코로나19 가을철 재유행 대비 긴급점검 좌담회: 시립병원 대응 중심

서울시공공보건의료재단에서는 9월 24일 코로나19 가을철 재유행 대비 긴급 좌담회를 개최하였다. ‘시립병원’을 중심으로 현재까지의 경험을 평가하고 다가올 재유행에 대비하기 위해 (1) 코로나 시대 공공의료의 전략적 의미 (2) 코로나 시대 시립병원의 역할 및 발전방향에 대해 논의하였다. 임준 국립중앙의료원 공공보건의료센터장, 소예경 서울시공공보건의료재단 선임연구위원이 발표를, 권영대 가톨릭대학교 교수, 권용선 서울시 시립병원운영팀장, 김병관 서울특별시보라매병원장, 박찬병 서울특별시서북병원장, 송관영 서울특별시서울의료원장, 윤상철 갈렙앤컴퍼니대표, 이병철 서울특별시시립병원혁신센터 설립추진단 부단장이 함께 토론을 하였다. 본고는 논의된 핵심내용을 재정리한 것으로, 코로나 시대 서울시립병원은 병원별 실질적 협력체계 구축, 경증환자 대상 생활치료센터 확보, 경증환자 대상 새로운 치료방식의 도입 등의 과제를 제시하였다.

갑상선 석회화의 방사선학적 양상

The purpose of this study was to analyse the various patterns of calcification demonstrated in the anterior and lateral neck roentgenograms of 213 unselected patients with thyroid enlargement. The patterns of thyroid calcifications were correlated with clinical, surgical and histological findings. The results were as follows : 1. Of 23 cases of thyroid enlargement, 180 cases were benign and 168 cases were female. 2. The calcification rate was high in the chronic thyroid enlargement. 3. The incidence of calcification was 30.2% in the malignancy and 17.2% in the benign disease. There was no calcification in the Hashimoto's diseases. 4. The nodular calcification was demonstrated in the both benign and malignant disease but curvilinear calcification was predominantly seen in benign disease.

MicroRNA-552 expression in colorectal cancer and its clinicopathological significance

Background: MicroRNA-552 (miR-552) has been reported to correlate with the development and progression of various cancers, including colorectal cancer (CRC). This study aimed to investigate miR-552 expression in cancer tissue samples compared to normal mucosal tissue and its role as a diagnostic or prognostic marker in CRC patients. Methods: Normal mucosal tissues and primary cancer tissues from 80 surgically resected CRC specimens were used. Quantitative real-time polymerase chain reaction was performed for miR-552 and U6 small nuclear RNA to analyze miR-552 expression and its clinicopathological significance. Immunohistochemistry for p53 and phosphatase and tension homolog (PTEN) was performed to evaluate their association with miR-552 expression. Results: miR-552 expression was significantly higher in primary cancer tissues compared to normal mucosal tissues (p < .001). The expression level of miR-552 was inversely correlated with that of PTEN (p = .068) and p53 (p = .004). Survival analysis showed that high miR-552 expression was associated with worse prognosis but this was not statistically significant (p = .255). However, patients with CRC having high miR-552 expression and loss of PTEN expression had significantly worse prognosis than others (p =. 029). Conclusions: Our results suggest that high miR-552 expression might be a potential diagnostic biomarker for CRC, and its combined analysis with PTEN expression can possibly be used as a prognostic marker.

사지기형증(Peromelia)의 1예

Amelia including hemimelia is a rare case in congenital anomaly of newborn infants. Authors experienced a case of peromelia (gestation period: 39 weeks, breech delivery), Who had multiple anomalies such as absence of external genitalia, anal atresia, duodenal obstruction and hemivertebrae. Autopsy findings revealed the complete duodenal obstruction between second and third portion, duodenal dilatation from pyloric ring to second portion of duodenum which filled with the greenish meconium and anal atresia. X-ray findings showed multiple bone anomalies such as hemivertebrae, dislocation of femur head due to incomplete formation of pelvis. We could not recognized the sex of this baby externally. We confirmed that this baby is a male by chromosome study and pathologic findings.

