL) following topical anesthesia with 4% lidocaine spray and remifentanil infusion. The glottis could not be identified by the GVL view. However, by entering toward the right side of the mass with bronchoscope, the glottis was found. Due to stiffness of the mass, we were unable to further enter the area using the bronchoscope. Alternatively,we attempted to expose the glottis by GVL blade and then successfully intubated the patient by manually pressing the cricoids cartilage. GVL is nonetheless an excellent instrument in airway management compared to fiberoptic bronchoscope for patients with a huge and fixed supraglottic mass.

        급개시 전단유동장에서 천연 다당류 잔탄검 농후용액의 과도적 레올러지 거동 : Wagner 구성방정식을 사용한 응력도약현상 예측

        The present study has been designed to theoretically predict the transientrheological behavior of concentrated xanthan gum systems in start-up shear flow fieldsusing the Wagner constitutive equation. Using an Advanced Rheometric Expansion System(ARES), a number of constant shear rates were suddenly imposed to aqueous xanthan gumsolutions with different concentrations and then the resultant shear stress responses weredetected with time. The linear and nonlinear stress relaxation moduli at various deformationmagnitudes were also measured to determine the damping function. The linear relaxationmodulus was characterized by a power-law expression to determine the memory functionand a time-strain separability of the nonlinear relaxation moduli was employed to predictthe nonlinear response. The experimentally obtained damping function was compared withthe fitted results calculated from the two mathematical forms of the Wagner and Soskey-Winter equations in order to examine the effect of damping function on the predictiveperformance of the Wagner model. The overall applicability of the Wagner model forpredicting the whole procedures of a transient rheological behavior at start-up of steadyshear flow was discussed in depth. The main findings obtained from this study aresummarized as follows : (1) The Wagner model has a predictive ability to qualitativelyexpress the whole procedures of a transient rheological behavior of concentrated xanthangum solutions for all shear rates imposed, regardless of selecting a damping function. (2)The values of the maximum reduced stress predicted by the Wagner model employing theWagner damping function exhibit an almost equal magnitude, irrespective of the shearrates imposed, whereas those predicted by the Wagner model employing the Soskey-Winter damping function are gradually decreased with an increase in imposed shear rate. (3) For all shear rates applied, the Wagner model having the Wagner damping function hasa fairly good ability to predict the time at which the maximum stress occurs, tmax, while theWagner model having the Soskey-Winter damping function predicts a much faster value oftmax. (4) The Wagner model using both of the Wagner and Soskey-Winter damping functionshas a weakness with respect to predicting a stress decay which always show a slowerdecrement than does the predicted results by the Wagner model using the two differentforms of a damping function. (5) The Wagner model adopting the Wagner dampingfunction exhibits a superior performance to the Wagner model adopting the Soskey-Winterdamping function for predicting the whole steps of a transient rheological behavior.

        Rheological Properties of Wool Fat : Role as a Main Ingredient in Skin-Care Cosmetic Products

        The principal objective of the present study has been to systematically investigate the rheological properties of wool fat as a main ingredient of skin-care cosmetic products. Using a strain-controlled rheometer, the steady shear flow properties of commercially available wool fat were measured over a wide range of shear rates, and the linear viscoelastic properties of the material in small amplitude oscillatory shear flow fields were measured over a broad range of angular frequencies. In addition, the temperature dependency of the linear viscoelastic behavior was investigated over a temperature range most relevant to usual human life. The main findings obtained from this study are summarized as follows : (1) Wool fat exhibits a finite magnitude of yield stress and a marked non-Newtonian shearthinning flow behavior. The appearance of a yield stress and a shear-thinning feature can be explained by the crystalline networks existing within the material. A yield stress is correlated with consistency whereas a shear-thinning viscosity is associated with spreadability. (2) The linear viscoelastic behavior of wool fat is dominated by an elastic nature rather than a viscous nature. This behavior can be interpreted by the orientation of hydrocarbon chains inside a monodomain. This linear viscoelastic feature could become a criterion to assess the storage stability of cosmetic products at rest or at very small deformations. (3) A modified form of the Cox-Merz rule provides a good ability to predict the relationship between steady shear flow properties (nonlinear behavior) and dynamic viscoelastic properties (linear behavior) for wool fat. (4) The storage modulus and loss modulus show a qualitatively similar tendency to sharply decrease with an increase in temperature. At a temperature range higher than 40°C, both of the two moduli exhibit a more pronounced temperature dependency while the difference between the two moduli becomes smaller. In the case of the storage modulus, an unexpected plateau region is observed over a temperature range between 25 and 30 °C.

        위상고정 회로를 이용한 X-band DRO 설계 및 제작에 관한 연구

        본 논문에서는 X-band(8 GHz)용 위상고정 유전체 공진 발진기를 설계 및 제작하였다. 직렬 궤환형 구조로 높은 선택도(Q)를 가지고 유전체 공진기와 주파수 동조를 위하여 바랙터 다이오드, loop filter와 10GHz까지 동작하는 prescaler를 이용하여 구성하였으며, 전압제어 발진기의 신호와 수정 발진기의 신호를 위상 검출기에 넣어 두 신호의 위상을 검출.고정함으로써 높은 안정도의 신호원을 얻을 수 있도록 구현하였다. 측정 결과 본 논문의 위상고정 유전체 공진 발진기는 8GHz에서 2.5dBm의 출력과 -26dBc의 고조파 억압과 중심주파수에서 10KHz offset에서 -64.33dBc의 위상잡음을 얻었으며, 전압제어 유전체 공진 발진기에 비하여 높은 주파수 안정도를 가지는 것을 볼 수 있었다. In this paper, the PLDRO is designed and implemented for X-band. It is comprised of tunable high Q resonator with a varactor diode for frequency tuning, loop filter and a 1/8 prescaler which up to 10GHz. Also, it is implemented a TCXO and a VCO signal into the phase detector and achieved a highly stable signal source. From the measurement, the designed PLDRO has the output power of 2.5dBm at 8GHz and phase noise of -64.33dBc at 10KHz offset from carrier. Its characteristic is 26 dBc. This PLDRO has much better temperature stability.

        Stickler 증후군 환자의 마취 경험 -증례 보고-

        Stickler syndrome is a connective tissue dysplasia disorder with characteristic midface hypoplasia, retromicrognathia, cleft palate, and a moon-shaped appearance. Incidence of the syndrome is estimated at around 1/10000 and the disorder is considered to be caused by mutations in the COL2A1, COL11A1, COL11A2, COL9A1 procollagen genes of type 2 and 11 collagen. Patients with a mandibular hypoplasia like Stickler syndrome present the anesthesiologist with considerable problems when mask ventilation or endotracheal intubation is attempted. We report a successful anesthetic experience, including blind endotracheal intubation with rigid laryngoscope without neuromuscular blockade, in a 9-year-old boy with Stickler syndrome for scleral buckling with cryotherapy.         Full-thickness rotator cuff tears are relatively uncommon in the young adults. One of the pathogenesis of such tear is thought to be closely related to the specific trauma event. Favorable outcome is expected in young patient rotator cuff tears when it is diagnosed early following prompt surgical repair. However, early detection is sometimes difficult when the acute rotator cuff tear is combined with other injuries especially around the shoulder joints such as ipsilateral humerus fractures. Authors report an uncommon case of acute traumatic rotator cuff tear accompanied by the midhumerus shaft fracture in young adult.
