국내 연안에서 분리한 Vibrio vulnificus의 특성

2002년 5월에서 7월사이 한국 근해로부터 총 가검물 50례 중에서 V. vulnificus은 5균주가 분리되었고, API 20E kit의동정률이 $89.4\~93.7\%$로 나타났다. V. vulnificus 16S rDNA를 증폭하여 얻은 산물을 cloning하여 염기서열을 결정하고 분석한 결과 V. vulnificus로 확인되었다. 동정된 분리균주들의 cell lysates을 SDS-PAGE로 분석하였고 표준균주로 사용된 V. vulnificus ATCC 27562와 분리 균주들을 비교해 본 결과 단백밴드pattern이 많은 차이를 보였으나 외막단백질은 V. vulnificus간에는 공통의 밴드가 확인되었으나 V. parahaemolyticus 와는 차이를 보였다. Five strains of Vibrio vulnificus, which cause serious septicemia in human worldwidely, was isolated from marine environments of Korea costal area from May to July of 2002. The isolated strains were identified by API 20E kit and partial 16S rDNA sequence analysis. 16S rDNA sequence of the isolates showed $99\%$ similarity with V. vulnificus ATCC 27562. The proteins of V. vulnificus isolates were examined by analyzing patterns of the cell lysates and outer membrane proteins (OMP). The OMP separated from cell lysates showed the common protein band. Therefore OMP profiles might be useful for the identification of V. vulnificus sp.

병원성 비브리오와 비병원성 비브리오 사이의 세포외 효소 활성 차이점을 비교하기 위하여 마우스를 이용하였다. 각 균주들을 마우스에 약 10^8 CFU로 복강 주사한 다음 다시 분리하여 세포외 효소 활성을 비교 검토하였다. 병원성 비브리오의 세포외 효소 탐색은 protease(BSA, casein, skim milk, gelatin), mucinase, hemolysin, hyaluronidase 및 chondroitin sulfatase 활성을 측정하여 수행하였다. 그리고 elastase 활성, fibrinolysis 활성을 측정하는 방법을 본 연구에 맞게 확립하였다. 치사율이 높게 나타나는 균주에서는 시험 대상의 세포외 효소의 활성이 모두 높게 나타났으며, 치사율이 낮아질수록 세포외 효소의 활성은 낮게 나타났다. 치사율에 비례하여 활성을 높게 보이는 효소는 대부분의 proteases, hemolysin이 뚜렷하였으며, 그 외의 대부분의 세포외 효소들도 병원성 균주에서 높은 활성을 보였다. Hemolysis의 경우는 치사율이 높은 V. vulnificus, V. parahemolyticus 등의 균주에서는 β 완전 용혈소의 분비가 뚜렷하게 관찰되는 반면, 치사율이 낮은 균주에서는 α 불완전 용혈소의 분비를 관찰할 수 있었다. V. harveyi는 비병원성 균으로 알려졌음에도 불구하고 병원성 균에서 분비하는 세포외 효소들을 대부분 분비하고 치사율도 매우 높음을 보여주었다. 결론적으로 병원성 비브리오균주의 높은 세포외 효소의 활성은 이 효소의 생산이 비브리오균들의 pathogenicity에 깊은 연관성이 있음을 시사한다고 볼 수 있다. The members of the genus Vibrio include harmless aquatic strains as well as strains capable of causing infections in human and fish. Pathogenic mechanisms are only understood for Vibrio cholerae O1 and O139 and not for the majority of Vibrio species. Twelve clinical and nonclinical strains were examined by in vitro and in vivo experiments for the importance of extracellular enzymes as a virulence determinant of Vibrio species. In vivo cytotoxicity assay was performed by injecting approximately 10^8 cells/mL into mice (BALB/c). V. harveyi and V. vulnificus showed 100% lethality within 3 hr after bacterial injection. V. fluvialis and four strains of V. parahaemolyticus showed 50% lethality within 4 hr. V. mimicus, V. alginolyticus and V. furnissii revealed 30% lethality within 9 hr. Nonclinical strains, V. campbellii and V. ordalii, did not show any lethality. In vitro protease and hemolytic activities were also good indicators for clinical and nonclinical strains of Vibrio species. The clinical strains showed much higher activities than nonclinical strains. The activity of some clinical strains of re-isolates was evidently increased. Most clinical strains had β hemolytic activity. The results demonstrate that the prevalent distribution of extracellular proteases in pathogenic Vibrio sp. implies their importance as a virulence determinant.

Purpose: Postoperative hypocalcemia is a common complication of thyroidectomy. This study evaluated the incidence and predisposing risk factors for postoperative permanent hypocalcemia after total thyroidectomy. Methods: There were 1,247 consecutive patients undergoing total thyroidectomy and complete treatment and observation for differentiated thyroid cancer between January 2012 to December 2012 who were enrolled in this study. Patients were divided into two groups, those remaining normalcalcemic (Group I-824 pts) and those who had hypocalcemia requiring treatment (Groups II-423 pts). Group II was subdivided into a transient hypocalcemic group (Group IIA-409 pts) and a permanent hypocalcemic group (Group IIB-14 pts). Results: Female gender, thyroiditis, preserved parathyroid number, lateral lymph node metastasis, RAI treatment, preoperative parathyroid hormone and preoperative vitamin D were significantly associated with the development of postoperative hypocalcemia by multivariate analysis. Comparing patients with transient versus permanent hypocalcemia, tumor size and multiplicity were significantly related to the development of permanent hypocalcemia by multivariate analysis. RAI treatment and parathyroid hormone level on the postoperative third day were significantly related to recovery from transient hypocalcemia to normo-calcemia. Conclusion: Risk factors of postoperative hypocalcemia were associated with preoperative patient factors and advanced thyroid cancer. Advanced thyroid cancer was a risk factor for permanent hypocalcemia. To prevent postoperative hypocalcemia, we should focus on patient condition and need to preserve parathyroid gland more carefully in thyroid surgery.
해양에서 분리한 Vibrio vulnificus 의 면역학적 특성

N/A Immunoglobulin G was purified by 40%(NH_4)_2SO_4 precipitation, DEAE-Sephadex, Sephadex G-150 column chromatographies from rabbit antiserum against V. vulnificus ATCC 27562 O antigen and used for immunological test for V. vulnificus isolates. The profiles of cell lysate total protein and outer membrane protein from the isolates were analyzed by SDS-PAGE and densitometry. The overall profiles in all isolates were similar. Distict protein band was observed in comparison with V. parahaemolyticus. Western Blotting with rabbit Immunoglobulin G against cell lysates and OMP of V. vulnificus isolates showed a strong antigenic response to antigen 66, 60, 54, 48, 33 and 26kDa which were common to all strains examined. The 26kDa antigen showed V. vulnificus specific antigen in comparison with Vibrio parahaemolyticus. A sandwich enzyme-linked immunosorbent assay was developed by using rat anti-V. vulnificus ATCC 27562 polyclonal antibodies as capture antibody, a purified rabbit IgG antibody as detector antibody, and goat anti-rabbit IgG-alkaline phosphatase conjugate as developer antibody. When four V. vulnificus isolates were tested, the reactivity showed from 50 to 70% by sandwich ELISA.