다문화청소년의 부모진로지지가 진로장벽에 미치는 영향: 사회적 위축, 진로태도 매개효과

배경: 다문화청소년은 일반청소년보다 진로발달에 있어서 더 큰 어려움을 겪고 있다. 또 부모의 영향이 또래보다 더 큰 영향을 미친다고 보고되고 있다. 그렇기에 다문화청소년이 경험하는 진로장벽과 부모의 영향 관계를 연구할 필요성이 있다. 목적: 지속적으로 증가하는 다문화가정의 부모진로지지가 다문화청소년의 진로장벽에 미치는 영향 관계를 살펴봄으로써 진로에 관한 다양한 방안을 수립하는 기초자료를 제공하고자 함이다. 방법: 본 연구에서는 한국청소년정책연구원에서 제공하는「다문화청소년패널 데이터」의 8차년도 데이터를 사용하였다. 결과: 첫째, 부모진로지지가 다문화청소년의 진로장벽에 부적인 영향을 미치는 것으로 확인되었다. 둘째, 부모진로지지가 다문화청소년의 진로장벽에 미치는 영향에서 사회적 위축의 매개효과는 유의한 것으로 확인되었다. 셋째, 부모진로지지가 다문화청소년의 진로장벽에 미치는 영향에서 부모진로지지는 진로태도에 정적인 영향을 미치는 것으로 확인되었다. 넷째, 부모진로지지가 다문화청소년의 진로장벽에 영향을 미치는데 사회적 위축과 진로태도를 매개로 하여 다문화청소년의 진로장벽에 영향을 미치는 간접경로가 통계적으로 유의한 것으로 확인되었다.

급성 백혈병 환자에서 항암제 투여 후 뇌 CT 소견

A study was performed to evaluate the atrophic changes of the central nerve system after chemotheraphy in the patients with acute leukemia. the computed tomographic findings and medical records 20 proven acute leukemia patients under 35 years-old who developed various CNS symptoms and signs during and/or after 2 coures of chemotheraphy were reviewed. The results were as follows : 1. Age distribution was 14 to 35 years (mean was 26 years) Male was 15. 2. Presenting clinical symptoms and signs were headache (16/20) nausea and vomiting (11/20) and loss of con-sciousness(5/20) 3. Brain atroply was noted in 16 patients including cortical and subcortical atrophy 15 cases and succortical atrophy 1 case. 4. Two cases of hemorrhage one each of intracranial hematoma and chronic subdural hematoma were found in addition to brain artophy. This showed that chemotherapeutic agents cause brain atrophy in a considerable number of the patients with symptomatic acute leukemia.

STRATEGIC CHOICE OF MANUFACTURING FLEXIBILITY IN INTERMEDIATE GOODS MARK

This paper examines the strategic choice of flexible versus dedicated manufacturing technologies in intermediate goods markets. We show that it may not be profitable for upstream firms to choose flexible technologies in a simple successive duopoly model. The reason is that the choice of flexible technologies reduces the profits from downstream captive firms. We find that downstream competition is an important factor affecting the choice of production technologies in the upstream market. 보험산업 사업구조개편 활성화를 위한 제도 개선 방안

○ 본고에서는 보험산업 사업구조개편과 관련된 주요국 사례를 살펴보고 국내 제도 개선 관련 시사점을 도출하고자 함 ㆍ 보험산업은 사업구조개편을 통해 성장이 둔화되고 있는 기존 사업분야의 자본을 향후 성장이 예상되는 신규 사업분야로 재배치함으로써 자본의 효율성(Capital Efficiency)을 제고할 필요가 있음 ○ 먼저, 영국의 경우에는 2000년 보험계약자의 동의 없이 법원의 승인하에 다른 보험회사로의 계약이전이 가능하도록 하는 제도를 도입했음 ㆍ 2000년 제도 도입 이후 2021년까지 300건 이상의 계약이전이 있었는데, 솔벤시Ⅱ 대응, 잔존계약의 처리, 운영효율성 및 자본효율성 제고 등이 계약이전의 주요 동기였음 ○ 영국에 비해 미국은 계약이전제도의 도입이 상대적으로 늦은 편이었는데, 2018년 오클라호마 주에서, 그리고 2021년 아칸소 주에서 영국과 유사한 형태의 제도를 도입했음 ㆍ 한편, 미국 일부 주에서는 보험계약자의 동의 없이 규제당국의 승인만으로 기업분할이 가능하도록 하는 제도를 도입했음 ○ 일본의 경우에는 보험산업 구조조정 활성화를 위해 포괄이전제도(책임준비금의 산출 기초가 같은 보험계약은 분리해서 계약이전하지 못하도록 하는 제도)를 2013년에 폐지했음 ㆍ 2000년 상법 개정에 따라 회사분할제도가 도입되면서 다른 산업에서는 이 제도를 활용한 다양한 형태의 구조조정이 이루어졌으나, 보험산업은 포괄이전제도로 인해 한계가 존재했음 ○ 주요국 사례분석의 시사점은 다음과 같음 ㆍ 첫째, 계약이전제도의 경우에는 주요국 사례를 참고하여 포괄이전으로 제한되어 있는 현행 보험업법 규정을 폐지하고 일부이전이 가능하도록 할 필요가 있음 ㆍ 둘째, 기업분할의 경우 현행 보험업법에는 명시적인 규정이 없는데, 상법에 의할 경우 사실상 기업분할에 어려움이 있기 때문에 미국 사례를 참고하여 보험계약자의 동의 없이 규제당국의 승인하에 기업분할이 가능하도록 하는 제도 도입 검토가 필요함
